Based on the examinations perfomed in 80 patients with coxarthrosis complicated by contractures, it is concluded that the pain is one of the most important pathogenetic factors of the contracture formation. A combined method of the treatment of coxarthrosis is developed, including laser therapy, acupuncture and intertrochanteric osteotomy using Ilizarovs apparatus. As many as 186 patients are operated. Positive results are obtained in 92% of the cases.

The structure, equipment and staff of the phoniatric centre are presented. The experience of the work from 1988 to 1993 is generalized. The quantitative characteristics of the work of polyclinic department of the centre and hospital, pointing to the significant, nearly 3 times, increase of the patients number taking phoniatric help in the dynamics are given. The combined treatment (microsurgical correction of structural elements of the larynx, phoniatric, logopedic, psychotherapeutic effect methods) allows to achieve positive results.

One of the known methods of treatment of patients with atopic dermatitis is laser therapy, however, the task of improving the efficiency of this method is still relevant. The objective of this study was the evaluation of the effectiveness of treatment technique of patients with atopic dermatitis, including alternation in a day the intravenous laser light of blood by low-intensity laser radiation with a wave length of 365 nm and 525 nm (green spectrum). 37 patients with atopic dermatitis (10 women and 27 men) aged 18 to 56 years (the average age 36,2), having a disease duration of 17 to 54 years, were observed. In the complex of therapeutic measures of the main group the laser therapy was included. It is the original technique by means of laser therapeutic apparatus "Lasmik- VLOK" (Registration certificate № RZN 2014/1410 from 06.02.2014) with the laser emitting heads KLVLOK- 365-2 (for LUFOK®) and KL-VLOK-525-2 for intravenous laser light of blood. For this technique the disposable sterile lights KIVL -01 on TU 9444-005-72085060-2008 made in Research centre "Matrix" (Russia, Moscow) were used. It is shown that the combined intravenous laser light of blood with a wavelength of 365 nm (VLOK-365 or LUFOK®) and radiation with a wavelength of 525 nm (green spectrum, VLOK-525) in a day, 10 sessions in combined treatment of patients with atopic dermatitis – allows to achieve complete regression of all acute inflammatory symptoms of disease - erythema, papules, desquamation, excoriations in 87,5% of patients with moderate-severe disease (the average SCORAD index – 57,5±4,0) and to lower at 3,4 times the average SCORAD index (to 21,3±4,0) in patients with severe disease (initial value – 72,8±3,0) with an overall positive dynamics.

The interest in the combined methods of treatment for mud therapy began to be debated in the press, at congresses and in societies relatively recently. On the question of so. called Duplication of procedures, the opinions of the authors still differ. Much attention is paid to the combined method for mud therapy in some of our Russian mud cure resorts; Thus, at the Kuyalnitsky estuary, with the participation of Professor EM Brusilovsky, even special schemes for the combined treatment of rheumatic diseases were created (Doctor, Delo, 1926, No. 9. Department of Consulting Practice).


Branch retinal vein occlusion (BRVO) includes occlusion of major branch retinal vein, macular branch vein, and peripheral branch vein. BRVO is the second most common retinal vascular disease after diabetic retinopathy. Macular edema is the leading cause of visual loss related to BRVO. Although there are many treatment options, effective treatment applications are limited. Laser therapy is one of these applications; that is used both in the development of neovascularization and in the presence of macular edema. Grid laser therapy doesn’t take place as much as the former in the primary treatment of macular edema; that still continues efficiency in combined treatment and selected cases.

One of the challenging problems for modern cosmetology is the cosmetic correction of the cutaneous scars of the skin that arises in the first place from the high prevalence of this condition and the lack of reliable and safe methods for its treatment The application of laser technologies including selective laser therapy as the method of fractional photothermolysis and the use of a neodymium (1064 nm) laser is considered to be a promising approach to address this problem. The present study included 18 patients presenting with hormone-dependent atrophic scars. All the patients were assigned for the treatment by selective photothermolysis (at the rate of 2 sessions per each treatment course performed with a 4 week interval) and irradiation by the neodymium laser (at the rate of 6-8 sessions conducted after fractional photothermolysis). The use of combined laser therapy in patients with hormone-dependent atrophic scars produced a pronounced effect as evidenced by the improvement of the clinical picture and was accompanied by the 78.1% reduction of the symptom index according to the DISC symptom scale and the 76.5% improvement of the dermatology life quality index (DLQI). The use of the combined method for the correction of atrophic cutaneous scars helps to restore the epidermal-dermal and epidermal structures of the skin in the association with the 1.4-fold, 1.7-fold, and 1.6-fold increase in the thickness of the epidermis, dermis, and hypodermis respectively; simultaneously, the acoustic density of the tissues increased by 1.8 times which corresponded to the improvement of the ultrasound picture by almost 75% when estimated in accordance with the Vancouver scar scale.

Analysis of projects for the elimination of open-pit mine workings in Russia has shown that most accidents associated with damage to the earth's surface are caused by critical values of rock pressure in empty spaces, a decrease in the bearing capacity of supports or their absence, as well as loss of stability of support structures. Rapid removal of the generated sinkholes does not always provide positive results, since often the material for filling them is burnt rock, and it was used without studying the properties of the earth's surface, where it is laid. This circumstance leads to the fact that the dips are re-activated. In this regard, the justification of the parameters of the method of eliminating sinkholes is an urgent scientific and practical task. The problem of stability of the abandoned mine workings, as well as geomechanical and environmental safety in the places where they are located, can only be solved using a systematic approach, which involves solving a set of issues to minimize the impact of each factor in the formation of sinkholes both in space and in time. The article describes the approaches to determining the technological parameters of the combined method of eliminating the sinkholes of mine workings, applied taking into account the geological characteristics of rocks and the mining and technical conditions of their location, the use of which will provide a reasonable calculation of the complex of engineering indicators for guaranteed removal.

Purpose. Evaluation of the effectiveness of the combined method in the surgical treatment of pterygium. Material and methods. There were 30 patients (30 eyes) under observation for primary progressive pterygium. To assess the condition of the ocular surface and the functional effectiveness of the new combined method of treating pterygium, the patients were divided into two groups. The main group consisted of 15 patients (15 eyes) who underwent surgical treatment of the pterygium according to the developed method. The comparison group consisted of 15 patients (15 eyes), who underwent pterygium removal using standard technology. Standard ophthalmological examination included: visometry, autorefractokeratometry, biomicroscopy with fluorescence test. The criteria for evaluating the effectiveness of treatment were: maximally corrected visual acuity; the degree of corneal astigmatism; the presence and degree of turbidity in the operation area; the frequency of relapse. Results. The use of the proposed method of combined treatment of pterygium, including surgical removal with plasty of the amnion in the zone of initial growth in combination with ultraviolet irradiation of the de-epithelialized corneal bed in the postoperative period, leads to the restoration of transparency and the correct shape of the cornea, therefore, and minimization of induced astigmatism, which is accompanied by an increase in functional parameters eyes. Conclusion. The results obtained, the simplicity and availability of the method developed by us allow us to recommend it for wider use in the surgical treatment of pterygium in order to increase the functional, cosmetic effect, surgical rehabilitation and prevention of disease recurrence. Background. The solution to the problem of correction of involutional changes in the periorbital region requires a multidisciplinary approach, as this location can comprise a broad range of problems that need to be addressed. For so many years, laser therapy methods have been the standard for correcting age-related changes. However, many issues concerning combined methods remain open, and the search and development of the most effective protocols for their use is therefore an urgent task of modern medicine. Objective. The goal of the study was to develop a combined method to correct involutional skin changes in the periorbital region in young and middle-aged patients using a fractional Nd:YAG and CO2 laser in conjunction with lower transconjunctival blepharoplasty, based on the study of clinical efficacy results and taking into account the indicators of the qualitative characteristics of the skin, as well as microcirculation. Material and methods. The study enrolled 108 patients with involutional changes in the skin of the lower eyelid. These patients were classified into groups based on the method of treatment. In the study, relevant methods were used, namely, the study of the qualitative characteristics of the skin (cutometry, TEVA-metry, and corneometry) and microcirculatory processes using laser flowmetry. Results. The effectiveness of the developed combined method is based on a substantial positive impact on active and passive hemodynamic mechanisms at all levels of the microvasculature, as demonstrated by an increase of 45.92% in the integral indicator of microcirculation compared to 26.35% and 5.77% in the comparison groups, while the coefficient of variation in the microcirculation indicator increased by 33.02% compared to 24.19% and 6.46% following the use of the method components. The use of the combined treatment method restores the qualitative characteristics of the skin, namely, the moisture index increased by 27% (p 0.05) compared to 14.1% and 7.8% in groups 1 and 2, respectively; the rate of transepidermal water loss decreased by 47% (p 0.05) compared to 35% and 12.7% in groups 1 and 2, respectively; and the elasticity index Ua/Uf increased by 23.2% (p 0.05) compared to 18.4% and 16.4% in groups 1 and 2, respectively. Conclusion. Based on the study, it can be concluded that the combined treatment method developed is highly effective.
