Összefoglaló. Bevezetés: A ritka betegséggel élők ellátásában fontos előrelépések történtek az elmúlt években. Egy következő lépés lehetne hazánkban a Ritka Betegségek Nemzeti Erőforrás (Uni-Versum) Központjának (a továbbiakban: Központ) létrehozása, amely az egészségügyi, szociális és köznevelési szolgáltatásokat összehangolva és kiegészítve személyközpontú ellátást nyújtana a betegek és támogatóik részére. Célkitűzés: Célunk az volt, hogy egy nemzetközi tudományos módszertan alapján javaslatokat tegyünk arra, hogy milyen eszközökkel lehet feloldani a Központ megvalósításának lehetséges korlátozó tényezőit. Módszer: A Központ megvalósíthatóságának értékelésére interdiszciplináris szakmai egyeztetést szerveztünk különböző érintett érdekcsoportok részvételével, a SELFIE H2020 kutatási projekt által kidolgozott módszertan alapján. Az előzetesen rangsorolt legfontosabb korlátozó tényezőkre lehetséges megoldási javaslatokat tettünk. Eredmények: A lehetséges korlátozó tényezőket a résztvevők relevánsnak tartották a Központ létrehozásával kapcsolatban, és ezekre összesen 17 olyan konkrét javaslat született, amelyben a résztvevők között egyetértés alakult ki. A javaslatok kiterjedtek az ellátás tartalmára, az alkalmazott technológiák támogató szerepére, a humánerőforrás-korlátok megoldására, a hatékony vezetés és szervezés kialakítására, az összetett finanszírozási struktúra kialakítására és a kutatási lehetőségek megteremtésére is. Megbeszélés: A Központ megvalósítása esetén a ritka betegséggel élők ellátása az egészségügyi, szociális és köznevelési tevékenységeket integráló megközelítés felé mozdulna el. A kutatás során megfogalmazott javaslatok hozzájárulhatnak a Központ létrehozásához, amennyiben megvan az ehhez szükséges szakpolitikai támogatás is. Ezen túlmutatóan, a leírt munkamódszer más integrált ellátási modellek bevezethetőségének elemzéséhez is mintaként szolgálhat. Következtetés: Összefoglalva megállapíthatjuk, hogy a Központ létrehozásához számos, előzetesen is látható korlátozó tényezőt kell feloldani. Az érdekcsoportok közös javaslatai alapján kialakítható egy olyan működési forma, amely az ellátórendszerek kiegészítésével és összehangolásával jelentős társadalmi értéktöbbletet eredményezhet. Orv Hetil. 2021; 162(45): 1818–1825. Summary. Introduction: In Hungary, significant achievements have been made in the care of patients with rare diseases in recent years. A next step could be the establishment of the National Resource Centre for Rare Diseases (hereinafter: Centre) to facilitate patient-centered complex care by the integration and supplementation of existing health, social and educational services. Objective: This research aimed to develop recommendations based on international scientific methodology to overcome potential implementation barriers of the aforementioned Centre. Method: To evaluate the feasibility of the implementation, we organized an interdisciplinary workshop with representatives of different stakeholder groups, adopting the methodology developed in the SELFIE H2020 research project. During the workshop, we discussed the previously ranked, most significant implementation barriers and made recommendations for potential solutions. Results: The potential implementation barriers were considered relevant by the participants and, reflecting on these barriers, altogether 17 recommendations were developed by consensus. These recommendations were related to the content of service delivery, use of supportive technologies, overcoming workforce issues, establishing effective leadership, implementing a complex financing structure and creating research opportunities. Discussion: Implementation of the Centre would shift the care of rare diseases towards personalized and integrated health, social and educational services. Our recommendations will contribute to the establishment of the Centre, subject to positive policy decision. Furthermore, our methodological approach could support the feasibility assessment of future integrated care solutions and programs. Conclusion: Several predictable barriers must be overcome to establish the Centre. Multi-criteria decision-making (MCDM) provides a suitable platform for groups as well as promotion of the participants' role in decision processes. This also enables the development of real participatory processes essential for the successful implementation and sustainable flood management programs. The present study contributes by applying two MCDM approaches for weighting the criteria related to the environmental impacts of flooding. Moreover, an attempt was made in this study by an extensive review of literature, and consultations with experts to identify the environmental impacts of flooding in Odisha State (India). Then, the Best Worst Method (BWM) followed by the Step-Wise Weight Assessment Ratio Analysis (SWARA) method was used to rank the environmental impacts which were considered as the risk factors. The result of this study will be useful to the governance system for an effective and proper planning, and implementation of flood mitigation projects.

Purpose This paper aims to identify and model the key barriers to implementation of project-based learning (PjBL) in higher educational institution. Design/methodology/approach Using the interpretive structural modelling (ISM) technique, the study has developed a hierarchical-based model, depicting the mutual relationships amongst the key barriers to implementation of PjBL. Additionally, the paper has performed Matrice d’ Impacts Croises Multiplication Appliqué an Classement (MICMAC) analysis to categorize the barriers in terms of their driving and dependence power. Findings The study has identified the key barriers to implementation of PjBL and presented an integrated model using ISM. Higher educational institutions need to pay attention to diagnose and overcome these hindrances for effective implementation of PjBL in their programmes. Research limitations/implications The study adopts a systematic way to model the relevant barriers to implementation of PjBL. The ISM-based model would help higher education institutions to prioritize the issues as the barriers are hierarchically structured. As the input to model development is based on the experts’ opinions, it may be biased, influencing the final output of the structural model. Originality/value The presentation of PjBL implementation barriers in the form of an ISM-based model is a new effort. The model would be useful to understand the barriers and overcome these for the successful implementation of PjBL in higher educational institutions.

The article summarizes the arguments within the scientific discussion about the search of alternative sources of research universities funding in the absence of sufficient support from the government. The main objective of the research is to define the possibility of using the endowment as a fundraising tool to attract alternative sources of funding for research universities in Ukraine. The relevance of solving this scientific issue lies in the fact that modern research universities are independent entities, which according to the Bologna Declaration assume the entire load on the future of the state and have to do it at the highest professional level. This can be ensured through a combination of traditional and modern sources of funding. The world experience of successful implementation of the endowment has been presented. The level of knowledge intensity of GDP, which is one of the key indicators that reflects the level of quality of higher education, has been analyzed. The endowment as one of the most effective fundraising tools is poorly developed in Ukraine. This fact has prompted us to research the main provisions of this phenomenon and to define the conditions for its implementation in Ukrainian research-based institutions of higher education by means of the strategic analysis. The conducted SWOT analysis has made it possible to systematize problem areas of the domestic market of educational services in the direction of creating and using the endowment and predicting possible strategies for its development. Since the Research Universities are considered to be the core of creation of intellectual and innovative products and the crucial mover of scientific and technological progress in the country and in the world, they have been chosen as the research object.  Keywords: fundraising, endowment, Research University, market of educational services, investment, alternative sources of funding

O presente trabalho tem como objetivo analisar o Atendimento Educacional Especializado, na modalidade domiciliar, considerando seu funcionamento, organização, formação do professor e impacto sobre a dinâmica familiar. Neste estudo adotou-se a pesquisa qualitativa, e como metodologia o estudo de caso e no desenvolvimento dos atendimentos pedagógicos domiciliares, foram adotados os pressupostos da pesquisa-ação. O estudo teve como participante uma aluna de 12 anos com deficiência múltipla, envolvendo as áreas cognitiva e motora. Esse estudo se desenvolveu durante o período compreendido de 2013-2015 e o acompanhamento variou de uma a duas vezes por semana. Observou-se que o Atendimento Educacional Especializado, na modalidade domiciliar, beneficiou tanto o desenvolvimento da aluna quanto auxiliou a família a entender o processo de ensino-aprendizagem. Nas considerações finais destaca-se a necessidade de uma formação/ capacitação mais específica para os professores atuantes nessa modalidade de atendimento educacional, além de recursos pedagógicos e materiais disponíveis para o ensino domiciliar.Palavras-chave: Atendimento educacional especializado. The study had participation ofone student with 12 years old with severe cognitive and motor impairment. This study had the duration of 2 years(2013-2015) and the accompaniment was once or twice a week. Therefore it is concluded that the teaching servicewas beneficial to the students development and also helped the family to understand the process of study of theirdaughter. As final consideration we see a need of a more specific formation for the teachers of this modality and theaddition of material resources for teaching at the specialized educational attendance at domiciliary mode.Keywords: Specialized educational services. En este estudio Para este trabajo, se utilizó la investigación cualitativa, como metodología el estudio decaso y en el desarrollo de los atendimientos pedagógicos domiciliarias, se adoptó el requisito de la investigación-acción.El estudio tuve como participantes una alumna de 12 años con discapacidades múltiples, involucrando áreascognitivas y motoras. Este estudio fue desarrollado por dos años (2013-2015) y el acompañamiento varió de una ados veces a la semana. Se observó que el Atendimiento Educacional Especializado, en la modalidad domiciliaria,benefició tanto en el desarrollo de la alumna, cuanto auxilió a la familia a entender el proceso de enseñanza-aprendizaje.En las consideraciones finales se destaca la necesidad de una formación/capacitación más específica paralos profesores que trabajan en este tipo específico de modalidad educacional, además de los recursos pedagógicosy materiales disponibles para la enseñanza domiciliaria.Palabras clave: Atendimiento Educacional Especializado. Objectives. Patient-Aligned Care Teams (PACTs) aim to reorganize primary care by putting the patient at the center of the care team that includes family and health care staff; embodying patient-centered care (PCC). Team members share information, and work together in a synergistic, supportive way to provide optimal care based on mutually negotiated goals to meet patient needs. Our objective was to evaluate the process, barriers, and facilitators to implementing PACT as part of a VA quality improvement effort.Study Design/Methods. Semi-structured interviews were conducted, transcribed verbatim and analyzed with constant comparative techniques using constructs from implementation and organizational frameworks. Respondent demographic and position-level data were also collected. Data were collected in 2013.Results. A purposive sample of 35 key leaders and staff involved in implementing and/or participating in PACT at VA facilities participated. Implementation barriers included: failure to gain staff buy-in for PACT early on and to alleviate existing concerns about time and resources, incongruent performance measurement, and lack of defined role/responsibilities. Implementation facilitators included: training and engaging staff in PCC delivery and support for team-building.Conclusions. Team-based approaches to care have strong literature support; less is known about implementation. This study provides insight into the implementation of team-based care. Implementation of a team-based care model should include staff preparation and support (awareness, education, and encouragement) in advance. Once implemented, continued attention to team-building efforts and staff engagement are needed for sustainability. Healthcare organizations must understand and address multiple organizational factors to successfully implement team-based care that leads to systemic and sustainable PCC delivery.

L’articolo ha l’obiettivo di mettere in luce potenzialità e criticità dell’inclusione della prospettiva dei pazienti nella ricerca sulle malattie rare e sui farmaci orfani. A tal fine, nella prima parte, si propone un’analisi epistemologica dell’utilizzo dei racconti dell’esperienza individuale della malattia nella ricerca scientifica e nei trial clinici, facendo emergere, anche attraverso gli strumenti della medicina narrativa, le sfide teoriche e operative poste dall’inclusione della soggettività del paziente e del vissuto di malattia nonché l’importanza della valorizzazione della prospettiva del paziente, sia in generale sia nella ricerca sulle malattie rare e sui farmaci orfani. Nella seconda parte, il testo analizza in particolare il ruolo degli esiti riportati dai pazienti o Patient Reported Outcomes (PROs), misure per la valutazione complessiva della salute basate sulla prospettiva dei pazienti stessi, incentrandosi sulla sperimentazione clinica nel campo delle malattie rare. In questo contesto, infatti, i racconti di malattia, raccolti e valorizzati da fonti istituzionali e associazioni di pazienti, hanno contribuito a far emergere importanti questioni critiche e difficoltà nell’impiego di outcome centrati sul paziente nello sviluppo di nuovi farmaci e trattamenti, generando una serie di documenti e raccomandazioni relative al loro utilizzo per il benessere della comunità dei malati rari. ---------- This paper aims to highlight the potentiality and criticality of including patients’ perspective in rare diseases and orphan drugs research. In the first part, we propose an epistemological analysis of individual narrations of disease experience as they are used in scientific research and clinical trials. With the help of narrative medicine approach, this analysis points out theoretical and operational challenges of a perspective that includes patient’s subjectivity and illness experience. Furthermore, it reveals the significance of patients’ standpoints in general and in rare diseases as well as in the orphan drugs research. The second part of our article focuses on the role of the Patient reported Outcomes (PROs) – which are measures for the health’s overall assessment based on patient’s perspective – by investigating the impact on clinical trials for rare diseases. In this context, illness stories, which are collected and promoted by institutional sources and patients’ associations, contribute to underline important critical issues at stake in the employment of patient-centered outcomes both in new drugs and in the treatments development. Moreover, these stories are crucial to elaborate documents and recommendations concerning the use of PROs for the rare patients’ community welfare.

Care management (CM) is a promising team-based, patient-centered approach “designed to assist patients and their support systems in managing medical conditions more effectively.” As little is known about its implementation, this article describes CM implementation and associated lessons from 12 Agency for Healthcare Research and Quality–sponsored projects. Two rounds of data collection resulted in project-specific narratives that were analyzed using an iterative approach analogous to framework analysis. Informants also participated as coauthors. Variation emerged across practices and over time regarding CM services provided, personnel delivering these services, target populations, and setting(s). Successful implementation was characterized by resource availability (both monetary and nonmonetary), identifying as well as training employees with the right technical expertise and interpersonal skills, and embedding CM within practices. Our findings facilitate future context-specific implementation of CM within medical homes. They also inform the development of medical home recognition programs that anticipate and allow for contextual variation.

Purpose The purpose of this paper is to present critical barriers and obstacles faced by Information Technology (IT) industry in the implementation of Lean Six Sigma (LSS) as the business improvement methodology. Design/methodology/approach A literature review of peer-reviewed journal articles, master and doctoral theses, paradigmatic books with managerial impact and survey reports was used to identify distinct barriers. An empirical survey, using 400 self-administered questionnaires, was then conducted. Data about 11 LSS barriers from 256 usable questionnaires, with a response rate of 64 per cent, were collected and analyzed by means of statistical data analysis software. Findings The challenges of “part-time involvement in Lean Six Sigma projects”, “time consuming”, “staff turnover in middle of project”, “difficulty in data collection” and “difficulty in identifying project scope” emerged as the most critical barriers in the context of IT industry. This research work advocates the development of a strategy for addressing the most critical barrier instead of focusing on all for successful implementation. Originality/value This paper will prove to be a fantastic resource for many researchers and practitioners who are engaged in research and applications of LSS in the IT industry. Moreover, the scarcity of literature specific to LSS in IT industry will be addressed to some extent.

Cities are critical actors in reducing global greenhouse gas (GHG) emissions; the level of knowledge, skills, and capacity, especially regarding innovative project planning such as sustainable energy (SE) projects, is fundamental for cities to meet their untapped potential. A range of indicator-based frameworks have been developed, focusing on general city economic competitiveness, yet these are often disconnected from the actual city operation and processes relevant to the investment cycle of SE projects. This paper proposes an integrated methodology for developing a City Capability Assessment Framework focusing on city planning, financing, and implementation capacity for SE projects. A principal component analysis (PCA) was applied to the results of a European city-survey to uncover areas highly relevant for the successful implementation of local SE projects. The insights gained aid policymakers to better understand implementation barriers, leading to the development of more effective actions and policy interventions towards the implementation of SE projects. The proposed framework can further enhance cities’ efforts with the implementation of local SE projects and guide potential investors to properly evaluate city capabilities and potential, when choosing local investment projects.
