Background. The solution to the problem of correction of involutional changes in the periorbital region requires a multidisciplinary approach, as this location can comprise a broad range of problems that need to be addressed. For so many years, laser therapy methods have been the standard for correcting age-related changes. However, many issues concerning combined methods remain open, and the search and development of the most effective protocols for their use is therefore an urgent task of modern medicine. Objective. The goal of the study was to develop a combined method to correct involutional skin changes in the periorbital region in young and middle-aged patients using a fractional Nd:YAG and CO2 laser in conjunction with lower transconjunctival blepharoplasty, based on the study of clinical efficacy results and taking into account the indicators of the qualitative characteristics of the skin, as well as microcirculation. Material and methods. The study enrolled 108 patients with involutional changes in the skin of the lower eyelid. These patients were classified into groups based on the method of treatment. In the study, relevant methods were used, namely, the study of the qualitative characteristics of the skin (cutometry, TEVA-metry, and corneometry) and microcirculatory processes using laser flowmetry. Results. The effectiveness of the developed combined method is based on a substantial positive impact on active and passive hemodynamic mechanisms at all levels of the microvasculature, as demonstrated by an increase of 45.92% in the integral indicator of microcirculation compared to 26.35% and 5.77% in the comparison groups, while the coefficient of variation in the microcirculation indicator increased by 33.02% compared to 24.19% and 6.46% following the use of the method components. The use of the combined treatment method restores the qualitative characteristics of the skin, namely, the moisture index increased by 27% (p 0.05) compared to 14.1% and 7.8% in groups 1 and 2, respectively; the rate of transepidermal water loss decreased by 47% (p 0.05) compared to 35% and 12.7% in groups 1 and 2, respectively; and the elasticity index Ua/Uf increased by 23.2% (p 0.05) compared to 18.4% and 16.4% in groups 1 and 2, respectively. Conclusion. Based on the study, it can be concluded that the combined treatment method developed is highly effective.

The use of the combined treatment method leads to the restoration of skin quality characteristics: the moisture index increased by 27% (p 0.05) compared to 14.1%, 7.8% in groups 1 and 2, respectively, the transepidermal water loss index decreased by 47% (p 0.05) (compared to 35%, 12.7% in groups 1 and 2, respectively), the ua/Uf elasticity index increased by 23.2% (p 0.05) (compared to 18.4%, 16.4% in groups 1 and 2, respectively). Conclusion. On the basis of the conducted research it can be concluded that the developed combined method of treatment is highly effective.

A retrospective assessment of the effectiveness of the application of a combination of radio-frequency radiation (needle lifting) and injections of platelet-enriched autologous human plasma was made in the correction of involuntary changes in the skin of the face and neck of 57 patients aged 40-65 years with signs of chronic aging and photodamage of the skin. Dermatological status was assessed before and after the reparative correction of involuntary changes in the skin through morphostructural analysis of the skin using the method of ultrasonography, visual analog scales, photography, studying the quality of life, and assessing patients’ satisfaction with the used treatment. To objectify the results, they were compared according to age: in the group of patients up to 49 years old and over 49 years of age. As a result of the application of the combined treatment method, the degree of manifestation of clinical signs of skin aging (the qualitative characteristics of the skin and the degree of wrinkles) markedly decreased, which contributed to an improvement in the quality of life of this category of patients. More than 90% of patients regardless of age noted good/very good efficacy and comfort of the combined method. Combined method of correction of involuntary changes in the face and neck skin, based on a combination of cellular technology with platelet activation by autologous thrombin and effect on the skin by needle lifting is a highly effective method of correcting involuntary skin changes.

The positive impact of mechanical treatment method on domain wall displacement is explained on the basis of Landgraf approach for dc loss analysis, and the effective dimension for eddy current in ac magnetic field is explained according to Bertotti approach for core loss analysis.

Background: The foreign body aspiration is very common while performing a dental procedure. Most of the patients are from lower age group as these group of people have high amount of sugar and starch containing diet which is not good for the oral health. But it is not limited to them and several aged patients are also victim of foreign body aspiration. Summary: Foreign bodies can be anything from cloth dam, barrier techniques itself to broken tooth. Due to age related delayed gag reflex and impact of anesthesia can be possible reasons behind the foreign body aspiration. Pulmonary aspiration is a serious cause of concern and cannot be neglected. It can go unnoticed which can brought to cognizance after careful study and observation of the radiographs. Endoscopic procedures are used to locate and extract the foreign body from the lungs. Conclusion: Proper guidelines regarding the extraction of foreign body extraction and identification must be followed as it is life threatening condition. Vulnerable age group must be treated with extra caution and every chance of mistake must be covered. Standard operating procedure must be strictly adhered to in order rot have maximum accuracy.

Based on the examinations perfomed in 80 patients with coxarthrosis complicated by contractures, it is concluded that the pain is one of the most important pathogenetic factors of the contracture formation. A combined method of the treatment of coxarthrosis is developed, including laser therapy, acupuncture and intertrochanteric osteotomy using Ilizarovs apparatus. As many as 186 patients are operated. Positive results are obtained in 92% of the cases.

Purpose: To evaluate the efficacy of a dexamethasone intravitreal implant in combination with intravitreal anti-vascular endothelial growth factor (VEGF) agents for treatment-resistant neovascular age-related macular degeneration (nvAMD). Methods: This study was designed as a single-center, retrospective interventional case series. Consecutive patients with treatment-resistant nvAMD underwent simultaneous combined injection of anti-VEGF agent and dexamethasone intravitreal implant. Eighteen patients with mean age of 81.5 years were included. Patients received an average of 26.3 anti-VEGF injections before dual therapy, with a mean follow-up of 8.2 months after dual therapy. Results: Dual therapy produced a significant mean decrease in central foveal thickness (126.3 μm), compared to a mean increase in 29.9 μm when treated with anti-VEGF monotherapy ( P = .0017). Patients also had a mean decrease in macular cube volume of −0.85 mm3 with dual therapy compared to anti-VEGF monotherapy ( P = .0014). There was a moderate correlation between the number of prior anti-VEGF injections and the magnitude of anatomic response, suggesting that shorter disease duration may positively influence response to combined treatment. Although there was a slight trend toward improved mean visual acuity after dual therapy, these differences did not reach statistical significance. Nevertheless, with combination treatment, 33% of patients gained 1 or more lines of vision. Dual therapy resulted in a significantly lower number of required anti-VEGF injections (4.25 vs 5.33) and an increase in the anti-VEGF injection-free interval to 1.41 months from 1.12 months during the 6 months following dual therapy compared to the same interval before dual therapy. Dual therapy was well tolerated; 2 eyes developed mild intraocular pressure elevation effectively managed with topical therapy and 1 patient developed worsening cataract. Conclusions: Combined treatment of anti-VEGF with dexamethasone intravitreal implant is a viable alternative for treatment-resistant nvAMD and may reduce treatment burden. Earlier treatment with dual therapy may be beneficial to maximize anatomic and visual outcomes in these patients.

Introduction: Vascular endothelial growth factor antibody therapy is an established treatment of exsudative age-related macular degeneration. Aim: The morphologic characterisation of the macular microvasculature after longstanding treatment. Method: Forty-eight patients (34 women and 14 men; age, 74.4 ± 8.0 years) were enrolled in the study. During follow-up time (53.8 ± 31.0 months), 7.6 ± 4.9 injections were administered in 56 eyes. Optical coherence tomography angiographic examination was performed with AngioVue (Optovue Inc. Fremont, CA, USA). Results: Distortion of the superficial retinal plexus and foveal avascular zone enlargement were noted in 5/56 eyes, deep retinal plexus defect was detected in 9/56 cases. Destruction of the choriocapillaries and the former neovascularisation could be found in 4 different patterns: 1. pigment epithelium and choriocapillary atrophy, 2. submacular scar, 3. active leaking choroidal neovascularisation, 4. intraretinal cysts. Conclusion: Optical coherence tomography angiography is a novel non-invasive method, which enables the follow up of macular degeneration. Orv. Hetil., 2016, 157(42), 1683–1690.

Purpose. To investigate whether adding video assistance to traditional verbal informed consent advisement improved satisfaction among cataract surgery patients. Methods. This trial enrolled 80 Chinese patients with age-related cataracts scheduled to undergo unilateral phacoemulsification surgery. Patients were randomized into two groups: the video group watched video explaining cataract-related consent information and rewatched specific segments of the video at their own discretion, before receiving traditional verbal consent advisement; the control group did not watch the video. Outcomes included patient satisfaction, refusal to consent, time to complete the consent process, and comprehension measured by a ten-item questionnaire. Results. All 80 enrolled patients signed informed consent forms. Compared with the control group, members of the video group exhibited greater satisfaction (65% versus 86%, p=0.035) and required less time to complete the consent process (12.3±6.7 min versus 5.6±5.4 min, p<0.001), while also evincing levels of comprehension commensurate with those reported for patients who did not watch the video (accuracy rate, 77.5% versus 80.2%, p=0.386). Conclusion. The video-assisted informed consent process had a positive impact on patients’ cataract surgery experiences. Additional research is needed to optimize patients’ comprehension of the video.

Postoperative cognitive dysfunction develops in the early and persists in the late postoperative period. Clinically, this condition manifests itself in the form of memory impairment, violation of concentration and violation of other higher functions of the cerebral cortex. The aim of this study is to study cognitive function in patients who underwent surgery with the use of general anesthesia in a remote period in a month.According to the results of the MMSE test, in the middle-aged patients the dynamics of the deterioration of the cognitive function was observed to a lesser degree than in the young patients, which is probably related to the age-related features of the plasticity of the cognitive function. In patients of middle age, according to the results of the FAB scale, at this period of the study was at the same level as those in young patients, which is associated with the age specificity of the plasticity of the cognitive function. According to the test drawing hours, there was no significant difference in the recovery rate, it was gradual, but no complete restoration was observed. For the 10-word memory test and the Schulte trial, the recovery was gradual, with a full recovery of the indicator to the level before the operation.
