Up to the end of 2012, Social Security showed the occurence of 103.074 work accident cases. 91.21% of the accident casualties were recovered, 3.8% were disabled, 2.61% partially disabled, and the rest died (2.419 cases) and were totally-permanently disabled (37 cases), with the average of 282 work accident cases per day. Due to the increasingly tight corporate climate, the company tries to make production cost efficiency (cost of production). One solution that may be taken is by applying outsourcing system. With this system, the company may save expenses in funding human resources (HR) who work in the company concerned. If the aspects of the application of Occupational Safety and Health (OSH) are not properly managed, the possibility of work accidents and environmental pollution that have a negative effect on the corporate image and the continuity of company product supply will increase. This may affect the company's loss, both material and corporate image. The role and responsibility of the safety officer is very influential on the achievement of zero accident, so many safety programs are developed, such as safety briefing and safety patrol, with the aim to improve the aspect of OSH. However, some employees have not complied with the provisions of OSH implementation in the company they work at.

Every production process in a company is inseparable from all the dangers of disease and occupational accidents for workers. So in a company it is necessary to implement occupational safety and health in order to reduce and prevent occupational diseases and accidents. In carrying out occupational safety and health at PT Pantja Simpati, PT Pantja Simpati has several problems that can cause illness and work accidents such as unavailability of personal protective equipment, non-functioning air vents, employees smoking in the production room, employees drinking in the production section. part. areas, fire fighting equipment lacks lighting, damaged ceilings or roofs, untidy cables, and slippery production floors. Ways to avoid work accidents such as providing PPE and APAR, repairing damaged equipment, and taking firm action against workers. From the problems found, the implementation of occupational safety and health at PT Pantja Simpati is still not good.Keywords— Occupational Safety and Health, 5W+1H

ABSTRACTUnsafe action is a trigger for work accident. Unsafe action happened for two reasons, such as unintentional mistakes and active errors or violation of a rule. Unsafe action influenced by the internal factor from workers itself, such as characteristic of workers. The objectivs of this research is to aims of this study are analyze the relationship between the characteristics of workers to unsafe action, in stevedor at PT. Terminal Petikemas Surabaya. This study was observational Descriptive. The sample was all stavedore in a group of which consisted of 60 people. The data presented in the form of distribution the frequency and tabulation cross then analyzed statistically the Spearman. The result of this study shownthat there was an relatively low association between the characteristic of workers with unsafe action, but there was one variable had the strong enough relationship, such as knowledge variable and unsafe action on stevedore in PT. Terminal Petikemas Surabaya with coefficient corelation 0.417. Based on the result of this study, suggestions that can be given to the HSE departement are scheduled refreshment through training and briefing to increase knowledge of occupational safety and health at work, meanwhile supervisor of field have to do a strict and firmly supervise to stevedore who performing unsafe action. So it expected to reduce the number of work accident in the process of loading and unloading.Keywords: Stevedore, unsafe action, characteristic of workers

K3 program aimed to provide security for employees in carrying out their work and reduce company costs. If work accidents and illness arise due to work relations, occupational safety and health (K3) must be a top priority in all companies in order to achieve satisfaction. The purpose of this study was to find out the description of the level of workforce satisfaction with the K3 program in the AMP (Asphal Mixing Plant) section of PT. Kerinci Development in 2017. The design of this study was descriptive. The population is all labor in PT. Prima Pembangunan Kerinci AMP section with 30 samples. The method of sampling with total sampling technique, the study was conducted in March - August 2017 and the research instrument using a questionnaire. Based on the results of the study as many as 13 workers (43.3%) were not satisfied with work safety and as many as 17 workers (56.7%) were not satisfied with the work health of the K3 program in the AMP (Asphal Mixing Plant) PT. Prima Pembangunan Kerinci in 2017. It is hoped that the company leaders will conduct routine health checks every month and also leaders pay more attention to employee safety by providing training on awareness of using personal protective equipment and introducing personal protective equipment and ways of using it, so that employees can use at any time in the event of a work accident.
