The article presents a review of current and alternative methods for planning the scope and financing of medical care in the subjects of the Russian Federation. The relevance of the issue is due to the imperfection of the current planning model, which stimulate representatives of the profile bodies and independent experts to develop alternative methods. The current approaches and legal documents regulating the procedure for distributing the scope of medical care and financial resources to pay for this care are discribed. The analysis of alternative methods shows how different demographic and infrastructural features of regions can be used to redistribute the scope of medical care and the corresponding financial support.

Objective. To analyze legislation regulating medical care for children with rheumatic diseases in the Russian Federation. Materials and methods. We analyzed regulatory documents of the Ministry of Health of Russia, including the rules of medical care for children with rheumatic diseases, the resolution regulating the distribution of medicines at a discount or for free, and regional legal acts regulating routing of children with rheumatic diseases. Results. The legislation regulating medical care for children with rheumatic diseases is imperfect and needs to be updated. We identified shortcomings in the system regulating preferential provision of certain groups of patients with medicines. The results of our analysis can be used as a rationale for changes and amendments to existing regulatory legal acts and for the development of new regulatory documents. Conclusion. This analysis of legal documents allows us to suggest amendments to the existing legal framework in order to reduce tensions (including legal tensions) associated with distribution of preferential medicines, reconsider establishment of disability in patients with juvenile idiopathic arthritis, expand opportunities to prescribe biologicals for non-disabled patients with systemic scleroderma, juvenile dermatomyositis, and systemic vasculitis in regions. We justified detailed description of regional legal documents regulating distribution of preferential medicines among patients with rheumatic diseases. The legislation in the field of e-health, adopted in 2017, opens fundamentally new opportunities in the development of medical care using telemedicine technologies. The article provides an analysis of regulatory legal documents that establish the legal framework for the provision of medical care using telemedicine technologies. An assessment is made of the current state of telemedicine legal regulation in Russia. The law on telemedicine requires adjustment and refinement of existing regulations, procedures, standards of medical care with a detailed regulation of tools and situations of their application.

The paper shows the principles of organizing the transition of Russian citizens to medical savings accounts (MSA). A simulation of the accumulation of financial resources on the MSA of all categories of citizens of the Russian Federation was performed, taking into account the need for a significant increase in the salary of medical personnel, namely: in accordance with the “may” Decrees of 2012, V. V. Putin, to bring the salary of doctors to a level twice higher than the average for the region by 2018. It is proved that for all categories of Russian citizens, including citizens, have chronic diseases, the funds accumulated on their personal MSA, will be sufficient to cover the costs of medical care given the salaries of medical personnel established in the “may” decrees of the President of the Russian Federation.

The article is devoted to the features of the organization and accounting of payment for medical care provided by medical institutions. The article highlights the trends in reforming the system of payment for medical care, which include the use of per capita payment based on the principles of Fund maintenance. The review of the current regulatory framework in the field of financial support of medical care was conducted. Based on the analysis of models of per capita financing of primary health care in the subjects of the Russian Federation, their insufficient focus on achieving final results is noted. The article deals with the organizational mechanism and accounting mechanism of payment for medical care in health care institutions.

Aim. To conduct an analytical review of the regulatory legal framework on the resources provision required in medical organizations for the provision of oncological medical care based on clinical guidelines.Materials and methods. Using Russian normative legal and regulative documents on the oncological medical care, the authors studied the normative regulation of the issues of both resources’ provision for medical organizations and resources’ demand assessment, including the need for financial support from various sources of the budgetary system of the Russian Federation. The analysis was carried out in two regulatory areas: financing and organization of the provision of medical care. The authors examined the main legislation acts relating to oncological medical care, including documents of the compulsory medical insurance, the annual decrees of the Government of the Russian Federation on the program of state guarantees of free provision of medical care to citizens for the coming year and the planned period, the decree of the Government of the Russian Federation regulating the provision of medicines and medical products, provisions on the organization of medical care by type of medical care, procedures for the provision of oncological medical care, regulatory documents on clinical guidelines and standards of medical care.Results. The content analysis of the studied materials showed that certain components of the resources’ provision, including the financial provision of oncological medical care, are regulated by various regulatory documents, and there is no methodology for assessing the resources’ demand based on clinical guidelines.Conclusion. New approaches to assessing the resources’ demand for the provision of medical care and the required financial support based on clinical guidelines need to be developed.

At the current stage of the Russian economic development, the small and medium-sized businesses are becoming an immediate priority in the development of the state financial policy for the long term. Strategic documents developed at the federal and regional levels necessarily include measures to promote small businesses followed by the establishment of professional associations, business development centers, business incubators, technology parks, introduction of new and improvement of existing legislation standards, etc. However, notwithstanding the efforts made by the authorities, increased “injections” of financial resources into this sphere of the economy as well as the direct interest of governments at all levels and a substantial public response, the development of small businesses in Russia is facing great difficulties. The data on the number of registered small enterprises indicate that the sharp rise of entrepreneurship observed in the early 1990s changed into the stagnation towards the end of the decade and the sluggish current dynamics at the present time. In this regard, the relevance of the research subject is substantiated by the need to improve the regulation of the financial support of small and medium-sized businesses in the Russian Federation in order to create favorable conditions for their development and ensure the country's economic growth. In the research framework the main tasks to be solved to ensure the achievement of the research objective were defined. Based on the research findings, proposals were made related to improvement of the legislation in the field of financial support of small and medium-sized businesses, including proposals to improve government programs for supporting the small and medium-sized business institution. Other proposals address the harmonization of planned and program-target documents defining activities aimed at financial support of small and medium-sized business entities in the Russian Federation, as well as proposals on how to make the currently applicable budgetary and tax methods of small and medium-sized business promotion in the Russian Federation more effective to ensure the accessibility of financial resources and improve the quality of the state regulation. The results obtained are presented in the form of conclusions and recommendations for state authorities concerned with the regulation of financial support of small and medium-sized businesses. The practical significance of the results of the work makes it possible to use them in the activities of the Economics and Finance Department of the Russian Federation Government, the RF Government Commission on Competition and Development of Small and Medium-Sized Businesses, and the Department of Small and Medium-Sized Business Development and Competition of the RF Ministry of Economic Development.

The article provides an overview and analysis of the regulatory legal acts of the Russian Federation that regulate the possibility of using video recording in the work of mobile ambulance and disaster medicine teams based on the example of our own experience of Antonius Medvision —  Ambulance LLC, an organization of a non-governmental healthcare system that provides emergency medical care outside a medical organization in the Kaluga and Moscow regions.

Providing high-quality and timely medical care is the duty of any state, regardless of the economic and political situation in the country. Thanks to the implementation of the decrees of the President of the Russian Federation and the national project “Healthcare”, the healthcare system is improving. However, the achievement of all target indicators is not always possible, in connection with these, it becomes necessary to carry out external state control annually to prevent and identify errors and violations.
