To measure nurses’ rankings of their electronic medical records (EMRs) on their job satisfaction over time, a retrospective analysis of a set of cross sectional data from a survey conducted by the United States’ California Registered Nursing Board in 2008, 2010, 2012, 2014, and 2016. Approximately 4,500 nurses ranked the usefulness of their EMRs in each of the five years.The EMR rankings increased steadily between 2008 and 2016 but the changes are small and the rates of change are very slow, suggesting that the problems with EMRs have been difficult to solve. The results show EMRs have a large impact: a one category increase in EMR rankings increased job satisfaction by as much as or slightly more than one-third for hospital and non-hospital nurses.The size of the effects and their persistence over eight years imply a substantial loss from poorly designed EMRs, and one which could have been avoided had EMR designs more closely matched nurses’ day to day work. The reductions in job satisfaction and potential effects on burnout are losses to be added to the more widely measured losses in productivity and negative effects of EMRs on patient-provider relationships.

Employee shirking, where workers give less than full effort on the job, has typically been investigated as a construct subject to organization-level influences. Neglected are individual differences that could explain why employees in the same organization or work-group might shirk. Using a sample of workers from the health care profession in the United States, the present study sought to address these limitations by investigating subjective well-being (a dispositional construct), job satisfaction, as well as other indiuidual-level determinants of shirking. Results indicate that whites shirk significantly more than nonwhites, and that subjective well-being, job satisfaction, and age have significant, negative effects on shirking. The implications of these results are discussed.

Abstract Background: Wide adoption of electronic medical records (EMR) systems in the United States can lead to better quality medical care at a lower cost. Despite the laws and financial subsidies by the U.S. government for service providers and suppliers, the adoption has been slow. Understanding the EMR adoption drivers for physicians and the role of policymaking can translate into increased adoption rate and enhanced information sharing between medical care providers. Methods: Physicians across the United States were surveyed to gather primary data on their psychological, social, and technical perceptions toward EMR systems. This quantitative study builds on the Theory of Planned Behavior, the Technology Acceptance Model, and the Diffusion of Innovation theory to propose, test, and validate an innovation adoption model for the health care industry. 382 responses were collected and data were analyzed via linear regression to uncover the effects of 12 variables on the intention to adopt EMR systems.Results: Regression model testing uncovers that government policymaking or mandates and other social factors have little or negligible effect on physicians’ intention to adopt an innovation. Rather, physicians are directly driven by their attitudes and ability to control, and indirectly motivated by their knowledge of the innovation, the financial ability to acquire the system, the holistic benefits to their industry, and the relative advancement of the system compared to others.Conclusions: A unidirectional mandate from the government is not sufficient for physicians to adopt an innovation. Government, health care associations, and EMR system vendors can benefit from our findings by working toward increasing the physicians’ knowledge of the proposed innovation, socializing how medical care providers and the overall industry can benefit from EMR system adoption, and solving for the financial burden of system implementation and sustainment.

The analysis of markets, courts, and civil litigation on the northeastern frontier of the United States provides a valuable opportunity to assess the evolution of institutions during economic development. The data set pools longitudinal and cross-sectional observations on 30,000 lawsuits filed in Maine during the critical period between 1700 and 1860. The earliest legal institutions moderated both social and economic norms, but courts quickly began to specialize in commercial issues. The residence of debtors and creditors and changes in spatial characteristics over time yield insights into the nature and extent of capital markets and impersonal exchange. The distribution and disposition of property and debt cases indicate that early markets were well developed and orderly; the evidence of “social tension” between debtors and creditors was minimal. The results do not support the standard claim of a transition from interactions based on community norms to impersonal market exchange late in the eighteenth century.

OBJECTIVES/SPECIFIC AIMS: Acculturation and discrimination are associated with negative health outcomes including functional disability. The effect of these on functional disability in older African immigrants in the United States is not well understood. The purpose of this study is to describe and examine the experiences of acculturation, racial discrimination and functional disability in older African immigrants. METHODS/STUDY POPULATION: This study will use cross-sectional quantitative and qualitative mixed methods design. We plan to recruit 150 older (>55 years) participants through the African Immigrant Health Study and community-based organizations serving African immigrants. We will use the PROMIS physical function measure to assess functional disability, the Everyday Discrimination scale and the Psychological Acculturation scale will be used to measure discrimination and acculturation respectively. Higher scores indicate greater severity. RESULTS/ANTICIPATED RESULTS: We have recruited 12 participants so far. The mean age is 57 years and mean length of stay in the United States is 23 years. Mean disability score is 6.5 (range 1–38). Mean discrimination is 8.2 (range 4–15). The prevalent acculturation strategy of these participants (7) is marginalization (neither identified with the American nor African cultures). DISCUSSION/SIGNIFICANCE OF IMPACT: Preliminary results indicate pervasive discrimination and marginalization of study participants. Exploring these experiences can inform preventive strategies of coping and health behaviors that can decrease the negative effects of discrimination, acculturation and functional disability in African immigrants.
