ABSTRAKGagal jantung merupakan salah satu penyakit kardiovaskuler yang paling sering terjadi di seluruh dunia yang mengakibatkan tingginya angka mortalitas, morbiditas dan juga berdampak secara finansial terutama bagi lanjut usia. Rehospitalisasi merupakan masalah umum yang sering terjadi pada pasien gagal jantung yang sebagain besar disebabkan oleh keterlambatan dalam pengenalan gejala, pengobatan dan ketidakpatuhan diet serta kurangnya pengetahuan dan keterampilan dalam melakukan perawatan diri. Panduan penanganan gagal jantung menekankan pentingnya perilaku perawatan diri untuk menurunkan kekambuhan dan rehospitalisasi pada pasien gagal jantung. Penelitian ini bertujuan untuk menggambarkan perilaku perawatan diri pada pasien gagal jantung yang mengunjungi poliklinik jantung di RSUD Panembahan Senopati Bantul. Penelitian ini merupakan penelitian deskriptif quantitatif dengan menggunakan pendekatan cross sectional. Sampel berjumlah 74 responden yang merupakan pasien rawat jalan di poliklinik jantung pada bulan Juni hingga Juli 2018. Alat ukur yang digunakan adalah Self-Care of Heart Failure Index (SCHFI), (skor ≥70 poin=adekuat). Uji statistik chi-square digunakan untuk mengetahui hubungan antara factor sosiodemografi dengan perilaku perawatan diri pada pasien gagal jantung. Hasil penelitian ini menunjukkan bahwa perilaku perawatan diri pada sebagian besar responden masih belum adekuat dengan frekuensi perilaku perawatan diri adekuat yang masih rendah (pemeliharaan: 45±13.1 (7.7%), pengelolaan: 46±20.4 (13.0%) dan kepercayaan: 69±16.5 (38.5%)). Hanya satu faktor yaitu penyakit penyerta yang berhubungan dengan perilaku perawatan diri pada pasien gagal jantung pada dimensi kepercayaan diri. Hasil penelitian ini menunjukkan bahwa sebagian besar responden memiliki perilaku perawatan diri yang tidak adekuat seperti juga pada negara-negara lainnya sehingga diperlukan upaya untuk meningkatkan perilaku perawatan diri pada pasien gagal jantung. ABSTRACTHeart failure (HF) is one of the most prevalent cardiovascular diseases in the world which associated with significant mortality, morbidity, and huge financial burden, particularly among aging population. Rehospitalization remain a concern in the care of the heart failure patient which largerly associated with delay in symptoms recognition, treatment, diet non-compliance and also lack of knowledge and skills in heart failure self-care. Guideline on HF emphasize the important of self care in preventing symptoms and hospital readmission. This study aims to describe self-care behavior in a sample of heart failure patients. This is a descriptive cross-sectional study with non-probabilistic sample of 74 in an out-patient heart clinic from June-July 2018. Self-care was measured using the Self-Care of Heart Failure Index (SCHFI), (scores ≥70 points=adequate self-care). Chi-square test was conducted to test whether participant’s characteristics associated with self-care behavior among heart failure patients in three scales. The result shows that self-care was inadequate in most scales (self-care maintenance: 43.4±11.8, self-care management: 49.4±18.5 and self-care confidence: 68,6±14.5. Low frequencies of participants with adequate self-care also observed in the study (self-care maintenance: 5.4%, self-care management: 15.4%) and self-care confidence: 36.5%)). In self-care confidence subscale, higher scores of the SCHFI were associated with no comorbid conditions (p=0.01). None of the sociodemographic and clinical characteristics of the sample were associated with increased self-care in other 2 sub-scales. In conclusion, study shows that self-care in HF is inadequate, interventions aimed at improving self-care espescially in self-care maintenance sub-scale are greatly needed. Additional studies are needed to determine the best approach for this.

Abstract Background Caregivers are major contributor to the self-care of patients with heart failure. The Caregiver Contribution to Self-Care of Heart Failure Index (CC-SCHFI) measures these contributions across three scales: self-care maintenance (symptom monitoring and treatment adherence); self-care management (dealing with symptoms); and confidence in contributing to the self-care (self-efficacy in managing self-care) of patients with heart failure. Informal caregivers play a vital role in supporting family members with heart failure in Thailand, yet no validated tool exists to measure their contribution. We examined the psychometric properties of the CC-SCHFI in a Thai population. Methods The CC-SCHFI was translated into Thai using a standard forward and backward translation procedure. A cross-sectional design was used to examine the psychometric properties of the Thai version of the CC-SCHFI in 100 family caregivers of heart failure patients in Southern Thailand. Confirmatory factor analysis was used to assess construct validity, and factor score determinacy coefficients were computed to evaluate internal consistency reliability. Results The Thai version of the CC-SCHFI demonstrated acceptable internal consistency (composite reliability of each scale ranged from 0.76 to 0.99). Reliability estimates were adequate for each scale (McDonald’s omega ranged from 0.75 to 0.96). Confirmatory factor analysis supported the original factor structure of the instrument, with good fit indices for all three scales (comparative fit index = 0.98–1.00; root-mean-square error of approximation = 0.00–0.07). Conclusions The Thai version of the CC-SCHFI appears to be a valid and reliable instrument for measuring caregiver contributions to self-care maintenance and self-care management as well as contributing to caregiver confidence in the self-care of Thai heart failure patients.

The purpose of this study was to assess the prevalence of self-care behavior and associated factors among heart failure patients. This institutional-based cross-sectional study design was done in Tigray region, Ethiopia. The data collection process was made between February and April 2018. An interviewer assisted structured questionnaire and medical chart review was used to collect the data. Data was entered and analyzed using Statistical Package for the Social Science (SPSS) version 22. Bivariate and multivariable logistic regression were conducted to identify the predictors ( p < .05). In this study, a total of 408 study subjects was included. The mean age of the participants was 45.4 (±19). The overall prevalence of good self-care behavior was 45.8% (95% CI: 40.9–50.5). Regression analysis revealed that age, educational level, co-morbidity, knowledge about heart failure, and social support were significantly associated with self-care behavior. Healthcare providers should strengthen patient education to improve self-care behavior.

Kualitas hidup secara umum bersifat subjektif dan bervariasi sesuai dengan persepsi individu terhadap kesehatan dan kemampuan untuk mempertahankannya. Adanya perubahan fisiologis dan kondisi kronis terhadap kesehatan sangat berpengaruh terhadap kualitas hidup seseorang khususnya pasien gagal jantung. Penelitian ini bertujuan untuk mengidentifikasi hubungan self caredan motivasi terhadap kualitas hidup pasien gagal jantung. Penelitian ini menggunakan metode analitis dengan desain studi potong lintang. Jumlah sampel penelitian sebanyak 73 sampel yang diambil secara purposive sampling. Self carediukur dengan self care of heart failure index (SCHFI), motivasi diukur dengan kuesioner yang dimodifikasi, dan kualitas hidup menggunakan minnesota living with heart failure questionnaire (MLHFQ). Uji statistik dilakukan dengan chi squaresehingga dihasilkan nilai p<0.05. Hasil penelitian didapatkan bahwa 41 (56,2%) responden memiliki self care yang kurang baik. Sebanyak 42 (57,5%) responden mempunyai motivasi yang rendah dan sebanyak 40 (54,8%) responden memiliki kualitas hidup pasien jantung yang kurang baik. Pada penelitian ini terdapat hubungan yang bermakna antara self caredan motivasi terhadap kualitas hidup pasien jantung. Hubungan yang signifikan antara self caredan kualitas hidup (p=0.001;) dan OR=6,000. Hubungan yang signifikan antara motivasi dan kualitas hidup (p=0.009) dan OR=4,056. Penelitian ini dapat disimpulkan bahwa self caredan motivasi berhubungan dengan kualitas hidup pasien jantung. Dapat disarankan untuk pasien gagal jantung untuk lebih memperhatikan kesehatan dan meningkatkan kualitas hidup melalui perawatan diri, dukungan, dan motivasi.Kata kunci:Gagal jantung, kualitas hidup, motivasi, self care AbstractIn general, the quality of life is subjective. It based on individual perceptions related to the ability of individual to keep their body healthy. The changes of physiological and chronical health conditions have influenced to the quality of life especially in heart failure patients. This study aimed to identify the relationship of self-care, motivation, and quality of life in heart failure patients. This study applied a cross sectional design. The samples were 73 subjects recruited by purposive sampling. Self-care was measured by self care of heart failure index (SCHFI), motivation was measured using a questionnaire. Quality o life (QoL) was assessed using minnesota living with heart failure questionnaire (MLHFQ). The data were analyzed using chi square test and set at p<0.05. The findings have shown that 41 (56.2%) of subjects performed a poor self-care, 42 (57.5%) of subjects had low motivation, and 40 (54.8%) of cardiac patient’s quality of life were poor. There were significant relationships among self-care, motivation, and the quality of life of heart failure patients. There were significant relationship between practices of self-care and QoL (p=0,001) and OR=6.00. The relationship between motivation and QoL were significant (p=0.009) and OR=4.056 . This research can be concluded that the self-care and motivation had contribute positively to QoL in heart failure patients. It can be suggested that cardiac patients should pay attention to their health condition and improving QoL through self care, support and motivation.Key words: Heart failure, motivation, quality of life, self care

AbstractObjective: to describe self-care behavior and its associated factors in a sample of heart failure Brazilian patients.Method: descriptive cross-sectional study with non-probabilistic sample of 116 ambulatory patients undergoing heart failure treatment. Self-care was evaluated using the Self-Care of Heart Failure Index, (scores ≥70 points=appropriate self-care). Association tests were applied, considering a descriptive level of 0.05.Results: the mean age of participants was 57.7 (SD =11.3) years; 54.3% were male; the mean schooling was 5.5 (SD = 4.0) years; and 74.1% had functional class II-III. The mean scores on the subscales of the Self-Care of Heart Failure Index indicated inappropriate self-care (self-care maintenance: 53.2 (SD =14.3), selfcare management: 50.0 (SD = 20.3) and self-care confidence: 52.6 (SD=22.7)) and it was found low frequencies of participants with appropriate self-care (self-care maintenance, 6.9%), self-care management (14.7%) and self-care confidence (19%). Higher scores of the Self-Care of Heart Failure Index were associated with: reduced left ventricular ejection fraction (p=0.001), longer time of experience with the disease (p=0.05) and joint monitoring by physician and nurse (p=0.007).Conclusion: investments are needed to improve the self-care behavior and the nursing can play a relevant role in this improvement.

Mutuality in patient–caregiver dyad may improve heart failure (HF) patient self-care and caregiver contribution to self-care, but literature is scarce. We evaluated the influence of mutuality and its dimensions on patient–caregiver dyad self-care. A sample of 366 HF patient–caregiver dyads was enrolled. The Actor–Partner Interdependence Model was used to identify the influence of individual patient and caregiver mutuality on their own self-care (actor effect) and on partner self-care (partner effect). The total mutuality score had an actor effect on patient self-care maintenance (i.e., behaviors aimed at maintaining HF stability) and on patient and caregiver self-care confidence. Total score of patient mutuality also had a partner effect on caregiver self-care management (i.e., responses to symptoms of HF exacerbation). Specific mutuality dimensions had different actor and partner effects on patient and caregiver self-care. Interventions to improve mutuality in HF patient–caregiver dyads may influence patient self-care and caregiver contribution to self-care.

Background: Patients with heart failure need to be engaged in adequate cardiac self-care behaviors helping to prevent the development of the disease and ameliorate their health status. However, the conceptual model of the present study has not been tested in previous studies among patients with heart failure. Objectives: The present study aimed to investigate the psychosocial determinants based on Pender’s health promotion model (HPM) affecting self-care behavior among outpatients suffering from heart failure. Methods: In this cross-sectional study, a total of 200 patients suffering from heart failure were selected from the outpatient clinics of Tabriz, Iran, using convenience sampling and assessed for self-self-care behaviors and major concepts of HPM via self-administered questionnaires. Path analysis was used in order to analyze the conceptual model Results: The present hypothetical model showed a good fit. Perceived benefits and activity-related effects directly affected self-care behaviors. Bootstrapping mediation analyses indicated that perceived self-efficacy, perceived barriers, perceived benefits, and activity-related effects indirectly affected self-care behaviors through commitment to action. Conclusions: The commitment to action can help to promote self-care behaviors among the subjects suffering from heart failure. The interventions should take the role of predictive variables of this study and commitment to action into account in these patients.

Background & Aim: Self-care in patients with heart failure can improve multiple outcomes and reduce mortality. Measuring self-care requires valid and reliable tools. This study aimed to determine the psychometric properties of the 9-items European Heart Failure Self-Care Behavior Scale. Methods & Materials: The present study is a psychometric evaluation. The original version of the tool was translated from English to Persian using the standard Backward-Forward method. The questionnaire's validity was assessed using face, content, criterion (the Self-Care of Heart Failure Index v.6 as a criterion), and construct validity.  An exploratory factor analysis approach was used to construct validity with a sample of 216 patients with chronic heart failure referred to Dr. Shariati and Imam Khomeini Hospital's heart clinics in Tehran; and Ayatollah Mousavi Hospital in Zanjan. The reliability of the questionnaire was evaluated by internal consistency and stability methods. Results: After the validation process (validity and reliability), all 9 items of the questionnaire were approved and remained. The content validity index of the total content of the questionnaire was calculated to be 0.96. Based on factor analysis, only one factor was confirmed for the questionnaire. The correlation between the present questionnaire and the Self-Care of Heart Failure Index v.6 was positive and significant (p<0.001, r=0.753). Internal consistency (α=0.728) and stability (0.897) of the questionnaire were also confirmed. Conclusion: The European Heart Failure Self-Care Behavior Scale is valid and reliable for measuring Iranian patients' self-care behaviors with heart failure.
