AbstractThe use of ionizing radiation, affects not only malignant cells but also healthy tissues, and promotes several side acute or late effects in the oral cavity. Among the acute effects, oral mucositis, xerostomia, hyposalivation, dysgeusia and dysphagia present a prominent role. The present study aims to conduct a narrative literature review on radiotherapy in the head and neck region as a therapeutic modality for cancer in this region and the main acute oral manifestations and their respective treatments. This was an exploratory literature review, through the database of Scielo, Pubmed, Medline and institutional websites using the crossing of the descriptors in English and Portuguese “head and neck neoplasms”, “radiotherapy”, “xerostomia”, “dysgeusia” and “dysphagia”. According to the established criteria, a total of 46 articles and 2 institutional websites were selected from the databases, and one book was added for presenting relevance on this theme. The results demonstrate that oral mucositis is the most prevalent acute effect and has a direct impact on patient’s quality of life, but there is no gold standard treatment. Dysphagia, xerostomia, hyposalivation and dysgeusia are common manifestations in irradiated patients, and present several therapeutic modalities. Given the importance of side effects of radiotherapy in head and neck region, further studies are needed to widely disseminate acute oral manifestations in irradiated patients. ResumoA utilização de radiação ionizante, no câncer de cabeça e pescoço, afeta não somente células malignas, mas também tecidos sadios, o que promove diversos efeitos colaterais em cavidade oral, classificados em agudos ou tardios. Dentre os efeitos agudos, a mucosite oral, xerostomia, hipossalivação, disgeusia e disfagia apresentam papel de destaque. O presente trabalho visou realizar uma revisão narrativa de literatura sobre a radioterapia em região de cabeça e pescoço como modalidade terapêutica para o câncer nesta região e as principais manifestações orais agudas decorrentes da radiação ionizante e seus respectivos tratamentos. Tratou-se de revisão de literatura do tipo exploratória, através das bases de dados da Scielo, Pubmed, Medline e sites institucionais utilizando o cruzamento dos descritores em inglês e português “neoplasias de cabeça e pescoço”, “radioterapia”, “mucosite oral”, “xerostomia”, “disgeusia” e “disfagia”. De acordo com os critérios estabelecidos, um total de 46 artigos e 2 sites institucionais foram selecionados nas bases de dados, e 1 livro foi acrescentado por apresentar relevância sobre a temática em questão. Os resultados demonstram que a mucosite oral é o efeito agudo mais prevalente e apesar de ter impacto direto na qualidade de vida, não há tratamento considerado padrão ouro para esta condição. A disfagia, xerostomia, hipossalivação e disgeusia são manifestações comuns em pacientes irradiados, e apresentam diversas modalidades terapêuticas que podem ser empregadas. Dada à importância dos efeitos colaterais da radioterapia de cabeça e pescoço, torna-se necessária a realização de mais estudos afim de divulgar amplamente as manifestações orais agudas em pacientes irradiados. A literature review reveals that gout has been described as affecting many sites in the head and neck region, both in the arthritic and tophaceous form. Gout can often mimic malignancy or infection, and has been described as causing acute airway problems requiring emergency tracheotomy. Here we describe the first published case of tophaceous gout affecting the soft tissues overlying the nasal bones. The patient presented with a bony, hard, dorsal hump and requested aesthetic rhinoplasty.We also describe an endoscopic technique for removal of tophi using a powered microdebrider system with a protected burr head. Endoscopic powered microdebrider blade excision of tophi affecting the limbs has already been described, with reduced complications when compared with conventional curettage and debridement techniques. This is the first such application to the nose.

AbstractLeiomyosarcoma is a malignant tumour of smooth muscle origin. These tumours are rarely encountered in the head and neck region. A case of leiomyosarcoma of the auricle is presented. There are two cases of external auditory canal leiomyosarcoma in the literature. To our knowledge the present case is the first example of leiomyosarcoma of the auricle.

An osseous choristoma is a benign tumor consisting of regular bone tissue in an irregular localization. Choristomas in the head and neck region are rare. Most frequently, they are found in the region of the tongue or oral mucosa. There are also very few reports on osseous choristomas in the submandibular region. We present the case of a woman with a large, caudal osseous choristoma within the lateral cervical triangle. Literature review is given about all of the reported cases in the region of the neck. The pathogenesis is yet unexplained. Our case supports the theory that the development of an osseous choristoma is a reaction to a former trauma. Cervical osseous choristomas are seldom, but they represent an important differential diagnosis when dealing with a cervical tumor.

Despite its effectiveness, radiochemotherapy treatment in the head and neck region is accompanied by acute oral complications such as oral mucositis, dysphagia, xerostomia, and dysgeusia. The aim of this study was to analyze and prospectively assess the frequency and evolution of acute oral complications during radiochemotherapy in patients diagnosed with squamous cell carcinoma in the head and neck region. We have analyzed oral complications of 20 patients during 6 weeks of radiochemotherapy treatment for squamous cell carcinoma. Oral mucositis was evaluated according to the World Health Organization criteria, dysphagia, and dysgeusia according to the National Cancer Institute Common Toxicity Criteria, and xerostomia according to parameters set by the Seminars in Radiation Oncology. Mucositis was first observed in the second week and all patients presented some degree of mucositis in the fourth week of radiotherapy. Xerostomia and dysphagia were initially reported already in the first week of radiotherapy. All patients presented xerostomia in the fourth week; however, dysphagia was observed in all patients, only in the sixth week. Dysgeusia was first observed in the second week, becoming more severe in the third week. Acute oral complications can be observed throughout the treatment, but the third week of radiotherapy seems to represent a critical week, regardless of the grade of the complication. The sixth week presents the worst grades of these complications. Knowledge about the natural course of oral complications during radiotherapy is important to develop better strategies for treatment and improve the patients’ quality of life.

Fourteen patients with malignant tumor in the head and neck region were treated with infusion of carboplatine microcapsules (CBDCA-mc) via percutaneous superselective catheterization. A microcatheter was advanced into a feeding artery using a coaxial catheter system. Eleven patients had over 30% reduction of the tumor size on CT within 1 month after embolization. Twelve patients had an increased amount of low attenuation tissue in the tumor on CT after embolization, suggesting increased necrosis in the tumor. No definite hematologic toxicity was found. A majority of patients complained of moderate pain in the embolized region immediately after embolization, easily relieved by i.v. analgesics. Chemoembolization using CBDCA-mc may be an effective therapeutic modality in advanced cases of head and neck cancer.

Hidrocystomas are benign cystic lesions which arise from the secretory part of sweat glands. Hidrocystomas can be either eccrine or apocrine and are often found in the head and neck region. In this paper we present a case of a large congenital hidrocystoma of the lower eyelid in a patient with possible orofacial digital syndrome (ODS). A special feature in our case was the presentation of bone remodeling of the anterior surface of the maxilla. The basic clinical and histological features of hidrocystomas are also reviewed.

AbstractHaemangiopericytoma is an uncommon vascular tumour that rarely appears in the head and neck region. Such a tumour arising from the middle ear has not been reported since it was first described in 1942 by Stout and Murray. The present case therefore illustrates the first example of a primary middle ear haemangioperictyoma.
