AbstractAlthough referral letters (RLs) form a nodal point in a patient’s care journey, little is known about their informative value in child and adolescent mental healthcare. To determine the informative value of RLs to child and adolescent psychiatry, we conducted a chart review in medical records of minors registered at specialized mental healthcare between January 2015 and December 2017 (The Netherlands). Symptoms indicated in RLs originating from general practice (N = 723) were coded and cross-tabulated with the best estimate clinical classifications made in psychiatry. Results revealed that over half of the minors in the sample were classified in concordance with at least one reason for referral. We found fair to excellent discriminative ability for indications made in RLs concerning the most common psychiatric classifications (95% CI AUC: 60.9–70.6 for anxiety disorders to 90.5–100.0 for eating disorders). Logistic regression analyses suggested no statistically significant effects of gender, age, severity or mental healthcare history, with the exception of age and attention deficit hyperactivity disorders (ADHD), as RLs better predicted ADHD with increasing age (OR = 1.14, 95% CI 1.03–1.27). Contextual problems, such as difficulties studying, problems with parents or being bullied were indicated frequently and associated with classifications in various disorder groups. To conclude, general practitioners’ RLs showed informative value, contrary to common beliefs. Replication studies are needed to reliably incorporate RLs into the diagnostic work-up.

(1) The fields of view of the photoreceptor cells are determined by the dimensions and anatomical arrangement of the optical part of the ommatidium. The dimensions, and therefore the fields of view of the ommatidia are also related across the eye. In the relation between structure and function there are many points that invite discussion, but the intention is to order our knowledge so that the gaps become obvious. (2) The first step has been to make maps of the eyes showing the maximum theoretical resolving power of the facets and also the interommatidial angle, the reciprocal of which is the maximum spatial resolution of combinations of facets. The ratio of these two resolutions at each point shows the minimum overlap of the visual fields. These maps can be made from the outside of the eye; they show the main types of eye. (3) The next step is to work out the optics of individual ommatidia so that the focal lengths and receptor widths can be measured. The field width can then be predicted from the facet size and the subtense of the receptor at the posterior nodal point. The final step is to measure the field widths of individual ommatidia experimentally as a test of the optical theory, and to make maps of the actual fields in their correct position on the eye in angular coordinates. (4) Three examples of maps of actual fields are given, and their anato­mical and diffraction components are separated. The maps are an essential step towards the electrophysiological analysis of the ganglia behind the eye. A theory of the origin of the fields in terms of anatomy and optics also opens the way to an analysis of mechanisms that change the field size upon adaptation to light. A comparative study of the fields in different eye regions and in different species can also be related to visual habits and behaviour.

In this study an attempt is made to predict displacements and stresses in face-hobbed spiral bevel gears by using the finite element method. A displacement type finite element method is applied with curved, 20-node isoparametric elements. A method is developed for the automatic finite element discretization of the pinion and the gear. The full theory of the generation of tooth surfaces of face-hobbed spiral bevel gears is applied to determine the nodal point coordinates on tooth surfaces. The boundary conditions for the pinion and the gear are set automatically as well. A computer program was developed to implement the formulation provided above. By using this program the influence of design parameters and load position on tooth deflections and fillet stresses is investigated. On the basis of the results, obtained by performing a big number of computer runs, by using regression analysis and interpolation functions, equations for the calculation of tooth deflections and fillet stresses are derived.

This entry on Maß (moderation, measure) explores a concept that has not received much attention in Goethe scholarship and makes a case for its usefulness and versatility in tracking how Goethe addresses a philosophical issue with a history stretching at least back to Aristotle’s conception of “the golden mean.” It shows how Goethe’s writings respond to numerous issues connected with the concept of moderation, ranging from the problem of self-moderation, when an individual’s own internal calibration comes in conflict with societal norms, to the more theoretical question of how to define the correct standard of measure (Maßstab). The discussion of moderation in Goethe’s work is, to be sure, coupled with its opposite, namely the potentially deadly threat of immoderation and excess, such as one finds in Die Leiden des jungen Werthers (1774; The Sorrows of Young Werther), Wilhelm Meisters Lehrjahre (1795; Wilhelm Meister’s Apprenticeship), and Torquato Tasso (1790). Such potential conflicts, which also raise questions of where to position the standard of measure (Maßstab) of behavior, lead naturally into contexts of scientific experimentation, as in Goethe’s essay “Der Versuch als Vermittler von Objekt und Subjekt” (1792; The Experiment as Mediator of Object and Subject), where such standards take on a different valence from their role in mathematically based natural sciences. In addition, Goethe’s novel, Die Wahlverwandtschaften (1809; Elective Affinities), provides a poetic model where conflicts between individually and socially calibrated notions of measure and moderation play out with major ethical consequences. The entry concludes with a reflection on different kinds of aesthetic experience, each with its particular understanding of Maß: the individual’s appreciation of the sublime, the theatrical performance, and the embodiment of the self through poetic meter. Throughout these examples, the entry will underscore the role of narrative constraints: regardless of whether the medium is prose or poetry, one finds that questions of Maß as moderation in Goethe’s writings are often accompanied by questions of narrative control and excess. The following overview and analysis of Maß in Goethe’s writing will show that this term is a nodal point of ethical, epistemological, and aesthetic concerns.

STING is a nodal point for cellular innate immune response to microbial infections, autoimmunity and cancer; it triggers the synthesis of the antiviral proteins, type I interferons. Many DNA viruses, including Herpes Simplex Virus 1 (HSV1), trigger STING signaling causing inhibition of virus replication. Here, we report that HSV1 evades this antiviral immune response by inducing a cellular microRNA, miR-24, which binds to the 3’ untranslated region of STING mRNA and inhibits its translation. Expression of the gene encoding miR-24 is induced by the transcription factor AP1 and activated by MAP kinases in HSV1-infected cells. Introduction of exogenous miR-24 or prior activation of MAPKs, causes further enhancement of HSV1 replication in STING-expressing cells. Conversely, transfection of antimiR-24 inhibits virus replication in those cells. HSV1 infection of mice causes neuropathy and death; using two routes of infection, we demonstrated that intracranial injection of antimiR-24 alleviates both morbidity and mortality of the infected mice. Our studies reveal a new immune evasion strategy adopted by HSV1 through the regulation of STING and demonstrates that it can be exploited to enhance STING’s antiviral action.

Injured mature CNS axons do not regenerate in mammals. Deletion of PTEN, the negative regulator of PI3K, induces CNS axon regeneration through the activation of PI3K-mTOR signaling. We have conducted an extensive molecular dissection of the cross-regulating mechanisms in axon regeneration that involve the downstream effectors of PI3K, AKT and the two mTOR complexes (mTORC1 and mTORC2). We found that the predominant AKT isoform in CNS, AKT3, induces much more robust axon regeneration than AKT1 and that activation of mTORC1 and inhibition of GSK3β are two critical parallel pathways for AKT-induced axon regeneration. Surprisingly, phosphorylation of T308 and S473 of AKT play opposite roles in GSK3β phosphorylation and inhibition, by which mTORC2 and pAKT-S473 negatively regulate axon regeneration. Thus, our study revealed a complex neuron-intrinsic balancing mechanism involving AKT as the nodal point of PI3K, mTORC1/2 and GSK3β that coordinates both positive and negative cues to regulate adult CNS axon regeneration.
