There is growing recognition that men as well as women need sexual health care (SHC) services. Despite this, male friendly sexual health services are not readily available in the United States, and men are underutilizing the services that are available. This situation needs to be rectified to improve sexual health outcomes for men and women. In this study we conducted 10 focus groups with young adult Latino and African American men to examine their perceptions of the factors influencing SHC utilization among the men they know, with an emphasis on how notions of what it means to be a man affects health care seeking. The findings both amplify and complicate the relationship between masculinity and SHC seeking. They suggest new directions for public health efforts to enhance men’s SHC utilization.

BACKGROUND African American men who have sex with men (MSM) and transgender women bear a disproportionate burden of HIV. Young MSM account for 75% of this burden for youth. When youths lack socially protective resources such as strong networks of adults, including parents, teachers, or community members, mentors may play a critical role in promoting health behaviors. This is especially true for youth at risk for HIV, such as African American youth with sexual and gender minority (SGM) identities. In the past decade, natural mentoring and mentoring programs have proliferated as a key prevention and intervention strategy to improve outcomes for young people at risk for poor academic, social, and health issues. Mentors appear to be able to facilitate health promotion among young SGM by modeling healthy behaviors; however, mentors’ knowledge and resource needs regarding sexual health topics including HIV are understudied, as is the potential role of mobile technology in enhancing mentoring relationships and the ability of mentors to learn about sensitive issues faced by youth. OBJECTIVE The aim of this study is to explore how mentoring plays a role in the sexual health of African American SGM youth and understand how mentoring relationships can be strengthened through mobile technology to promote youth HIV prevention behaviors. METHODS In-depth interviews were conducted with African American SGM youth mentees (n=17) and mentors (n=20) to such youths in 3 Mid-Atlantic cities. Mentee interviews focused on discussions regarding sexual health and HIV and how a mentor could broach such topics. Mentor interviews explored whether sexual health and HIV are currently mentoring topics, mentors’ knowledge and confidence in mentoring on these issues, and barriers to discussions. All participants were asked if a mobile app could help facilitate mentoring on sensitive health issues, particularly HIV and sexual health. Data were transcribed, coded, and analyzed for relevant themes. RESULTS Sexual health was a common topic in mentoring relationships, occurring more in natural mentorships than in mentoring program pairs. Mentors and mentees felt positive about such discussions. Mentors expressed having limited knowledge beyond condom use and HIV testing, and expressed a need for more complete resources. Both mentors and mentees had mixed comfort levels when discussing sexual health. Sufficient trust and shared lived experiences made discussions easier. Mentees have multifaceted needs; however, mentors stated that an app resource that provided self-training, resources, support from other mentors, and tips for better mentoring could prove beneficial. CONCLUSIONS For the African American SGM community, access to natural mentors is crucial for young people to learn healthy behaviors. A mobile resource to assist mentors in confidently having discussions with mentees may be a promising way to promote healthy practices.

BACKGROUND African American young adults have low rates of uptake and engagement with health technologies, which may further widen sexual health inequalities. OBJECTIVE We examined factors influencing uptake and engagement for a consumer health informatics (CHI) intervention for HIV/STI prevention among African American young adults using the diffusion of innovation theory, the trust-centered design framework and O’Brien and Toms’ model of engagement. METHODS This community-based participatory research, mixed-methods study included surveys at four time points (N=315; 280 African-American) of young adults aged 18 to 24 involved in an HIV/STI prevention intervention described as “parties”. Qualitative interviews were conducted with a subset of participants (N=19) after initial surveys, website server logs, and social media accounts indicated low uptake and engagement. A generalized linear mixed-effects model identified predictors of eIntervention uptake, server logs were summarized to describe use over time, and interview transcripts were coded and thematically analyzed to identify factors affecting uptake and engagement. RESULTS Self-reported eIntervention uptake was low, but increased significantly over time, Demographic factors and HIV/STI-related behaviors were not significantly correlated with uptake. The most frequent activity was visiting the website, followed by visiting the Facebook page. Factors driving uptake were the desire to share HIV/STI prevention information with others, trust in the intervention, and gender homophily. Factors undermining uptake were personal and group distrust online. Factors driving initial engagement were audience-targeted website aesthetics and appealing visuals; long-term engagement was impeded by insufficiently frequent updates. CONCLUSIONS To encourage uptake, CHI interventions for African-American young adults can leverage users’ desire to share information about HIV/STI prevention with others. Ensuring implementation through trusted organizations is also important, though there is a need for vigorous promotion. Visual appeal and targeted content foster engagement at first, but ongoing engagement may require continual content changes. A thorough analysis of CHI intervention use can inform the development of future interventions in order to promote uptake and engagement. To guide future analyses, we present an expanded uptake and engagement model for CHI interventions targeting African American young adults based on the empirical results presented here.

Background Media is a social determinant of HIV and sexually transmissible infection (STI) risk. However, limited empirical data have examined men’s media exposure and their sexual attitudes and behaviour towards women. Methods: Eighty heterosexual African-American men were assessed on their exposure to music videos, sexual attitudes and behaviour. They also were tested for STIs. Results: Findings indicated that men influenced by music videos reported more sexual adventurism, more condom barriers, more lifetime sexual partners, more condom request refusals, substance abuse and a history of incarceration. Conclusions: Further longitudinal research is needed to better understand this relationship and to address the role of media in HIV and STI prevention interventions.
