Objective: The aim of our study was to assess the comparative patient satisfaction and side-effects of the currently prescribed drugs – tamsulosin and silodosin for benign prostatic hyperplasia (BPH). Methods: A prospective study was conducted in a total of 110 BPH patients from the Department of Urology for a period of 6 months. Fifty-five patients in each group received silodosin 8 mg or tamsulosin 0.4 mg once daily. Data were collected using a suitably designed pro forma and the patient satisfaction was assessed with patient’s perception of study medication (PPSM) scale. International prostate symptom score (IPSS) was used for assessing the severity of symptoms. Results: The current study found that the treatment had a significant effect on improving scores of PPSM and IPSS at which all changes were significant at p<0.01 (paired t-test). An independent t-test showed that silodosin group had a greater improvement in PPSM scores – PPSM total by 40.4%, PPSM global by 43.7%, and PPSM pain by 0.39% which was supported by corresponding decline in IPSS scores. The side effects reported for tamsulosin were headache (5.5%), dizziness (5.5%), dry mouth (3.6%), and postural hypotension (14.4%) and those reported for silodosin were myalgia (5.5%), dizziness (7.3%), diarrhea (1.8%), and postural hypotension (10.9%). Conclusion: Patient satisfaction was improved by both the alpha blockers but silodosin showed a significantly greater increase in patient satisfaction than tamsulosin. Thus, silodosin is the better drug of choice.

Objective: The present study was aimed to analyze the side effects of olmesartan medoxomil and enalapril in hypertensive subjects. Methods: The study consisted of newly diagnosed hypertension categorized according to 7th report of Joint National Commission on prevention, detection, evaluation, and treatment of high blood pressure. The subjects were divided into two groups. The Group A subjects received olmesartan, and Group B subjects received enalapril. Pressure was recorded both in supine and sitting positions. The appearance of side effects was observed in the follow-up, i.e., dry cough, headache, postural hypotension, angioedema, dizziness, skin rashes, taste alterations, and urticaria. A statistical data were prepared on the basis of information obtained and analyzed thoroughly for antihypertensive effects and side effects of olmesartan and enalapril. SPSS software was used for analysis. Results: There was observed an increase in the incidence of taste alteration with drug therapy in Group B (Enalapril). There was observed an increase in the incidence of postural hypotension with drug therapy in both groups. In Group A (Olmesartan), the incidence of postural hypotension at the beginning of trial, 4 weeks, and 8 weeks was 0%, 2%, and 2%, respectively. In Group A (Olmesartan), there was no incidence of a headache at the beginning of trial, at 4 weeks and 8 weeks. Conclusion: From the study, it can be concluded that both olmesartan and enalapril are effective in Stage I and Stage II hypertension, but olmesartan is tolerated well with lesser side effects.

Background: Patients who are refractory to initial management of trochlear dysplasia, which consists of bracing and physical therapy, may be candidates for trochleoplasty. Indications: Indications for trochleoplasty include Dejour classification type B or D trochlear dysplasia, supratrochlear spur height ≥7 mm, and a positive J sign on examination. Technique Description: The thick shell sulcus-deepening trochleoplasty technique involves removing the supratrochlear spur by creating a 5-mm–thick osteochondral shell with underlying cavity, dividing the thick shell into medial and lateral leaflets, and securing the leaflets with absorbable sutures. This establishes a new trochlear groove that is flush with the anterior cortex of the femur. Results: A review of 21 studies with length of follow-up from 8.8 months to 15 years found postoperative dislocation was present in 0% to 15% of patients and the patient satisfaction ranged from 81.0% to 94.4%. A prospective study with a minimum 2-year follow-up found no cases of recurrent dislocation, no progression of radiographic arthritis, 84.8% of patients returned to sport, and the patient satisfaction was a 9.1/10. Discussion/Conclusion: Sulcus-deepening trochleoplasty for trochlear dysplasia provides patellar stability and excellent patient satisfaction.

Abstract Introduction Surgeons must establish and maintain effective relationships with patients and, where appropriate, with their supporters. Before surgery, surgeons should strive to have an honest and sensitive discussion with patients about their options for treatment that leads to informed and deliberate consent. The aim of this study is to compare our consent practice in the vascular department in Royal Stoke Teaching Hospital against consent guideline in the Best surgical practice and to if we are meeting patient satisfaction. Method A prospective study of 30 patients from 21/09/2020 to 21/10/2020. Questionnaire form was designed based on the best surgical practice guideline and was given to the patient to fill. We include patients admitted for elective vascular procedures, wide range ages. The questionnaire Results Most of the of the patient (90%) were consented by the professionals who provide the treatment. 63% of the patients asked for further information in the form pf leaflets or websites. 70% of them request to know the recent guidelines of their procedures. Only 13% of them had a copy of the consent prior to surgery. Recommendations We must provide the patients with further information, Recent guidelines, and a copy of the consent to take home prior to surgery day. This can be achieved by consenting them before the operation day.
