The study was carried out to analyze database of registered diseases of peripheral arteries in patients of Yekaterinburg during 2009-2013. The database of the territorial foundation of mandatory medical insurance of the Sverdlovsk region in 2009-2013 was used as study material. The number of registered cases of treatment of diseases of peripheral arteries was analyzed. The sampling of patients with diseases of peripheral arteries taking treatment in twenty-four-hours and day-time hospitals. The statistical processing was applied to all cases ranged by years: patients with compensated blood circulation, critical ischemia of extremities and mortification. The audit was applied to randomly selected 40 medical records of patients with diseases of peripheral arteries, receiving treatment in twenty-four-hours and day-time hospitals during 2014 and 50 medical records ofpatients with diseases ofperipheral arteries against the background of diabetes mellitus from register of the oblast podiatry consulting room. The total number of treated patients with diseases ofperipheral arteries in Yekaterinburg during five years made up to 31,309, including 19% ofpatients with diabetic affection of lower extremities. In twenty-four-hours hospital 12,716 (40.6%) patients were treated and 18,593 (59.4%) were treated in day-time hospital. The study established increasing in day-time hospitals number of patients with arteriosclerosis obliterans up to 1.8 times, with diabetic angiopathy up to 3.6 times. At increasing of number of treated patients number ofpatients with critical ischemia of lower extremities during 5 years decreased insignificantly and number of large amputations have a certain trend to decreasing (up to 2.5%). The study established deviations under application of medicinal therapy in patients with diseases of peripheral arteries at pre-specialized stage and absence of achievement of target values of main laboratory parameters. The database of registered diseases of peripheral arteries can be used as a tool for data analysis and further management decision making with purpose of enhancing quality of medical care of patients residing in municipality.

Despite the development of modern methods of diagnosis and treatment of obliterating atherosclerosis of the lower extremities in 38-65% of patients, the disease progresses with the development of critical ischemia of the lower extremities. This often requires amputation at various levels. Postoperative mortality in this case reaches 20% or more. The aim of this work was to study the causes of death in patients after amputation of the lower extremities for critical ischemia. We studied 296 medical records and 52 autopsy reports of the deceased, which were used to amputate the lower extremities. It turned out that the mortality rate in women is 1.9 times higher than in men. The risk of death is increased by the presence of coronary heart disease in a patient with grade 2B circulatory failure and atrial fibrillation. The most common immediate cause of death after amputation is multiple organ failure.

At present, the study of the peculiarities of the reproductive history of patients planning pregnancy is considered to be a promising area for the effective ensuring the timely and highly successful implementation of their reproductive intentions. Goal — to conduct an analytical retrospective assessment of the reproductive history of patients with childbirth in the pelvic presentation. A retrospective analysis of medical records has been conducted on the basis of the Odessa Regional Perinatal Center, medical records of 300 primipara and secundipara with pregnancy with one fetus in the breech presentation have been studied: I — the main group (MG) (n=150), women who gave birth in the pelvic presentation; ІІ — control group (CG) (n=150), women who gave birth in the cephalic presentation. Statistical processing of the results of the study was carried out using the statistical packages of the program Statistica for Windows (version 6.1) by the means of parametric and nonparametric statistics. The study of submitted medical documents showed a statistically less favorable gynecological history of women from MG: while the majority of women with MG were multipara — 96 (64%), in CG, the number of patients was lower — 67 (44.6%) cases (p<0.05). The number of cases of urgent labor in the MG — 137 (91.3%) was greater than in the control group — 118 (78.7%) (p <0.05). The frequency of operative birth in MG was 102 (68%) cases compared to 44 (29.3%) (p<0.05) in CG. The operative birth in CG occurred only in 5 (11.4%) cases, whereas in MG there were 13 cases (100%) (p<0.05). Therefore, the prevalence of cases of operative delivery and less amount of cases of preterm births have been indicated in MG.

Abstract. Background: Previous suicide attempts increase the risk of a completed suicide. However, a large proportion of patients with deliberate self-wrist cutting (DSWC) are often discharged without undergoing a psychiatric interview. Aims: The aims of this study were to investigate the differences in the characteristics and outcomes of patients with DSWC and those with deliberate self-poisoning (DSP) episodes. The results of this study may be used to improve the efficacy of treatment for DSWC patients. Method: We retrospectively reviewed the medical records of 598 patients with DSWC and DSP who were treated at the emergency department of Seoul Saint Mary's Hospital between 2008 and 2013. We assessed sociodemographic information, clinical variables, the reasons for the suicide attempts, and the severity of the suicide attempts. Results: A total of 141 (23.6%) patients were included in the DSWC group, and 457 (76.4%) were included in the DSP group. A significantly greater number of patients in the DSWC group had previously attempted suicide (p = .014). A total of 63 patients (44.7%) in the DSWC group and 409 patients (89.5%) in the DSP group underwent psychiatric interviews. Conclusion: More DSWC patients had previously attempted suicide, but fewer of them underwent psychiatric interviews compared with the DSP patients.

Abstract. Based on responses from 158 reward managers located at the headquarters or subsidiaries of multinational enterprises, the present study examines the relationship between the centralization of reward management decision making and its perceived effectiveness in multinational enterprises. Our results show that headquarters managers perceive a centralized approach as being more effective, while for subsidiary managers this relationship is moderated by the manager’s role identity. Referring to social identity theory, the present study enriches the standardization versus localization debate through a new perspective focusing on psychological processes, thereby indicating the importance of in-group favoritism in headquarters and the influence of subsidiary managers’ role identities on reward management decision making.

Subject. This article considers the issues of provision of supplementary education services for children and methodological tools for the formation of educational environment. Objectives. The article aims to assess the level of development of the system of supplementary education of children in Krasnoyarsk and offer a methodological approach to improving the management decision-making procedure in the formation of a portfolio of supplementary education services at the municipal level. Methods. For the study, we used the methods of theoretical, empirical, and logistic analyses, and sociological studies. Results. The article offers concrete solutions to the lack of a methodological approach to providing supplementary education services, considering one of the micro-districts of Krasnoyarsk as a case in point. It also offers tools that can be used by public authorities to organize educational space at the municipal level. Conclusions. A unified methodological approach should be used to provide a system of supplementary education, taking into account financial means, as well as differentiation in the distribution of educational facilities throughout the area. The relevance of the set of supplementary education services and consumer preferences should be taken into account, as well.

Currently, atherosclerosis, which affects the vascular bed of all vital organs and tissues, is considered as a leading cause of death. Most commonly, atherosclerosis involves coronary and peripheral arteries, which results in acute (e.g., myocardial infarction, lower extremities ischemia) or chronic (persistent ischemia leading to severe heart failure) consequences. All of them have a marked unfavorable impact on the quality of life and are associated with increased mortality and morbidity in human populations. Lower extremity artery disease (LEAD, also defined as peripheral artery disease, PAD) refers to atherosclerotic occlusive disease of the lower extremities, where partial or complete obstruction of peripheral arteries is observed. Decreased perfusion can result in ischemic pain, non-healing wounds, and ischemic ulcers, and significantly reduce the quality of life. However, the progressive atherosclerotic changes cause stimulation of tissue response processes, like vessel wall remodeling and neovascularization. These mechanisms of adapting the vascular network to pathological conditions seem to play a key role in reducing the impact of the changes limiting the flow of blood. Neovascularization as a response to ischemia induces sprouting and expansion of the endothelium to repair and grow the vessels of the circulatory system. Neovascularization consists of three different biological processes: vasculogenesis, angiogenesis, and arteriogenesis. Both molecular and environmental factors that may affect the process of development and growth of blood vessels were analyzed. Particular attention was paid to the changes taking place during LEAD. It is important to consider the molecular mechanisms underpinning vessel growth. These mechanisms will also be examined in the context of diseases commonly affecting blood vessel function, or those treatable in part by manipulation of angiogenesis. Furthermore, it may be possible to induce the process of blood vessel development and growth to treat peripheral vascular disease and wound healing. Reactive oxygen species (ROS) play an important role in regulation of essential cellular signaling pathways such as cell differentiation, proliferation, migration and apoptosis. With regard to the repair processes taking place during diseases such as LEAD, prospective therapeutic methods have been described that could significantly improve the treatment of vessel diseases in the future. Summarizing, regenerative medicine holds the potential to transform the therapeutic methods in heart and vessel diseases treatment.
