Cardiogenic shock (CS) is the most severe complication of myocardial infarction, manifested by an acute tissue hypoperfusion as a result of impaired contractile function of the heart. CS occupies a leading place in the patterns of mortality in patients with myocardial infarction, despite all the advances in medicine. This review presents a modern classification of CS and a risk assessment score, considers the main aspects of epidemiology and pathophysiology of CS, discusses issues of its diagnosis and treatment.

Purpose. The purpose of the current study was the search for evaluation criterion for the radiosensitivity of bone metastases of various origins. Materials and methods. The first part of the study presents a modern classification of pain, method of its evaluation, mechanisms and features of metastatic bone pain. The radiobiological patterns of pain relief in low-dose anti-inflammatory radiation therapy were also considered. The analysis of these features testifies in favor of the antitumor mechanism of pain relief in high-dose irradiation. To identify the relationship between pain and other clinical symptoms, 810 cases of bone metastases of various localization and morphology were analyzed. Results. The severity of pain directly correlates with gender, lesion length and little affected by the presence of pathological fracture. The intensity of pain does not depend on the primary tumor and bone metastases localization and does not correlate with the presence of neurological deficit. Conclusion. Completeness and persistence of the analgesic effect may reflect the real radiosensitivity of bone metastases, but taking into account the patient's gender and lesion length in the irradiated area.

Esophageal atresia is a malformation in which the upper and lower segments of the esophagus are separated. In this case, one or both segments may have a connection with the trachea.Purpose: to familiarize medical practitioners with a rare clinical case of esophageal atresia. The clinical variant of esophageal atresia presented in the publication has not yet been described in the medical literature; according to the clinical characteristics, it is most consistent with type 3B according to the classification of E. Vogt. Diagnosis and treatment of this malformation is carried out according to the previously developed standard protocol for diagnosis and treatment. The key diagnostic method is thoracoscopy, in many cases the malformation is detected intraoperatively (the main diagnostic method). The history of a sick child, a characteristic of clinical symptoms, data of paraclinical research methods, methods and favorable treatment results are provided.
