This study evaluates a number of power-saving measures that were applied on individual electric appliances to investigate their contributions to energy savings of the electricity network. In order to carry out the study, power-saving measures were applied on all appliances under study for certain period periods at the Universiti Malaysia Sarawak (UNIMAS). The appliances are the air conditioner (AC), computers, lightings and closed-circuit television (CCTV). As a means of validating the accuracy of models developed for electricity costs, a comparison of was done between measurements taken from the electricity network and those taken from PowerLogic PM5350 power meter (PM5350) installed for the purpose of this research. The results from model analysis show significant cost savings of 39.9%, 20.3%, 8% and 0.6% when control strategies were applied to AC, lightings, computers and CCTV

Problems. Windows in the summer let through a large amount of solar energy into the room, which causes an additional cost of cooling the air by conditioning. It is known that the limit of comfort is the temperature of 26 oC. To reduce the temperature, use air conditioners, which are required 0,3...0,5 kW of electrical power for 10 m2 of housing. The study deals with the capture of solar energy by water and its use for domestic water purposes (DHW). The goal of the research. Experimental verification of patented developments and calculations of hot water quantity obtained per day, energy savings and economic indicators. Methods of implementation. Experiments were conducted on the south window of the room, with water pumping by a pump and periodic measurement of air and water temperatures at the outlet of the system by mercury thermometer. The calculations were performed using the methods developed by the authors. The studies were conducted within three days of November. The temperature inside ranged a room from 19 to 23 °C. The system was operated in circulating mode on a water battery tank located below the absorber. Isolation of the absorber from the side of the room was made of a transparent food film. Research results. Water temperature reached 45 °C per 1,5-2 hours. Water consumption is enhanced by the thermosiphon effect when water moves from the bottom up. On a clear day of spring and autumn, you can heat for 50-70 % more water than the average for the average day of months of the warm season. For preparing DHW with 1 m2 of absorber, it is possible to get 45-50 kW∙h of heat for each month from March to September, taking into account cloudiness. The savings from the use of hot water and from reducing the consumption of electricity in the air conditioner are calculated. Conclusions. The payback period of the system, taking into account the cost of the heat for DHW and electricity savings for an electric air conditioner, is approximately 4-5 years. The cost of the system is close to the cost of a home air conditioner, for a premise with a single window oriented south. To the energy-saving factor, an additional advantage is the environmental friendliness of the system compared to the air conditioner.

Purpose 5G systems will enable an improved transmission performance and the delivery of advanced communication services. To meet the expected requirements, operators will need to invest in network modernisation, with the radio access network being the most expensive network component. One possible way for operators to reduce this investment would be via sharing of resources by means of a multi-tenancy concept. This implies that a mobile service provider may use the common infrastructure of one or various infrastructure providers, whereby it provides services to multiple tenants. This paper aims to study the expected cost savings in terms of capital expenditures (CAPEX) and operational expenditures (OPEX) that can be achieved when using a cloudified 5G multi-tenant network. Design/methodology/approach A cost model was used. The study period is 2020-2030 and the study area consists of three local districts in central London, UK. Findings This paper describes that the total cost reduction achieved when using multi-tenancy for a 5G broadband network in comparison with the case where operators make the investment independently ranges from 5.2% to 15.5%. Research limitations/implications Further research is needed to assess the cost implications of network sharing for 5G on a regional or nationwide basis. Originality/value Very little quantitative research about the cost implications of network sharing under 5G networks has been published so far. This paper sheds light on the economic benefits of multi-tenancy in a 5G broadband network.

BACKGROUND: Endoscopic ultrasound (EUS) is a safe alternative to endoscopic retrograde cholangiopancreatography (ERCP) for diagnostic biliary imaging in choledocholithiasis. Evidence linking a decline in diagnostic ERCP with the introduction of EUS in clinical practice is limited.OBJECTIVE: To assess the clinical impact and cost implications of a new EUS program on diagnostic ERCP at a tertiary referral centre.PATIENTS AND METHODS: A retrospective review was performed of data collected during the first year of EUS at the University of Alberta Hospital (Edmonton, Alberta). Patients were referred for ERCP because of suspicion of choledocholithiasis based on clinical, biochemical and/or radiological parameters. If they were assessed to have an intermediate probability of choledocholithiasis, EUS was performed first. ERCP was performed if EUS suggested choledocholithiasis, whereas patients were clinically followed for six months if their EUS was normal. Cost data were assessed from a third-party payer perspective, and cost savings were expressed in terms of ERCP procedures avoided.RESULTS: Over 12 months, 90 patients (63 female, mean age 58 years) underwent EUS for suspected biliary tract abnormalities. EUS suggested choledocholithiasis in 20 patients (22%), and this was confirmed by ERCP in 17 of the 20 patients. EUS was normal in 69 patients, and none underwent a subsequent ERCP during a six-month follow-up period. One patient had pancreatic cancer and did not undergo ERCP. The sensitivity and specificity of EUS for choledocholithiasis were 100% and 96%, respectively. A total of 440 ERCP procedures were performed over the same 12-month period, suggesting that EUS resulted in a 14% reduction in ERCP procedures (70 of 510). There were no complications of EUS. The cost of 90 EUS procedures was $42,840, compared with $108,854 for 70 ERCP procedures. The cost savings for the first year were $66,014.CONCLUSION: EUS appears to be accurate, safe and cost effective in diagnostic biliary imaging for suspected choledocholithiasis. The impact of EUS is the avoidance of ERCP in selected cases, thereby preventing the risk of complications. Diagnostic ERCP should not be performed in centres and regions with physicians trained in EUS.

This major research project focuses on reducing the energy consumption, by modelling a radiant floor heating system with phase change materials, in the Toronto climate. Computer generated simulations were performed using DesignBuilder software, using an example of a typical condominium in Toronto .Two south facing suites and two north facing suites were investigated. Of those suites, one north facing suite had PCM below the finished floor, as well as one south facing suite. The objective of these simulations was to determine the impact of using PCM in the condo suites. Three different types of PCM were used, in order to determine which type had the biggest energy savings. The PCMs were M91/Q21, M51/Q21 and M27/Q21. The final results showed that the suites with the M27/Q21 PCM had the lowest energy usage. A cost savings comparison was performed based on the rate of energy used and the cost of the energy, provided by the Ontario Energy Board.

5112 Background: Angiogenesis inhibitor (AI) therapies are promising novel treatments for patients with RCC. However, IV therapy may impose transportation burdens for patients, infection risk from IV catheters and increased costs. The incremental costs associated with IV vs. oral administration of selected AI therapies (oral sunitinib or sorafenib, or IV bevacizumab off-label) for the treatment of RCC were evaluated. Methods: Patients with ≥ 2 RCC claims (ICD-9 189.0, 198.0) were identified from a large US commercial health insurance claims database from 1/02 to 12/07. Patients receiving bevacizumab (N=109) were matched 1:1 to patients receiving sorafenib or sunitinib, and observed from their first AI therapy claim until the last treatment date. Inpatient, outpatient and pharmacy costs were calculated on a per- patient per-month (PPPM) basis over the treatment period. Cost comparisons between the IV AI group and each separate oral AI group were performed using multivariate Tobit regressions for each category separately, adjusting for age, gender, region, health plan, time from first RCC claim and first metastasis claim to treatment initiation. Results: Mean total medical cost was $13,351, $6,998, and $8,213 PPPM for bevacizumab, sorafenib, and sunitinib, respectively (p<0.05 for equality). Adjusted incremental total medical cost for the bevacizumab group was $5,045 PPPM higher than for sorafenib (p<0.05), and $4,902 PPPM higher than for sunitinib (p<0.05). Bevacizumab patients incurred higher incremental PPPM outpatient services cost by $7,687 and $7,995 compared to sorafenib and sunitinib, respectively (both p<0.05) largely driven by the cost of bevacizumab itself. Conclusions: Patients with RCC treated with bevacizumab incur an incremental $58,826 to $60,546 total medical cost per patient per year compared to those treated with sunitinib or sorafenib. Oral AI therapies may result in cost savings to patients and healthcare payers over IV therapies. [Table: see text] [Table: see text]

The present study examines the effect of agency-level implementation variation on the cost-effectiveness of an evidence-based parent training program (Positive Parenting Program: “Triple P”). Staff from six community-based agencies participated in a five-day training to prepare them to deliver a 12-week Triple P parent training group to caregivers. Prior to the training, administrators and staff from four of the agencies completed a site readiness process intended to prepare them for the implementation demands of successfully delivering the group, while the other two agencies did not complete the process. Following the delivery of each agency’s first Triple P group, the graduation rate and average cost per class graduate were calculated. The average cost-per-graduate was over seven times higher for the two agencies that had not completed the readiness process than for the four completing agencies ($7,811 vs. $1,052). The contrast in costs was due to high participant attrition in the Triple P groups delivered by the two agencies that did not complete the readiness process. The odds of Triple P participants graduating were 12.2 times greater for those in groups run by sites that had completed the readiness process. This differential attrition was not accounted for by between-group differences in participant characteristics at pretest. While the natural design of this study limits the ability to empirically test all alternative explanations, these findings indicate a striking cost savings for sites completing the readiness process and support the thoughtful application of readiness procedures in the early stages of an implementation initiative.

One-solving methods and techniques necessary to avoid inefficiencies and not economic costs as well as reduce the cost of housing construction is the method of Value Engineering. Value engineering is a method and cost control techniques to analyze a function to its value at the lowest cost alternative (most economical) without reducing the quality desired.At the writing of this study used a comparison method by comparing the initial design to the design proposal of the author. In the housing projects Upgrading Tirto Penataran Asri type 70, the application of Value Engineering conducted on the job a couple walls and roofs pair by replacing some work items with a more economical alternative but does not change the original function and high aesthetic level and still qualify safe. For that performed the step of determining a work item, the alternative stage, the analysis stage, and the stage of recommendations to get a Value Engineering application and cost savings against the wall a couple of work items and partner roof.The proposed design as compared to the initial design. Work items discussed was the work of a couple wall having analyzed obtained savings of Rp. 2,747,643.56 and the work of the roof pair obtained savings of Rp. 2,363,446.80. Thus the total overall savings gained is Rp 5,111,090.36 or savings of 0048%.

Now that oxygen production facilities can be controlled to match the requirements of the dissolution system, improved oxygen dissolution control can result in significant cost savings for oxygen activated sludge plants. This paper examines the potential cost savings of the vacuum exhaust control (VEC) strategy for the City of Houston, Texas 69th Street Treatment Complex. The VEC strategy involves operating a closed-tank reactor slightly below atmospheric pressure and using an exhaust apparatus to remove gas from the last stage of the reactor. Computer simulations for one carbonaceous reactor at the 69th Street Complex are presented for the VEC and conventional control strategies. At 80% of design loading the VEC strategy was found to provide an oxygen utilization efficiency of 94.9% as compared to 77.0% for the conventional control method. At design capacity the oxygen utilization efficiency for VEC and conventional control was found to be 92.3% and 79.5%, respectively. Based on the expected turn-down capability of Houston's oxygen production faciilities, the simulations indicate that the VEC strategy will more than double the possible cost savings of the conventional control method.

Background: Asthma and Chronic Obstructive Pulmonary Diseases (COPD) are well known respiratory diseases affecting millions of peoples in India. In the market, various branded generics, as well as generic drugs, are available for their treatment and how much cost will be saved by utilizing generic medicine is still unclear among physicians. Thus, the main aim of the current investigation was to perform cost-minimization analysis of generic versus branded generic (high and low expensive) drugs and branded generic (high expensive) versus branded generic (least expensive) used in the Department of Pulmonary Medicine of Era Medical University, Lucknow for the treatment of asthma and COPD. Methodology: The current index of medical stores (CIMS) was referred for the cost of branded drugs whereas the cost of generic drugs was taken from Jan Aushadi scheme of India 2016. The percentage of cost variation particularly to Asthma and COPD regimens on substituting available generic drugs was calculated using standard formula and costs were presented in Indian Rupees (as of 2019). Results: The maximum cost variation was found between the respules budesonide high expensive branded generic versus least expensive branded generic drugs and generic versus high expensive branded generic. In combination, the maximum cost variation was observed in the montelukast and levocetirizine combination. Conclusion: In conclusion, this study inferred that substituting generic antiasthmatics and COPD drugs can bring potential cost savings in patients.
