Background / Aim. Patient satisfaction, when it comes to anesthesia, is more difficult to assess than in any other medical specialty. The aim of this study was to construct a tool for assessing patients' satisfaction with anesthesia, and then examine the effects of post-operative care provided by anesthesiologists on patients? satisfaction. Methods: The study included patients undergoing general anesthesia, who were considered ASA 1 and ASA 2 class. Patients were divided into three groups: Group 1 included 74 patients who had postoperative visit performed by an attending anesthesiologist; Group 2 included 70 patients who had postoperative visit performed by a nurse anesthetist after surgery; and Group 3 included 74 patients who did not have postoperative visit during post-operative care by an anesthesiologist nor a nurse anesthetist. The tools used in the research were Anesthesia patient satisfaction questionnaire specially designed for this study and Post Anesthetic Recovery Scoring System (PAS). ANOVA and Pearson?s correlation coefficient were used for estimating the statistical significance of the obtained results between the groups. Results: Association between an objective assessment of the post-operative status of patients on day zero and satisfaction with the anesthesiologist's patient management shows statistical significance (p<0.05). Patients who had a post-operative visit by an anesthesiologists tolerated better preoperative and postoperative physical symptoms. Patients visited by an anesthesiologist were most satisfied with post-operative care (p<0.05). Conclusion: Use of a highly reliable questionnaire for evaluation of patient?s satisfaction with anesthesia could improve the post-operative condition of patients and enable faster recovery during post-operative period.

Abstract Background Mastopexy combined with silicone implant placement is one of the most frequently performed plastic surgeries. Various techniques have been developed to increase the degree of patient satisfaction after surgery. The goal is to give the breasts a harmonious shape, generating the smallest scars possible. The Breast Evaluation Questionnaire 55 (BEQ 55) was developed in 2006 as a tool for assessing the satisfaction and comfort of patients with the general appearance of their breasts. Objectives The objective of this study was to compare the degree of satisfaction of patients undergoing mastopexy by the inverted-T technique vs the short-scar technique by the BEQ 55. Methods The BEQ 55 was administered pre- and postoperatively to 200 patients who underwent mastopexy performed by the first author between October 2013 and September 2017. One hundred patients underwent the short-scar technique, and 100 patients underwent the inverted-T technique. Results The first analysis used descriptive statistics. Relative frequencies were used to track the responses corresponding to each technique. There was an increase in the level of patient comfort and satisfaction with breast appearance after surgery in both groups. The short-scar technique was shown to be superior, with statistical significance (P &lt; 0.05) found for the questions evaluating comfort with overall appearance naked, comfort with breast appearance naked, and satisfaction with breast appearance. Conclusions Both techniques improved the patients’ satisfaction and comfort with their breasts. When the short-scar technique was used, patients were more comfortable with their breast appearance and with their overall appearance naked. Level of Evidence: 4

Quality in health care is commonly measured by patient satisfaction. This study assessed asthmatic patients’ satisfaction with the pharmaceutical care services rendered in 2 Nigerian tertiary hospitals. This 3-arm intervention study was single-blinded, prospective, and randomized. The 3 arms were Usual Care, Individual Intervention, and Caregiver-assisted Intervention. Intervention arms received education for 6 months, whereas the Usual Care arm received no education. The Patient Satisfaction with Pharmaceutical Services questionnaire was utilized. Data were analyzed using the IBM SPSS Version 25.0 with statistical significance set as P < .05. Seventy-eight asthma patients participated in the 3-arm study. The majority of the patients (82.1%) were happy with the services provided by the pharmacists. More of the patients who received Individualized Intervention were highly satisfied, compared to those in the Caregiver-assisted Intervention arm and Usual Care arm (52.6% vs 44.7% vs 2.6%, χ2 = 32.124, P < .001). The Individualized Intervention satisfied patients better than the Caregiver-assisted Intervention. Pharmacists should strive to have direct communication with their patients despite the involvement of caregivers.

Abstract Background: To promote the integration of medical resources, Beijing has built medical alliances since 2012, but this reform has not changed the state of disordered medical treatment. Patients are still willing to go to high-level hospitals for medical treatment. What causes patients to prefer high-level hospitals? To explore the reasons behind this preference for high-level medical treatment among patients and to guide patients to make visits in an orderly manner, we conducted this study and compared patients' satisfaction with hospitals of different levels in the context of medical resource integration.Methods: This study conducted a questionnaire survey among 1,250 patients who were selected in 18 medical alliances in Beijing from October to December 2016. The study type is a comparative study based on cross-sectional data. Patient satisfaction was the main outcome, and descriptive analysis, chi-square tests, nonparametric tests and binary logistic regression analysis were used. The level of statistical significance was set at p<0.05.Results: The overall satisfaction score of the medical alliances was 3.375, and the satisfaction scores of core hospitals and cooperative hospitals were 2.77 and 3.07, respectively. The overall patient satisfaction rate was 44.62%, and the satisfaction rates of core hospitals and cooperative hospitals were 34.34% and 50.43%, respectively. The type of hospital and understanding of medical alliance policy were the factors associated with patient satisfaction with the medical alliance.Conclusions: The patients’ satisfaction with cooperative hospitals was higher than their satisfaction with core hospitals. Although the patients were more satisfied with the service attitude of the cooperative hospitals, they still preferred core hospitals due to their higher expectations for their medical treatment and greater trust in their medical technology. It is necessary to explore the establishment of closed medical alliances under the unified management of human and financial resources to enable medical alliances to become a community of common interests and provide integrated medical services for patients.

ABSTRACT This pilot study was designed to determine the serum cytokine profile of acute otitis media (AOM) due to Streptococcus pneumoniae and the impact of clarithromycin (Abbott Laboratories, Inc). Serum levels of interleukin-1β (IL-1β), tumor necrosis factor alpha (TNF-α), IL-6, and IL-8 were measured at diagnosis and 3 to 5 days after start of antibiotic treatment in 10 patients (mean age, 18.3 ± 13.9 months) who had middle ear fluid culture positive for S. pneumoniae. The mean concentrations of all cytokines were elevated at diagnosis of AOM compared to levels in healthy controls, yet only IL-6 reached statistical significance (P = 0.05). IL-6 showed a statistically significant decrease in mean serum concentration at visit 2 (P = 0.03). IL-8 displayed a similar pattern to IL-6, but the difference between samples from day 1 and day 2 did not reach statistical significance. The cytokines IL-1β and TNF-α appear to be elevated in the serum of patients with S. pneumoniae AOM, but there was no significant change between mean serum levels obtained pre- and postinitiation of antibiotic treatment in the time frame studied. The results suggest a systemic inflammatory response as evidenced by increased IL-6. A significant decrease of IL-6 and improvement of clinical symptoms were observed. Determining cytokine levels, especially IL-6, in AOM could offer a powerful tool for objective assessment of response to treatment, minimizing unnecessary treatment of asymptomatic children who may still have some otoscopic findings suggestive of AOM at follow-up visits.

BACKGROUND: In recent years’ medical management with misoprostol is an effective alternative to surgical evacuation. But there is a dearth of evidence to reveal the effectiveness of the different routes of misoprostol and satisfaction rate among the patients treated with these routes. AIM: This study was conducted to compare the effectiveness and patient’s satisfaction rate of vaginal versus oral misoprostol. METHODS: It was a prospective non-interventional study. One hundred women of having missed abortion confirmed by ultrasonography examination were enrolled in the trial. Fifty-eight subjects were administered 200 mcg of oral and 42 subjects received 200 mcg of vaginal misoprostol every four hours up to four doses. If complete expulsion did not occur 12 hours after the last dose, the surgical evacuation was done. RESULTS: There was no significant statistical difference between the effectiveness of treatment with vaginal (78.57%) and oral misoprostol (79.31%) (p = 0.928). The difference between Patients’ satisfaction at the time of discharge for the vaginal group (64.29%) and oral group (65.52%) was not statistically significant (P = 0.991). There was an increase in patients’ satisfaction for both groups at the follow-up session, but still, the difference was not significant (P = 0.897). CONCLUSION: This study confirms that there is no statistical difference between the effectiveness and patient satisfaction of oral and vaginal misoprostol in the treatment of missed abortion.

Abstract Background: With the development of technology in recent years, booking appointment software systems have gained special and significant popularity among physicians and patients. As Medical university of Isfahan is one of the leading universities of the country the results of this study could be very important for the use of other health systems in different states of country. This research aimed to assess the patients’ satisfaction of the booking appointment system services of Isfahan dentistry faculty, considering its different types of booking systems and patient satisfaction. Methods: The study was descriptive and cross-sectional. The questionnaire was prepared using Zhang study in 2014 and the standard Forward-Backward translation method. In order to investigate the validity, the opinions of the professors of Isfahan Dental School were taken and the professional opinions of the individuals were applied. The questionnaire was distributed among patients referring to the dental school who had undergone the initial examination. After completing the data, SPSS software version 20 and descriptive and analytical statistical tests were used to analyze the data (a = 0.05).Results: In this study, independent t-test, Kruscal-Wallis test and Pearson correlation test were used to investigate the data. The Kruscal-Wallis test showed that overall satisfaction was significantly higher in patients who booked appointment online than in other patients (P = 0.003). The independent t-test showed that the mean scores of patient satisfaction with staff behavior (P = 0.03) and satisfaction with the waiting time (P = 0.01) were significantly different between native, non-native and passenger patients.Conclusion: Most of the people surveyed were generally relatively satisfied with the booking appointment services. Most of them surveyed came in person to queue. Most people cited cheap services as the main reason for their visits. Most people cited telephone busy signal as the reason for not using the telephone booking system.

Abstract   Impaired gastric conduit perfusion is a risk factor for anastomotic leak after esophagectomy. Most studies evaluating conduit perfusion have been qualitative with limited impact on post-operative care. The aim of this study is to evaluate the feasibility of intraoperative quantitative assessment of gastric conduit perfusion with indocyanine green (ICG) fluorescence angiography as a predictor for cervical esophagogastric anastomotic (CEGA) leak after esophagectomy. Methods ICG fluorescence angiography using the SPY elite® (Stryker, MI, USA) system was performed in patients who had undergone a transhiatal or McKeown esophagectomy CEGA from July 2015 through December 2020. Fluorescence angiography assessed Ingress (dye uptake) and Egress (dye exit). Ingress Index, Ingress Time, Egress Index, and Egress Time at two anatomic landmarks (tip of the conduit, and 5 cm from tip) were calculated from the measured curve of fluorescence (Figure). The collected data between the leak (L) group and the no-leak (NL) group were compared by both univariate and multivariable analyses to analyze risk factors potentially associated with CEGA leak. Results 304 patients were evaluated. There was no significant difference in patients' demographic and post-operative complications between the groups (L n = 73; NL n = 231), except for anastomotic stricture (42.5 vs 9.1%, p &lt; 0.01). 5 cm and Tip Ingress Index were significantly lower in L (35.0 vs 45.1% and 17.4 vs 25.7%, p &lt; 0.01). 5 cm Ingress Time was significantly higher in L (70.6 vs 56.8 sec, p &lt; 0.01). On multivariable analysis, these variables retained statistical significance, suggesting that these three variables can be used to predict future leak. Conclusion This study revealed that gastric conduit perfusion correlates with the incidence of CEGA leak. Intraoperative measurement of gastric conduit perfusion may be predictive for CEGA leak following esophagectomy. These variables can be easily collected intraoperatively with the SPY study and used to make clinical decisions which may avert CEGA leak.

Abstract Background Various treatment modalities have been applied to treat striae alba with low satisfaction rate. Fractional Microneedling radiofrequency provides deep dermal coagulation, thereby inducing collagen synthesis and tissue tightening. The addition of platelet lyophilized growth factors may add to the effect of fractional Microneedling radiofrequency. Objectives To evaluate and compare the efficacy and safety of fractional Microneedling radiofrequency alone or combined with lyophilized growth factors in the treatment of striae alba. Methods In this left-right comparative study, 25 patients suffering from striae alba on the abdomen received sessions of fractional microneedling radiofrequency with lyophilized growth factors on one side and with saline (as placebo) on the other side at 4 weeks interval. Patients‘ satisfaction was evaluated at the end of study using a questionnaire (Patients were asked the following questions: Are you satisfied with this treatment (completely satisfied, satisfied, partially satisfied, or not satisfied)?. Results There was a significant improvement in striae alba after treatment in both the lyophilized growth factors (p = &lt;0.0001) and placebo sides (p = &lt;0.0001), the lyophilized growth factors being superior (p = 0.002). Patient satisfaction was more on the LGF side (p = 0.034),where 6 versus 3 patients were completely satisfied. Erythema and hyperpigmentation were less on the lyophilized growth factors side. Conclusion The combination of fractional microneedling radiofrequency and lyophilized growth factors improves striae alba outcome.

In complex systems, such as healthcare, patient satisfaction is identified as the patient experience of care and has been referred to as the “indispensable outcome.” The main goals of ED practitioners are the patient satisfaction along with maintaining optimal outcomes. Patient satisfaction has become a very important outcome measure when assessing healthcare systems performance. Nevertheless, it is a complex confusing concept. Some providers suggest several activities in order to enhance the satisfaction without being sure if these actions really improve satisfaction or no. Also, patient satisfaction enhancement activities should not conflict with cost efficacy. Several factors fall under patient satisfaction. Interviews with physicians prove that patients have specific expectations during a clinical encounter; being aware of these expectations, physicians can fulfill patient satisfaction. The goal of this chapter is to determine the contribution and effect of these factors in influencing patient satisfaction.

Abstract Introduction Accurate models are a fundamental prognostic tool for risk stratification, therapy guidance, resource allocation, and comparative effectiveness research. Enhanced recovery after surgery protocols are developed to increase early post-operative recovery rates in surgical patients. Due to the unique nature of burn injuries and post-operative care, there is a need to develop a protocol unique to burn surgery, enhanced recovery after burn surgery. Methods The PubMed, Embase, Cochrane, and Web of Science databases were systematically searched. Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) and Cochrane guidelines were strictly followed throughout the study. Search terms were utilized to capture the relevant studies relating to early ambulation of adult burn patients (&gt;18 years of age) and their post-surgical outcomes such as graft take, time to discharge, pain levels, VTEs, and length-of-stay. Results Thirteen of 888 studies retrieved from the search query were eligible for systematic review and meta-analysis. Patients with delayed ambulation, after 5 or more days were found to have increased pain levels at rest (p=0.02) and when ambulating (p=0.08). One study found an increased infection rate in late ambulatory patients (p=0.22). Most results from studies did not have significant data that was relevant to our extraction. For example, only one study assessed pain levels and only three studies notes zero venous thromboembolisms (without statistical significance). Conclusions Limited evidence exists relating to thromboembolic events and time-to-ambulation in post-operative burn patients. There are no significant differences in the number of events between early and late ambulation groups. Early ambulation should be included as part of the ERABS protocol for lower risks of hospital-acquired infections due to shorter lengths-of-stay. Decreased associated pain levels could lead to decreased risk for opioid dependence. Due to limited literature references, these conclusions are immature and more studies should be performed in order to develop more accurate and effective protocols. Applicability of Research to Practice Burn surgery recovery patients are unique. Therefore, specialized protocols must be developed to enhance their post-operative care.
