Introduction. Metastases to the oral cavity are uncommon, accounting for only 1% of all oral malignant tumors. When they occur they mostly originate from primary tumors of the lungs, kidney, breast and prostate. Oral metastases from the primary colorectal carcinoma are much more infrequent. Case Outline. We present an unusual case of a 78-year-old man with a soft tissue oral metastasis originating from the primary colorectal carcinoma. The patient was referred to the Department of Otorhinolaryngology, Head and Neck Surgery with an intraoral mass on the right side of the maxilla. The diagnosis was confirmed by histopathologic examination and immunohistochemical analysis. Conclusion. Oral metastases occur rarely and often can mimic much more common benign lesions, therefore they should be considered as a possibility in a differential diagnosis.

✓Cholesterol granulomas (CGs) are benign lesions resulting from an inflammatory reaction to cholesterol and hemosiderin. These masses most often arise within the temporal bone or nasal sinuses; intracerebral CGs are extremely rare. In this report the authors present an unusual case of a CG arising within the lateral ventricle. The patient presented with transient hemiparesis and numbness. Computed tomography and magnetic resonance imaging demonstrated a cystic partially enhancing midline mass within the right lateral ventricle, expanding the ventricle and displacing the septum pellucidum. The patient underwent an interhemispheric, transcallosal resection of the lesion. Microscopic examination revealed a granulomatous inflammatory lesion containing cholesterol clefts, macrophages, and hemosiderin. Embedded within the granulomatous response were foci of tiny cystlike structures lined by nonciliated flattened cuboidal epithelium, consistent with the diagnosis of CG. To the authors' knowledge this is the first reported case of CG presenting as an intraventricular mass. The origin of this lesion is unclear, but it may relate to prior traumatic brain injury. The authors describe the presentation, imaging findings, histopathological characteristics, and surgical treatment of this rare lesion and related pathological entities.

The differential diagnosis of a congenital cutaneous vascular-appearing mass in a newborn is broad and includes both benign and malignant tumors. We report the case of a newborn who presented with an erythematous exophytic skin nodule on the right upper leg. Excision was performed due to ulceration, concern for bleeding, and for diagnosis. Pathology revealed the mass to be an infantile myofibroma. This case highlights the importance of considering a broad differential diagnosis in a newborn with a cutaneous mass. While history, physical exam, and imaging can help diagnose some cases, a biopsy or excision is often needed to distinguish benign lesions from more concerning lesions.

Metastases from a variety of malignant tumors can involve the ureters, but ureteral involvement by lung cancer is extremely rare and usually described at autopsy. We report a rare case of a 76-year-old man who presented with a three-month history of right flank dullness and was noted to have a nonhomogeneous retroperitoneal mass with hydronephrosis of the right kidney on computed tomography of the abdomen. Computed tomography of the thorax showed a nodule in the lower lobe, measuring3×2 cm, in the right lung. After excluding the presence of other primary tumors and metastases, we reached a final diagnosis of solitary retroperitoneal metastasis of adenocarcinoma of the lung. Although rare, in patients of non-small cell lung cancer, presence of hydronephrosis should alert the physician to the possibility of metastasis.

Objective: To determine the expression of BRAF V600E in tissue samples of colorectal carcinoma and to correlate it with various clinico-pathological parameters. Study design and setting: Cross-sectional study was conducted at department of Pathology, Pakistan Navy Station Shifa hospital Karachi from 1st March 2016 to 28th February 2019 Methodology: Total of 51 cases of colorectal cancer were analyzed for immunohistochemical staining using BRAF antibodies on representative tissue blocks. Clinical and pathological records were retrieved for data collection. The results of immunohistochemical analysis were correlated with the recorded clinico-pathological parameters. Results: In this study 51 cases of colorectal cancer were analyzed for immune expression of BRAF V600E. The age of the patients ranged from 14 to 85 years with the mean age of 60.96 years. Among the 51 cases, 37(72.5%) cases were males and 14(27.4%) were females. 37(72.5%) were localized to left side colon and 14(27.4%) were found in the right colon. For BRAF V600E, positive expression was seen in 20(39.2%) cases, whereas 31(60.7%) cases showed negative expression of BRAFV600E. No significant association was seen between BRAF V600E expression and histological variants like age, gender, tumor location and glandular carcinomas. Conclusion: BRAF V600E immunosuppression was seen in 39.2% of colorectal carcinoma in this study. No significant association was seen in BRAF V600E expression and histological variants

The clinical behavior of various types of thryroid tumor have been much studied during the past several decades, and the histologic features, surgical management, and prognostic factors of follicular and papillary tumors in particular have been clarified to a considerable degree. On the other hand, there is still controversy concerning management of Hurtle cell tumor (HCT) of the thyroid. HCT is not a common disease, making it impossible to obtain sufficient clinical data at a single institution. It has been reported that all HCT are aggressive and should be treated as malignant tumors. It is believed that an accurate differential diagnosis can be made between cancer and adenoma on the basis of pathological studies. We describe a patient with HCT of the thyroid extending into the right atrium. To our knowledge, after a Medline search, this is the first such case in the medical literature. The interesting features are described, and the relevant literature is briefly reviewed.

Colorectal carcinoma is one of the most common malignant tumors of gastrointestinal tract and is a contributing factor of cancer mortality in Myanmar. Matrix metalloproteinase-9 is a proteolytic enzyme which breaks down extracellular matrix leading to tumor progression, contri-buting as a potential prognostic marker nowadays. The study was aimed to determine MMP-9 immunoexpression in colorectal adenocarcinoma and its association with Astler-Coller staging. A cross-sectional descrip-tive study was done on 42 specimens of colorectal adenocarcinoma. All tissue specimens were studied with haematoxylin and eosin to categorize histological types and grades. Out of 42 cases, 21.4% were well differentiated, 57.2% were moderately differentiated and 21.4% were poorly differentiated. According to Astler-Coller staging, 11.9% were found to be in stage B1, 40.5% in stage B2, 7.1% in stage C1, 33.3% in stage C2 and 7.1% in stage D. 81% (34/42) of colorectal carcinoma showed positive MMP-9 immunoexpression. Positive MMP-9 immuno-expression was seen in 91% of conventional adenocarcinoma, 40% of mucinous carcinoma and no cases signet ring carcinoma. The findings of the study pointed out that MMP-9 immunoexpression was positively associated with histological types of colorectal adenocarcinoma (p=0.001). MMP-9 immunoexpression was positive in 88.9% of well differentiated adenocarcinoma, 91.7% of moderately differentiated adenocarcinoma and 44.4% of poorly-differentiated adenocarcinoma (p=0.007). Regarding the immunoexpression of MMP-9 in different Astler-Coller staging, positive MMP-9 immunoexpression was seen in 60% of the cases in stage B1, 76.5% in stage B2, 66.7% in stage C1, 92.9% in stage C2 and 100% in stage D. The findings of the study can be extrapolated to predict prognosis and help in better management of colorectal adenocarcinoma by introducing targeted therapy against MMP-9 in future.

Despite cardiac metastases are found in about 20% of cancer deaths, the presence of primary cardiac tumors is rare. Most primary tumors are benign, and malignant tumors comprise about 15%. We report a 21-year-old man with fever, dyspnea, and hemoptysis that was diagnosed with angiosarcoma of the right atrium and pulmonary metastasis. Patient was submitted to surgical tumor resection without adjuvant therapy and died four months after diagnosis.

This report describes an unusual case of bizarre parosteal osteochondromatous proliferation of bone (BPOP) which developed in the foot of an eight year-old child. Also described as Nora's lesion, BPOP is a rare benign bone tumor found most commonly in the hands and feet with a radiographic appearance occasionally confused with malignancy. Resembling osteochondromas at first glance, these lesions have a distinct radiographic and histologic appearance that permits differentiation from other benign lesions or low-grade malignant tumors. Treatment of BPOP consists of simple excision, although there is high rate of recurrence. Despite this high recurrence rate, there have been no reported metastases and local excision is still recommended.

Abstract Background & Objective Colorectal cancer is a major cause of morbidity and mortality throughout the world. In Egypt, it is the seventh commonest cancer representing 3.47% of male cancers and 3% of female cancers. Although the diagnosis of colorectal cancer is straight forward in primary site, yet it may represent a diagnostic problem in metastatic tumour of unknown primary origin. Pathologists are often called upon to suggest a site of origin in patients with metastasis of an unapparent primary tumour site. This is because some specific tumour types may respond well to targeted molecular therapies. Most tumours of unknown primary site are evaluated with immunohistochemical analysis in combination with morphologic assessment and correlation with clinical data. Therefore, establishment of reliable diagnostic markers that confirm or rule out colorectal origin are mandatory especially in adenocarcinomas of unknown primary site which is one of the most clinical problems. The aim of this work is to study the expression of Cadherin 17 & CDX2 in colorectal carcinoma to evaluate their roles when metastatic colorectal carcinoma is suspected in cancer of unknown primary site. Material & Methods This retrospective study included (65) cases of adenocarcinomas: 35 cases of colorectal adenocarcinoma (whether primary or metastatic) and 30 cases of non colorectal adenocarcinoma from other different sites. Our cases were retrieved from the archives of Pathology Department of Ain Shams University Hospitals and Ain-Shams University Specialized Hospital during the period from (2010 to 2015). Immunohistochemical study was performed using monoclonal cadherin 17 & CDX2 antibodies. Results The sensitivity of CDX2 and cadherin 17 were 97.1% and 100% respectively in detecting colonic adenocarcinoma. The specificity of CDX2 was 53.3% while cadherin 17 showed a specificity of 50%. The PPV, NPV and overall accuracy of CDX2 versus cadherin 17 were 70.8%, 94.1% and 76.9% versus 70%, 100% and 76.9% respectively. Conclusion Cadherin 17 is a more sensitive marker than CDX2 in diagnosis of carcinoma of unknown primary site especially when colorectal carcinoma is suspected.
