Enteric septicaemia of catfish was first detected in 1976 as an economically significant disease associated with commercial catfish production. Initially, Edwardsiella ictaluri was a host specific pathogen of catfish species but has also been reported from other hosts other than the catfish such as the zebrafish. E. ictaluri has not been isolated in humans hence it is not a zoonotic infection. There has been no previous report of isolation of this organism in humans. This was a case report of a 5 year old boy who presented with fever, vomiting, passage of bloody stool of 6 days and abdominal pain of a day duration. In the case of this 5 year old boy who presented with features of dysentery, blood culture using BACTEC™ grew E. ictaluri. E. ictaluri may be a pathogen which can infect humans just like another closely related species, Edwardsiella tarda. Although, E. ictaluri has not been reported in humans, could this be the first case? Non availability of diagnostic technique appropriate for its diagnosis may explain the rare incidence of the organism in humans, hence many cases would have been treated without isolating the organism.

Introduction: Conjunctival melanoma is extremely rare in children and has low rates of resolution. Definitive histopathological diagnosis based exclusively on microscopic findings is sometimes difficult. Thus, early diagnosis and adequate treatment are essential to improve clinical outcomes. Clinical case: We present the first case in which the fluorescent in situ hybridization (FISH) diagnostic technique was applied to a 10-year-old boy initially suspected of having amelanotic nevi in his right eye. Based on the 65% of tumor cells with 11q13 (CCND1) copy number gain and 33% with 6p25 (RREB1) gain as measured by the FISH analysis, and on supporting histopathological findings, the diagnosis of conjunctival melanoma could be made. Following a larger re-excision, adjuvant therapy with Mitomycin C (MMC), cryotherapy and an amniotic membrane graft, the patient has remained disease-free during 9 years of long-term follow-up. Case discussion: Every ophthalmologist should remember to consider and not forget the possibility of using FISH analyses during the differential diagnosis of any suspicious conjunctival lesions. Genetic techniques, such as FISH, have led to great advances in the classification of ambiguous lesions. Evidence-based guidelines for diagnosing conjunctival melanoma in the pediatric population are needed to determine the most appropriate strategy for this age group.

Two cases of Sarcophaga myiasis are reported from Montreal, Quebec. In the first case, larvae were passed in a bloody stool of a 5-year-old child and in the second, larvae were removed from a sore on the toe of a middle-aged man. The external morphology, cephalo-pharyngeal sclerites, prothoracic respiratory organs, and the posterior spiracular pit of the larvae ate described. Case I larvae agree with descriptions of S. haemorrhoidalis Fallén. Those from case II are probably S. crassipalpis Macquart. Both species are known to occur in Quebec. Probable modes of infection are discussed.

Abstract Background: Currently, there is no available study about the occurrence of enterocolitis associated with the coronavirus disease 2019 (COVID-19) among adults. Case presentation: A 56-year-old man had lower abdominal pain, distension, diarrhea, and bloody stool. Moreover, the patient presented with fever and general fatigue. Laboratory examination did not reveal leukocytosis nor neutrophilia. However, the patient’s C-reactive protein level increased to 4.9 (normal range: 0.0–0.3) mg/dL. Both sputum and stool cultures had normal results. However, abdominal computed tomography (CT) scan revealed wall thickness mainly in the submucosal layer of the left part of the large intestine. Thus, real-time polymerase chain reaction test of throat swabs was performed for the diagnosis of COVID-19, and positive results were obtained. Conclusions: We report the first case of enterocolitis correlated to COVID-19, and the patient had positive abdominal CT scan findings. This imaging modality could be effective for the diagnosis of enterocolitis associated with COVID-19.

ABSTRACT The bacterial pathogen Edwardsiella ictaluri causes enteric septicemia of catfish (ESC), an economically significant disease of farm-raised channel catfish. Commercial catfish production accounts for the majority of the total fin fish aquaculture in the United States, with almost 300,000 tons produced annually, and ESC is the leading cause of disease loss in the industry. We have demonstrated the survival and replication of E. ictaluri within channel catfish cells and identified a secretion system that is essential for E. ictaluri intracellular replication and virulence. We have also identified nine proteins encoded in the E. ictaluri genome that we believe are actively transferred from the bacterium to the cytoplasm of the host cell and act to manipulate host cell physiology to the advantage of the bacterium. The data presented here confirm that the proteins are actually transferred during an infection, which will lead to further work on approaches to preventing or controlling ESC. Edwardsiella ictaluri, a major pathogen in channel catfish aquaculture, encodes a type III secretion system (T3SS) that is essential for intracellular replication and virulence. Previous work identified three putative T3SS effectors in E. ictaluri, and in silico analysis of the E. ictaluri genome identified six additional putative effectors, all located on the chromosome outside the T3SS pathogenicity island. To establish active translocation by the T3SS, we constructed translational fusions of each effector to the amino-terminal adenylate cyclase (AC) domain of the Bordetella pertussis adenylate cyclase toxin CyaA. When translocated through the membrane of the Edwardsiella-containing vacuole (ECV), the cyclic AMP produced by the AC domain in the presence of calmodulin in the host cell cytoplasm can be measured. Results showed that all nine effectors were translocated from E. ictaluri in the ECV to the cytoplasm of the host cells in the wild-type strain but not in a T3SS mutant, indicating that translocation is dependent on the T3SS machinery. This confirms that the E. ictaluri T3SS is similar to the Salmonella pathogenicity island 2 T3SS in that it translocates effectors through the membrane of the bacterial vacuole directly into the host cell cytoplasm. Additional work demonstrated that both initial acidification and subsequent neutralization of the ECV were necessary for effector translocation, except for two of them that did not require neutralization. Single-gene mutants constructed for seven of the individual effectors were all attenuated for replication in CCO cells, but only three were replication deficient in head kidney-derived macrophages (HKDM). Fasciolosis is a zoonotic infection caused by the trematoda Fasciola hepatica and Fasciola gigantica. Human fasciolosis is endemic in some parts of South America, Africa, Eastern Asia and Europe. High prevalence of human fasciolosis does not necessarily occur in areas where fasciolosis is a major veterinary problem. Infection with Fasciola hepatica has not been reported in humans in Serbia and former Yugoslavia, although a large variety of animals, such as sheep and cattle show infection rates that may reach significant proportions in some areas. Humans can become accidental hosts of this parasite by ingesting contaminated drinking water or plants in an endemic area. Infection with Fasciola hepatica has a variable clinical presentation depending on the stage of the disease. Typical symptoms that may be associated with fascioliasis can be divided by the phases of the disease including the acute or liver phase, the chronic or biliary phase, the obstructive phase, and ectopic or pharyngeal fascioliasis. The diagnosis of human fasciolosis may be problematical and delayed, especially in non-endemic areas, because physicians rarely encounter this disease and a long list of other diseases must be included in the differential diagnosis. The diagnosis of fasciolosis is complex and requires the application of direct an indirect methods of diagnostics: clinical diagnosis, haematological and biochemical findings, parasitological diagnosis, immuno-diagnosis, imaging procedures, liver biopsy. At the Clinic for Infectious and Tropical Diseases in 2005, we recorded the first case of human fasciolosis in a woman from Belgrade, a citizen of Serbia, who developed clinical symptoms of acute fasciolosis after several months of living in Bosnia-Herzegovina. This article reviews the epidemiology and diagnostics of human fasciolosis. .

Osteosarcoma is the most common primary bone tumour with a peak occurrence in adolescence. The occurrence of osteosarcoma in preadolescents is rare with a paucity of data in the developing world. Metastasis of osteosarcoma to the orbit is even a rarer presentation with few cases reported in the literature but to the best of the researchers’ knowledge, none of the previous cases had contralateral orbital metastasis to osteosarcoma. This is the first case report of orbital metastasis of osteosarcoma in Nigeria. The present case is presented for its rarity, to increase awareness and add to knowledge on the possibility of metastasis to contralateral orbit in osteosarcoma.

Abstract Background Granulomatosis with polyangiitis (GPA) is a syndrome of refractory vasculitis involving the upper respiratory tract, lungs, kidneys, and systemic small and medium-sized arteries that affects all age groups. No pediatric case with a bloody pericardial effusion resulting in cardiac tamponade and co-existing hematochezia has been reported. Case presentation A 14-year-old boy was referred for evaluation of prolonged fever, chest pain, and intermittent hematochezia. Diagnostic imaging showed a prominent pericardial effusion. Immediately after admission, his systolic blood pressure decreased. Emergent pericardiocentesis resulted in aspiration of a massive amount of bloody pericardial fluid. This was diagnosed as cardiac tamponade because his blood pressure recovered immediately after the drainage. The patient had an elevated serine proteinase 3-anti-neutrophil cytoplasmic antibody (PR3-ANCA) level on serological examination. Head MRI showed thickening of nasal and sinusoidal mucosa and a cystic mass in the left sphenoid sinus. After ruling out malignancy based on the cytology of the effusion, chest MRI, and gallium scintigraphy, total colonoscopy showed multiple irregular-shaped aphthae from the right transverse colon to the cecum on the contralateral side of the mesenteric attachments. Biopsy specimens of aphthous lesions confirmed necrotizing granulomatous inflammation. A diagnosis of GPA was made based on these findings, and oral prednisolone (PSL) and azathioprine were started. The hematochezia disappeared rapidly, and no recurrence of pericardial effusion was seen after PSL tapering was completed. The PR3-ANCA level decreased into the normal range immediately after the initial therapy. Conclusions Pericarditis is a common cardiac complication of GPA, but there have been no reports of resultant cardiac tamponade. This is the first case of pediatric GPA with cardiac and gastrointestinal complications preceding the common symptoms such as respiratory or renal symptoms. A case of pediatric GPA with hematochezia is also extremely rare. In conclusion, serial measurement of ANCA levels is important in patients with persistent fever and bloody stool, such as in inflammatory bowel disease, to make the diagnosis of a vasculitic syndrome.
