The Limb Body Wall Complex (LBWC) aka. Body Stalk Syndrome is an uncommon congenital disorder characterized by severe malformations of limb, thorax, and abdomen, characterized by the presence of thoracoschisis, abdominoschisis, limb defects, and exencephaly. This condition is extremely rare with an incidence of 1 per 14,000 and 1 per 31,000 pregnancies in large epidemiologic studies. Majority of these malformed fetuses end up with spontaneous abortions. We present this rare case with occurrence in a preterm infant of 35 weeks gestation. Our report highlights majority of the clinical presentations as reported in previous literature, but the significant pathological findings of absent genitalia and malformed genitourinary, anorectal malformations make this case presentation an even more rare occurrence. Infant karyotyping was normal male and there is no specific underlying genetic correlation in this condition which has fatal prognosis.

Abstract Dermatophytosis includes all fungal infections caused by dermatophytes in humans. Some risk factors for the development of subtypes of dermatophytosis have been studied; however, large-scale epidemiologic studies on risk factors for total dermatophytosis are scarce. We investigated the risk factors of dermatophytosis using a nationwide study. Total 4,532,655 subjects with dermatophytosis aged between 20 to 40 years were examined using data from the Korean National Health Insurance Service from 2009 to 2018. Women showed a lower risk of development of dermatophytosis compared to men (hazard ratio [HR], 0.848; 95% confidence interval [CI], 0.843–0.853). Subjects with elevated waist circumference (HR, 1.057; 95% CI, 1.048–1.065), heavy drinking (HR, 1.053; 95% CI, 1044–1.061), engaging in mild-to-heavy exercise (HR, 1.071; 95% CI, 1.064–1.077) had a higher risk of dermatophytosis. In addition, subjects with body mass index (BMI) of more than 30 kg/m2 exhibited a higher risk of dermatophytosis (HR, 1.36; 95% CI, 1.342–1.378) compared to those with BMIs in the range of 18.5 to 23 kg/m2. In this study, the risk of developing dermatophytosis significantly increased in individuals with elevated waist circumference or high BMI. Lifestyle modifications, including weight management, are suggested to be important in preventing dermatophytosis.

AbstractDespite significant advances in the treatment and care of people with HIV (PWH), several challenges remain in our understanding of disease pathogenesis to improve patient care. HIV infection can modify the host epigenome and as such can impact disease progression, as well as the molecular processes driving non-AIDS comorbidities in PWH. Epigenetic epidemiologic studies including epigenome-wide association studies (EWAS) offer a unique set of tools to expand our understanding of HIV disease and to identify novel strategies applicable to treatment and diagnosis in this patient population. In this review, we summarize the current state of knowledge from epigenetic epidemiologic studies of PWH, identify the main challenges of this approach, and highlight future directions for the field. Emerging epigenetic epidemiologic studies of PWH can expand our understanding of HIV infection and health outcomes, improve scientific validity through collaboration and replication, and increase the coverage of diverse populations affected by the global HIV pandemic. Through this review, we hope to highlight the potential of EWAS as a tool for HIV research and to engage more investigators to explore its application to important research questions.

Sports-related injuries, such as concussion, during childhood may result in considerable morbidity, including a range of negative developmental consequences. Auditory dysfunction is generally recognized to be a possible sequela of sports-related concussion; however, few epidemiologic studies have quantified the association between hearing quality and sports-related activity in the pediatric population. The National Health and Nutrition Examination Survey for the 2015-2016 cycle was utilized to determine the association of sports activities and hearing quality. Subjective abnormal hearing quality was more frequent among children who played football than those that did not (36.5% vs 26.8%; odds ratio, 1.56 [95% CI, 1.23-2.00]; P = .001). Other sports, such as basketball and soccer, did not have this association with hearing quality ( P = .496 and P = .852, respectively). Our findings suggest a notable association between practice of a potentially high concussion sport and hearing quality. Children practicing football are 1.56 times more likely to report abnormal hearing quality. Level of Evidence: 4.

Optimal use of aerosol optical depth (AOD)-PM2.5 fused surfaces in epidemiologic studies requires homogeneous temporal and spatial fused surfaces. No analytic method is currently available to evaluate the spatial dimension. The temporal case-crossover design was modified to assess the association between Community Multiscale Air Quality (CMAQ) lag grids and four respiratory-cardiovascular hospital events. The maximum number of adjacent lag grids with the expo-sure-health outcome association determined the size of the homogeneous spatial area. The largest homogeneous spatial area included 5 grids (720 km2) and the smallest 2 grids (288 km2). PMC and PMCK analyses of ED asthma, IP asthma, IP MI, and IP HF were significantly higher in rural grids without air monitors than in urban with air monitors at lag grids 0, 1, and 01. Grids without air monitors had higher AOD-PM2.5 concentration levels, poverty percent, population density, and environmental hazards than grids with air monitors. ED asthma, IP MI, and HF PMCK ORs were significantly higher during the warm season than during the cold season at lag grids 0, 1, 01, and 04. The possibility of elevated fine PM and other demographic and environmental risk factors contributing to elevated respiratory-cardiovascular diseases in persons residing in rural areas was discussed.

Objective: This study aimed to evaluate the longitudinal association between changes in depressive symptoms and hazardous drinking among South Korean adults.Participants/Methods: This study was conducted using a sample drawn from participants enrolled in the Korea Welfare Panel Study (KoWePS) from 2013 to 2018. Hazardous drinking was defined as a score of 11 points for men and seven points for women on the Korean version of the Alcohol Use Disorders Identification Test. Depressive symptoms were evaluated using an 11-item version of the Center for Epidemiologic Studies Depression Scale. A generalized estimating equation model was used to analyze the association between changes in depressive symptoms and hazardous drinking.Results: Of the 12,878 people registered with KoWePS and without follow-up losses from 2013 to 2018, a total of 2,341 were included in this study, excluding those under the age of 19 and those with missing data. Persistent depressive symptoms (men, odds ratio [OR]: 2.20, 95% confidence interval [CI]: 1.48–3.21; women, OR: 2.64, 95% CI: 1.66–4.22) and the changes from non-depressive symptoms to depressive symptoms (men, adjusted OR: 2.18, 95% CI: 1.80–2.64; women, OR: 1.71, 95% CI: 1.27–2.29) increased the likelihood of hazardous drinking.Conclusions: Persistent depressive symptoms and changes from non-depressive to depressive symptoms are associated with increased prevalence of hazardous drinking. This suggests that an evaluation of the underlying mental illness or emotion should be made when counseling for abstaining from alcohol for chronic drinkers or the general public.

Between 2007 and 2017, Brazil registered 99,826 outbreaks of foodborne diseases and 0.84% of those were associated with fish meat intake. It is estimated that approximately 56 million infection cases occur worldwide due to raw or undercooked fish meat containing several disease-causing parasites. Hence, this study aimed to review the literature concerning diseases caused by ingestion of contaminated fish meat. Reviews, case reports and epidemiologic studies were searched in Portuguese, Spanish and English in the databases LILACS, Pubmed, Science Direct, SciElo and Scholar Google using as keywords: transmissive diseases, contaminated fish and human infections were used to retrieve papers from 2014 to 2020. Nine papers, including seven reviews, one case report and one case-control study fulfilled inclusion criteria and presented several consequences of contaminated raw or undercooked fish meat ingestion, which ranged from nemathode, bacterial and toxin diseases that may cause gastrointestinal problems to allergic reactions, lung infection, endemic acute myalgia, bacteremia, meningitis and death. Growing fish meat intake in several dishes presents significant health risk due to the pathogenic potential of toxins and parasites that remain when food is consumed raw or undercooked. Tighter sanitary surveillance, population health education, training and sensitization of health professionals in recognizing and notifying cases might contribute to minimize risk.

Importance: Epidemiologic studies suggest maternal immune activation during pregnancy may be associated with neurodevelopmental effects in offspring. Objective: To determine whether in utero exposure to the novel coronavirus SARS-CoV-2 is associated with risk for neurodevelopmental disorders in the first 12 months after birth. Design: Retrospective cohort Participants: Live offspring of all mothers who delivered between March and September 2020 at one of six Massachusetts hospitals across two health systems. Exposure: SARS-CoV-2 infection confirmed by PCR during pregnancy Main Outcome and Measures: Neurodevelopmental disorders determined from ICD-10 diagnostic codes over 12 months; sociodemographic and clinical features of mothers and offspring; all drawn from the electronic health record. Results: The cohort included 7,772 live births (7,466 pregnancies, 96% singleton, 222 births to SARS-CoV-2 positive mothers), with mean maternal age of 32.9 years; offspring were 9.9% Asian, 8.4% Black, and 69.0% white; 15.1% were of Hispanic ethnicity. Preterm delivery was more likely among exposed mothers (14% versus 8.7%; p=.003). Maternal SARS-CoV-2 positivity during pregnancy was associated with greater rate of neurodevelopmental diagnoses (crude OR 2.17 [95% CI 1.24-3.79, p=0.006]) as well as models adjusted for race, ethnicity, insurance status, offspring sex, maternal age, and preterm status (adjusted OR 1.86 [95% CI 1.03-3.36, p=0.04]). Third-trimester infection was associated with effects of larger magnitude (adjusted OR 2.31, 95% CI 1.16-4.21, p=0.01) Conclusion and Relevance: Our results provide preliminary evidence that maternal SARS-CoV-2 may be associated with neurodevelopmental sequelae in some offspring. Prospective studies with longer follow-up duration will be required to exclude confounding and confirm these effects.
