<p><strong>Objective: </strong>The aimed of this study is to investigate the changes in serum levels of tumor necrosis factor (TNF-α) in the patients with acute pancreatitis treated with octreotide and its clinical significance. <strong>Method: </strong>Total of 65 patients of acute pancreatitis were selected as a case study, in which 30 patients with mild acute pancreatitis (MAP) and 35 severe acute pancreatitis (SAP) patients were treated with octreotide. 60 healthy subjects as control group and 65 case group was subjected to performed double antibody sandwich enzyme-linked immunosorbent assay (ELISA) to detect the serum levels of TNF-α. <strong>Results</strong><strong>: </strong>The serum TNF-α level in the case group was (12.67 ± 3.45) pg/mL and the control group was (1.56 ± 0.57) pg/mL. Case group was significantly higher than control group (<em>p</em> &lt; 0.05). Serum level of acute pancreatitis (AP) before treatment was (8.96 ± 2.12) pg/mL. After treatment, SAP group was (17.34 + 4.56) pg/mL, MAP group was significantly lower than SAP group, and the difference was statistically significant (<em>p</em> &lt; 0.05). <strong>Conclusion: </strong>The serum levels of TNF-α in patient with acute pancreatitis were significantly higher than those of normal healthy people, and their serum level was closely related to the severity of illness.</p>

Цель. Изучить закономерности изменения экспрессии интерлейкина-10 и интерлейкина-24, обладающих иммуномодулирующим эффектом, при развитии B-клеточного хронического лимфолейкоза. С учетом этого выявить информативные прогностические критерии развития гемобластоза и/или нового подхода к терапии заболевания. Методы. У 120 больных с разными стадиями В-клеточного хронического лимфолейкоза методом твердофазного иммуноферментного анализа исследована динамика уровней интерлейкина-10 и интерлейкина-24 в сыворотке крови. Результаты. Обнаружено закономерное повышение содержания интерлейкина-10 и интерлейкина-24 в сыворотке крови пациентов уже на начальной стадии B-клеточного хронического лимфолейкоза и сохранение их достоверно высоких уровней на последующих стадиях заболевания. Заключение. Обнаруженный нами факт повышения содержания интерлейкина-10 в сыворотке крови пациентов с В-клеточным хроническим лимфолейкозом является фактором риска снижения противоопухолевой защиты организма вследствие подавления им механизмов клеточного иммунитета и способности ингибировать апоптоз малигнизированных клеток. Напротив, повышение экспрессии интерлейкина-24, обладающего проапоптотической активностью и стимулирующего дифференцировку клеток, может способствовать повышению эффективности механизмов противоопухолевой резистентности организма. Устранение дисбаланса продукции и/или содержания указанных цитокинов в сыворотке крови может создать условия повышения эффективности терапии пациентов с В-клеточным хроническим лимфолейкозом. Aim. To study serum levels of immunosuppressive cytokines (interleukin (IL)-10 and IL-24) in patients with B-cell chronic lymphocytic leukemia for assessment of the disease progression and elaboration of a new treatment strategy. Methods. 120 patients with B-cell chronic lymphocytic leukemia were enrolled in the study and divided into four groups according to the disease stage (Rai stage I-IV). Control group included 30 healthy volunteers. Concentrations of IL-10 and IL-24 were measured in serum using the enzyme-linked immunosorbent assay (ELISA). Results. Serum levels of IL-10 and IL-24 levels were significantly increased in all patient groups compared to the control. No difference in the cytokines levels between the patient groups was observed. Conclusion. In patients with B-cell chronic lymphocytic leukemia, the increased serum level of IL-10 might impair the antitumor defence by inhibiting the cell immune response and preventing apoptosis of malignant lymphocytes. On the other hand, the increased serum level of IL-24 might oppose these effects by promoting cellular differentiation and inducing apoptosis in malignant cells. Therefore, correction of IL-10/IL-24 imbalance may be a beneficial therapeutic strategy for patients with B-cell chronic lymphocytic leukemia.

Objective: The purpose of this study was to explore the application of dexmedetomidine (Dex) in cardiopulmonary bypass. Methods: A total of 60 patients undergoing elective cardiopulmonary bypass were divided into control (C) group and Dex group. In the Dex group, appropriate amount of Dex was added into the membrane lung prefilling solution before anesthesia induction, while those in control group were given normal saline. The levels of mean arterial pressure (MAP) and heart rate (HR) at different times were measured. The levels of cardiac troponin I (CTNI), malondialdehyde (MDA), interleukin 6 (IL-6), and tumor necrosis factor α (TNF-α) at different points (T0/T1/T2/T3/T4) in both groups were measured by enzyme-linked immunosorbent assay kits. Results: The intraoperative and postoperative levels of MAP and HR in the 2 groups were significantly lower than those preoperatively ( P < .05). The levels of MAP and HR in the Dex group were significantly lower than those of the C group ( P < .05). The levels of CTNI/MDA/IL-6/TNF-α at different points in both groups were significantly higher than those at T0 ( P < .05). The serum levels of CTNI, MDA, IL-6, and TNF-α in the Dex group at T1/T2/T3/T4 were significantly lower than those in the C group ( P < .05). The rate of arrhythmia in the Dex group was significantly lower than that in the C group ( P < .05). Conclusion: Dexmedetomidine has a stable effect in cardiopulmonary priming solution.

PURPOSE: To investigate the effects of pentoxifylline (PTX) in experimental acute pancreatitis (AP) starting drug administration after the induction of the disease. METHODS: One hundred male Wistar rats were submitted to taurocholate-induced AP and divided into three groups: Group Sham: sham-operated rats, Group Saline: AP plus saline solution, and Group PTX: AP plus PTX. Saline solution and PTX were administered 1 hour after induction of AP. At 3 hours after AP induction, peritoneal levels of tumor necrosis factor (TNF)-α, and serum levels of interleukin (IL)-6 and IL-10 levels were assayed by Enzyme-Linked Immunosorbent Assay (ELISA). Determinations of lung myeloperoxidase activity (MPO), histological analysis of lung and pancreas, and mortality study were performed. RESULTS: PTX administration 1 hour after induction of AP caused a significant decrease in peritoneal levels of TNF-α and in serum levels of IL-6 and IL-10 when compared to the saline group. There were no differences in lung MPO activity between the two groups with AP. A decrease in mortality was observed in the PTX treatment compared to the saline group. CONCLUSIONS: Administration of PTX after the onset of AP decreased the systemic levels of proinflammatory cytokines, raising the possibility that there is an early therapeutic window for PTX after the initiation of AP.

Introduction. Preeclampsia (PE) is an important cause of mortality and morbidity for mothers, fetuses, and the newborns. Placenta plays a pivotal role in pathogenesis of PE. Hepatic growth factor (HGF) is a cytokine expressed by the mesenchymal stalk of placental villi during pregnancy and assumes a paracrine role in trophoblasts which express its receptor (c-MET). In the present study, we investigate the diagnostic value of s-Met (the soluble form of the receptor) in the first and second trimesters of pregnancy for early diagnosis of preeclampsia. Method and Materials. This is a case-control study conducted on 95 pregnant women. The serum level of s-Met was measured in the first and second trimesters, and the participants were followed until delivery. 44 individuals with preeclampsia (the case group) and 51 individuals without preeclampsia (the control group) were evaluated. Results. Serum level of s-Met in preeclamptic participants was lower than that of the control group in both the first and the second trimesters (P<0.0001). In addition, serum levels of s-Met were significantly lower during the first and second trimesters in patients with early, severe preeclampsia compared to those with late, mild preeclampsia (P<0.0001). The sensitivity and specificity of s-Met in the first and second trimesters were, respectively, (83%, 94%) and (77%, 94%) for early preeclampsia and (88%, 92%) and (86%, 98%) for severe preeclampsia. Conclusion. Considering our findings, serum level of s-Met may be used as a predictive factor for early detection of preeclampsia. Further research is required to corroborate the functional and therapeutic value of s-Met in preeclampsia.

Background: T helper 17 (Th-17) cell, a proinflammatory subset of CD4 T cells, have an essential role in immunity against Helicobacter pylori (H. pylori) infection. Objectives: This study aimed to evaluate expression of selected Th-17 cells associated cytokines (IL17, IL-21, IL-22, IL-23, IL-26 and TNF-α) in H. pylori-infected patients and to recognize their responsibility in H. pylori associated chronic gastritis with different severity. Methodology: This study is a case control study. The case group included 25 H. pyloripositive patients suffering from chronic gastritis. The control group included 25 age and sex-matched healthy individuals without any dyspeptic symptoms and negative for H. pylori. Infection with H. pylori in all participants was determined by detection of H. pylori stool antigen by enzyme-linked immunosorbent assay (ELISA) kit. Certain cytokines expression (IL-17, IL-21, IL-22, IL-23, IL-26 and TNF-α) in serum samples from all participants were tested using ELISA. Results: Comparing the serum cytokines expression in cases and controls, IL-17, IL-21 IL-23 and TNF-α were significantly higher in cases while IL-22 and IL-26 were higher in cases but not statistically significant. Both serum IL-17 and TNF-α expressions were statistically significant higher in cases with moderate or severe forms of chronic gastritis than in cases with mild form of chronic gastritis. However, the levels of IL-21, IL-22, IL-23 and IL-26 showed insignificant variation regarding chronic gastritis severity. Conclusion: Th-17 cells are responsible for the pathogenesis of H. pylori infection and the severity of gastritis. So, down regulation of Th-17 cells associated cytokines offers a promising therapy to diminish H. pylori associated gastritis.

AbstractThe aim of this study was to evaluate the serum NF-κBp65, TLR4 (Toll-like receptor 4) expression in patients of preeclampsia and its diagnostic value as biomarkers.MethodsThirty patients with preeclampsia (case group) and 30 normal pregnant women (control group) were included in this study. The serum level of NF-κBp65 and TLR4 were examined by enzyme linked immunosorbent assay (ELISA), and compared between the two groups. The diagnostic sensitivity, specificity and area under the receiver operating characteristic (ROC) curve were calculated by STATA11.0 statistical software.ResultsThe expression level of TLR4 and NF-κBp65 in serum of preeclampsia patient group was 3.76±1.07ng/ ml and 183.20±49.19ng/ml, whereas that in the serum of the normal pregnant group was 2.43±0.69ng/ml and 98.68±29.80ng/ml. The expression of TLR4 and NF-κBp65 in serum of preeclampsia patient group was significantly higher than that of the normal pregnant group (P<0.05); The Pearson correlation test showed that the TLR4 expression in the serum of preeclampsia patients and normal pregnant women was positively correlated with their NF-κBp65 expression [rpreeclampsia=0.46, (P<0.05), rnormal=0.48, (P<0.05)]. When TLR4 and NF-κBp65 were selected as the reference indexes, the diagnostic sensitivity of preeclampsia was 86.67% (95%CI:69.28%-96.24%) and 90.33% (95%CI:73.47%-97.89%), and the specific ity was 70.00% (95%CI:50.60%-85.27%) and 83.33% (95%CI:65.28%-94.36%). The area under the ROC curve was 0.84 and 0.89.ConclusionSerum levels of TLR4 and NF-κBp65 was significantly higher in patients with preeclampsia which may involve in the pathogenesis of preeclampsia, and can be used as biomarker for predicting preeclampsia.

Introduction: More than 500 million people throughout the world are infected with Toxoplasma gondii. On the other hand, migraine is known as the most common pain syndrome. The aim of this study was to detect anti-Toxoplasma gondii IgG in the serum of individuals with and without migraine. Methods: In this descriptive-analytical study, 50 person (7 male & 43 female, in the age range of 20-60 years) with history and symptoms of migraine (case group), and 50 individuals (7 male & 43 female, in the age range of 20-60 years) without migraine (control group) were selected randomly. Blood samples (5 ml) were collected from all the selected people, and the serum level of anti-Toxoplasma gondii IgG were determined using Enzyme-Linked Immunosorbent Assay (ELISA) technique. In this test, 10 IU/ml of anti-Toxoplasma gondii IgG was considered as the minimum titer. For statistical analysis SPSS Inc, Chicago, IL; Version 18 software and -Chi-square and t-tests were used. Results: 38% of the patients with migraine and 32% of the control group had anti-Toxoplasma gondii IgG above 10 IU/ml. The mean amount of anti-T. gondii IgG in the serums of case group was 173.42 IU/ml, while it was 68.25 IU/ml in the control group, the mean amount of Toxo-IgG in migraine positive group was 2.5 time higher than the amount in the control group (p<0.05). Conclusion: According to the results of this study it is concluded that the serum level of anti-Toxoplasma gondii IgG in migraine positive persons is significantly higher than the migraine negative individuals. Therefore, it is recommended that the patients disordered with migraine be tested for chronic toxoplasmosis.

Objective: Study the influence of doxycycline versus doxycycline with vitamin C drugs in the management of acne vulgaris on some immunological parameters which include ( IL-1β, IL-8, IFN-γ, TNF-α, and TLR2) and the following objectives were adapted: 1-Selection of population for the study ( control, patients) groups and follow up the patients after one month of treatment, the first group receive doxycycline and the second group receive doxycycline with vitamin C,2- Blood samples collection and separation of serums for immunological analysis, 3- Statistical analysis. Methods: This study is a randomized clinical trial carried out in clinical dermatology in Merjan Medical City in Babylon from September 2018 to March 2019. The number of subjects enrolled in the present study was 60; their age was between (14- 30 years), among whom 30 were acne patients, the remaining 30 subjects were apparently healthy individuals, and they were served as control. A dermatologist diagnosed a total of 30 acne patients to having moderate to severe acne and divided into two groups (15 patients in each group). Patients in the first group were treated with doxycycline (100mg) once daily after meal for 30 days, while in the second group patients were treated with doxycycline (100mg) capsule once daily after a meal in combination with vitamin C (500mg) chewable tablets once daily. After 1-month of therapy, the response was evaluated clinically and immunologically by measure the concentration of pro-inflammatory cytokines (IL-8, IL-1β, IFN-γ, TNF-α) and ( TLR-2 ) by using enzyme-linked immunosorbent assay (ELISA) and the results were compared to their levels before treatment and that in the control group. Results: Significant elevation in the serum levels ( p ≤ 0.001) of immunological parameter IL-8, IL-1β, IFN-γ, TNF-α, and TLR-2 among acne patients in comparison to the control. The clinical response in the first group was good, moderate and poor in 5 (33%), 7(47%) and 3(20%) respectively, while in the second group was good, moderate and poor in 7 (47%), 7(47%) and 1(6%) respectively, the immunological results showed that the serum levels to the (IL-1β, IL-8, IFN-γ, TNF-α) and (TLR-2 ) were more reduced in the second group compared to their levels in the first group. Conclusion: Significant elevation (p less than 0.001) in the serum levels of (IL-8, IL-1β, IFN-γ, TNF-α, and TLR-2) among moderate to severe acne patients in comparison to control group. Clinically the combination of doxycycline plus vitamin C was more efficient as therapeutically in comparison to doxycycline alone. Immunologically doxycycline plus vitamin C was more effective in reducing serum levels of (IL-8, IL-1β, IFN-γ, TNF-α, and TLR-2) in comparison to doxycycline alone.

Background: T helper type 2 (Th2) cells are critical cellular elements in allergic rhinitis. Interleukin-33 (IL-33) produces Th2-related cytokines and binds to the ST2 receptors. This is expressed strongly in mastocytes and discerningly in Th2 cells. Through Th2 cells, IL-33 may also have partly involved in immune responses. Objectives: This study aimed to measure the IL-33 serum levels in children suffering from allergic rhinitis and investigate its relationship with the disease. Methods: This case-control study was conducted on a population with the age range of 7-18 years, who referred to the Pediatric Clinic of the Shahid Beheshti Hospital in Kashan, Iran, in 2017. The study sample encompassed 57 patients with allergic rhinitis (case group) and 57 subjects with no allergic rhinitis (control group). The ELISA assay was used to measure the serum level of IL-33 in the case and control groups. Allergic rhinitis was diagnosed by a pediatric immunologist considering the patient’s history and the guidelines set out by the Allergic Rhinitis and its Impact on Asthma (ARIA). All study data were analyzed with SPSS software version 22. Results: There were significant differences between the two groups in terms of age (P = 0.001), gender (P = 0.0144), family history of atopy (P < 0.001), symptoms duration (P < 0.001), and comorbidities (e.g., atopic dermatitis and asthma) (P < 0.001). Furthermore, compared to the control group, the case group exhibited significantly higher IL-33 serum levels (P < 0.001). Conclusions: The high serum levels of IL-33 exhibited in patients with allergic rhinitis indicate its involvement in the pathogenesis of the concerned disease.
