The experience with the distant remote rehabilitative treatment of the patients presenting with the severe injury and multiple lesions of the musculo - skeletal system: a review of the clinical reports

The problem of providing a continuous staged rehabilitation treatment of patients with severe associated injuries musculoskeletal system is an urgent and important task. This article describes two clinical cases of successful comprehensive medical rehabilitation of disabled persons with severe combined trauma and multiple injuries skeleton in need of long-term continuous rehabilitation program which was successfully launched at the stage of hospital care and continued remotely, through on - line technologies in a familiar patient home medium. As a result of rehabilitation treatment was observed: increased mobility of patients and level of independence in daily life, increased range of motion in the affected joints, increase muscle strength of the affected limbs, improving psycho - emotional status of patients resocialization and adaptation to the familiar home environment.

In pre-revolutionary China most disabled persons had no opportunity to receive education or obtain employment, and many of them experienced social discrimination. After the founding of the People's Republic, the government adopted a number of measures aimed at guaranteeing their livelihood. Efforts were made to give those who maintained the ability to work the opportunity to do so; those who were unable to work were provided with emergency relief or, if they had no family members who could support them, placed in orphanages, long-term psychiatric hospitals, and other types of welfare institution. Once the basic livelihood needs of the disabled were met, the next objective was to provide rehabilitative services. The evolution of these services has depended on three separate but related developments: the maturation of an organisational structure for the co-ordination of the services; the promulgation of laws that safeguard the rights and privileges of disabled persons; and the formation of academic societies of medical rehabilitation. This paper first considers these three developments and then examines how they have influenced the evolution of psychiatric rehabilitation in China.

The paper presents the experience of rehabilitation treatment for servicemen who had suffered from acute nonrheumatic myocarditis. The treatment was conducted in the 1469 Naval Clinical Hospital of the Northern Fleet with using an 8-modular system of medical rehabilitation activities: a laboratory-diagnostic cluster with a patient’s rehabilitation and expert evaluation and a rehabilitation prognosis; drug therapy, diet therapy, psychosomatic diagnosis and correction of the emotional state; therapeutic exercise with restoration of physical performance and adaptive health parameters; information cluster — providing the patient with a complete and accessible understanding of information about a healthy lifestyle; physiotherapeutic non-pharmacological correction of physiological disorders, activation of endogenous protective systems of the body; functional diagnostics complex: a study of restoration of functioning parameters for cardiovascular and respiratory systems, the development of the body’s adaptation to physical exertion. As a result of using the modular system, a decrease in the average bed-day during the rehabilitation phase for patient after mild acute myocarditis was 16,6% (from 17,5±1,3 to 14,6±2,7 days); after moderate myocarditis — 8.1% (from 22,3±2 to 20.5±1,8 days); and after severe — 5,9% (from 27,2±3,5 to 25,6±1,2 days). The rehabilitation program, which has been developed basing on principles of the theory of modules, is a dynamic, easily simulated system with a combination of variable and permanent elements, that allows to reduce the cost of treatment and rehabilitation in mild myocarditis by 10,6±0,9%, in moderate myocarditis — by 7,2±0,8%, and in severe myocarditis — by 3,7±0,4%.

Abstract Tourist services represent a category of services in which the inseparability of production and consumption, the inability to be storable, the immateriality, and last but not least non-durability, induces in tourism management a number of peculiarities and difficulties. Under these circumstances the development of medium-term strategies involves long-term studies regarding on the one hand the developments and characteristics of the demand, and on the other hand the tourist potential analysis at regional and local level. Although in the past 20 years there has been tremendous growth of on-line booking made by household users, the tour operators agencies as well as those with sales activity continue to offer the specific services for a large number of tourists, that number, in the case of domestic tourism, increased by 1.6 times in case of the tour operators and by 4.44 times in case of the agencies with sales activity. At the same time, there have been changes in the preferences of tourists regarding their holiday destinations in Romania. Started on these considerations, paper based on a logistic model, examines the evolution of the probabilities and scores corresponding to the way the Romanian tourists spend their holidays on the types of tourism agencies, actions and tourist areas in Romania.

The use of Customer Relationship Managers to foster customers loyalty has become one of the most common business strategies in the past years.  However, CRM solutions do not fill the abundance of happily ever-after relationships that business needs, and each client’s perception is different in the buying process.  Therefore, the experience must be precise, in order to extend the loyalty period of a customer as much as possible. One of the economic sectors in which CRM’s have improved this experience is retailing, where the personalized attention to the customer is a key factor.  However, brick and mortar experiences are not enough to be aware in how environmental changes could affect the industry trends in the long term.  A base unified theoretical framework must be taken into consideration, in order to develop an adaptable model for constructing or implementing CRMs into companies. Thanks to this approximation, the information is complemented, and the outcome will increment the quality in any Marketing/Sales initiative. The goal of this article is to explore the different factors grouped by three main domains within the impact of service quality, from a consumer’s perspective, in both on-line and off-line retailing sector.  Secondly, we plan to go a step further and extract base guidelines about previous analysis for designing CRM’s solutions focused on the loyalty of the customers for a specific retailing sector and its product: Sports Running Shoes.

Coronavirus disease 2019 (COVID-19) causes significant mortality and morbidity, with an unknown impact in the medium to long term. Evidence from previous coronavirus epidemics indicates that there is likely to be a substantial burden of disease, potentially even in those with a mild acute illness. The clinical and occupational effects of COVID-19 are likely to impact on the operational effectiveness of the Armed Forces. Collaboration between Defence Primary Healthcare, Defence Secondary Healthcare, Defence Rehabilitation and Defence Occupational Medicine resulted in the Defence Medical Rehabilitation Centre COVID-19 Recovery Service (DCRS). This integrated clinical and occupational pathway uses cardiopulmonary assessment as a cornerstone to identify, diagnose and manage post-COVID-19 pathology.

AbstractThe aim of this study was to estimate the long-term results of complex and supervised rehabilitation of the hands in systemic sclerosis (SSc) patients. Fifty-one patients were enrolled in this study: 27 patients (study group) were treated with a 4-week complex, supervised rehabilitation protocol. The control group of 24 patients was prescribed a home exercise program alone. Both groups were evaluated at baseline and after 1-, 3-, 6-, and 12-months of follow-up with the Disability of the Arm, Shoulder and Hand Questionnaire (DAHS) as the primary outcome, pain (VAS—visual analog scale), Cochin Hand Function Scale (CHFS), Health Assessment Questionnaire Disability Index (HAQ-DI), Scleroderma-HAQ (SHAQ), range of motion (d-FTP—delta finger to palm, Kapandji finger opposition test) and hand grip and pinch as the secondary outcomes. Only the study group showed significant improvements in the DASH, VAS, CHFS and SHAQ after 1, 3 and 6 months of follow-up (P = 0.0001). Additionally, moderate correlations between the DASH, CHFS and SHAQ (R = 0.7203; R = 0.6788; P = 0.0001) were found. Complex, supervised rehabilitation improves hand and overall function in SSc patients up to 6 months after the treatment but not in the long term. The regular repetition of this rehabilitation program should be recommended every 3–6 months to maintain better hand and overall function.

Abstract Aim: With increasing demands on the healthcare system, a central (and currently essential) push for remote consultations, and an increasing number of co morbidities in the surgical population, the aim was to create an application that placed the focus on pre-hospital optimisation, education and autonomy – addressing these sector trends to enhance outcomes for patients and ease the burden on healthcare settings. Method After obtaining baseline data, I worked alongside students from the Queensland University of Technology to produce an application suitable for both iOS and android platforms. Results The application has four facets which mirror a patient’s journey from their initial outpatient appointment through to recovery. The application is easy to use, free to download and readily accessible. On opening the application, the user can select their planned operation and navigate along a personalised decision tree. Conclusions The application has been designed to educate and empower a patient to become an active participant in their care, leading to long-term changes in healthcare. Assessment is ongoing but early indications suggest that this will be a valuable tool in optimising outcomes for patients undergoing vascular surgery – leading to fewer post-operative complications and earlier restoration of functional status.

This case study documents a 13-year-old female who presented to our intensive inpatient chronic pain rehabilitation program with complex regional pain syndrome (CRPS) of her left leg, which was significantly interfering with her normal daily functioning. She participated in a full day of traditional interdisciplinary therapies, including physical and occupational therapy for 3 hours daily. As assistive equipment was altered or weaned her physical mobility, balance, and tremors worsened and/or increased. As she began advancing her legs more independently (versus requiring physical assist), she demonstrated more variable functional strength and stability, inconsistent balance reactions, and a more unsteady gait pattern. The team was treating her according to her incoming CRPS diagnosis; however, as treatment progressed, her physical and psychological presentation seemed more aligned with diagnostic criteria of functional neurologic symptom disorder (FND). Staff then treated according to the FND diagnosis resulting in successful long-term outcomes. The clinical impact from this case study includes highlighting the commonalities between CPRS and FND clinically, discussing differentiating treatment suggestions depending on the diagnosis, and emphasizing key components of family/patient education.
