Abstract Objectives: To know the aspects related to breastfeeding from the perspective of women in a city in the inland of Rio de Janeiro and to discuss the family support network created as a facilitation strategy for women to breastfeed their children. Methods: A qualitative, descriptive and exploratory research. For data collection, a semi-structured interview script instrument was developed. The focus groups were filmed, and the speeches transcribed in full. Data was divided and grouped into thematic categories. A total of ten volunteer women participated. The reference used was the National Breastfeeding Policy. Results: The following categories emerged from the focus groups: Breastfeeding: facilities and difficulties faced; Prenatal specificities and identification of the support network during breastfeeding. Conclusion and implications for practice: Breastfeeding is an action based on subjectivity, women's experience and among members of the social network. The results point to the need for effective participation of prenatal nurses who promote breastfeeding and the inclusion of the father for better participation in the entire breastfeeding process and family members, where both play a key role in promoting breastfeeding, and continued breastfeeding. RESUMOObjetivo: compreender as experiências vivenciadas por cuidadores familiares de pacientes elegíveis para cuidados paliativos. Método: estudo qualitativo, descritivo e exploratório, fundamentado na fenomenologia social. Entrevistaram-se 20 cuidadores cadastrados na atenção primária à saúde. Realizou-se a coleta de dados por meio de entrevista fenomenológica e interpretada através das fases de descrição, redução e compreensão. utilizou-se a técnica de Análise de Conteúdo na modalidade Análise Ttemática categorial, com o auxílio do programa ATLAS TI 7. Resultados: identificaram-se as categorias: Redes de apoio ao familiar cuidador; Dinâmicas e rotinas diárias de cuidados prestados pelo familiar; Sobrecargas físicas e mentais do cuidador familiar. Conclusão: vivenciaram-se pelos cuidadores importantes mudanças em suas rotinas diárias, o que acabou por ocasionar uma sobrecarga física e mental aos mesmos, levando à necessidade de redes de apoio formais e informais para estes cuidados. Observou-se a inexistência de uma rede de apoio formal aos cuidadores e a falta de treinamento e orientação suficientes para realização do cuidado junto a seu familiar. Ressalta-se a importância da atuação dos equipamentos da APS, por meio do apoio às necessidades biopsicossociais destes cuidadores. Data collection was performed through a phenomenological interview and interpreted through the description, reduction and understanding phases. As for the analysis of the data, the categorical thematic modality was used, with the help of the program ATLAS TI 7. Results: the following categories were identified: Support networks for the caregiver family; Dynamics and daily routines of family care; Physical and mental overloads of the family caregiver. Conclusion: The caregivers experienced important changes in their daily routines, which led to a physical and mental overload, leading to the need for formal and informal support networks for these care. There was a lack of a formal support network for caregivers and a lack of adequate training and guidance for the care of the family member. The importance of the performance of PHC equipment is highlighted, by supporting the biopsychosocial needs of these caregivers. Conclusión: los cuidadores vivieron importantes cambios en sus rutinas diarias, lo que acabó por ocasionar una sobrecarga física y mental a los mismos, llevando a la necesidad de redes de apoyo formales e informales para estos cuidados. Se observaron la inexistencia de una red de apoyo formal a los cuidadores y la falta de entrenamiento y orientación suficientes para realización del cuidado junto a su familiar. Se resalta la importancia de la actuación de los equipamientos de la APS, por medio del apoyo a las necesidades biopsicosociales de estos cuidadores. ABSTRACT Purpose: to analyze the work process of the Family Health teams in the municipality of Caxias do Sul, in Rio Grande do Sul State, Brazil. Methods: this is a qualitative, descriptive and exploratory research conducted in four Family Health Strategy (FHS) units, with the participation of 22 health professionals. The data were collected through a semi-structured interview and analyzed through the theme content analysis, aided by the Atlas.ti software. Results: the results were organized in three categories: (1) Roles, reality and idealization of working in the FHS, in which health prevention and curative practices appeared as the focus of the work process; (2) Elements of working in the FHS, which dealt with the purpose of the work connected to health prevention, related instruments, hard technologies and work object, in relation to professional specificities; and (3) Challenges and perspectives of working in the FHS, which emphasizes the fragilities, due to the shortage of professionals and the need for managerial improvement, as well as commitment and teamwork as potentialities. Conclusion: in the work process in health, the preventive professional actions with curative focus were predominant. The challenges were centered on redirecting the work process toward the Family Health approach.

A survey was carried out of almost 50% of Black inpatients in a state psychiatric hospital to evaluate the level of accessibility of the family network of the patients. Staff were interviewed on the problems they have with contacting families. The survey shows the extent of inadequate access and identifies reasons for the problem.

OBJECTIVE: to describe and to analyze the experience from families of children with cerebral paralysis living under circumstances of social vulnerability. METHOD: six resident families in area with this characteristic were interviewed. It was opted to use the Symbolic Interactionism as theoretic reference and the Thematic Content Analysis of Bardin as analysis method for the data. RESULT: the experience of such families is represented in the subjects: Reorganizing the Life, with the categories "Discovering the way" and "Accommodating the routine", and Stopping a Constant Fight with the categories: "Primary Carer being overcharged", "Coexisting with the preconception", "Having locomotion difficulty" and "Living with financial difficulties". CONCLUSION: the social vulnerability influences how the family bears the chronic condition. Professionals and strategies of public health are a power to minimize impacts including those related to the family budget, but they have not been effective. They need to be sensitized to become supporting resources, to offer and to guide the access to the support networks and to spur the social service in action when necessary. This study adds knowledge to the already existing by pointing out peculiarities of the family experience in situations regarding two variables of difficult handling: chronicity and social vulnerability, evidencing the role of the professional in search of the solution for the confrontation of demands and sufferings together with the family.

This exploratory descriptive study, using a qualitative approach, aimed to characterize the social support of nurses in the care of their own children. The participants were ten nurses who were mothers, selected through a snowball method. Data collection occurred from November 2011 to January 2012 through semi-structured interviews and construction of families' genograms and ecomaps. Data were analyzed through Bardin content analysis, leading to the establishment of two categories: (1) Returning to work: the importance of family support and (2) The family and their interactive contexts: types of bonds. The social support network of the family is essential to the lives of these women, who need support, assistance and guidance in directing their activities in everyday overload.

ABSTRACTthis study aimed to know the strategies of families in the care of children with chronic diseases. A descriptive and exploratory research was carried out using a qualitative approach, in a Pediatric Unit of a university hospital in the south of Brazil, during the first and second halves of 2013. The study involved 15 participating families. Data were collected by means of interviews subjected to thematic analysis. It was identified as strategies: balance work, study and care, share tasks throughout the day, adapt the physical area of the house, engage the child in their own care, seek resources in the network of social support and faith, seek assistance in another city; and engage in physical activity to reduce stress. It was concluded that it is necessary that the nurses participate of the family support network, as a support source for the strengthening of the family to the care, acquiring skills and expertise to a positive coping of the child's chronic disease.

RESUMOObjetivo: avaliar a dinâmica de uma família ribeirinha. Método: estudo qualitativo, tipo descritivo, realizado com uma família utilizando o Modelo Calgary de Avaliação Familiar (MCAF). Os dados foram obtidos por entrevista semiestruturada e analisados pela técnica de Análise de Conteúdo. Resultados: a aplicação deste Modelo investigou os principais aspectos de sua estrutura, desenvolvimento e funcionamento identificando os vínculos familiares, redes de apoio social e as funções diárias. Conclusão: o núcleo familiar estudado mostrou-se com vínculos fortes e relacionamento positivo, fortes redes de apoio social familiar e comunitário, porém com fragilidade com relação ao serviço de saúde, uma vez que os serviços oferecidos na ilha são incipientes e há dificuldade de locomoção da ilha para a busca de atendimento de saúde em outro local. O Modelo Calgary é uma ferramenta útil por considerar a família no seu contexto, além de ser um modelo próprio da enfermagem. Este estudo pode contribuir, futuramente, para que outros enfermeiros possam propor à família intervenções para a melhoria da qualidade de vida familiar, colaborando para encontrar soluções e lidar com as dificuldades do cotidiano. Results: the application of this model investigated the main aspects of its structure, development and functioning, identifying family ties, social support networks and daily functions. Conclusion: the family nucleus under study showed strong bonds and positive relationships, strong family and community social support networks, but with fragility in relation to the health service, since the services offered on the island are incipient and there is difficulty locomotion in the island to seek health care elsewhere. The Calgary Model is a useful tool for considering the family in its context, besides being a nursing model. This study may contribute, in the future, for other nurses to propose to the family interventions aimed to improve the quality of family life, collaborating to find solutions and deal with daily difficulties. Siblings of children with cancer need support to ameliorate the challenges they encounter; however, little is known about what types and sources of support exist for siblings. This study addresses this gap in our understanding of the social networks and sources of support for adolescents with a brother or sister who has cancer. Additionally, we describe how the support siblings receive addresses what they feel are the hardest aspects of being a sibling of a child with cancer. During semi-structured interviews, siblings (ages 12–17) constructed ecomaps describing their support networks. Data were coded for support type (emotional, instrumental, informational, validation, companionship) and support provider (e.g., mother, teacher, friend). Network characteristics and patterns of support were explored. Support network size ranged from 3 to 10 individuals (M = 6 ± 1.9); siblings most frequently reported mothers as sources of support (n = 22, 91.7%), followed by fathers (n = 19, 79.2%), close friends (n = 19, 79.2%) and siblings (with or without cancer) (n = 17, 70.8%). Friends and brothers or sisters most often provided validation and companionship while instrumental and informational supports came from parents. This study provides foundational knowledge about siblings’ support networks, which can be utilized to design interventions that improve support for siblings of children with cancer.

Being embedded in social networks is crucial for well-being and health. While this is particularly the case for people with Down syndrome (DS), our knowledge of how their support networks are developed is limited. This article investigates the role of family members in developing and maintaining the social support networks of their adult children with DS. Based on 29 interviews with family members, a grounded theory study was conducted. The Family Building and Connecting (BAC) framework was developed, which distinguishes a “building” and a “connecting” approach. The building approach includes strategies that rely on family members and close friends for building a support network for the person with DS. The connecting approach includes strategies that connect the person with DS to external and often professional resources and services. Distinguishing these approaches is important for future research and for strengthening the support networks of people with DS and their families.

This paper reviews the Reagan administration's attack on the US welfare system during the 1980s. The paper considers the origins, provisions and impact of Reagan's three major pieces of retrenchment legislation: the Omnibus Budget Reconciliation Act of 1981, the Social Security Amendments of 1983, and the Family Support Act of 1988. It is argued that Reagan's record in retrenching welfare was limited in budgetary terms, but was successful in making welfare programmes more restrictive. Reagan's welfare legacy is assessed in terms of his attempts at restructuring social provision and shifting the welfare debate to the right. The paper concludes by asserting that Reagan's critique of, and attack on, social provision was accepted by his presidential successors, George Bush and Bill Clinton.
