ABSTRACTBackgroundEarly life adversity exposure impacts preterm infants’ neurodevelopment and early intervention protocols may modulate neurodevelopmental outcomes.Neuronal genomes are plastic in response to environment and mobile genetic elements, including LINE-1 (L1), are source of brain genomic mosaicism. Maternal care during early life regulates L1 methylation and copy number variations (CNVs) in mice. Here, we sought to identify the effects of maternal care and positive multisensory stimulation (Early Intervention) on L1 methylation and neurodevelopment in preterm infants.MethodsVery preterm infants were randomized to receive Standard Care or Early Intervention. L1 methylation was measured at birth and at hospital discharge. At 12 months infants’ neurodevelopment was evaluated with the Griffiths Scales. L1 methylation and CNVs were measured in mouse brain areas at embryonic and postnatal stages.ResultsWe demonstrated that L1 is hypomethylated in preterm versus term infants at birth. Early Intervention contributes to restore L1 methylation and positively modulates neurodevelopment. We showed that L1 methylation is developmentally-regulated in mice, decreasing in early postnatal life stages, which turns into an increased L1 CNVs specifically in hippocampus and cortex.ConclusionsHere we demonstrated that L1 dynamics can be modulated by Early Intervention, in parallel with ameliorated neurodevelopmental outcomes. SummaryAge at menarche is associated with anthropometry in adolescence. Recently, there has been growing support for the hypothesis that timing of menarche may be set early in life but modified by changes in body size and composition in childhood. To evaluate this, a cohort of 255 girls aged <5 years recruited in 1988 were followed up in 2001 in Matlab, Bangladesh. The analysis was based on nutritional status as assessed by anthropometry and recalled age at menarche. Data were examined using lifetable techniques and the Cox regression model. The association between nutritional status indicators and age at menarche was examined in a multivariate model adjusting for potential confounding variables. Censored cases were accounted for. The median age at menarche was 15·1 years. After controlling for early-life predictors (birth size, childhood underweight, childhood stunting) it appeared that adolescent stunting stood out as the most important determinant of age at menarche. Adolescent stunting still resonates from the effect of stunting in early childhood (OR respectively 2·63 (p<0·01 CI: 1·32–5·24) and 8·47 (p<0·001 CI: 3·79–18·93) for moderately and severely stunted under-fives as compared with the reference category). Birth size was not a significant predictor of age at menarche. It is concluded that age at menarche is strongly influenced by nutritional status in adolescence, notably the level of stunting, which is in turn highly dependent on the level of stunting in early childhood. A ‘late’ menarche due to stunting may be detrimental for reproductive health in case of early childbearing because of the association between height and pelvic size.

Infants who fail to grow much occur in the third month or fourth month of early life. Early breastfeeding of the ASI causes mothers not to give breast milk and cause infectious diseases that result in stunting in infants. The purpose of this research is to find out the relationship of early breastfeeding with nutritional status (PB/U) age 4-7 months in Ratu Samban Sub-district of Bengkulu City.  This research uses analytic observational with a cross-sectional design. The results showed the nutritional status of PB / U short category 18.3% and MP ASI early 66.7% while the statistical test showed probability (p) of 0.273 (p> 0.05).  There is no correlation between early breastfeeding of MP and nutritional status of PB / U age 4-7 months in Ratu Samban Sub-district of Bengkulu City. Need to do re-research a using Cohort method and with other variables.Bayi yang gagal tumbuh banyak terjadi pada bulan ketiga atau bulan ke empat awal kehidupan. Pemberian MP ASI dini mengakibatkan ibu tidak berusaha memberikan ASI dan menyebabkan terjadinya penyakit infeksi yang mengakibatkan stunting pada balita. Tujuan dari penelitian ini adalah untuk mengetahui hubungan pemberian MP-ASI dini dengan status gizi (PB/U) pada bayi usia 4-7 bulan di Kecamatan Ratu Samban Kota Bengkulu. Jenis penelitian ini menggunakan observasional analitik dengan desain cross sectional. Hasil penelitian menunjukan status gizi PB/U kategori pendek 18,3% dan MP ASI dini 66,7% sedangkan uji statistik menunjukkan nilai probabilitas (p) sebesar 0,273 (p > 0,05).  Tidak ada hubungan antara pemberian MP ASI dini dengan status gizi PB/U usia 4-7 bulan di Kecamatan Ratu Samban Kota Bengkulu . Hal ini berkaitan dengan pemberian makanan pendamping ASI sementara sebagai makanan prelaktal sebelum ASI ibu keluar dan diberikan dalam jumlah sedikit. Lebih baik dilakukan penelitian lanjutan dengan menggunakan metode Kohort dan dengan variabel lain.

Retinol palmitate oral administration is convenient, but it is difficult to assess/monitor its nutritional status in preterm infants and literature is controversial about the administration route and the effectiveness of vitamin A supplementation. We primarily evaluated retinol plasma levels to assess the vitamin A nutritional status in preterm infants (<1500 g; 32 weeks) after 28 days of oral supplementation (3000 IU/kg/day, retinol palmitate drops), in addition to vitamin A standard amount as suggested by European Society of Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) guidelines. We then observed the rate of typical preterm pathologies in the supplemented group (31 newborns) and in 10 matching preterm infants, hospitalized in neonatal intensive care unit (NICU) in the same period, who received neither vitamin A supplementation nor parents allowed plasma sampling. Oral integration resulted in constant retinol plasma concentration around the desired level of 200 ng/mL, but without statistical increase during the study period. Due to the complexity of vitamin A metabolism and the immaturity of preterm infant’s organs, retinol supplementation may had first saturated other needy tissues; therefore, plasmatic measures may not be consistent with improved global vitamin A body distribution. Therefore, achieving a constant retinol concentration is a valuable result and supportive for oral administration: decreasing levels, even after parenteral/enteral supplementation, were reported in the literature. In spite of favourable trend and no adverse events, we did not report statistical difference in co-morbidities. This investigation confirms the necessity to perform further trials in preterm newborns, to find an index reflecting the complex nutritional retinol status after oral administration of vitamin A, highlighting its effectiveness/tolerability in correlated preterm infant’s pathologies.
