<p class="abstract"><strong>Background:</strong> Lower levels of lipid in proliferating tissues and in blood compartments may occur due to ongoing process of oncogenesis. The evidence regarding hypolipidemia as a predisposing factor for malignancy is scarce. Therefore, this study was undertaken to study alteration of plasma lipid levels in patients diagnosed with oral squamous cell carcinoma (OSCC) and its oral pre-cancerous (OPC) lesions and also to compare the plasma lipid profile in different grades of OSCC.</p><p class="abstract"><strong>Methods:</strong> A single centre prospective cross sectional study was carried out among 150 consecutive patients of OSCC and OPC lesions. The diagnosis of all lesions was confirmed histopathologically and grading was also done for each lesion based on degree of differentiation and keratinization of tumour cells. Serum total cholesterol, HDL, TG, LDL was estimated by using semi-automated chemical analyzer. The lipid levels were compared by using unpaired t-test between two strata whereas one way analysis of variance (ANOVA) with Tukey’s post-hoc tests was used among more than two strata.  </p><p class="abstract"><strong>Results:</strong> Total cholesterol, TG, HDL, and LDL were significantly reduced in OSCC and OPC groups when compared with the reference values. Serum TC, LDL and HDL levels were found to be decreased with the loss of differentiation in histological grading but with no statistical significance.</p><p class="abstract"><strong>Conclusions:</strong> It can be inferred that there is an inverse relationship between the serum lipid profile values of TC, HDLC, TG, LDL and OSCC as well as OPC patients. The lower serum lipid status may be considered a useful indicator for initial changes occurring in neoplastic cells.</p>

ABSTRACT Aim Altered lipid profile patterns have been associated with malignancies because lipids play a pivotal role in the maintenance of cell integrity. The present study evaluated alterations in serum lipid profile in oral squamous cell carcinoma patients as well as its association with the habit of tobacco consumption. Materials and methods The study included 30 oral squamous cell carcinoma patients, 20 patients with the habit of tobacco consumption and 20 controls. Serum lipids, including (i) total cholesterol, (ii) LDL cholesterol (LDLC), (iii) HDL cholesterol (HDLC), (iv) VLDL cholesterol (VLDLC) and (v) triglycerides, were analyzed by spectrophotometric kits. Results A significant decrease in serum total cholesterol (TC) levels, triglyceride levels (p = 0.007, p = 0.029 respectively) were observed in oral squamous cell carcinoma patients as compared to the healthy control group. The mean serum HDLC levels (p = 0.003) were significantly lowered in the tobacco habituates when compared to the healthy controls. The mean serum total cholesterol levels were significantly lower in subjects with oral squamous cell carcinoma (p = 0.000) as compared to the tobacco habituates. Likewise, LDLC levels and TC:HDLC ratios (p = 0.000 and p = 0.000 respectively) were significantly decreased in oral squamous cell carcinoma patients as compared to the tobacco habituates. Our data strengthens the evidence of an inverse relationship between serum lipid levels and oral squamous cell carcinoma. Conclusion The lower level of serum cholesterol and other lipid constituents in the patients is thought to be due to their increased usage by tumor cells for new membrane biogenesis. Clinical significance The lower serum lipid profile status serves as a useful indicator for gauging initial changes occurring in tumor cells. A detailed study of the cholesterol carrying lipoprotein transport mechanisms helps in better understanding of the underlying mechanisms of the regulation of serum cholesterol concentrations in cancer. Introduction: Dengue Virus (DENV) infection now remains as the most rapidly spreading viral disease worldwide. Lower lipid level occurs in critically ill patients with DENV infection and is postulated as an independent predictor of the clinical outcome. Aim: To find the correlation between serum lipid levels and severity in children admitted with various grades of Dengue Haemorrhagic Fever (DHF) and secondarily to correlate serum lipid levels with factors like platelet count and duration of hospital stay. Materials and Methods: This was a prospective descriptive study done in the Paediatric ward from October 2019 to December 2019. Febrile Children with a platelet count of less than 1 lakh/cu.mm and positive for DENV IgM antibodies by Enzyme Linked Immuno Sorbent Assay (ELISA) formed the study group. Demographic details, number of days of fever on admission, duration of stay in hospital and severity grading of DENV infection as per the National guidelines were collected. These children were evaluated with Complete Blood Counts (CBC), fasting lipid profile, X-ray chest and ultrasound abdomen. Mean and standard deviation was used for categorical variables. Chi-square test was used to assess the strength of association between lipids and severity of DENV infections. Pearson correlation coefficient was used to assess R value between length of stay and serum lipid levels. Results: The Total Cholesterol, Low Density Lipoprotein (LDL), Triglyceride and Very Low Density Lipoprotein (VLDL) showed a simple linear decline as the severity grade advanced from grade I to grade IV. However, statistical significance was found only for total cholesterol and LDL with p-values of 0.046 and 0.041 respectively. The distribution of High Density Lipoprotein (HDL) across the grades of severity did not follow any pattern and was randomly distributed. On analysis of Pearson correlation between number of days of hospital stay and lipid profile, statistical significance was observed for LDL alone (p=0.023). Conclusion: The serum levels of total cholesterol and LDL were significantly reduced in severe grades of DHF. Serum levels of total cholesterol and LDL can be used as an economical surrogate marker to predict the severity of DHF at the time of admission.

Abstract Objective This study aimed to determine the correlation between the vascular mast cells and inflammatory changes in the potentially malignant disorders, compared to the oral squamous cell carcinoma (OSCC) in varying degrees of dysplasia. Materials and Methods Thirty samples were selected: 10 of OSCC, 10 of oral leukoplakia, and 10 of actinic keratosis. The toluidine blue technique was used on the mast cells, and hematoxylin and eosin were used for analyzing the lymphocytes, mitosis, and vessels. The quantification was performed using the ImageJ software after obtaining the images by light microscopy with a × 40 objective. Analysis of variance with p < 0.05 was considered for statistical significance. Results Mast cells (p < 0.0158), vessels (p < 0.9431), lymphocytes (p < 0.0001), and mitoses (p < 0.0009) were found in OSCC. In potentially malignant disorders, a lower density of these structures and a higher concentration of mitosis in the actinic keratosis were observed. Conclusion The results showed evidence of a positive correlation between mast cells and vascularization in the OSCC, suggesting the aggression of the disease. Intense inflammatory infiltrate indicates that other molecular events are involved in the carcinogenesis process, and further studies are necessary for a better understanding of it.

Introduction: Oral submucous fibrosis (OSF) is a potentially malignant disorder of the oral cavity. Aims and Objectives: The core objective of the study is to analyze the oral submucous fibrosis with and without associated oral squamous cell carcinoma (OSCC). Materials and Methods: This cross sectional study was conducted in Baqai Dental College Karachi during October 2020 to June 2021. The data was collected from 100 patients of both genders who were confirmed with OSF on the basis of history and clinical examinations were included in the study. Demographic and clinical information were recorded. Results: The data were collected from 100 patients of both females and males. The age distribution of the sample was analyzed. The patients were mostly above 50 years of age in both categories. However, the number of patients who were with OSCC at an age below 50 was greater in the OSF-positive group (23.8% vs. 21.9%). Similarly, the mean age of OSF + OSCC patients was 57.5 years while this value was 59.5 years for those without. Conclusion: It is concluded that tumors show well-differentiated histology and less lymph node involvement. However, a statistical significance was not observed among these variables, when comparing the OSCC patients with OSF to those who are without.

Background: Oral squamous cell carcinoma is prevalent in South Asian countries with rising cases of its incidence and mortality. Despite advancements in treatment, survival and recurrence rates are poor. Immunotherapy is a novel therapeutic modality in immunooncology. Immune checkpoint proteins are under investigation for clinical implications amongst which Programmed Death Ligand-1 has shown valuable results in certain malignancies. Aims: To determine the immunohistochemical expression of Programmed Death Ligand-1(PD-L1) in oral squamous cell carcinoma and to find an association of Programmed Death Ligand-1 with stage and clinicopathological parameters of oral squamous cell carcinoma. Study Design: Cross-sectional study. Place and Duration of Study: Ziauddin Medical University, Karachi, 1 Year duration during 2018-2019. Methods: A total number of 140 biopsy confirmed cases of oral squamous cell carcinoma were recruited in the study. Immunohistochemical expression of Programmed Death Ligand-1 was evaluated and associated with the clinicopathological parameters of oral squamous cell carcinoma (OSCC). The data was statistically analyzed through Descriptive statistics and Chi square test by using SPSS v.20. Results: Out of 140 participants, 74% were males (n=103) and 26% were females (n=37). Programmed Death Ligand-1 positivity was observed in 62.1% of cases (n=87). The Mean age of the participants was 48.91 ± 11.7 years.  The most common site of cancer involvement was buccal mucosa and majority of participants were habitual of consuming chewable products i.e. Pan, Gutka and betel nut (89; 64%). Stage III and IV tumours comprised a major portion of cases in our study. (52; 37%), (56; 40%). A statistically significant p-value was noted for the association of Programmed Death Ligand-1 with stage II and IV tumours. (P-values: 0.029, 0.001)The association of Programmed Death Ligand-1 with other variables such as age, gender, ethnicity, sites or habits was not statistically significant. Conclusion: This study concludes that the statistical significance of Programmed Death Ligand-1 expression with tumour stage is suggestive of worsening prognosis and might have detrimental effects as tumour progresses in advanced stage. Programmed Death Ligand-1 positivity in patients having oral squamous cell carcinoma could be useful in future research in the light of cancer immunotherapy which has shown success in oncology.
