Turning is an attentionally demanding task for stroke survivors especially when it involves challenging environments. The aim of this study was to investigate turning characteristics under dual-task conditions (dual motor and dual cognitive) in stroke survivors and healthy controls. We compared the turning characteristics based on the following: (1) staggering, (2) number of steps, (3) time was taken, and (4) strategy used to complete a 180º turn. We found that stroke survivors presented with turning difficulty, especially under dual cognitive task. Forthcoming research is needed to investigate further the social impact of stroke survival in their current environment.© 2016. The Authors. This study examines the social impact of foreign immigrants on urban communities in Taman Taming Jaya, Selangor.The scope covers the overall social impact on the presence of foreign immigrants on urban communities,family relationship,the relationship among the local community, the relationship between locals and foreigners,the acceptance of foreigners by local residents,sense of belonging and the safety aspect.A total of 95 questionnaires survey were carried out among local respondents.Besides,direct observation was conducted to investigate the condition of the study area.The findings revealed that most of the respondents felt that the presence of foreign immigrants has negatively impacted their life. This study aimed to investigate the dual-task cost of both motor and cognitive performances in patients with multiple sclerosis (PwMS) and in healthy controls and to determine their relationships with clinical features in PwMS. The participants performed motor tasks (postural stability, walking, and manual dexterity) and cognitive tasks (mental tracking and verbal fluency) under single- and dual-task conditions. The results showed that postural stability under dual-task conditions did not change, whereas walking and manual dexterity deteriorated, regardless of the concurrent cognitive task, in PwMS (median Expanded Disability Status Scale score: 1) and the healthy controls. Verbal fluency decreased during postural stability, whereas it increased during walking, and it was maintained during manual dexterity in both groups. Mental tracking did not change during walking; it declined during manual dexterity in both groups. Mental tracking during postural stability deteriorated in PwMS, while it did not change in the healthy controls. In general, dual-task costs were associated with baseline performances of tasks rather than clinical features. Therefore, baseline performances of both tasks should be increased for improving dual-task performance in PwMS.

Background: Deficits in motor functioning, including walking, and in cognitive functions, including attention, are known to be prevalent in multiple sclerosis (MS), though little attention has been paid to how impairments in these areas of functioning interact. Objectives: This study investigated the effects of performing a concurrent cognitive task when walking in people with MS. Level of task demand was manipulated to investigate whether this affected level of dual-task decrement. Method: Eighteen participants with MS and 18 healthy controls took part. Participants completed walking and cognitive tasks under single- and dual-task conditions. Results: Compared to healthy controls, MS participants showed greater decrements in performance under dual-task conditions in cognitive task performance, walking speed and swing time variability. In the MS group, the degree of decrement under dual-task conditions was related to levels of fatigue, a measure of general cognitive functioning and self-reported everyday cognitive errors, but not to measures of disease severity or duration. Conclusions: Difficulty with walking and talking in MS may be a result of a divided attention deficit or of overloading of the working memory system, and further investigation is needed. We suggest that difficulty with walking and talking in MS may lead to practical problems in everyday life, including potentially increasing the risk of falls. Clinical tools to assess cognitive—motor dual-tasking ability are needed.

Patient education is the process of influencing patient behaviour with using a combination method such as teaching, counselling and behaviour modification techniques to increasing the knowledge and maintain or improve health. Patient education is one way of communication between patient and nurses, which provides the necessary knowledge for enhancing patient care and potentially affecting their health outcomes. By doing the exploration on the current patient education practice, the study perhaps to overcome the issue of hospital readmission and patient able to manage their activities of daily living after receiving effective patient education. The study aims to investigate the perception and practice of patient education among nurses in one of the public hospital in Malaysia. The objective of this study was to identify the perception and factors influencing the practice of patient education among public hospital nurses. Stratified Random Sampling was used for the sampling technique. A total of 246 staff nurses were involved in this study. A self-administered questionnaire was used in this study for obtained data. Statistical Package for the Social Sciences (SPSS) for Windows, version 21 was used to analyse the data. The study revealed that most of the nurses perceived patient education as a high priority in nursing care and it was nursing responsibility to provide patient education. Findings indicated that lack of time, inadequate staffing and lack of knowledge were significant factors that influenced the practice of patient education. Years of working experience among the nurses was found to be significantly associated with priority for patient teaching. In conclusion, public hospital nurses were having a positive attitude towards patient education but could not practice completely. In order to enhance patient education, a more strategy is required to address inadequate staffing, lack of time, and lack of knowledge. Abstract Background: The aim of this study was to evaluate the effects of cognitive and physical loads on dynamic and static balance of older adults under single, dual and multi-task conditions. Methods: The effects of single versus combined (dual-task and multi-task) cognitive (to speak out the name of the weekdays in a reverse order) and physical (with three levels including handling weights of 1kg, 2kg and 3kg in each hand) loads on dynamic and static balance of 42 older adults (21 males and 21 females), aged ≥ 60 years were studied. Dynamic and static balance measures were evaluated using the Timed Up and Go (TUG) and stabilometer (sway index) tests, respectively. Results: The TUG speed of female participants was generally slower than that of male participants. Cognitive task influenced the participants’ dynamic balance during the dual-task conditions, while the static balance was not affected in this phase. The dynamic and static balance measures were more influenced when performing the multi-tasks than when doing the dual-tasks. The effects of various levels of physical demand on the dynamic balance varied greatly under dual- and multi-task conditions. Conclusions: The findings add to the understanding of the factors influencing the elderly balance and control under cognitive and physical functioning.

Background. Individuals with multiple sclerosis (MS) experience deficits in motor and cognitive domains, resulting in impairment in dual-task walking ability. The goal of this study was to compare performance of forward walking and backward walking in single- and dual-task conditions in persons with MS to age- and sex-matched healthy controls. We also examined relationships between forward and backward walking to cognitive function, balance, and retrospective fall reports. Methods. All measures were collected in a single session. A 2×2×2 mixed model ANOVA was used to compare differences in forward and backward walking in single- and dual-task conditions between MS and healthy controls. Spearman correlations were used to examine relationships between gait and cognitive function, falls, and balance. Results. Eighteen individuals with relapsing-remitting MS and 14 age- and sex-matched healthy controls participated. Backward walking velocity revealed significant differences between groups for both single-task (p=0.015) and dual-task (p=0.014) conditions. Persons with MS demonstrated significant differences between single- and dual-task forward and backward walking velocities (p=0.023; p=0.004), whereas this difference was only apparent in the backward walking condition for healthy controls (p=0.004). In persons with MS, there were significant differences in double support time between single- and dual-task conditions in both backward (p<0.001) and forward (p=0.001) directions. More falls at six months were significantly associated with shorter backward dual-task stride length (r=−0.490; p=0.046) and slower velocity (r=−0.483; p=0.050). Conclusion. Differences in MS and age- and sex-matched healthy controls are more pronounced during backward compared to forward walking under single- and dual-task conditions. Future work with a larger sample size is needed to validate the clinical utility of backward walking and dual-task assessments and mitigate the limited sensitivity of the current dual-task assessments that primarily rely upon forward walking.

Abstract This study was designed to investigate the effects of anxiety and dual-task on reach and grasp motor control in chronic stroke survivors compared with age- and sex-matched healthy subjects (HC). Reach and grasp kinematic data of 68 participants (high-anxiety stroke (HA-stroke), n = 17; low-anxiety stroke (LA-stroke), n = 17; low-anxiety HC, n = 17; and high-anxiety HC, n = 17) were recorded under single- and dual-task conditions. Inefficient reach and grasp of stroke participants, especially HA-stroke were found compared with the control groups under single- and dual-task conditions as evidenced by longer movement time (MT), lower and earlier peak velocity (PV) as well as delayed and smaller hand opening. The effects of dual-task on reach and grasp kinematic measures were similar between HCs and stroke participants (i.e., increased MT, decreased PV that occurred earlier, and delayed and decreased hand opening), with greater effect in stroke groups than HCs, and in HA-stroke group than LA-stroke group. The results indicate that performing a well-learned upper limb movement with concurrent cognitive task leads to decreased efficiency of motor control in chronic stroke survivors compared with HCs. HA-stroke participants were more adversely affected by challenging dual-task conditions, underlying importance of assessing anxiety and designing effective interventions for it in chronic stroke survivors.

Everyday cognitive tasks are frequently performed under dual-task conditions alongside continuous sensorimotor coordinations (CSCs) such as driving, walking, or balancing. Observed interference in these dual-task settings is commonly attributed to demands on executive function or attentional resources, but the time course and reciprocity of interference are not well understood at the level of information-processing components. Here we used electrophysiology to study the detailed chronometry of dual-task interference between a visual oddball task and a continuous visuomanual tracking task. The oddball task’s electrophysiological components were linked to underlying cognitive processes, and the tracking task served as a proxy for the continuous cycle of state monitoring and adjustment inherent to CSCs. Dual-tasking interfered with the oddball task’s accuracy and attentional processes (attenuated P2 and P3b magnitude and parietal alpha-band event-related desynchronization), but errors in tracking due to dual-tasking accrued at a later timescale and only in trials in which the target stimulus appeared and its tally had to be incremented. Interference between cognitive tasks and CSCs can be asymmetric in terms of timing as well as affected information-processing components. NEW & NOTEWORTHY Interference between cognitive tasks and continuous sensorimotor coordination (CSC) has been widely reported, but this is the first demonstration that the cognitive operation that is impaired by concurrent CSC may not be the one that impairs the CSC. Also demonstrated is that interference between such tasks can be temporally asymmetric. The asynchronicity of this interference has significant implications for understanding and mitigating loss of mobility in old age, and for rehabilitation for neurological impairments.

Abstract Research Objective Dual-task performance, in which individuals complete two or more activities simultaneously, is impaired following mild traumatic brain injury. The aim of this project was to develop a dual-task paradigm that may be conducive to military utilization in evaluating cognitive-motor function in a standardized and scalable manner by leveraging mobile device technology. Methods Fifty healthy young adult civilians (18–24 years) completed four balance stances and a number discrimination task under single- and dual-task conditions. Postural stability was quantified using data gathered from iPad’s native accelerometer and gyroscope. Cognitive task difficulty was manipulated by presenting stimuli at 30, 60, or 90 per minute. Performance of cognitive and balance tasks was compared between single- and dual-task trials. Results Cognitive performance from single- to dual-task paradigms showed no significant main effect of balance condition or the interaction of condition by frequency. From single- to dual-task conditions, a significant difference in postural control was revealed in only one stance: tandem with eyes closed, in which a slight improvement in postural stability was observed under dual-task conditions. Conclusion The optimal dual-task paradigm to evaluate cognitive-motor performance with minimal floor and ceiling effects consists of tandem stance with eyes closed while stimuli are presented at a rate of one per second.

This paper focuses on the issue of vitality in urban public spaces, streets in particular, as a major indicator of their success and as one of the determinants of livable cities. The study first provides a theoretical framework for understanding the social value and role of urban public spaces in quality of urban life. Second, it discusses essential components of vitality in streets. Finally, it investigates vitality and its determinants in two main streets in Famagusta and Kyrenia, in North Cyprus. The paper highlights some issues such as proper connection and proximity to important magnets, along with physical and social attributes in the street area.
