Abstract Background: Preventing or reducing the attacks of acute otitis media (AOM) is a major worldwide healthcare problem in children. It is therefore urgent to evaluate the additional, unintended benefits of social segregation in cutting down the prevalence of AOM in children of various ages, especially during coronavirus disease 2019 (COVID-19) periods.Methods: Children aged from 6 months to 12 years who suffer from AOM and attended the outpatient clinic of tertiary hospital in the Eye Ear Nose Throat Hospital Affiliated to Fudan University in Shanghai were investigated from January 2015 to December 2020. The change in daily outpatient attendance for AOM and time-series analyses were estimated following both non-pandemic and COVID-19 period, and age and gender were taken into account. Results: 24543 outpatients (male:13529, female:11014) were enrolled in this clinic population. The epidemic trend of AOM in 2015-2019 was characterized by seasonal fluctuations, with a higher incidence in December and January but relatively lower outpatient records in winter and summer vacations (February, June and August). There was an estimated 63.6% (54.32%-70.36%) reduction in the daily outpatient attendance of AOM associated with the introduction of social segregation in COVID-19 period. When age was taken into account, children aged 4-6 who are in kindergarten were more vulnerable to AOM (66.2%), especially during the school period. Meanwhile, the outpatient attendance of AOM in preschoolers (before the age of three) was mainly in autumn and winter, with no significant relationship with semester change. And age structure of AOM in COVID-19 period broadly in line with that in non-pandemic period. Conclusions: Data from non-pandemic and COVID-19 periods indicated that AOM might spread into susceptible populations by close contact, and social segregation could effectively reduce the likelihood of catching AOM. In tandem with broader consequences, certain interpersonal distance, mask effects and good hand hygiene can significantly reduce the incidence of AOM, which provides a preventive and therapeutic point of view for AOM.

Objectives: To present the otologic findings of a patient with COVID-19 and complicated acute otitis media, evaluate for the presence of SARS-CoV-2 in middle ear fluid, and assess whether suctioning of middle ear fluid may be aerosol- generating. Methods: The case of a man with SARS-CoV-2 infection and complicated acute otitis media with facial paralysis is presented to illustrate unique clinical decisions made in context of the COVID-19 pandemic. A cadaveric temporal bone was used to simulate droplet spread during suctioning of fluorescein-labelled middle ear fluid and visualized with a blue-light filter. Results: A 23-year-old male who presented with complicated acute otitis media with facial paralysis was found to have an acute infection with SARS-CoV-2, with positive viral PCR of nasopharyngeal swab, and a negative PCR of the middle ear fluid. He was placed on isolation precautions and treated with myringotomy, topical and systemic antibiotics, and antivirals. Consistent with observations during endonasal suctioning, suctioning of middle ear fluid was not found to be aerosol or droplet generating. Conclusion: The case of a patient with active COVID-19 presenting with complicated acute otitis media in whom middle ear fluid was sampled to evaluate the etiology of the infection and the potential middle ear predilection of SARS-CoV-2 is described. This study has implications for the clinical management of patients with both known and unknown SARS-CoV-2 infection who present with ear disease. While middle ear suctioning may not be aerosol-generating, the risk of coughing or prolonged close contact requires heightened precautions during otologic procedures in patients with suspected or confirmed COVID-19.

AbstractObjective:To describe a relatively unknown clinical entity – inflammatory cast of the tympanic membrane after acute otitis media – and its simple out-patient treatment.Study design:Retrospective review of case series.Setting:Subspecialty practice at a tertiary hospital.Patients:Seven patients diagnosed previously with acute otitis media with perforation or otitis externa, and with persistent ear discomfort.Methods:Retrospective chart review.Results:The patients presented with weeks to months of persistent hearing loss after acute otitis media with perforation or acute otitis externa. Visits to their primary care physicians had been uninformative. After comparison of the affected and unaffected tympanic membranes, a thin, hard cast was identified and removed from the affected tympanic membrane. Improvement in hearing was documented in the three patients who underwent audiometric testing; the remainder had subjective improvement without audiometric evaluation.Conclusion:Otolaryngologists should be aware of the possibility of an inflammatory cast of the tympanic membrane following acute otitis media with perforation or otitis externa, and should carefully compare the unaffected and affected ears in such cases. Treatment – removal of the rigid cast – is both simple and effective.

Objectives: To determine the prevalence of complications of cochlear implant surgery in children with congenital profound sensorineural hearing loss. Methods: This study retrospectively & consecutively reviewed charts of children who underwent cochlear implantation from July 2015 to July 2019 at Cochlear Implant Centre of Otolaryngology Department of Capital Hospital, Islamabad Pakistan. These included cases of both genders aged one to 12 years operated at least one year before the time of data collection. Basic demographic data, complications including major and minor complications and treatment received was noted and statistically analyzed using SPSS-23. Results were presented using descriptive statistics. Results: Current study included a sample of N=251 having a mean age of 4.05±2.15 years including 154(61.4%) males and 97(38.6%) females revealed a prevalence of complications of 16(6.4%) with 4(1.6%) major and 12(4.8%) minor complications. Wound infection and acute otitis media with frequency of 3(1.2%) each were the commonest complications, followed by , facial nerve twitching, tinnitus and vertigo, infection and extrusion; and device failure in 2(0.8%) each. However, there was no significant association of complications with age group and gender with P=0.344 and P=0.519 respectively. Conclusion: Present public sector implant program is characterized with a very low prevalence of complications of 16(6.4%) with 4(1.6%) major and 12(4.8%) minor complications.
