Background: Blood requisitions received in blood bank for elective and emergency procedures from surgery, trauma, Obstetrics and Gynaecology Department are often associated with excessive demand for cross matching of blood which is often more than the required blood and blood components. In this study, our goal was to use the blood stocks more efficiently and reduction in the wastage due to over dating and to prevent injudicious pre-operative cross-matching and utilization of blood.Methods: This was a prospective study conducted in the Department of Blood Transfusion and Immunohematology of Sher-i-Kashmir Institute of Medical Sciences (SKIMS), Soura, Srinagar over a period of one year with effect from September 2017 to August 2018. Source was requisition forms and blood bank records of patients who underwent elective procedures in the hospital. For the purpose of analysis, departments were categorized into surgical and allied branches i.e Cardiovascular Surgery (CVTS), Neurosurgery, General Surgery and Obstetrics and Gynaecology. Department wise utilization of blood Components cross matching to Transfusion ratio (C/T), transfusion probability (T%) and Transfusion Index (TI) were calculated. Data was entered and analyzed using Microsoft excel window 2010. The obtained data was evaluated and presented in the tabular and diagrammatic forms.Results: A total of 3940 requests for cross matching of blood and its components were received for 3072 patients. Out of these, 2048 units were transfused. The total C/T Ratio, Transfusion Probability (T%) and Transfusion Index (TI) was 1.92, 42.5% and 0.6 respectively.Conclusions: To reduce the injudicious usage of blood, blood transfusion services need to adopt blood conserving policies. Efforts should be made to adopt more conservative transfusion thresholds, periodic feedback to improve blood ordering, conduct regular auditing, handling, distribution and utilization practices of this scarce resource.

Although cytomegalovirus (CMV) infection has been recognized as a potential hazard of blood transfusion since 1966,1 few transfusion services provide blood components that have been selected specifically for low risk of infectivity with CMV. The objective of this commentary is to provide a review of the epidemiology of posttransfusion CMV infections with special reference to recent recommendations for its control by donor antibody screening and other blood bank procedures. In the healthy immunocompetent child or adult, posttransfusion CMV infection usually results in asymptomatic seroconversion or is manifested as a mild heterophile-negative mononucleosis syndrome. There is minimal morbidity associated with these transfusion-related infections and, therefore, minimal need for special CMV-free blood components.2

Abstract Introduction/Objective Although requesting for blood is a very common practice for patients for surgical interventions, the average requirement for a particular procedure is usually based on subjective anticipation of blood loss rather than on evidence based estimates. Over-ordering of blood components results in wasting of technical time, reagents and imposes extra cost on patients. The objective of this study is to assess blood ordering and utilization in a Nigerian Tertiary Hospital. Methods A review of one year data collected from the records of all discharged surgical patients and blood bank registers was done. Blood utilization was calculated using crossmatch to transfusion ratio (C/T), transfusion probability (%T), and transfusion index (TI). Results A crossmatch request for 1,228 units for 569 patients was made. Requests was higher for gynaecological cases (24.8 %) followed by orthopaedic procedures and least for cardiothoracic and urological surgeries (1.4 % each). Out of the crossmatched units, 880 units was transfused for 373 patients. The overall of C/T, %T, and TI were 1.4, 65.6%, and 1.55 respectively. The highest C/T was observed in elective caesarean sections (2.25) and neurosurgeries (2.2) while the lowest C/T was observed in oral maxillofacial surgeries (1.09) Conclusion Unnecessary crossmatching with minimal transfusion practice was observed in many elective surgical cases. Blood request pattern for elective surgeries needs to be revised and unnecessary requests minimized. There is a need for a Hospital blood transfusion committee which should come up with blood request policies and transfusion guidelines for elective surgical procedures and also conduct regular blood transfusion service audits.

Objectives: Blood and its components are an important part of patient management treatment protocols and like drugs have property to cause adverse reactions in the recipients. To maximize the effectiveness, safety and utility clinicians and intravenous therapists should be knowledgeable about the potential risk of blood component therapy. Hence, regular audit of blood and its component usage is essential to access the blood utilization pattern and set ideal policies in all the blood using specialties. Material and Methods: This is a prospective Study conducted in department of Transfusion Medicine at Vinayaka Mission Kirupananda Variyar Medical College & Hospital for a period of one year. Source of data was blood bank requisition forms and blood bank registers of patients who underwent elective or emergency procedures in the hospital, for which blood was ordered. Results: The mean age of the study subjects was 41.8 years. The male : female ratio was 1.6 : 1. Majority of the study subjects were in the surgery department followed by Ortho and OBG. Majority of the study subjects belong to B+ve blood group followed by O+ve group and only 20% of the subjects belong to negative blood group. 70% of the subjects required blood transfusion for some kind of surgical intervention and only 28% had required blood transfusion related to medical causes. Majority required four units of PRC transfusion. Majority of the subjects had the haemoglobin levels in the range of 6–7 and the mean level was 6.56 gms%. Majority of the packed red cell was stored for 2 weeks or 5 weeks and the mean duration of storage was 4.3 weeks. A statistical significant improvement was observed in the mean haemoglobin levels in the post-transfusion period compared to the pre-transfusion haemoglobin. Only 5% of the times the reaction related to fever or anaphylaxis had occurred among the entire study subjects. 50% had completed the entire blood transfusion in less than 4 hrs and the mean duration was 4.3 hrs. 65.9% of the patients had appropriate blood transfusion based on the guideline and the remaining 34% had inappropriate blood transfusion. It is inferred from the table that the CTR, transfusion probability and the transfusion index was found to be above the guideline value to be considered as effective blood utilisation. Conclusion: Regular audit of blood components is crucial so that appropriate measures can be taken for proper usage. Continuous medical education regarding the transfusion services for the clinicians and staff nurses have major role in improvement for the clinical transfusion practices in the hospitals.

OBJECTIVES: To evaluate the indications of blood transfusion in the Obstetrics and Gynecology Department of Government NaseerUllah Khan Babar Memorial Hospital. METHODOLOGY: This retrospective observational study was performed on indoor gynecology and obstetrics patients for the period of one year, a total of 100 patients were included in this study that received blood transfusion. Samples were collected by non-random convenience sampling after getting approval from the hospital ethical committee. Data was analyzed by using SPSS version 20. RESULTS: In this study a total of 100 patients who received blood transfusion were analyzed, out of 100 patients 78% of patients received transfusion due to obstetrical causes and 22% patients got transfused for gynecological causes. Most common blood group transfused was B+ and O+ and mean hemoglobin level at which patients received blood transfusion was 9.7g/dl. Blood components preparation can provide components to treat two to three patients from a single donor. The use of packed cell transfusion should be promoted instead of whole blood transfusion that is not even needed in most of the cases. CONCLUSION: Blood transfusion practice has been used aggressively in gynecology and obstetrics in some cases even without proper indications. There is a need to modify this practice by correcting anemia through drugs to avoid the inappropriate use of blood. Use of blood components should be encouraged.

Background: With the advent of the new pharmacological drugs and surgical advances compared to yesteryears, that the requirement of blood transfusion in obstetrics and gynaecology has decreased. Earlier obstetrical haemorrhage had been the commonest reason for blood transfusion. This trend seems to be changing. To evaluate this an observational study was done at a peripheral hospital to assess various indications for blood transfusion in maternity ward over a period of one year.  Methods: A total of 129 transfusions were studied in a period of one year and requirement of transfusion was assessed by the same team of doctors as per AABB guidelines for blood transfusion.  Results: A total of 87 obstetric patients required transfusion out of which 51.2% patients were transfused for anaemia near term. Postpartum haemorrhage constituted only 1.15% and ante partum haemorrhage only 4.6%. 17.24% of patients were transfused for incomplete abortion after taking medical abortion. 42 patients were transfused blood for various gynaecological reasons of which puberty menorrhagia constituted 19.01% and patients with perimenopausal bleeding were 28.57%.Conclusions: This study highlights the changing trends in requirement of blood transfusion and the need to emphasise on antenatal nutrition, supplements and contraception.

INTRODUCTION: Blood transfusion is an important concern for the society, as it is life saving for patients with bleeding disorders, accidents, surgeries, inherited/acquired hematological diseases and malignancies. Generally, donors are classied into the following categories: voluntary, family replacement, remunerated or paid donors, and autologous donor. AIMS & OBJECTIVES:To understand the importance of Blood & its safe Transfusion practice in a Teaching Hospital. METHODOLOGY: An Observational study was conducted over a period of 12 months from January 2019 to December 2019 in a 750 –bedded Tertiary Care Hospital of Jammu(UT). OBSERVATIONS: In addition to providing Blood-Transfusion Services to the patients admitted in SMGS Hospital Jammu & Other Associated Hospitals of GMC Jammu, the Blood-Bank is also catering to the needs of Registered Private Nursing Homes & Hospitals of Jammu(UT).This Blood-Bank has exceptionally maintained a record of consuming the whole stock of Blood without wasting even a single pint of blood. DISCUSSION: The Aim of Blood Transfusion Services is to supply good Quality of Blood & its Components to the Patients & avoid any risk to the Donors as well as Recepients. Hence it is extremely essential to institute strict Quality Control Measures RECOMMENDATIONS: Recommended that Upgradation of Blood-Bank is essential to cater with the needs of Additional bed-strength that SMGS Hospital is going to acquire in the coming future.

Background. There is a paucity of data on asymptomatic carriage of Plasmodium parasite among adult population in Eastern Uganda, an area of perennial high transmission of malaria. In this study, we estimated the prevalence of Plasmodium parasites in donor blood units at Mbale Regional Blood Bank (Mbale RBB), a satellite centre of the Uganda Blood Transfusion Service (UBTS). Method. This was a cross-sectional descriptive study in which 380 screened donor blood units were examined for the presence of Plasmodium parasites. A systematic random sampling technique using the interval of 7 was used in selecting the screened blood units for testing. Two experienced malaria slide microscopists (MC1 and MC2) independently examined each thick and thin blood slide under high power magnification of X400 and then X1000 as stated on the study standard operation procedure (SOP). Each slide was examined for 100 oil immersion fields before the examiner declared them negative for Plasmodium parasites. The results by each microscopist’s examination were tallied separately, and finally, the two tallies were compared. The third independent microscopist (MC3) was blinded to the results from MC1 and MC2, but whose role was to perform quality control on the slides randomly sampled and read 38 (10%) of all the slides and was available to examine any slides with inconsistent findings by MC1 or MC2. Results. All the microscopists were unanimous in all the slide readings. Five of the thick smears (1.3%) confirmed the presence of Plasmodium parasites among donor blood units. Of these, 4/5 were from male donors. Plasmodium falciparum was identified in 4 positive samples, while Plasmodium malariae was identified in one of the donor units. Conclusion. The 1.3% prevalence of Plasmodium malaria parasites in screened donor blood units represents risk of malaria blood transfusion transmitted infection and a pool of community transmittable malaria infections, respectively.
