Objetivo: analisar os aspectos bioéticos intrínsecos de um serviço especializado no atendimento em transplante hepático em um hospital universitário da região Sul do Brasil. Método: pesquisa qualitativa exploratória, analítica, com coleta de dados realizada por entrevista semiestruturada com profissionais da equipe multiprofissional. A análise de dados deu-se por análise temática. Resultados: constatado que o paciente inserido no serviço de transplante hepático recebe atendimento diferenciado quando comparado aos demais pacientes atendidos na instituição. Conclusões: os atendimentos embasados na consolidada política pública dos transplantes de órgãos e os atrativos repasses financeiros aos procedimentos disparam a qualidade da assistência, em comparação aos atendimentos alicerçados em outras referências de remuneração.Descritores: Liver Transplantation. El análisis de datos se produjo mediante el análisis temático. Resultados: encontrado que el paciente ingresó en el servicio de trasplante de hígado recibe servicio diferenciado en comparación con el resto de los pacientes tratados en la institución. Conclusiones: la atención basada en la política pública consolidada de los trasplantes de órganos y transferencias financieras atractivas a los procedimientos, desencadenar la calidad de la atención en comparación con las llamadas a tierra en otras referencias de compensación.Descriptores: Trasplante de Hígado; Bioética; Políticas Públicas de Salud.

ABSTRACT Objective: to understand the challenges and strategies adopted by nurses for the exercise of Transformational Leadership in a university hospital. Methods: qualitative and exploratory study, in which 25 nurses working in a university hospital in the state of Bahia, Brazil participated. Data collection took place through semi-structured interviews and categorized according to Thematic Analysis, using Nvivo software. Results: The prevalent challenges involved: lack of encouragement from the institution for the training of leaders; professional inexperience and young age; resistance to leadership and insubordination. The strategies adopted by nurses consist of acting as team examples and establishing dialogic relationships. Final considerations: The practice of Transformational Leadership has been relevant in the daily lives of nurses and contributes to the quality of care.

Este trabalho objetivou identificar as percepções dos enfermeiros de um hospital universitário do sul do país acerca dos efeitos do trabalho noturno sobre a saúde e vida social. Trata-se de uma pesquisa qualitativa utilizando questionários e análise temática de Bardin. Participaram do estudo 16 enfermeiros. Conclui-se que o trabalho afeta diretamente o equilíbrio psíquico do trabalhador, uma vez que não há como dissociar a saúde psicológica da física, e por isso o trabalho noturno demanda uma reformulação geral de hábitos de vida.Descritores: Trabalho Noturno, Esgotamento Profissional, Saúde Ocupacional.Perception of the nurses on the effects of night work in their livesThis study aimed to identify the perceptions of nurses at a university hospital in southern Brazil, about the effects of night work on their health and social lives. ABSTRACT Objective: was to describe the flow of a specialist service in the care of liver transplant in a university hospital. Method: a qualitative research in the form of a case study, performed in a transplant service in southern Brazil. Data collection occurred from November 2013 to February 2014 through the triangulation of data, document analysis, structured interviews with 11 professional and semi direct observations interviews. Data analysis was performed by analysis of thematic content. Results: describes the flow of service and revealed the involvement of a multidisciplinary team in a cohesive manner, with competence recognized by patients and other sectors of the institution and structural deficiencies in care service for immunosuppressed patients. Conclusion: it was found that there is a need for studies that address the structures of care in liver transplantation services and to evaluate the impact of the quality of the life expectancy and proper recovery of persons undergoing liver transplantation.

<sec><title>OBJECTIVE:</title><p> To evaluate the acceptability of an educational project using A porcine model of airway for teaching surgical cricothyroidotomy to medical students and medical residents at a university hospital in southern Brazil.</p></sec><sec><title>METHODS:</title><p> we developed a teaching project using a porcine model for training in surgical cricothyroidotomy. Medical students and residents received lectures about this surgical technique and then held practical training with the model. After the procedure, all participants filled out a form about the importance of training in airway handling and the model used.</p></sec><sec><title>RESULTS:</title><p> There were 63 participants. The overall quality of the porcine model was estimated at 8.8, while the anatomical correlation between the model and the human anatomy received a mean score of 8.5. The model was unanimously approved and considered useful in teaching the procedure.</p></sec><sec><title>CONCLUSION:</title><p> the training of surgical cricothyroidotomy with a porcine model showed good acceptance among medical students and residents of this institution.</p></sec>

Background and Hypothesis: The impact of chronic liver diseases on patients and their family member is often understated and understudied. Chronic liver diseases can sometimes progress to a need for Liver transplant (LT). While recent studies have described quality of life (QOL) at different stages of liver disease, the impact of the patient’s QOL in LT survivors has not been examined. The importance of studying QOL in patients is due to its effect on the survivorship of LT recipients. We hypothesize that QOL in LT patients is lower than the general population. Our aim was to describe predictors of QOL in a well-described cohort of LT patients. Methods: Patients were enrolled at the Digestive and Liver Disease Liver clinic at Indiana University Hospital. All patients over the age of 18 were approached, if patients consented to the study, they were then enrolled during their liver follow up visit. The PROMIS survey was administered on an iPad and completed during the clinic visit. Survey were then scored and analyzed. Results: The T-scores for post liver transplant patients are lower in physical function, anxiety and depression, but higher in general life satisfaction compared to the general population. LT recipients have similar T-scores in Fatigue, Sleep disturbance, ability to participate in social activities, and pain interference compared to the general population. Conclusion and Potential Impact: Previous diagnosis of PBC, HCC, diagnosis of depression, household income, insurance status, Charlson Comorbid Index and number of non-transplant related medications have the highest association with quality of life. Further enrollment is needed to increase the power of the study. However, this can inform physicians the importance to taking these factors in to consideration in order to improve the QOL in LT recipients.

Nurse practitioners (NPs) provide medical care equivalent to that of physicians and facilitate access to healthcare. Although Japan’s first NP graduated in 2010, how Japanese expert NPs work effectively in clinical contexts is yet to be investigated. We aimed to identify the competencies that make expert NPs in Japan effective. Twelve Japanese expert NPs were purposely selected. The average age of the participants was 44.8 years, average NP experience was 7.5 years, and eight participants were women. Semi-structured interviews were conducted online from March to May 2021. Interviews were recorded, transcribed verbatim, and analyzed using thematic analysis. Thematic analysis revealed five themes: working in physicians’ contexts, interprofessional collaboration, involvement in nurses’ work, contribution to healthcare, and personal qualities for effective working. Japanese expert NPs can function effectively in clinical settings by flexibly and humbly collaborating with other medical professionals who have autonomous positions. They can improve the quality of healthcare by proposing practical solutions to problems faced by patients and medical organizations. These explored competencies can be applied to other aging and more complex societal contexts, and in updating the required competencies of Japanese NPs.

ABSTRACT Objective: To evaluate quality of life, religiosity and anxiety and depressive symptoms in liver transplant candidates. Method: An epidemiological and cross-sectional study carried out with liver transplant candidates attended at the outpatient clinic of a University Hospital from 2014 to 2016. Results: Fifty (50) patients with a mean age of 52.5 years old participated in the study, predominantly male (58.0%), having access to primary education (48.0%), Model for End-Stage Liver Disease between 10-19 and having viral hepatitis as the main etiology. They presented an average quality of life score (4.1), high intrinsic religiosity index (5.6) and the presence of anxiety (52.0%) and depressive symptoms (48.0%). It was possible to observe an association between religiosity and quality of life in the worry domain, with higher non-organizational religiosity leading to higher quality of life; anxiety and depressive symptoms were not associated with quality of life and religiosity. However, patients with higher levels of education were more likely to present depressive symptoms. Conclusion: The analysis of quality of life and religiosity was significant, reinforcing the need for the care team to consider religiosity as a coping strategy for the disease.

Objective: Successive reorganizations of healthcare system around the globe have placed enormous pressure on the work of nurse managers (NMs) and this has eroded their quality of work life (QWL). However, little is known about the meaning of NMs’ QWL.Aim: Inspired by Watson’s Human Caring Science perspective, this study aimed to describe and understand the meaning of QWL among NMs working in a affiliated-university hospital.Methods: A descriptive phenomenological method the Relational Caring Inquiry (RCI) was conducted to describe and understand the experiential meaning of QWL. This qualitative method was used to collect and analyze data from two semi-structured interviews with 14 NMs in an affiliated-university hospital in Quebec, Canada.Results: The results have identified the following five Eidos themes to describe and understand the experiential meaning of QWL: (1) actualizing leadership and political skills to improve the quality of nursing care; (2) contextual elements conducive to humanization of the organization; (3) organizational support promoting socioprofessional and personal fulfillment; (4) learning culture within the organization to encourage the development of nursing management skills; and (5) personalized support addressing the specific needs of new NMs. For NMs, the essence of the QWL experience is defined as a socioprofessional emancipation of NMs in their clinical-administrative practice in humanist organizations.Conclusions: Taking an organizational humanization perspective, the results reveal sustainable and practical potential strategies aimed at optimizing QWL implementation programs.

Abstract Objectives: Individuals with acquired brain injury (ABI) may present with challenging behaviours (CB) that place themselves and others at risk of harm and impact their community integration. It is crucial for community ABI therapists to successfully train in and implement behaviour interventions. The current study aimed to investigate community ABI therapists’ experiences of using, training in and implementing behaviour interventions. An additional aim was to determine these therapists’ understanding of Positive Behaviour Support (PBS), one approach to addressing CB with a focus on improving quality of life. Methods: Semi-structured interviews were conducted with 24 Australian community ABI therapists about their experiences of using, training in and implementing behaviour interventions and understanding of PBS. Inductive thematic analysis and content analysis were performed on interview transcripts. Findings: The thematic analysis resulted in the generation of six themes which described the difficulties participants faced in training in and delivering behaviour interventions and identified their training and implementation needs. The content analysis resulted in 10 categories that characterised participants’ understanding of PBS, which centred around the absence of consequences, a focus on antecedents, person-centred practice and encouraging prosocial alternatives to CB. Conclusions: The findings highlight a need and desire for more practical and interactive clinician training in behaviour interventions for individuals with ABI. Moreover, the findings suggest a limited understanding of PBS amongst community ABI therapists. Important considerations for the development of clinician training in ABI behaviour interventions and subsequent implementation into community practice are discussed.

Approximately 50% of autistic children experience high anxiety. Intolerance of uncertainty (tendency to react negatively to uncertain situations) is a key mechanism in the development and maintenance of anxiety. This study aimed to provide a descriptive, thematic analysis of the impact on uncertainty on the daily lives of autistic children and their families. Semi-structured interviews were conducted with parents and caregivers of 53 autistic children (including 3 dyads) aged 6–16 years to elicit examples of uncertain situations that the children found challenging and to explore the impact of these situations on family life. Thematic analysis revealed five overarching themes: child’s reactions to uncertainty, trying to reduce uncertainty, the impact of difficulties with uncertainty, the impact of uncertainty on parenting and the impact on parents. These findings provide a crucial understanding into the range of uncertain situations autistic children with anxiety experience. Anxiety-reduction approaches based on increasing tolerance to everyday uncertain situations could help improve well-being, quality of life and participation for autistic children and their families. Lay abstract Anxiety is common in autistic children. Research shows that this may be related to intolerance of uncertainty, which is a tendency to react negatively to uncertain situations. Understanding when, why and how autistic children respond to uncertainty is important in the development of anxiety programmes. We asked 53 (including 3 dyads) parents of autistic children about the types of uncertain situations that cause difficulties for their child and how uncertainty impacts on daily life for them and their families. We found that uncertain situations made autistic children and their families feel sad, worried, frustrated and angry through the themes: child’s reactions to uncertainty, trying to reduce uncertainty, the impact of difficulties with uncertainty, the impact of uncertainty on parenting and the impact on parents. There are lots of situations that are anxiety provoking for autistic children because of uncertainty, such as school. Programmes to reduce anxiety and increase autistic children’s ability to cope with everyday uncertain situations could improve quality of life for autistic children and their families.
