The Impact of Cognitively Impaired Patients and Shift on Nursing Assistant Stress

This study compared the stress experienced by nursing assistants (NAs) under four work conditions: high or low proportion of cognitively impaired patients and day or other shift. Five standard measures of caregiver stress served as the dependent variables in this study: burden, reaction to patient behaviors, workload, and two measures of burnout. A 2 × 2 multivariate analysis of variance found an interaction effect of type and shift on the stress measures. Univariate tests found that Burden and Depersonalization accounted for this effect. A further multivariate analysis of simple main effects found significant differences for each independent variable within each level of the other independent variable. Univariate analyses found that NAs who care for cognitively impaired patients on the day shift show significantly higher scores on specific stress measures. The article concludes with a discussion of how institutions can respond to the stresses faced by NAs who care for cognitively impaired patients.

The purpose of this paper is to investigate the impact of capital structure decisions on the performance of the firm. The investigation has been performed using a data of 62 listed non-financial Egyptian firms over a period of fourteen years from 2003-2016. This study used two measures for performance the dependent variable which are ROA and ROE, the most common used measures agreed upon on the majority of previous studies. Whereas, for the independent variable “the capital structure, the study uses the three measures of capital structure which are total debt to total assets (TD), total short-term debt to total assets (STD), and total long-term debt to total assets (LTD). The results showed when using ROA as a measure of performance, a significant negative impact of capital structure (TD, STD, and LTD) exists; while in case of using ROE as a measure of performance, there’s a significant negative impact of capital structure only when using STD, otherwise a positive significant impact of capital structure exist.

Background: In 2016 veterinary nursing assistants (VNAs) were introduced as an additional tier to New Zealand veterinary practice. Aim: This study explores the utilisation of VNAs in New Zealand veterinary practices to ascertain the impact of an additional staffing layer to patient outcomes, workload management and staff wellness. Method: Through focus groups and semi-structured interviews with 30 participants, three themes emerged allowing evaluation of the Allied Veterinary Professionals Regulatory Council (AVPRC) Scope of Practice (SP) (AVPRC, 2020) and development of delegation guidelines (DG). Results: Analysis identified weak processes in delegation. The practice-based perspectives of VNA staff utilisation supports the AVPRC SP. Conclusion: Effective communication of the SP and DG for veterinary practice utilisation could contribute to reducing workload pressure. Additionally, individual practice staff discussions regarding own and colleague job expectations, along with review of contractual job descriptions, could further evolution of multi-tiered practices leading to improved patient outcomes, team wellness and business success.

This chapter extends the principles of bivariate analysis to multivariate analysis, which takes into account more than one independent variable and the dependent variable. With multivariate analysis, it is possible to investigate the impact of multiple factors on a dependent variable of interest, and to compare the explanatory power of rival hypotheses. Multivariate analysis can also be used to develop and test multi-causal explanations of political phenomena. After providing an overview of the principles of multivariate analysis, and the different types of analytical question to which they can be applied, the chapter shows how multivariate analysis is carried out for statistical control purposes. More specifically, it explains the use of ordinary least squares (OLS) regression and logistic regression, the latter of which builds on cross-tabulation, to carry out multivariate analysis. It also discusses the use of multivariate analysis to debunk spurious relationships and to illustrate indirect causality.

The use of nursing assistants has increased across health systems in the past 20 years, to alleviate licensed nurses' workload and to meet rising health care demands at lower costs. Evidence suggests that, when used as a substitute for licensed nurses, assistants are associated with poorer patient and nurse outcomes. Our multimethods study evaluated the impact of a policy to add nursing assistants to existing nurse staffing in Western Australia's public hospitals, on a range of outcomes. In this article, we draw the metainferences from previously published quantitative data and unpublished qualitative interview data. A longitudinal analysis of patient records found significantly higher rates adverse patient outcomes on wards that introduced nursing assistants compared with wards that did not. These findings are explained with ward-level data that show nursing assistants were added to wards with preexisting workload and staffing problems and that those problems persisted despite the additional resources. There were also problems integrating assistants into the nursing team, due to ad hoc role assignments and variability in assistants' knowledge and skills. The disconnect between policy intention and outcomes reflects a top-down approach to role implementation where assistants were presented as a solution to nurses' workload problems, without an understanding of the causes of those problems. We conclude that policy makers and managers must better understand individual care environments to ensure any new roles are properly tailored to patient and staff needs. Further, standardized training and accreditation for nursing assistant roles would reduce the supervisory burden on licensed nurses.

Background: Fever on presentation has been demonstrated to be an independent predictor of poor-outcome after ICH. However, prospective data with post-discharge outcomes are limited. Methods: Associations between fever (≥38°C) on presentation and 3-month outcomes were sought among patients in ERICH, a multicenter, triethnic case-control study of ICH. Patients with missing CT or Glasgow Coma Score (GCS) and those lost to 3-month follow-up were excluded. Rates of fever on presentation were compared across admission characteristics, hospital complications, and 3-month outcomes. Additionally, a multivariate analysis was performed to determine factors associated with fever on presentation. Results: A total of 1644 patients out of 2276 enrolled by 12/31/2013 met inclusion criteria for the analysis. Fever on presentation with ICH was present in 2.4% of patients. In univariate analyses, fever was associated with (all data are % febrile in each group): ICH volume (<15mL: 1.7% , 15-30mL: 2.3%, 30-45mL: 4.7%, 45-60: 6.6%, >60: 2.9%, p=0.03), higher ED GCS (3-4: 1.8%, 5-12: 3.0% 13-15: 6.6%, p=0.004), and WBC > 10 (yes, 3.4% vs. no, 1.7% p=0.03). Additionally, borderline significance was seen with race (white 3.1%, black 2.9%, Hispanic 1.1%, p=0.06) and ICH Location (lobar 3.9%, primary IVH 3.8%, Deep 1.6%, Brainstem 2.2%, cerebellum 1.7%, p=0.06). Fever on presentation was not associated with 3-month outcomes (mRS 0-2: 2.1%;3-5:1.9%; 6: 3.2%, p=0.45). In a multivariate analysis, fever was associated with GCS (OR 0.9, 95% CI 0.83-0.97) and white or black race (reference Hispanic; white OR 3.0 95% CI 1.14-7.88; Black OR 2.68, 95% CI 1.04-6.96). Conclusion: Fever is relatively uncommon at admission and does not appear to be associated with poor outcomes.

The effects of the employment relationship (superior vs coworker), recipient's response (positive vs negative), and sex of the rater (male vs female) on perceptions of sexual harassment, appropriateness of the initiator's behavior, and appropriateness of the recipient's behavior were investigated. Subjects were 82 female and 76 male undergraduate students. The results of a 2 × 2 × 2 multivariate analysis of variance indicated main effects for response of the recipient and sex of the rater. Univariate analyses indicated that women rated the initiator less favorably than men, that men perceived the situation as more harassing and approved of the recipient more than women when the recipient's responses were positive, although there was no difference between men and women when the recipient's responses were negative.

AbstractWe reviewed research that examines racism as an independent variable and one or more health outcomes as dependent variables in Black American adults aged 50 years and older in the USA. Of the 43 studies we reviewed, most measured perceived interpersonal racism, perceived institutional racism, or residential segregation. The only two measures of structural racism were birth and residence in a “Jim Crow state.” Fourteen studies found associations between racism and mental health outcomes, five with cardiovascular outcomes, seven with cognition, two with physical function, two with telomere length, and five with general health/other health outcomes. Ten studies found no significant associations in older Black adults. All but six of the studies were cross-sectional. Research to understand the extent of structural and multilevel racism as a social determinant of health and the impact on older adults specifically is needed. Improved measurement tools could help address this gap in science.

Subject. This article explores the issues of business tourism clustering in Greater Moscow. Objectives. The article intends to justify the need to create a business tourism cluster in Greater Moscow to improve the investment climate in the region. Methods. For the study, I used a multivariate analysis, forecasting, and extrapolation. Results. The article shows a certain relationship between the efficient functioning of the business tourism cluster and the economy's development. Conclusions and Relevance. Certain types of tourist clusters can serve as platforms for attracting investors and implementing marketing plans. The business tourism cluster is a link between buyers and sellers in various industries. The results of the study can be used to improve the effectiveness of the cluster initiative in business tourism, as well as find ways of cooperation between the State and private investors when creating the business tourism cluster in Greater Moscow.

This study dealt with the effect of foreign direct investment on the market value of the company during the period of time (2010-2017). This issue was studied through a sample of oil fields in southern Iraq in which the company operates within the first and second licensing contracts rounds and according to the circumstances and variables of the investment environment as it is. Although this investment often achieves high returns, it is also characterized by a high degree of risk and for the purpose of evaluating the impact of foreign direct investment on the market value of the company's stock prices for the period (2010-2017). The statistical scale (T-TEST) was used to indicate the significance of the correlation hypotheses. Between the return on investment as the independent variable and the market value as the dependent variable, and the use of the coefficient of determination (R2) that measures the effect of the independent variable (foreign direct investment) on the dependent variable (market value) and the F-Test to demonstrate acceptance or rejection of the hypothesis of the return on investing in the market value of the oil company, and if the company achieves a high return in foreign direct investment, the market value of it will be affected positively. The study was based on a set of goals, including determining the attractiveness of Iraq to foreign investments, especially the oil sector, and the study reached a number of conclusions, the most prominent of which is the existence of a strong inverse correlation between the return on investment and the market value of the company. And the existence of a slight impact of the return on investment on the market value of the company, and the study reached a number of recommendations, the most important of which is activating the investment climate through political stability and the clarity and stability of laws and legislation regulating investment, which is one of the most important factors affecting the investment decision.
