Psychometric Properties of the Hospital Ethical Climate Survey: A Cross-Sectional Study in Greek and Cypriot Cancer Care Settings

Background and Purpose:Ethical climate provides the context in which ethical behavior and decision-making occur. To test the psychometric properties of the Greek version of the Hospital Ethical Climate Survey (HECS) in cancer care settings.Methods:This was a cross-sectional study with 235 cancer nurses. Principal component analysis (PCA) and confirmatory factor analysis (CFA) were examined. Reliability was investigated with Cronbach’s coefficient α.Results:Cronbach’s α was 0.86 for the HECS total and ranged from 0.71 to 0.85 for the five subscales. PCA revealed that five components accounted for 61.09% of the variance which were comparable to those produced in the original validation study. The CFA with the five factors identified, produced a model with a good fit.Conclusion:The Greek version of the HECS is valid and reliable for use within the cancer care context.

Background/aimsCancer burden is predicted to double by 2030 in sub-Saharan Africa; access to healthcare services for cancer management is a priority in the region. In Nigeria, National Cancer Control Plan aims to ensure >50% cancer screening of eligible populations by 2022 for all Nigerians. We describe healthcare utilisation, cancer screening activities and potential barriers to accessing cancer care within an understudied rural community-based adult population in South West Nigeria.MethodsIn April 2018, we conducted a cross-sectional study of community-based adults (>18 years) ~130 km east of Ibadan, 250 km from Lagos in Osun State, South West Nigeria. Participants completed a face-to-face survey in local dialect. We used a questionnaire to assess demographics, health status, income, medical expenditures, doctor visits and cancer screening history.ResultsWe enrolled 346 individuals: with median age of 52 years and 75% women. Of the entire cohort, 4% had medical insurance. 46% reported a major medical cost in the last year. Cancer screening activities were infrequent in eligible participants: 1.5% reported having had cervical cancer screening, 3.3% mammogram and 5% colonoscopy screening. Cancer screening assessment was less frequent in those with less income and lower education levels. Using a multivariable logistic regression model including personal income, insurance status and education, higher personal income was associated with more cancer screening activity (OR 2.7, 95% CI 1.3 to 5.7, p<0.01). Despite this, most individuals had contact with a primary healthcare doctor (52% in the last year), and over 70% access to radio and TV suggesting the opportunity to expand community-based screening interventions and awareness exists.ConclusionsDespite national increases in cancer cases, we highlight a deficiency in cancer screening and universal healthcare coverage within a community-based adult Nigerian population. Subject to availability of governmental resources, increasing financial risk protection, awareness and targeted resource allocation may help expand access in Nigeria.

Background: Azo compounds, containing naphthol and diazonium salts, are synthetic dyes widely used in the batik industry. Azo compounds are considered toxic when they are exposed to human tissue. The purpose of this study was to analyze buccal cell DNA exposed to azo compounds in batik workers. Methods: A cross-sectional study involving 20 male subjects divided into two groups (n=10 group), namely azo-exposed and non-exposed (control group). Inclusion criteria were batik workers of the colouring division who have been exposed to azo for at least 5 years. Buccal cells were taken using cytobrush then DNA were isolated from buccal cell. DNA isolation was done by buccal DNA kit, while the purity and concentration of the DNA was determined using spectrophotometer and electrophoresis. Results: The azo-exposed group revealed higher purity DNA than those in the control group. The purity of the DNA in the azo-exposed group and control group was 0.61±0.93 and 0.21±0.09, respectively, while the concentration of DNA was of 59.02 and 19.35 ng/UL, respectively. The ratio at 260/280 nm was 1.84-1.94 (azo-exposed) and 1.85-1.92 (control). Principal component analysis using the first principle component (PC1) and second principle component (PC2) could successfully classify subjects in the control and azo-exposed groups. Conclusion: Characteristics of DNA could be used as an indication of exposure to azo compounds in workers of batik industries.

Background: The Ten-Item Personality Inventory (TIPI) is a validated brief instrument measuring the five-factor model (FFM) personality dimensions, developed for instances where more comprehensive FFM instruments are impractical to use. The TIPI has been translated into several languages, but psychometric properties of the Norwegian version (N-TIPI) have not been systematically explored.Objectives: This study aimed to explore the psychometric properties of the N-TIPI, in terms of internal consistency and structural validity.Methods: In a cross-sectional study, responses on the N-TIPI were collected from 5,009 Norwegian master graduates. Descriptive statistics for the subscales and correlations between subscales were calculated. Internal consistency was assessed with inter-item correlations, Cronbach’s α and Spearman-Brown coefficients. Structural validity was explored with principal component analysis, parallel analysis, and visual scree plot inspection. Results for the N-TIPI were compared with those previously reported for the original TIPI as well as the German, French, Spanish, and Portuguese versions.Results: Compared with the original and non-English versions of TIPI, results for N-TIPI showed comparable subscale rank order of means, standard deviations, and pattern of correlations between subscales, as well as inter-item correlations and Cronbach’s α. The 10 N-TIPI items were adequately reduced to five components, theoretically corresponding with the FFM personality domains.Conclusion: The N-TIPI demonstrated acceptable internal consistency and satisfactory structural validity. Although further research is warranted, the instrument stands out as feasible when it is essential to minimize participants’ response burden in studies that aim to explore personality as one among several concepts or utilize personality traits as covariates.

IntroductionPatient-derived xenografts (PDXs) have the potential to transform personalised cancer care, however, little is known about the acceptability of using PDXs to guide treatment decision-making. Given that patient and community preferences can influence satisfaction with care as well as the success of new technologies, we will evaluate the acceptability of PDXs in individuals affected by cancer and community comparisons.Methods and analysisThis comparative cross-sectional study will recruit 323 individuals affected by cancer (cancer survivors (of childhood or adult cancer) and parents of childhood cancer survivors) and 323 community comparisons (adults and parents). We will collect data via structured interviews and questionnaires. To determine the acceptability of PDXs, we will assess five domains: willingness to use PDXs when/if diagnosed with cancer, perceived advantages and disadvantages of PDXs, maximum acceptable out-of-pocket costs per patient, maximum acceptable turnaround time to receive results and maximum acceptable number of mice sacrificed per patient. The primary endpoint will be participants’ decisional balance ratio (calculated as participants’ advantages ratings divided by perceived disadvantages ratings).Ethics and disseminationThe study protocol has been approved by the South Eastern Sydney Local Health District Human Research Ethics Committee (HREC:12/173) and UNSW Sydney (HC15773). The results will be disseminated in peer-reviewed journals and at scientific conferences. A lay summary will be published on the Behavioural Sciences Unit website.
