A Pretest – Posttest Pilot Study for the Development and Preliminary validation of a tool for the clinical assessment of Radioiodine Induced Sialadenitis

Abstract Background: The clinical assessment of radioiodine -induced sialadenitis is relied on the observer-defined toxicity grading model. However, this model has significant limitations, the major one being the lack of systematic assessment based on objective criteria. The main aim of this study was the development and testing of an assessment tool which could examine the severity of post irradiation sialadenitis.Methods: The development of the Sialadenitis Assessment Tool proceeded through three phases. The first and second phase included a literature review and the development of the tool which derived from the review, respectively. The third phase involved a pilot testing of the Assessment Tool to a sample of 34 patients undergoing I131. The assessment was carried out by two independent HCPs, pre- and post-radioiodine therapy. The results of the assessment tool were compared to other scales, including the DIRIX and EORTC H&N35.Results: The Cohen’s kappa test, suggested that the Sialadenitis Assessment Tool (SAT) is a reliable tool for the assessment of sialadenitis (Cohen’s K =1). The concurrent and internal validity tests, showed a tendency of association with most variables (p <0.001) in the DIRIX s and EORTC QLQ-HN35 scales.Conclusions: Preliminary evidence show that Sialadenitis Assessment Tool is a valid and reliable tool to assess radioiodine-induced sialadenitis in patients undergoing I131 therapy post thyroidectomy.

ZusammenfassungRegelmäßige körperliche Betätigung trägt zu einer gesunden Knochendichte bei, Leistungssportlerinnen in einigen Sportarten weisen jedoch eine erhöhte Prävalenz erniedrigter Knochendichte sowie vermehrt Stressfrakturen auf. Häufig liegt dem ein relatives Energiedefizit (RED), verbunden mit hoher mechanischer Belastung und intensivem Trainingsalltag, zugrunde. Das Vorliegen eines RED im Sport ist oft unterdiagnostiziert, daher hat sich eine systematische multidisziplinäre Zusammenarbeit unter Zuhilfenahme standardisierter Tests, z. B. des RED‑S CAT (Relative Energy Deficiency in Sport Clinical Assessment Tool) als hilfreich erwiesen. Therapieansätze zielen primär auf eine Behebung der negativen Energiebilanz der Patientin als auch auf psychotherapeutische Interventionen ab. Darüber hinaus kann in schweren Fällen eine medikamentöse Therapie zur Unterstützung sowohl einer ausgeglichenen hormonellen Situation als auch des Knochenstoffwechsels notwendig sein.

BACKGROUND Assessment of pain is critical to its optimal treatment. There is a high demand for accurate objective pain assessment for effectively optimizing pain management interventions. However, pain is a multivalent, dynamic, and ambiguous phenomenon that is difficult to quantify, particularly when the patient’s ability to communicate is limited. The criterion standard of pain intensity assessment is self-reporting. However, this unidimensional model is disparaged for its oversimplification and limited applicability in several vulnerable patient populations. Researchers have attempted to develop objective pain assessment tools through analysis of physiological pain indicators, such as electrocardiography, electromyography, photoplethysmography, and electrodermal activity. However, pain assessment by using only these signals can be unreliable, as various other factors alter these vital signs and the adaptation of vital signs to pain stimulation varies from person to person. Objective pain assessment using behavioral signs such as facial expressions has recently gained attention. OBJECTIVE Our objective is to further the development and research of a pain assessment tool for use with patients who are likely experiencing mild to moderate pain. We will collect observational data through wearable technologies, measuring facial electromyography, electrocardiography, photoplethysmography, and electrodermal activity. METHODS This protocol focuses on the second phase of a larger study of multimodal signal acquisition through facial muscle electrical activity, cardiac electrical activity, and electrodermal activity as indicators of pain and for building predictive models. We used state-of-the-art standard sensors to measure bioelectrical electromyographic signals and changes in heart rate, respiratory rate, and oxygen saturation. Based on the results, we further developed the pain assessment tool and reconstituted it with modern wearable sensors, devices, and algorithms. In this second phase, we will test the smart pain assessment tool in communicative patients after elective surgery in the recovery room. RESULTS Our human research protections application for institutional review board review was approved for this part of the study. We expect to have the pain assessment tool developed and available for further research in early 2021. Preliminary results will be ready for publication during fall 2020. CONCLUSIONS This study will help to further the development of and research on an objective pain assessment tool for monitoring patients likely experiencing mild to moderate pain. Aim: To verify, in the scientific production, the degree of reliability of the  Semmens-Weinstein  monofilament  as  a  risk  assessment  tool  for  diabetic  foot. Method:  This  is  an  integrative  literature  review  conducted  from  consultation  of  the electronic  databases  CINAHL,  MEDLINE,  SCOPUS  and  SCIELO.  Results:  Six  articles comprising  five  cross-sectional  studies  and  one  cohort  study  were selected.  The  six articles included in the review were taken from medical journals; no nursing publication was  found  that  met  the  goal.  Conclusion:  The  Semmens-Weinstein  monofilament  is  a reliable tool which has the best performance for assessing the risk for diabetic foot and its applicability is extremely important in consultations.

Purpose The clinical assessment of intelligibility must be based on a large repository and extensive variation of test materials, to render test stimuli unpredictable and thereby avoid expectancies and familiarity effects in the listeners. At the same time, it is essential that test materials are systematically controlled for factors influencing intelligibility. This study investigated the impact of lexical and articulatory characteristics of quasirandomly selected target words on intelligibility in a large sample of dysarthric speakers under clinical examination conditions. Method Using the clinical assessment tool KommPaS , a total of 2,700 sentence-embedded target words, quasirandomly drawn from a large corpus, were spoken by a group of 100 dysarthric patients and later transcribed by listeners recruited via online crowdsourcing. Transcription accuracy was analyzed for influences of lexical frequency, phonological neighborhood structure, articulatory complexity, lexical familiarity, word class, stimulus length, and embedding position. Classification and regression analyses were performed using random forests and generalized linear mixed models. Results Across all degrees of severity, target words with higher frequency, fewer and less frequent phonological neighbors, higher articulatory complexity, and higher lexical familiarity received significantly higher intelligibility scores. In addition, target words were more challenging sentence-initially than in medial or final position. Stimulus length had mixed effects; word length and word class had no effect. Conclusions In a large-scale clinical examination of intelligibility in speakers with dysarthria, several well-established influences of lexical and articulatory parameters could be replicated, and the roles of new factors were discussed. This study provides clues about how experimental rigor can be combined with clinical requirements in the diagnostics of communication impairment in patients with dysarthria.

In clinical nursing, students apply knowledge gained from classroom experience to real life situations. This educational model is experiential in nature, where patient learning experiences are selected based on the successful learning process. Hence, the competencies anticipated at a certain level are achieved in accordance with the objectives of knowledge. Purpose: This study was, therefore, aimed at developing a valid clinical assessment tool to guide the education system, and evaluate nursing students’ performance during practice in Indonesia. Methods: This research employed an exploratory sequential mixed method research design. Result: The transcripts generated from Focus Group Discussions and in-depth interviews showed three major areas of competence to be achieved while undergoing clinical practice. These ought to be combined with learning outcomes on the Competency-Based Curriculum in Indonesia, and subsequently reprocessed, to establish the final results, comprising 37 items. The study shows IN-CAT as acceptable (Scale-CVI= 0.9), indicating the agreement is adequate (Cohen’s kappa= 0.795), and satisfactory (The Cronbach’s alpha= 0.949). Conclusion: The results initially specify the scale as a reliable and valid measurement tool, with the potential for use in the assessment and evaluation of clinical competence amongst nursing students. Based on the feedback from teachers and students, The assessment tool demonstrates clarified learning objectives, improved focus, and enhanced the evaluation objectivity.
