<p class="abstract">According to De Van, the preservation of what remains is of utmost importance rather than the meticulous replacement of what has been lost. Need based use of unconventional approach is a growing demand in prosthodontics in India. The increasing demands of the patients have led to innovative techniques for fabricating complete dentures. Conventional techniques may provide satisfactory results in most patients but may not be suitable in all cases. Cu-sil denture is one of the transitional dentures which is easy to fabricate, saves time as well as reduces the cost of treatment. These dentures provide a psychological boost to the patients and serve as a viable alternative. They not only promote the alveolar ridge integrity but also help in retaining the proprioceptive ability of the periodontium. This paper presents a case of fabrication of Cu-sil denture in a patient with two teeth remaining in the maxillary arch.</p>

The present prime concern in dentistry is on preservation of remaining natural teeth. Presence of few teeth in oral cavity help in preserving alveolar ridge integrity, maintain the proprioception, and gives psychological benefit to the patient. Overdenture has proven to be the mainstay of conservative prosthodontic treatment when proper diagnosis, treatment planning, and most importantly, patient compliance are achieved. This paper presents a case report of rehabilitation of a 65 year old patient with multiple missing teeth with tooth supported overdenture in both maxilla and mandible.

Objective: According to the manufacture, fewer than 1 in 10 capsule associated with overdentures on natural teeth separated from the denture base during an 8-year clinical trial; more interestingly, none experience loss of magnetic attraction.Methods: A 50 year old female patient came to Dental Hospital Makassar Hasanuddin University wanted to fix a loose denture, which has been used for 9 years. Chief complaint was a mobile mandibular denture. Treatment for the restoration of oral function, including mastication, using mandibular implant-retained overdenture with magnets in total edentulous. Implant supported retained with magnetic attachment constitute an accurate and predictable treatment option and achieve a higher patients satisfaction.Results: The surface of both magnet and attachment keeper were coated with titanium nitride (TiN). Self-curing resin was used for luting between  magnets and denture base resin.Conclusion: Magnetic attachments can be used to retain mandibular implant overdenture.Key words: Implant supported overdenture with magnets, resorbed alveolar ridge, total edentulous

ABSTRACT Over denture is favoured treatment modality for an elderly patient with few remaining teeth, because of adaptation to wearing is facilitated. Roots maintained under the denture base preserve the alveolar ridge, provide sensory feedback and improve the stability of the dentures. In comparison with complete dentures, overdentures may provide better functions. The most common problem of mandibular complete denture is retention & stability and alveolar ridge resorption; hence care must be taken to preserve it.

The aim of this study was to present a case report of the surgical removal of hyperplasia in the oral cavity, using carbon dioxide (CO2) laser radiation and rehabilitation with a complete denture. Epulis fissuratum occurs in complete denture patients, because a constant irritative action induces the mucosa to grow under poorly fitting dentures. These lesions must be removed, and to avoid a relapse, new complete dentures should be made to maintain healthy surgical tissues. The clinical sequence presented in this case shows a completely edentulous patient with epulis fissuratum on the lower alveolar ridge extending to the vestibular sulcus of the anterior region of mandible. Immediate complete dentures were made prior to the lesion removal with CO2 laser radiation, providing satisfactory results in oral function and tissue health.

AbstractA removable dental prosthesis that covers and rests on two or more remaining natural teeth, the roots of natural teeth, and/or dental implants is known as overdenture. This leads to less resorption of residual alveolar ridges. Restoration and modified teeth coronally can also be used as multiple abutments. In most cases, abutments are treated endodontically. In most patients with complete dentures, mandibular denture is difficult to adapt, and there is loss of retention because of less surface area. This case report represents prosthetic rehabilitation of a mandibular edentulous patient with cast coping with short dowels supported overdenture.

Purpose: For making denture in maxillectomy cases is very difficult and challenging to get the retention tomake the denture stable in its position during functioning. This case report describes a clinical condition inwhich patient was treated with a maxillary obturator with zygomatic suspension wiring due to insufficientretention in the palate. Materials and Methods: A 63-year-old patient had gone to a subtotal maxillectomybecause following myofibroblastic sarcoma andwas issued with bilateral circum-zygomatic wiring hooksimmediately after surgery. The patient had less than a third of their alveolar ridge remaining and did notprovide sufficient retention on its own. The wires were used for the retention because natural retentioncould not get due to inadequate maxillary ridge. While the denture was being fabricated, a temporaryfeeding plate was provisioned to the patient. Counter hooks were implemented on the definitive upperdenture, posterior to molars, to attach to the zygomatic suspension hook. Final upper denture was furtherreinforced with denture adhesive on it. A lower denture was also fabricated for the said case followingconventional protocols of jaw relation determination. Results: The obturator provided with adequate sealand leak proof phonetics. The zygomatic wiring coupled with denture adhesive were sufficient to provideadequate retention. Conclusion: Zygomatic suspension wires coupled with counter hooked obturatorprosthesis provide rehabilitation of patients with palatal defects with inadequate maxillary arch forretention.However long-term repeated use of such wires might expose the patient to secondary infectionsand should be kept in consideration Clinical Significance: Circum-zygomatic suspension wiring providesufficient retention to sustain the upper obturator prosthesis in place. However, the retention was stillinadequate, so denture adhesives were used to make it more stable. Bangladesh Journal of Medical Science Vol.19(3) 2020 p.582-585

Cu-Sil denture is a simplified approach which helps in preserving the few remaining natural teeth .It is a new type of transitional denture which clasps the neck of the tooth providing space for the remaining teeth to emerge into the oral cavity through the denture. Preserving the remaining teeth not only preserves the alveolar ridge but also maintains proprioception providing psychological benefit for the patient. Cu-Sil denture is one such simpler clinical approach which offers many advantages. This case report represents a simple chair-side clinical approach to fabricate Cu-Sil denture by preserving the few natural teeth. Key words: Cu-Sil denture, transitional denture, proprioception.

ABSTRACT A basic concern of edentulous patients is retention of their dentures. This has led to considerable experimentation and research in efforts to perfect dentures that compensate for the loss of natural teeth. The use of multiple miniature suction cups made from a soft material lining the denture satisfies the requirements of retention and stability. A technique describing the fabrication of a maxillary and mandibular multicup dentures which are more beneficial than conventional complete dentures is discussed below with case report.

The prosthetic treatment of patients with an edentulous maxilla opposing mandibular natural teeth is one of the most challenging endeavors that face clinicians. Occlusal forces from the opposing natural teeth may cause fractures in the maxillary prosthesis and also result in advanced bone loss of the edentulous maxilla. With the presence of extreme gagging reflex, the treatment may become more complicated. This article describes and illustrates the 2-stage surgical and prosthetic treatment of a patient with an edentulous maxilla opposing natural teeth. In the beginning, the patient was treated with 4 implants and a maxillary implant-supported overdenture. The extreme gagging reflex and the occlusal forces from the mandibular natural teeth obligated the team a second stage surgical and prosthetic treatment, which included increasing the number of implants after bilateral sinus lifting in the posterior maxilla and fabricating a maxillary fixed hybrid prosthesis made of micro-ceramic composite that yielded a satisfactory result.
