Background: In veterinary surgical operations for cats and dogs, inhalation anaesthesia is known to be a good option for general anaesthesia in long operations or emergency cases. Studies have revealed that during inhalation anaesthesia, the heart and respiratory functions of the animals are more stable compared to injectable anaesthesia. However, there are few studies performed with the use of inhalation anaesthesia in ruminants. In this research, the goal is to evaluate the effects of isoflurane and sevoflurane after induction with propofol in calves based on clinical and physiological parameters and to find a more reliable strategy for general anaesthesia in ruminants.Materials, Methods & Results: The research was carried out on 30 calves, from new-born up to 3-months-old, undergoing surgery operation. Each group consisted of 15 animals that were divided as isoflurane and sevoflurane. For premedication, atropine was administered at a dose of 0.04 mg/kgvia subcutaneous injection in calves. For induction, 15 min after atropine application, propofol was given at 5-6 mg/kgvia intravenous infusion. Then, endotracheal intubation was performed and inhalation anaesthesia began at 5 min after induction. In the isoflurane group, the onset of anaesthesia concentration was set to 3-5% and the maintenance was set to a concentration of 1.5-3%. In the sevoflurane group, the onset of anaesthesia concentration was set to 5-7% and the maintenance was set to a concentration of 2.5-4%. To monitor the calves, the heart rate (HR), the pulse rate (PR), the systolic blood pressure (SBP), diastolic blood pressure (DBP), the respiratory rate (RR), the rectal temperature (RT) and the electrocardiogram (ECG) measurements were recorded before anaesthesia, the premedication, the induction periods, at 5, 15, 30, 45, 60, and 75 min after the onset of inhalation anaesthesia during the operation period. Blood samples were taken before anaesthesia, premedication, induction periods, at 30 and 75 min during the operation and the red blood cell (RBC) count, white blood cell (WBC) count, the amount of haemoglobin (HGB), the haematocrit concentration (HCT), and the platelet (PLT) count were evaluated. Based on the findings, isoflurane and sevoflurane suppressed the cardiovascular system minimally. Both anaesthetic agents caused decreases that did not exceed the physiological limits compared to the measurements taken before the initiation of anaesthesia. Although no significant differences were detected between the groups (P > 0.05) at clinical parameters, the RT differences were regarded as statistically significant according to the measurements that were taken before anaesthesia at all measurement times (P < 0.05). In both groups, the changes that occurred in the P wave, the PR interval, and in the QRS, R and T waves remained within the reference values given in the literature. Both anaesthetic agents have similar effects on the ECG. The dramatic no changes throughout anaesthesia in terms of the amount of HGB and HCT, the RBC, the WBC, and the PLT were not statistically significant, which may be associated with good tissue perfusion.Discussion: Since no serious complications occurred with either anaesthetic option, we have concluded that isoflurane and sevoflurane have similar effects on the cardiopulmonary systems of calves. In addition, the effects of propofol, which is used for induction, have been evaluated and observed and this drug is an effective and reliable option to initiate inhalation anaesthesia. The research on the use of this anaesthetic in calves is limited and most of the existing studies investigated its combination with injectable anaesthetics. As a result, in this research, we concluded that isoflurane and sevoflurane induce similar physiological measurement results, blood parameters, and ECG results and that both anaesthetics cause minimal complications.

Introduction: Propofol (Diprivan®) is now a mainstay drug during anaesthesia in human and small animal surgeries and this is as a result of its good quality anaesthesia, rapid onset, and short duration of action with rapid recoveries. There is paucity of information in Nigeria as per the use of general anaesthesia to carry out procedures in pregnant red Sokoto Does. The aim of this study is to assess the haematological profile of pregnant red Sokoto doe following propofol anaesthesia and also ascertain the safety of the drug during gestation. Methods: Ten apparently healthy pregnant Red Sokoto does were used for this study. The animals were acclimatised for two weeks and pregnancy was diagnosed using B-mode Real time® transcutaneous ultrasonography. The animals were grouped into two (A and B) comprising five animals each in their first and second trimesters respectively. Propofol at 4 mg/kg and 5 mg/kg was administered to the does in groups A and B respectively. Blood samples were collected via the jugular vein and analysed using full automated blood counter (Ehmma® PCE 210). Results: The result showed that the onset and duration of propofol anaesthesia at the two doses in both groups were smooth and rapid without much excitation. There were no significant (p < 0.05) differences in the blood parameters in both group when compared with the control. Significance: Our study lends credence to the fact that propofol anaesthesia is suitable and safe anaesthetic agents for induction and maintenance of general anaesthesia in pregnant red Sokoto does.

Traumatic brain injuries (TBI) represent a high impact public health problem due to a high rate of death , long term disability and occurrence especially in young adults. Despite several promising animal studies, several parameters were proposed as biological markers and were assessed for this aim. Our study proposes the study of the early biochemical changes in association to hematological parameters for severe TBI patients prognosis. 43 patients with acute TBI were included in study based on clinical, laboratory and imagistic findings. The severity of the TBI was established by Glasgow Coma Scale GCS 3-8. In all patients were evaluated hematologic parameters (Red blood cell count - RBC, Hematocrit, blood Hemoglobin, White blood cell - WBC, Platelet count and biochemical parameters (glucose, urea, creatinine, electrolytes). Outcome was expressed as Glasgow Outcome Scale (GOS), between 1-5. Values were compared to control group -15 cases. Significant early differences in body temperature, heart rate, and systolic blood pressure were observed in TBI group versus control (p[0.05). After correlation, laboratory findings significantly associated to severe outcome - GOS = 1, 2 - (p[0.05) were plasma Na decrease and significant glucose increase. An early increase of temperature and decrease of Na may predict a severe outcome in patients with acute TBI; association with shifts in heart rate and blood pressure, imposes aggressive treatment measures.

AbstractConventional blood pressure (BP) measurement methods have different drawbacks such as being invasive, cuff-based or requiring manual operations. There is significant interest in the development of non-invasive, cuff-less and continual BP measurement based on physiological measurement. However, in these methods, extracting features from signals is challenging in the presence of noise or signal distortion. When using machine learning, errors in feature extraction result in errors in BP estimation, therefore, this study explores the use of raw signals as a direct input to a deep learning model. To enable comparison with the traditional machine learning models which use features from the photoplethysmogram and electrocardiogram, a hybrid deep learning model that utilises both raw signals and physical characteristics (age, height, weight and gender) is developed. This hybrid model performs best in terms of both diastolic BP (DBP) and systolic BP (SBP) with the mean absolute error being 3.23 ± 4.75 mmHg and 4.43 ± 6.09 mmHg respectively. DBP and SBP meet the Grade A and Grade B performance requirements of the British Hypertension Society respectively.

Background: Psoriasis is associated with accelerated non-calcified coronary plaque burden (NCB) by coronary computed tomography angiography (CCTA). Machine learning (ML) algorithms have been shown to effectively identify cardiometabolic variables with NCB in cross-sectional analysis. Objective: To use ML methods to characterize important predictors of change in NCB by CCTA in psoriasis over 1-year of observation. Methods: The analysis included 182 consecutive patients with 80 available variables from the Psoriasis Atherosclerosis Cardiometabolic Initiative, a prospective, observational cohort study at baseline and 1-year using the random forest regression algorithm. NCB was assessed at baseline and 1-year from CCTA. Results: Using ML, we identified variables of high importance in the context of predicting changes in NCB. For the cohort that worsened NCB (n = 102), top baseline variables were cholesterol (total and HDL), white blood cell count, psoriasis area severity index score, and diastolic blood pressure. Top predictors of 1-year change were change in visceral adiposity, white blood cell count, total cholesterol, c-reactive protein, and absolute lymphocyte count. For the cohort that improved NCB (n = 80), the top baseline variables were HDL cholesterol related including apolipoprotein A1, basophil count, and psoriasis area severity index score, and top predictors of 1-year change were change in apoA, apoB, and systolic blood pressure. Conclusion: ML methods ranked predictors of progression and regression of NCB in psoriasis over 1 year providing strong evidence to focus on treating LDL, blood pressure, and obesity; as well as the importance of controlling cutaneous disease in psoriasis.

A transport and monitoring unit for fully anaesthetized piglets (weight 20-30 kg) was made. The unit provided easy transport for short or longer distances while at the same time making continuous monitoring of invasive blood pressure (IBP), temperature and HR possible. The animals stayed in the unit for the duration of the experiments which required that the anaesthetized animal be kept in exactly the same position for several hours. This, as well as the experimental animal's welfare, was ensured by the unit. Such a unit can be used for any lengthy transportation of experimental animals to research facilities located separately from animal laboratories.

Intravenous anaesthesia is gradually becoming popular in veterinary practice. Traditionally, general anaesthesia is induced with intravenous drugs and then maintained with inhalation agents. Inhalation anaesthetic agents cause more significant dose-dependent cardiorespiratory depression than intravenous anaesthetic drugs, creating a need to use less of the inhalation anaesthetic agents for maintenance of general anaesthesia by supplementing with intravenous anaesthesia drugs. Better still, if anaesthesia is maintained completely with intravenous anaesthetic drugs, autonomic functions remain more stable intra-operatively. Patient recovery from anaesthesia is smoother and there is less pollution of the working environment than happens with inhalation anaesthetic agents. Recently, a number of drugs with profiles (pharmacokinetic and pharmacodynamic) suitable for prolonged intravenous anaesthesia have been studied, mostly in humans and, to a certain extent, in dogs and horses. There is currently very little scientific information on total intravenous anaesthesia in goats, although, in the past few years, some scholarly scientific articles on drugs suitable for partial intravenous anaesthesia in goats have been published. This review article explored the information available on drugs that have been assessed for partial intravenous anaesthesia in goats, with the aim of promoting incorporation of these drugs into total intravenous anaesthesia protocols in clinical practice. That way, balanced anaesthesia, a technique in which drugs are included in anaesthetic protocols for specific desired effects (hypnosis, analgesia, muscle relaxation, autonomic stabilisation) may be utilised in improving the welfare of goats undergoing general anaesthesia.

The aim of this study was to investigate whether insulin resistance is independently associated with early manifestations of atherosclerosis. To this end, 176 normotensive offspring of type 2 diabetic patients were subjected to euglycemic hyperinsulinemic clamp to assess insulin sensitivity. Early atherosclerosis was studied by ultrasonography of the common carotid artery. Of the total 176 subjects, 145 were glucose tolerant, 18 had impaired fasting glucose, and 13 had impaired glucose tolerance. Univariate correlations showed that age, body mass index, waist, blood pressure, 2-h postchallenge glucose, fasting insulin, triglycerides, interleukin-6, fibrinogen, and white blood cell count were significantly correlated with carotid intima-media thickness (IMT), whereas HDL cholesterol and glucose disposal showed a negative correlation. A stepwise multivariate regression analysis including sex, age, waist circumference, smoking status, systolic blood pressure, diastolic blood pressure, triglyceride, HDL cholesterol, 2-h postchallenge glucose, plasma IL-6, fibrinogen, white blood cell count, insulin-stimulated glucose disposal, and fasting insulin showed that the four variables that remained significantly associated with carotid IMT were waist circumference, insulin-stimulated glucose disposal, white blood cell count, and diastolic blood pressure, accounting for 33.7% of its variation. These findings support the concept that insulin sensitivity, rather than plasma insulin levels, is associated with early atherosclerosis in nondiabetic normotensive offspring of type 2 diabetic patients.

The present study aimed to explore the relationship between electrocardiographic (ECG) and pulse wave analysis variables in patients with hypertension (HT) and high normal blood pressure (HNBP). A total of 56 consecutive, middle-aged hypertensive and HNBP patients underwent pulse wave analysis and standard 12-lead ECG. Pulse wave velocity (PWV), heart rate, intrinsic heart rate (IHR), P wave and QT interval durations were as follows: 7.26 ± 0.69 m/s, 69 ± 11 beats/minute, 91 ± 3 beats/minute, 105 ± 22 mm and 409 ± 64 mm, respectively. Significant correlations were obtained between PWV and IHR and P wave duration, respectively, between early vascular aging (EVA) and P wave and QT interval durations, respectively. Linear regression analysis revealed significant associations between ECG and pulse wave analysis variables but multiple regression analysis revealed only IHR as an independent predictor of PWV, even after adjusting for blood pressure variables and therapy. Receiver-operating characteristic (ROC) curve analysis revealed P wave duration (area under curve (AUC) = 0.731; 95% CI: 0.569–0.893) as a predictor of pathological PWV, and P wave and QT interval durations were found as sensitive and specific predictors of EVA. ECG provides information about PWV and EVA in patients with HT and HNBP. IHR and P wave durations are independent predictors of PWV, and P wave and QT interval may predict EVA.
