Identifying teaching material is the process of  selecting, adapting and evaluating of teaching based on specific necessities of the students. Need Analysisis really needed to identify teaching materials for  English for Specific Purpose. The researchobjective is toidentify the ESP materials for teaching 3rd semestermedical record students at Academy of Medical Record and Health information Citra Medika (APIKES CITRA MEDIKA) Surakarta. The work used qualitativeand quantitative methods. The outcomesof this study showed the real necessities of students in learning English to prepare their future at the field of medical record and health information. Findings of the need analysis demonstrate that all four of the language skills were necessary for their academic studies and their target career. There are certain topics related to English for medical record such as medical record staff’ duties, ethical and legal issues in medical record, Hospital statistics, Medical record filling system, Health information system, and so on. Accordingly, this study proposes new ESP materials based on the stakeholders’ needs.It is suggested that textbook or handout of English for Medical Record will be made based on the Need Analysis by ESP designers and ESP lecturers involve actively recognizing the progressive needs of medical record students.

The internet offers unprecedented power to provide users with health information for patients, health professionals, and professionals. Maintaining the integrity, data systems, and confidentiality of individual health information, quality of content, and consumer protection and the commercial interests of the health industry against unethical practices, are areas of greatest concern in the implementation and use of the Internet. However, there is no national and international legislation for regulating the use of online-based health services. This research is a Literature review that aims to explore ethical and legal issues in the use of online-based health services (E-Health). The review process begins by identifying journal articles that are relevant to the research topic. This study concludes that the use of online-based health services (E-Health) is an important public health issue. E-Health emerged as a tool for developing new diagnostics and therapeutic interventions. Ethical issues related to crossing clinical practice and online communication about health services. This allows discriminatory or unethical behavior and is not following the professional code of ethics. E-Health licensing standards and regulations have not been implemented in many countries. So that health workers are required to code of ethics in the use of online-based health services (E-Health).

Health Information System is a system that integrates the collection, processing, reporting of data, and use of information needed to increase the effectiveness and efficiency of health services through better management at all levels of health services. Kulon Progo Health Office is one of the Health Services that has utilized the Health Information System in organizing its health transactions. However, the implementation of the Health Information System still has shortcomings, namely that it was found that a patient has many medical record numbers or often referred to as duplicated medical record data. Community service activities are carried out through the use of appropriate technology at the Kulon Progo Health Office. This activity aims to implement data cleansing techniques using the "RESIK" framework  to help prevent and detect duplication of medical records and provide training to medical recorders in cleaning data. The training was attended by 105 participants, each of whom was a representative of the Puskesmas staff in the Kulon Progo Health Office area. The “RESIK” framework  was then piloted at Puskesmas Sentolo 2 as the location for the implementation of the system. From this activity, duplicate medical record data can be found at Puskesmas Sentolo 2, and then cleaning is carried out. Kulon Progo Health Office is advised to implement data cleansing using the "RESIK" framework  at all Puskesmas in the Kulon Progo area.

Health Information System aims at improving and enhancing the delivery of quality, data availability and administrative effectiveness of people’s health. Medical record has come under severe threat as a result of the manual system of medical record keeping in spite of its important functions. This system of record-keeping involves taking down patient data on pieces of paper, which are put into files and kept in cabinets. In fact, this is an improper means of documentation resulting in loss and mismatch of patient data, and time wastage. It is alsocumbersome, bulky and consumes a lot of the office space. In this paper, a complete web-based health information system is designed to solve these problems so as to enable users handle details on policies efficiently and effectively. A test of the system over various network topologies reveals that time taken to move a packet and received acknowledgment for standalone, LAN, WAN and Intranet is 3ms, 4ms, 8ms and 10ms respectively. These short periods of time show faster and efficient delivery of health activities. The Web Based HealthInformation System thus provides significant benefit to institutions as it can capture data and store it in the developed database for future use. Citation: Kwasi, T. The purpose of this chapter is to explore ethical and legal concerns that relate to the mandatory reporting of child maltreatment including informed consent, confidentiality, reasonable suspicion, and penalties for false or nonreporting. Informed consent with individuals with limited English language skills is specifically addressed. In addition, the chapter provides an overview of the perceived benefits and challenges of mandatory reporting and who is considered a mandatory reporter in Canada. Documentation is discussed, and a case example is provided. The chapter discusses mandatory reporting legislation and both the benefits and criticism of this legislation and recording suspicions of child maltreatment will conclude the chapter.

Electronic patient records are a central feature of many national policies on health information technology. The present article provides an overview of definitions for various degrees of medical record computerisation and current obstacles to implementation. A summary of progress in the areas of standards, specific technological developments and legal issues is provided. The wide ranging benefits of electronic patient records and the implications for health information managers and clinical coders are discussed.
