The compromised tooth structure after endodontic treatment necessitates the use of post and core for structural durability. Management of the flared root canal is still a tough obstacle for the clinicians. Reinforcing the root canal space with the customised anatomic post thus regains the lost structure within the canal and aids in establishing a great strength and stability to the tooth structure. The report of this case scenario aims on clinical technique of fabricating anatomic post with glass - fibre posts and composite resin on a central and lateral incisor.

ABSTRACT Loss of tooth structure occurs due to caries, endodontic treatment, or fracture caused by trauma; restoring these teeth is difficult task for a clinician. Post and core is an option for teeth when excessive tooth structure is lost and teeth need to be restored with crown and bridge procedure. They can be prefabricated post with composite built up or a one-piece custom-made post. The custom-made cast metal post and core have disadvantage due to their color that imparts grayish hue to overlying ceramic restorations. Masking the metal core can enhance the esthetic of all ceramic restorations that are placed over the cast post and core. ABSTRACTBackground: Broken instruments affect the outcome and prognosis. A broken instrument will prevent the procedure of cleaning and shaping, irrigation and obturation of the root canal on the obstructed root section causing failure in endodontic treatment. There are some treatments for broken instrument including retrieving a broken fragment file and bypassing it on the root canal. the aim of the case report aims to provide an alternative for retrieving broken instruments using the H-file braiding technique. Method: A 38-year-old female patient came to the Dental Conservation Clinic RSGM Prof. Soedomo FKG UGM. A patient got a history of initial root canal treatment in a clinic, which ended with a separated file on canine teeth left upper. No complain pain, from the investigation radiographic there is an instrument broken in half apical. The teeth 23 seen temporary restoration cavity class I is still in good. Retrieving broken instruments using the H-file braiding technique, preparation with step back techniques, obturation with warm vertical condensation technique and class I composite resin restorations with fiber-reinforced retention. Result: No complain pain after 2 weeks retrieving broken instrument.Conclusion: The technique of the H-file braiding technique is one technique to retrieve a broken instrument, this procedure is simple, cost-effective, harmless to the teeth and gives root canal sealing.

ABSTRACTThis case report describes the repositioning of a laterally luxated primary central incisor with occlusal interference, using a composite inclined plane. The patient was a 4-year-old girl who applied to our clinic three days after the injury. Because of the time delay between injury and presentation, it was not possible to reposition the tooth with pressure. Following a root-canal treatment, an inclined plane was prepared on the lower primary incisors, using composite resin. The tooth was repositioned in two weeks, and the inclined plane was then removed. After 1 year of follow-up, the treatment was found to be successful, both clinically and radiographically. The use of a composite inclined plane, accompanied by careful follow-up, is an effective alternative to extraction for laterally luxated primary incisors with occlusal interference. ABSTRACT Self-introduced foreign object by an adult into the root canal of a lower incisor is seldom reported in the endodontic literature. This case report describes a self-introduced unusual foreign body and its retrieval from the root canal of lower left lateral incisor. A persistent sinus tract resistant to calcium hydroxide intracanal medicament was then successfully treated with the use of triple antibiotic paste. Postendodontically, nonvital bleaching and composite resin restoration was used to restore the form, function and esthetics of the tooth in a conservative manner. Selama prosedur preparasi saluran akar, kemungkinan patahnya instrumen selalu ada. Saat ini instrumen yang patah dapat dikeluarkan dengan menggunakan alat ultrasonik seperti jarum Miller yang dihubungkan dengan tip ultrasonik endo, selain itu diperlukan juga akses dan visibilitas yang baik sehingga memudahkan operator untuk mengeluarkan instrumen yang patah tersebut. Tujuan laporan kasus ini adalah untuk melaporkan keberhasilan pengambilan  lentulo patah di dalam saluran akar menggunakan jarum miller yang dihubungkan dengan tip ultrasonik endo dikombinasikan dengan hedstroem no 25. Dalam makalah ini dilaporkan satu kasus perawatan saluran akar gigi molar satu kiri bawah pulpitis irreversibel pada pasien wanita 20 tahun, namun terjadi patah lentulo saat pengaplikasian bahan sterilisasi saluran akar. Pengambilan lentulo berhasil dilakukan pada kunjungan kedua dengan menggunakan jarum miller yang dihubungkan dengan tip ultrasonik endo serta hedstroem file no 25. Visibilitas didapatkan dengan melakukan coronal flaring menggunakan heroshaper dari mikro mega yang dihubungkan dengan alat rotary. Setelah 1 minggu, gigi diobturasi dengan teknik single cone pada saluran akar distal dan teknik kondensasi lateral pada saluran akar mesiobukal dan mesiolingual. Pada kunjungan berikutnya, gigi direstorasi dengan resin komposit disertai pasak dentatus screw. Setelah 2 bulan diamati secara radiografis dan klinis, tidak ada keluhan dari pasien.Broken Lentulo Removal During Root Canal Treatment On The First Molar Mandible Sinistra With Pulp Necrosis. During root canal preparation procedure, there is always potential for instrument breakage. Nowadays, broken instruments can be removed using ultrasonic instruments such as a needle miller connected to endo ultrasonic tip, but it needs good access and visibility in order to make it easier for the operator to remove the broken instruments. The aim of this case report is to present the successful removal of a broken lentulo left in a root canal by using smooth broach connected to a ultrasonic endo tip combined with an hedstroem file no 25. This paper reports a case of molar root canal treatment of the lower left irreversible pulpitis in 20 year-old female patient, but the incident of broken lentulo occured while applying root canal medicament. The effort to remove lentulo was successful on the second visits using a needle miller connected to endo ultrasonic tip and headstrom file no 25. Visibility was obtained by using a coronal flaring of micro mega hero shaper associated with the rotary tool. In the following week, the teeth was obturated with single cone technique on the distal root canal applying the lateral condensation technique on mesiolingual and mesiobuccal root canal. In the next visit, the teeth were restored with composite resin with dowel dentatus screw. After 2 months of being observed radiographically and clinically, there is no more complaint from the patient

A grossly decayed tooth sometime poses difficulty in fabrication of FDP, The post core is often required to gain support from the remaining tooth structure, In a multirooted tooth where more then one root were used as support the path of insertion of post is difficult, in those cases a custom split post and core has to be fabricated. This case report describes a simple and scientific multidisciplinary approach towards management of grossly decayed tooth. The technique seems to be effective for extensively damaged teeth that lack sufficient tooth structure to create an adequate ferrule of 1.5 to 2 mm for the final crown.

Dens evaginatus is an uncommon developmental anomaly of human dentition characterized by the presence of tubercle on the occlusal surface of mandibular premolars and lingual surface of anterior teeth. Due to occlusal trauma this tubercle tends to fracture thus exposing the pathway to the pulp chamber of teeth. This case report is about the presentation of dens evaginatus in mandibular premolars bilaterally; among them tooth 44 was associated with chronic apical periodontitis. Fractured tubercle of three premolars was sealed with composite resin. Root canal treatment was performed with tooth 44. Routine endodontic treatment did not result in remission of infection. Therefore, culture and sensitivity tests were performed to identify the cause and modify treatment plan accordingly. Triple antibiotic paste was used as an intracanal medicament to disinfect the root canal that resulted in remission of infection.

Background: Trauma to the anterior teeth is common in young children and in adolescents. Uncomplicated crown fracture to the permanent teeth has an intense effect not only on the patient’s esthetics, but also on speech and function. Aim: Performing an economical, a minimally invasive, time-saving, long lasting, functional and natural alike direct composite resin restorations in a single visit. Conclusion: As restoring a fractured tooth structure is a complex procedure, this technique can prove to be a simple, effective and appropriate technique that will fulfill all the requirements of dental practioner without requiring special skills in providing the patients with direct composite restorations. Clinical Significance: This technique is economical which requires less chair side time compared to indirect restorations.

Introduction. Knowledge of root canal morphology is one of the main prerequisites for successful endodontic treatment. We present the case of a patient with multiple canals in lower anterior teeth which were treated endodontically. Case Report. To a 45-year-old patient endodontic treatment was applied on lower left lateral incisor, lower right central and lateral incisor and lower right canine. These teeth were previously prepared for prosthetic crowns. All involved teeth had two canals which were not visible on the primary radiographs. Gracility of remaining tooth structure and canals and the presence of two canals in each tooth required additional radiographs for verification of multiple canals. Careful biomechanical instrumentation and irrigation with use of lubrication due to narrow canals was performed. The canals were obturated with gutta-percha using lateral compaction technique and Acroseal (Septodont) paste. On the control checkup after eight months, the patient was asymptomatic, while clinical finding was normal. Conclusion. Complexity of presented case was even more significant because these teeth were prepared to support prosthetic superstructure and crowns, and every ?forgotten? canal is an iatrogenic error.

Abstract Aim The purpose of this case report is to describe the clinical procedure for fabricating fiber-reinforced composite (FRC) onlay composite resin restorations using a FRC as the substructure. Background A variety of therapeutic modalities are available to restore teeth with moderate coronal defects in the posterior region of the mouth. For patients who refuse complete crown restorations or when minimal tooth reduction is preferred, a FRC restoration can be a good alternative to conventional restorative techniques. Case Report A 42-year-old female patient presented with an endodontically treated mandibular right first molar with extensive destruction of the coronal tooth structure. To conserve the remaining tooth structure a FRC resin core substructure was fabricated and veneered with Ceramage dentin and enamel hybrid composite to create the final restoration. Summary FRC restorations using adhesive technology appears to be a promising restorative option. However, further clinical investigation will be required to provide additional information on this technique. Based on the clinical and radiographic findings in the present case, the fabrication of a conventional crown was avoided in order to conserve the remaining tooth structure. Clinical Significance The restoration of badly damaged teeth is a challenge for clinicians when cast crown restorations are not an option for the patient.
