In the article is represented the analysis of the primary and repeated disablement of adult population on the classes and the separate diseases in g. To Moscow in 2017-2018 the yr., which showed that an increase in the dynamics of the number of persons VPI the decrease of the number PPI. In the structure VPI the leading positions occupied the invalids as a result of the diseases of the system of blood circulation with the decrease of their specific weight; as a result of the malignant new formations with an increase in their specific weight; as a result of the diseases of osteomuscular system and connective tissue with the decrease of their specific weight and invalids as a result of the mental disorders and the disorders of behavior with the tendency of an increase in their specific weight. In the dynamics was noted a decrease of the portion of invalids as a result of the diseases of nervous system, diseases of endocrine system, tuberculosis, consequences of injuries and poisonings and an increase in the specific weight of invalids as a result of the diseases, caused HIV, the diseases of eye and its additional apparatus, diseases of ear and mammiform branch, diseases of the organs of respiration and urino-genital system. In the structure PPI the leading nosologic forms were the diseases of the system of blood circulation with the decrease of their specific weight, an increase in the portion of malignant new formations, mental disorders and disorders of behavior, the decrease of the portion of invalids as a result of the diseases of osteomuscular system and connective tissue, by an increase in the share of the diseases of nervous system. In the dynamics was noted a insignificant increase in the portion of invalids as a result of tuberculosis, diseases, caused HIV, diseases of eye and its additional apparatus, diseases of ear and mammiform branch and decrease of the specific weight of invalids as a result of the diseases of endocrine system, diseases of the organs of respiration, digestion, consequences of injury and poisonings.

The article presents results of the study ofproblems of the repeated disability due to diseases of the musculoskeletal system and connective tissue in retirement age citizens of the Russian Federation in the period of from 2005 to 2014. There were analyzed the number of this category of invalids, their specific weight among re-recognized as disabled cases with account of classes of diseases and the age, structure of the repeated disability in groups, the level of repeated disability and ranking places of subjects of the Russian Federation on this index in 2014. The results of the executed analysis indicate that only in the Russian Federation for 10 years (2005-2014) there were re-recognized as disabled due to diseases of the musculoskeletal system and connective tissue 487.1 thousand people from citizens of retirement age with distinct dynamics of the increase as their absolute number (from 22.1 thousand persons in 2005 to 69.3 thousand people in 2014) and the index of disability level (respectively from 7.6 to 20.5 per 10 thousand of the corresponding adult population). This pathology was established to determine a high level of repeated disability in this age group (20.5 per 10 thousand population). Comparative analysis by age revealed that in the Russian Federation in the structure of the repeated disability due to diseases of the musculoskeletal system and connective tissue in the adult population invalids of the retirement age prevail (42.8% in 2014). Along with diseases of the circulatory system (of rank 1) and malignant neoplasms (rank 2), diseases of the musculoskeletal system and connective tissue form the contingent of cases repeatedly recognized as disabled of retirement age (3th rank place). A ranking ofall subjects of the Russian Federation by the level of repeated disability of citizens of retirement age due to diseases of the musculoskeletal system and connective tissue allowed to select 70 ranking places, that with bearing in mind the present demographic situation, has a practical orientation for planning the organization of medical and social care for the given large category of the population.

In view of the observed cases of various diseases of the nervous system, which are associated with single poisoning with carbon monoxide, the number of which in the medical literature is increasing every year, as well as in the form of nervous and mental disorders in chronic poisoning with the same gas, Maureau, Е. Вескеr, E. V. Hardin and me, it seemed very interesting to trace the course of blood circulation in the brain during the process of poisoning itself, all the while I gratefully accepted the proposal and permission of the highly respected prof. V. M. Bekhterev to carry out research in his laboratory on this issue.

AbstractNervous system diseases are the subject of intensive research due to their association with high mortality rates and their potential to cause irreversible disability. Most studies focus on targeting the biological factors related to disease pathogenesis, e.g. use of recombinant activator of plasminogen in the treatment of stroke. Nevertheless, multiple diseases such as Parkinson’s disease and Alzheimer’s disease still lack successful treatment. Recently, evidence has indicated that physical factors such as the mechanical properties of cells and tissue and topography play a crucial role in homeostasis as well as disease progression. This review aims to depict these factors’ roles in the progression of nervous system diseases and consequently discusses the possibility of new therapeutic approaches. The literature is reviewed to provide a deeper understanding of the roles played by physical factors in nervous system disease development to aid in the design of promising new treatment approaches. This systematic review aims to identify probiotics and prebiotics for modulating oral bacterial species associated with mental disorders. Using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guideline, we search the electronic MEDLINE database published till January 2021 to identify the studies on probiotics and/or prebiotics for preventing and treating major oral dysbiosis that provokes mental disorders. The outcome of the search produces 374 records. After excluding non-relevant studies, 38 papers were included in the present review. While many studies suggest the potential effects of the oral microbiota on the biochemical signalling events between the oral microbiota and central nervous system, our review highlights the limited development concerning the use of prebiotics and/or probiotics in modulating oral dysbiosis potentially involved in the development of mental disorders. However, the collected studies confirm prebiotics and/or probiotics interest for a global or targeted modulation of the oral microbiome in preventing or treating mental disorders. These outcomes also offer exciting prospects for improving the oral health of people with mental disorders in the future.

Background: Transverse myelitis (TM) is an inflammatory disorder that can be idiopathic or associated with central nervous system autoimmune/dysimmune inflammatory diseases, connective tissue autoimmune diseases, or post-infectious neurological syndromes. Prognosis of initial TM presentations is uncertain. Objective: To identify outcome predictors in TM. Methods: Retrospective study on isolated TM at onset. Scores ⩾3 on the modified Rankin scale (mRS) marked high disability. Results: A total of 159 patients were identified. TM was classified as follows: idiopathic (I-TM, n = 53), post-infectious (PI-TM, n = 48), associated with multiple sclerosis (MS-TM, n = 51), or neuromyelitis optica spectrum disorders/connective tissue autoimmune diseases/neurosarcoidosis ( n = 7). At follow-up (median, 55 months; interquartile range, 32–80), 42 patients were severely disabled, and patients with I-TM or PI-TM showed the worst outcomes. Predictors of disability were infectious antecedents, sphincter and pyramidal symptoms, high mRS scores, blood–cerebrospinal fluid barrier damage, lumbar magnetic resonance imaging (MRI) lesions on univariate analysis, and older age (odds ratio (OR), 1.1; 95% confidence interval (CI), 1.0–1.1), overt/subclinical involvement of the peripheral nervous system (PNS) (OR, 9.4; 95% CI, 2.2–41.0), complete TM (OR, 10.8; 95% CI, 3.4–34.5) on multivariate analysis. Conclusion: Our findings help define prognosis and therapies in TM at onset. Infectious antecedents and PNS involvement associate with severe prognosis. Nerve conduction studies and lumbar MRI could improve the prognostic assessment of this condition.
