Pediatricians face a problem of functional constipation in children and adolescents in one-third of cases of observation. Functional disorders of the gastrointestinal tract are based on disregulatory processes of the cortical-vegetative-endocrine complex. Magnesium deficiency is one of the factors influencing the formation of functional constipation in children, and an important link in the pathogenesis of the disease. Correction of magnesium level by means of magnesium-containing mineral water is one of the safe, effective and complex factors influencing a number of pathogenetic mechanisms of formation of functional constipation in children and teenagers.

The prevalence of celiac disease (CD) is increasing in the world. In Morocco, its incidence is also increasing considerably. The gluten-free diet (GFD) remains the only effective treatment so far. Its effectiveness depends essentially on good adherence to this diet. However, several factors hinder adherence by contributing to diet failure. Within this framework, this study aims to evaluate adherence to this diet in celiac children and adolescents continuing in a tertiary health center. The article also looks to identify the different factors influencing adherence to GFD in Morocco. The results show from serological and/or biopsy tests on 238 celiac patients that half had good adherence and 13% had poor adherence. The questionnaire aimed at 127 parents or proxies of celiac children and adolescents shows that several factors are responsible for the failure to adhere to the diet such as the role of the mass media, cultural and psychological factors. The exorbitant price, nutritional quality, and low availability of gluten-free products play a major role in GFD failure. Low socio-economic factors have a negative influence on the adherence to the GFD.

The purpose of the review is to acquaint general practitioners, therapists, gastroenterologists with the possibilities of diagnosing functional disorders of the gastrointestinal tract associated with constipation, as well as to draw attention to the need for additional examination methods for a comprehensive assessment of functional disorders of the gastrointestinal tract in patients with constipation on pathogenesis.Functional constipation is an urgent problem today due to its prevalence among different age groups, a negative impact on the quality of life, as well as the variability of the clinical forms of diseases associated with this symptom. The modern diagnostic algorithm for chronic constipation consists of several stages, including the identification of signs of the disease, according to the Rome criteria of the IV revision (2016), the identification of symptoms of “red flags” and laboratory and instrumental research. It should be borne in mind that functional constipation is often based on the pathology of the anorectal zone, which is an important factor in the diagnosis and development of further therapeutic strategies. The article provides an overview of modern and promising methods for diagnosing functional disorders of defecation, taking into account functional disorders of the anorectal zone associated with constipation.Functional methods such as anorectal manometry, rectal sensory test and balloon expulsion test are the  most studied and obligatory tests for an objective assessment of the sensorimotor function of the anorectal zone. In Russia, these methods are used very little, which requires extensive familiarization of practitioners with the advantages of these techniques. Knowledge and application of the presented examination algorithms will help the clinician to increase the overall diagnostic efficiency and apply the most justified tactics of managing patients with functional pathology of the digestive system associated with constipation.

Background. Functional constipation continues to be one of the urgent problems in the practice of a pediatrician and pediatric gastroenterologist. The frequency of constipation in children is about 2530%. A regular, painless bowel movement with a stool of soft consistency is possible only with a whole range of measures, including the use of laxatives. The possibility of using therapeutic magnesium-containing mineral water as an osmotic laxative in children and adolescents has not been adequately addressed in the literature. Aim: to study the effectiveness of the use of magnesium-containing mineral water in treatment and prevention of functional constipation in children and adolescents. Methods. The design was a randomized prospective, open, comparative study. The study included 95 children and adolescents aged 1016 years (average age 12.7 0.2 years) who were receiving sanatorium therapy with a diagnosis of functional constipation. Follow-up was performed for 18 days. The patients received basic therapy in the form of a sparing-exercise regimen, therapeutic nutrition, health-improving massage, and physiotherapy exercises. The subjects were divided into 2 groups by common randomization. The group 1 consisted of 55 patients who, along with basic treatment, took therapeutic mineral magnesium-containing water. The group 2 (control) consisted of 40 pediatric patients who received only basic treatment. Patient groups were comparable by disease pattern, gender, and age. The examination included a general medical examination, filling out special questionnaires reflecting the frequency and nature of the defecation; assessment of the vegetative status and functional reserves of the body on the Health Sources hardware complex. Results. In the study group, by the day 5 of therapy, daily defecation was achieved in 49.0% of patients; and by the end of the course of treatment, defecation regularity was registered in 89.1% of children. In the control group, the positive changes were less pronounced and amounted to 55% by the end of the treatment course (p 0.05). When taking mineral water, the nature of the defecation changed, namely in 89.1% of patients, it became of soft consistency with free painless bowel movement. When assessed according to the Bristol scale, the defecation corresponded to the types 3 and 4 with the initial type 1. In patients who did not receive a course of mineral water, during the entire follow-up period, no significant changes in the nature of defecation were detected, while the type 3 was noted in only 18% of patients by the end of the course of treatment. When taking mineral water, most patients of the main group developed a persistent reflex to morning defecation after ingestion of mineral water. Conclusion. Studies have shown that magnesium-containing mineral water is an effective, safe and easy to use means of correcting functional constipation in children and can be used both in outpatient practice and in sanatorium treatment.

The article is devoted to the influence of magnesium on the homeostasis of the body and, in particular, on the cardiovascular system. It describes the importance of the presence and effects of magnesium on various key processes and functions occurring in the body. The reasons for the lack of magnesium and ways to replenish it both in the natural way (eating, certain foods) and magnesium preparations are considered. The article provides examples of large randomized studies that prove the importance of the influence of normal magnesium levels on human health in general and on the state of the cardiovascular system. These studies show how magnesium deficiency increases the risk of cardiovascular diseases and how it can be reduced. It is also shown which trace elements and vitamins are closely related to magnesium metabolism, and how they (in particular, potassium and vitamin B6) improve and facilitate the normalization of magnesium levels. It is noted how comorbidity decreases with the normalization of magnesium level – the higher the magnesium level in the blood plasma (closer to the upper limit and more), the less comorbidity and longer life expectancy. Magnesium is an absolutely essential ion and a good medicine. Magnesium deficiency and hypomagnesemia are quite common, difficult to diagnose (due to underestimation and rare level control) and accompany many diseases of the cardiovascular system and beyond. The widespread use of organic magnesium salts would improve the situation as a whole, due to their universal multiple effect on many processes in the body. This is an integral part of therapeutic and preventive measures in patients with already existing diseases and in people who do not have diseases, but who are at risk due to existing hypomagnesemia.

Magnesium takes part in metabolic processes and is an indispensable element for the functioning of the nervous, cardiovascular, respiratory systems and the gastrointestinal tract. The article analyzes the role of magnesium in the life support processes of the body, presents to the attention of readers the possibility of diagnosing magnesium deficiency, and substantiates the need for magnesium supplement using mineral complexes in various pathological processes and in order to prevent magnesium deficiency.
