Kata kunci:Gagal jantung, kualitas hidup, motivasi, self care AbstractIn general, the quality of life is subjective. It based on individual perceptions related to the ability of individual to keep their body healthy. The changes of physiological and chronical health conditions have influenced to the quality of life especially in heart failure patients. This study aimed to identify the relationship of self-care, motivation, and quality of life in heart failure patients. This study applied a cross sectional design. The samples were 73 subjects recruited by purposive sampling. Self-care was measured by self care of heart failure index (SCHFI), motivation was measured using a questionnaire. Quality o life (QoL) was assessed using minnesota living with heart failure questionnaire (MLHFQ). The data were analyzed using chi square test and set at p<0.05. The findings have shown that 41 (56.2%) of subjects performed a poor self-care, 42 (57.5%) of subjects had low motivation, and 40 (54.8%) of cardiac patient’s quality of life were poor. There were significant relationships among self-care, motivation, and the quality of life of heart failure patients. There were significant relationship between practices of self-care and QoL (p=0,001) and OR=6.00. The relationship between motivation and QoL were significant (p=0.009) and OR=4.056 . This research can be concluded that the self-care and motivation had contribute positively to QoL in heart failure patients. It can be suggested that cardiac patients should pay attention to their health condition and improving QoL through self care, support and motivation.Key words: Heart failure, motivation, quality of life, self care

ABSTRACTHeart failure (HF) is one of the most prevalent cardiovascular diseases in the world which associated with significant mortality, morbidity, and huge financial burden, particularly among aging population. Rehospitalization remain a concern in the care of the heart failure patient which largerly associated with delay in symptoms recognition, treatment, diet non-compliance and also lack of knowledge and skills in heart failure self-care. Guideline on HF emphasize the important of self care in preventing symptoms and hospital readmission. This study aims to describe self-care behavior in a sample of heart failure patients. This is a descriptive cross-sectional study with non-probabilistic sample of 74 in an out-patient heart clinic from June-July 2018. Self-care was measured using the Self-Care of Heart Failure Index (SCHFI), (scores ≥70 points=adequate self-care). Chi-square test was conducted to test whether participant’s characteristics associated with self-care behavior among heart failure patients in three scales. The result shows that self-care was inadequate in most scales (self-care maintenance: 43.4±11.8, self-care management: 49.4±18.5 and self-care confidence: 68,6±14.5. Low frequencies of participants with adequate self-care also observed in the study (self-care maintenance: 5.4%, self-care management: 15.4%) and self-care confidence: 36.5%)). In self-care confidence subscale, higher scores of the SCHFI were associated with no comorbid conditions (p=0.01). None of the sociodemographic and clinical characteristics of the sample were associated with increased self-care in other 2 sub-scales. In conclusion, study shows that self-care in HF is inadequate, interventions aimed at improving self-care espescially in self-care maintenance sub-scale are greatly needed. Additional studies are needed to determine the best approach for this.

ABSTRACT Objective: To analyze sociodemographic and clinical characteristics, depressive symptoms and quality of life of patients with heart failure and associate quality of life with depressive symptoms. Method: A cross-sectional study conducted with outpatients and inpatients. Sociodemographic data were collected and questionnaires were applied to assess quality of life (Minnesota Living with Heart Failure Questionnaire) and depressive symptoms (Beck Depression Inventory). Results: The sample consisted of 113 patients. Outpatients were retired (p=0.004), with better education (p=0.034) and higher ventricular ejection fraction (p=0.001). The inpatient group had greater depressive symptoms (18.1±10 vs 14.6±1.3; p=0.036) and lower quality of life (74.1±18.7 vs 40.5±3.4; p<0.001) than the outpatient group. Outpatients with depressive symptom scores from 18 points had worse quality of life scores in 17 of the 21 questions. Conclusion: Inpatients had worse depressive symptoms and quality of life, which was more affected in the physical dimension in those with moderate/severe depressive symptoms. Outpatients with more severe depressive symptoms had worse quality of life in all dimensions.

Long term of heart failure symptoms will affect the patient functional status and ability to do self-care appropriately. Decreasing functional status and self-care ability will certainly affect the quality of life of patients. This study aimed to determine the relationship of self-care with the quality of life of heart failure patients that undergoing treatment at Heart Polyclinic of Mangusada Hospital. The research design was descriptive correlational using cross sectional approach. The study was conducted in January-February 2019. Total respondent participated were 61 respondents and were taken using a purposive sampling technique. Self care of heart index (SCHFI) questionnaire and the Minnesota Living With Heart Failure Questionnaire (MLHFQ) were used. The results showed that the average of self-care score was 45.25 and the average score of the quality of life was 60.67. Analysis using the Spearman Rank test obtained p-value = 0,000 and r = 0.506. It can be concluded that self care had a significant correlation on quality of life patients with heart failure in Mangusada Hospital. We recommended that nursing service providers play an active role in providing education and motivation to heart failure patients to improve self-care abilities.

Background: The purpose of this systematic review was to (a) examine the effects of interventions delivered by a heart failure professional for mild cognitive impairment and dementia on cognitive function, memory, working memory, instrumental activities of daily living, heart failure knowledge, self-care, quality of life and depression; and (b) identify the successful elements of these strategies for heart failure patients with mild cognitive impairment or dementia. Methods and results: During March 2018, an electronic search of databases including CINAHL, MEDLINE, EMBASE and PsycINFO was conducted. All randomised controlled trials, which examined an intervention strategy to help heart failure patients with mild cognitive impairment or dementia cope with self-care, were included. An initial search yielded 1622 citations, six studies were included ( N= 595 participants, mean age 68 years). There were no significant improvements in cognitive function and depression. However, significant improvements were seen in memory ( p=0.015), working memory ( p=0.029) and instrumental activities of daily living ( p=0.006). Nurse led interventions improved the patient’s heart failure knowledge ( p=0.001), self-care ( p<0.05) and quality of life ( p=0.029). Key elements of these interventions include brain exercises, for example, syllable stacks, individualised assessment and customised education, personalised self-care schedule development, interactive problem-solving training on scenarios and association techniques to prompt self-care activities. Conclusions: Modest evidence for nurse led interventions among heart failure patients with mild cognitive impairment or dementia was identified. These results must be interpreted with caution in light of the limited number of available included studies.
