The article describes the problem of corruption occurring in the relationship between doctor and patient. The doctor–patient relationship, including the provision of health services, is one of several potential areas of corruption in the health care system. Among the reasons for the existence of corruption in these relationships are the need to obtain better health care for the patient, and higher earnings in the case of a doctor. Indications of corruption are utilitarian (action for personal advantage without ethical aspects), but may also be (actually or in the patient’s opinion) the only way to obtain services and save health and even life. Corruption between the doctor and the patient can be limited by better organization of the health care system, including the financing of benefits and education of medical personnel and patients, as well as traditional legal measures, such as prevention or the application of criminal sanctions.

Patient–doctor communication, and the image of physicians and public health entitiesThe text is an attempt to analyse the problem of the image of health care system and its personnel mainly doctors in the context of communication. Author shows that the basic question in the context of doctor–patient relationship is the question of reliance and trust. Reliance is consequently the matter of communication, so the better communicative competences the doctor has, the bigger is the trust of patients form him/her.

Objectives Self-management of chronic diseases is rather complex, especially for patients with limited health literacy. In this review, we aim to disentangle the specific difficulties patients with limited health literacy face in relation to self-management and their associated needs with respect to self-management support. Methods We performed a literature search in five databases. We used a broad definition of health literacy and self-management was categorized into four types of activities: medical management, changing lifestyle, communicating and navigating through the health care system and coping. Included reviews described the relationship between health literacy and different domains of self-management and were published after 2010. Results A total of 28 reviews were included. Some clear difficulties of patients with limited health literacy emerged, predominantly in the area of medical management (especially adherence), communication and knowledge. Other associations between health literacy and self-management were inconclusive. Barriers from the patients’ perspective described mainly medical management and the communication and navigation of the health care system. Discussion Patients with limited health literacy experience difficulties with specific domains of self-management. For a better understanding of the relationship between health literacy and self-management, a broader conceptualization of health literacy is warranted, including both cognitive and behavioural aspects.

One vital goal of medical education is to promote the development of desirable professional qualities among future physicians, such as compassion, empathy, and humanism. Future physicians must finish their training prepared to meet the changing health needs of society, yet in reality many students graduate from medical school more cynical and less empathetic than when they began. During clinical clerkships, many students experience an “ethical erosion” as they transition in to real world clinical settings. Through innovative longitudinal integrated curricular designs focusing on continuity, medical students participate in the comprehensive care of patients over time and have continuous ongoing learning relationships with the responsible clinicians. As patients place increasing importance on the doctor-patient relationship, learning models that foster stronger connections between medical students and their patients, as well as with their teachers and communities, are needed in order to better prepare the next generation of physicians to serve a changing health care system.

Restrictions to health services in Poland have been an inspiration to establish Watch Health Care Foundation (WHC). The fundamental disease of the system is namely the disproportion between the amount of the funds and the contents of the package. It causes everywhere the same ’symptoms’ and leads to the same pathological phenomena: queues and other forms of rationing (’guaranteed’) health benefits, corruption, making use of privileges. Foundation uses the potential of information society and available infrastructure (web portal http://www.watchealthcare.eu) and all activities are presented on the website with the aim of influencing the health care system. On the basis of reports of limited access to health services, a ranking is created at WHC web portal, which aims to show what the biggest gaps in access to health services are - this is the way of showing the patient and health care system needs and also one possible approach of continuous education of the health care services consumers targeted at health care systems improvement.
