Intussusception of the appendix is the introduction of the appendix into the lumen of the cecum may be accompanied by the development of acute appendicitis, or proceed without it. The combination of intussusception and acute inflammation of the appendix makes the diagnosis of appendicitis difficult due to the absence of classic clinical symptoms. The article presents the case of successful treatment of a patient with an untypical clinical presentation of acute appendicitis that developed in an invaginated vermiform appendix.

Abstract: Appendicitis is an inflammation of vermiform appendix. Acute inflammation of the appendix needs to be treated immediately to prevent fatal complications. The incidence among females and males is slightly comparable, however, the incidence is higher among males than females in the age range between 20-30 years. The fundamental clinical decision in the diagnosis of a patient with suspected appendicitis is whether to do an operation or not. The meaningful evaluation of acute appendicitis balances early operative intervention to prevent operative risks. This study aimed to obtain the incidence of appendicitis at Prof. Dr. R.D Kandou Hosiptal Manado from October 2012 to September 2015. This was a  retrospective descriptive study using data of the Department of Medical Record Prof. Dr. R.D Kandou Manado Hospital. The results showed that there were 650 patients. Most patients had acute appendicitis as many as 412 patients (63%) meanwhile chronic appendicitis was found in 38 patients (6%). Of 650 patients, 200 patients had complications; 193 patients (30%) with perforated appendicitis and 7 patients (1%) with appendicular mass. The most frequent age group to develop appendicitis was 20-29 years. The number of male patients was higher than the females. Peradangan akut pada apendiks memerlukan tindak bedah segera untuk mencegah komplikasi yang umumnya berbahaya. Insidens pada perempuan dan laki-laki umumnya sebanding, kecuali pada umur 20-30 tahun insidens pada laki-laki lebih tinggi. Keputusan klinis mendasar dalam mendiagnosis pasien dengan dugaan apendisitis ialah apakah perlu dilakukannya operasi atau tidak.  Evaluasi yang baik dari kasus apendisitis akut dapat mengurangi intervensi untuk operasi awal, dengan harapan dapat mengurangi risiko operasi yang tidak diperlukan. Penelitian ini bertujuan untuk mengetahui angka kejadian apendisitis di RSUP Prof. Dr. R. D, Kandou Manado periode Oktober 2012 – September 2015. Metode penelitian yang digunakan ialah deskriptif retrospektif dengan menggunakan data di Bagian Rekam Medik RSUP Prof. Dr. R. D. Kandou Manado. Hasil penelitian menunjukkan bahwa selama periode Oktober 2012 – September 2015 terdapat 650 pasien. Jumlah pasien terbanyak ialah apendisitis akut yaitu 412 pasien (63%) sedangkan apendisitis kronik sebanyak 38 pasien (6%). Dari 650 pasien, yang mengalami komplikasi sebanyak 200 pasien yang terdiri dari 193 pasien (30%) dengan komplikasi apendisitis perforasi dan 7 pasien (1%) dengan periapendikuler infiltrat. Kelompok umur tersering yang menderita apendisitis ialah 20-29 tahun. Jumlah pasien laki-laki lebih banyak daripada perempuan. Kata kunci: apendisitis, angka kejadian

In pediatric patients appendicitis is the most common cause of abdominal pain and surgery. Torsion of vermiform appendix is a rare cause, clinically indistinguishable from appendicitis with usually an intraoperative diagnosis. The first description of vermiform appendix torsion was made by Payne in 1918. Clinical presentation is similar to acute appendicitis. Preoperative investigations play a minimal role. Etiology of this condition is unclear, but is possible to distinguish a primary and a secondary torsion. We report a case of 5-years-old boy who presented with right lower quadrant abdominal pain. His clinical signs, symptoms and investigations mimicked an acute appendicitis. Intraoperatively we found a 720° appendix torsion on its base with its mesentery rotated in counter-clockwise direction. The appendix was gangrenous in appearance. A video-assisted trans-umbilical appendectomy was performed. We describe clinical presentation and management of this rare condition reviewing the literature.

Abstract Background Abdominal pain and other gastrointestinal symptoms are common presenting features of multisystem inflammatory syndrome in children (MIS-C) and can overlap with infectious or inflammatory abdominal conditions, making accurate diagnosis challenging. Case Presentation We describe the case of a 16-year-old female who presented with clinical symptoms suggestive of appendicitis and an abdominal computed tomography (CT) that revealed features concerning for appendicitis. After laparoscopic appendectomy, histopathology of the appendix demonstrated only mild serosal inflammation and was not consistent with acute appendicitis. Her overall clinical presentation was felt to be consistent with MIS-C and she subsequently improved with immunomodulatory and steroid treatment. Conclusions We note that MIS-C can mimic acute appendicitis. This case highlights MIS-C as a cause of abdominal imaging with features concerning for appendicitis, and MIS-C should be considered in the differential for a patient with appendicitis-like symptoms and a positive COVID-19 IgG. Lab criteria, specifically low-normal white blood cell count and thrombocytopenia, appears to be of high relevance in differing MIS-C from acute appendicitis, even when appendix radiologically is dilated.

Acute appendicitis is the most common acute surgical infection during pregnancy. Although usually pyogenic in origin, parasitic infections account for a small percentage of cases. Despite the relatively high prevalence of acute appendicitis in our environment, it is not commonly associated with schistosomiasis. We report here the association of pregnancy and appendicitis caused by Schistosoma haematobium. Schistosomiasis is very common complication of pregnancy in hyperendemic areas. Schistosome egg masses can lodge throughout the body and cause acute inflammation of the appendix, liver and spleen. Congestion of pelvic vessels during pregnancy facilitates passage of eggs into the villi and intervillous spaces, causing an inflammatory reaction. Tourism and immigration make this disease a potential challenge for practitioners everywhere.

Potential Role of Cytokines in Children with Acute Appendicitis and Acute Mesenteric Lymphadenitis Although, AAP and AML have different etiological factors, clinical symptoms are very much similar but treatment tactics in both the disease differ a lot. In case of AML, treatment is more conservative and does not require hospitalization while in case of AAP immediate hospitalization and maybe further surgery can be mandatory. With the identification of group of cytokines serum inflammatory mediators IL-8, IL-10, IL-12[p70], IL-17, TNF-a and MCP-1, it is believed early and proper diagnosis of AAP in the near future. Research of cytokines-serum inflammatory mediators has opened new opportunities for an early detection and differentiation of these two diseases in children.

Acute pancreatitis (AP) is an acute inflammation of the pancreas, usually occurs suddenly with a variety of clinical symptoms, complications of multiple organ failure and high mortality rates. Objectives: To determine the value of combination of HAP score and BISAP score in predicting the severity of acute pancreatitis of the Atlanta 2012 Classification. Patients and Methods: 75 patients of acute pancreatitis hospitalized at Hue Central Hospital between March 2017 and July 2018; HAP and BISHAP score is calculated within the first 24 hours. The severity of AP was classified by the revised Atlanta criteria 2012. Results: When combining the HAP and BISAP scores in predicting the severity of acute pancreatitis, the area under the ROC curve was 0,923 with sensitivity value was 66.7%, specificity value was 97.1%; positive predictive value was 66.7%, negative predictive value was 97.1%. Conclusion: The combination of HAP and BISAP scores increased the sensitivity, predictive value, and prognostic value in predicting the severity of acute pancreatitis of the revised Atlanta 2012 classification in compare to each single scores. Abstract De Garengeot hernia is a rare subtype of femoral hernia whereby the vermiform appendix is located within the hernial sac. Even rarer is the presence of appendicitis within the hernia sac. De Garengeot’s hernia is difficult to diagnose pre-operatively and can prove technically difficult at operation particularly with regards to mobilization of the caecum and appendix in order to perform appendicectomy. Laparoscopic, open, with and without mesh repair of de Garengeot hernia have all been described in the literature with varying degrees of success. We present a case of an 82 year old lady presenting with an acutely painful right sided groin lump. CT scan revealed the presence of de Garengeot hernia with acute appendicitis. We describe in text and photo format our approach to the hernia repair, appendicectomy and provide a short review of the literature with regards to the different operative approaches to such a patient.
