Ameloblastoma is a benign odontogenic tumor with local, invasive growth and late recurrence. The aim of this paper is report a case with recurrence after 32 years. The patient was 52 year-old, female and presented to the Santa Casa Hospital Complex with a complain of oral swelling of 5 years duration, in the right mandibular body region. The area produced sleep apnea, mastigatory and speech problems. Microscopic exam reveled ameloblastoma with cystic degeneration and dystrophic calcification.

The Gardner’s syndrome is described as a rare genetic disease resulting from mutations of the Adenomatous Polyposis Coli (APC) gene located in chromosome 5q21. This syndrome is a variant of familial adenomatous polyposis associated with intestinal polyps, mandible osteomas, epidermoid cysts, impacted teeth, and supernumerary among other clinical findings. The aim of this study is to report a clinical case of a 12-year-old female patient diagnosed with the Gardner’s syndrome who searched the Oral and Maxillofacial Surgery and Traumatology Service of Vitoria da Conquista General Hospital complaining of aesthetic alteration in the face. Clinically, there was a hardened mass in the mandibular body region in the right side, painless to palpation. Through computed tomography it was possible to identify the presence of bone lesions, the largest one located in the right mandibular body. With a diagnostic hypothesis of osteoma, the patient was submitted to surgery for mandible tumors resection with subsequent follow-up by the surgeon. The case highlights the importance of knowledge and proper management of the oral and maxillofacial alterations involved in this pathology.

Angiosarcoma is a malignant mesenchymal tumor characterized by the presence of vascular endothelial cells. Although rare, angiosarcoma developing in the mammary glands has a poor prognosis. We report a case of breast angiosarcoma with a preoperative diagnosis of late recurrence of breast cancer. A 78-year-old woman noticed a tumor in her right breast and visited our hospital. The patient had undergone breast-conserving surgery and axillary lymph node dissection from the right breast 12 years before the visit. The tumor was diagnosed as T4bN0M0, stage IIIB. Anastrozole was administered as postoperative adjuvant therapy for 5 years; the patient also received 50-Gy whole-breast radiation therapy after surgery. Physical examination during her visit revealed an elevated lesion with blue purpura around the nipple in the right breast. We performed breast ultrasound and detected a well-defined 19.6 × 16.4 × 10.7 mm hypoechoic tumor in the left subareolar area. The patient underwent core needle biopsy (CNB). Based on the CNB specimen findings, she was suspected to experience late local recurrence after surgery. Therefore, she underwent total mastectomy after breast-conserving surgery. A dark-red tumor sized 18 × 12 mm was found in a specimen from the nipple. The pathological diagnosis of the specimen revealed short spindle-shaped tumor cells with strong nuclear pleomorphism and a significant interstitial fibrosis. Immunohistochemistry using D2-40 and CD31 antibodies showed irregular luminal proliferation at the anastomosis, infiltration into the surrounding tissue, and massive necrosis, thereby leading to the diagnosis of breast angiosarcoma. We have reported a case of breast angiosarcoma with a preoperative diagnosis of late recurrence of breast cancer.

Abstract Introduction/Objective Adenomatoid odontogenic tumor (AOT) is an uncommon benign odontogenic tumor representing 3 to 7% of all odontogenic tumors. This tumor was first reported as adeno-ameloblastoma by Bernier and Tiecke in 1950 as it was initially assumed to be a type of ameloblastoma. In 1969, Philipsen and Bern proposed the term adenomatoid odontogenic tumor which was subsequently adopted by WHO and became the accepted terminology for this tumor. AOT is classified as a tumor of odontogenic epithelium but occasionally abnormal hard tissues consistent with dentinoid material may also be present as part of the tumor. AOT tends to occur in younger patients and 50% of the cases are diagnosed in teenagers. It occurs twice as commonly in females and frequently involves the anterior maxilla. Radiographically, AOT can appear as a radiolucent or mixed lucent-opaque lesion and may be associated with an impacted tooth. Methods/Case Report We report two unusually large expansile lesions of AOT that presented in the mandible. In the first case, the tumor presented as an expansile radiolucent lesion involving the right posterior mandible in a 32 year old female. In the second case, the tumor presented as an expansile mixed lucent-opaque lesion involving the left anterior mandible in a 21 year old female. The clinical presentation, radiographic and imaging findings, histopathologic features and treatment of these two cases will be discussed. The recommended treatment for AOT is surgical excision. The prognosis is good as this tumor seldom recurs after excision. Results (if a Case Study enter NA) NA Conclusion AOT is considered to be a non-aggressive, non-invasive and slow growing benign neoplasm. It is usually discovered on routine radiographic examination as the lesion is usually small and asymptomatic at the time of diagnosis but occasional cases that are larger in size have been reported in the literature.

Introduction. Intentional partial odontectomy or coronectomy is primarily advocated in situations with intimate relationships between the roots of the tooth indicated for extraction and the inferior alveolar nerve (IAN). The aim of this report was to present a patient with two neighbouring infraoccluded teeth in the right mandible indicated for extraction prior to prosthetic rehabilitation, which were coronectomied as to prevent injuring of the IAN and causing iatrogenic fracture of the mandibular body. Case report. Coronectomy of both teeth was performed as recommended in the literature. The patient had no special discomfort after the operation or deficit in sensitive nerve function, and the wound healed uneventfully. The patient was followed regularly during a 2- year period. Conclusion. The presented case suggests coronectomy as quite beneficial solution for avoiding serious problems (injuring of the IAN and possible fracture of the mandible) and making feasible forthcoming prosthetic rehabilitation.

To study respiratory timing mechanisms in patients with occlusive apnea, inspiratory and expiratory times (TI and TE) were calculated from the diaphragmatic electromyogram obtained in seven patients during non-rapid-eye-movement (NREM) sleep. Peak diaphragmatic activity (EMGdi) had a curvilinear relationship with TI during the ventilatory and occlusive phases such that TI shortened as EMGdi decreased during the ventilatory phase (r = 0.87, P less than 0.05) and it prolonged as EMGdi increased during the occlusive phase (r = 0.89, P less than 0.02). However, EMGdi vs. TI for the occlusive phase was shifted to the right of that for the ventilatory phase, reflecting the relatively longer TI during upper airway occlusion. TI also had a linear relationship with pleural pressure (r = 0.94, P less than 0.001) that remained unchanged during the ventilatory and occlusive phases such that it prolonged as negative inspiratory pressure increased. These results indicate that respiratory timing is continuously modified in patients with occlusive apnea as inspiratory neural drive fluctuates during NREM sleep and suggest that this modification is due to the net effects of changing inspiratory neural drive and afferent input predominantly from upper airway mechanoreceptors.

We report a case of recurrent Pindborg tumor (calcifying epithelial odontogenic tumor) of the maxilla. The patient was a 34-year-old woman who had been previously diagnosed with Pindborg tumor and treated with curettage. She was subsequently referred to us for evaluation of nasal obstruction. Examination revealed the presence of a mass lesion in the right nasal cavity and right maxilla, which was identified as a recurrence of her earlier Pindborg tumor. The patient was treated with maxillectomy with orbital preservation. Pindborg tumor is a rare odontogenic tumor; when it does occur, it is more often seen in the mandible than in the maxilla. While this tumor is often treated with curettage alone, the aggressive nature of the recurrence in our patient necessitated radical surgery. We report this case to highlight the need to be suitably aggressive in treating these types of tumors in order to avoid recurrence.

An attempt was made to develop a valuation index for draught cattle breeds of Tamil Nadu, which would help the farmers in getting the right price for their animals sold. The data were collected through personal interview using pre-tested interview schedule from 225 draught cattle owners from selected three districts of Tamil Nadu during the year 2010-11. A multiple linear regression was fitted for arriving at the valuation index of draught cattle breeds viz., Burgur, Kangayam and Umbalacherry. The adjusted R2 for overall draught cow valuation index was 0.818. The factors like presence of whirls in acceptable body region, skin and coat, body weight, milk yield and presence of pregnancy were found to be significant and had influenced the value of animal to the tune of Rs. 1642.40, Rs.1250.21, Rs. 36.25, Rs. 679.30 and Rs. 1241.24, respectively from their mean value. The overall draught bullock valuation index was also fitted and its adjusted R2 was 0.428. The variable body weight was found to be significant at one per cent level, whereas age was found to be significant at five per cent level and had negative influence on bullock value. The duration of work was found to be significant at five per cent level and it hiked the value at the rate of Rs.528.26. The variables like skin and coat, activeness, whirls, right pair, disease tolerance and breed were found to be non-significant.

Renal cell carcinoma (RCC) has a propensity for distant organ metastasis and late recurrence, involving not only the ipsilateral but also contralateral kidney. Lingual metastasis by RCC is rare. We present an unusual case of bilateral asynchronous RCC. Involvement of the right kidney was discovered only after a metastatic tongue lesion was diagnosed. The original RCC had been treated by left nephrectomy 14 years previously. Due to end-stage primary pulmonary malignancy, and poor function of the remaining kidney, immunotherapy was unsuitable. Palliative local resection of the lingual metastasis alleviated functional difficulties and was preventative against airway obstruction, but the patient died five months later.
