Abstract Quality of sleep directly impacts quality of life, whilst lifestyle significantly impacts night rest. Cigarette smoking is a serious threat to overall health, including sleep. Nicotine in cigarettes affects the nervous system, whilst the respiratory system is impacted by carcinogenic substances, such as dusts and tarred substances, inhaled with smoke. Cigarettes are assumed to be a risk factor in sleep disorders, including breathing sleep disorders. This study’s aim was to analyze the impact of cigarette smoking on adult men’s polysomnographic parameters. Polysomnographic records for 94 adult men were obtained from the polysomnography laboratory in Opolskie Province, Poland. Additionally co-morbidities, height, weight and frequency of smoking were also noted. Three groups of men were categorised according to the frequency with which they smoked: non-smokers, smoking less than a pack a day, smoking more than a pack a day. Compared to non-smokers and mild smokers, men who were heavy smokers also exhibited the longest sleepless time (H=8.11; p=0.017), the maximum waking time following the onset of sleep (H=7.99; p= 0.018), the lowest sleep efficiency across the three groups (H=7.96; p=0.019), the greatest number of apnea events per hour of sleep (H=6.23; p= 0.045), the lowest Oxygen Level Nadir (H=11.44; p= 0.003) and the highest rate of limb movements per hour of sleep (H=9.81; p=0.007). The dose effect was identified, which correlated more cigarettes men smoked per day with lower sleep quality.

Background: Night time reux has been shown to be associated with fragmented sleep. However, few studies have assessed the quality of sleep on gastroesophageal reux and the impact of gastroesophageal reux on reported quality of sleep and quality of sleep on gastroesophageal reux. The aims of this study were to evaluate the quality of sleep and other parameters in patients with gastroesophageal reux disease.50 Subjects with typical GERD symptoms ≥3 times a week and All subjects were administered 3 questionnaires: PSQI , ESS, GERD-HRQL . All the subjects underwent nocturnal polysomnography and completed a all 3 questionnaire before NPSG. Results: Overall ,the mean percentage of N1 was 9.10±9.74 ,N2 was 83.97±13.81 , N3 was 3.44±4.16 , N4 was 0.60±2.04 , REM was 2.14±3.79 , PSQI was 12.48±1.23 , ESS was 11.80±0.76 , TST (hr) was 5.19±0.74 , SPT(hr) was 6.58±0.77 , sleep efciency(%) was 52.50±27.16 , Latency (mint) was 22.79±30.85 , REM Latency (mints) was 96.99±151.86 ,and Microarousal index was 36.05±25.93 . To assess the potential impact of sleep quality via nocturnal polysomnography on severity of gastroesophageal reux , we performed correlations between the GERD questionnaire and nocturnal polysomnography reports . We observed that PSQI(r= -0.285, p<0.045) , ESS(r=0.206, p=0.05) , N1(r=0.202 , p>0.160) , N2(r=- 0.045 ,p>0.758) , N3(r=-0.079, p>0.583) , N4(r-0.209 ,p>0.145),REM(r=0.045 ,p>0.756) , TST(r=0.036 ,p>0.803), SPT(r=0.015,p>0.917) , Sleep Efciency (r=-0.113,p>0.435) , Sleep Latency (r=-0.045 ,p>0.756), REM Latency (r=0.165, p >0.253), Microarousal index (r=0.058 , p>0.683). Conclusions: Gastroesophageal reux disease and sleep disorders are extremely prevalent conditions, and it seems intuitive that there must be some overlap between the two. Sleep disorders may in fact be one of the most prevalent of the extraesophageal complications of GERD and often goes unrecognized.

Healthy sleep is not only the most important source of restoring the body's strength, but also a way to overcome traumatic events and stress, as well as a resource for the effective functioning of the human nervous system. Many studies confirm the relationship between the duration and quality of sleep and the health status of the population. At the same time, in modern conditions, socio-demographic characteristics of the population are of great importance in determining sleep. The main sleep disorders are recognized not only as a reduction in its duration, but also as a deterioration in quality (problems with falling asleep, a feeling of unreconstructed strength). The purpose of this article is to study the quantitative and qualitative characteristics of sleep in the population (including in the context of the main socio-demographic groups), the prevalence of factors of its disorders and their impact on subjective health assessments. The information base was data from monitoring of physical health of the Vologda oblast population in 2020 (n = 1500). The study identified risk categories for quantitative and qualitative sleep disorders; confirmed the relationship of stress factors, noise pollution and poor housing conditions with the frequency of sleep disorders; proved the impact of sleep problems on self-assessment of health.

Background: Insomnia is an important problem in medical sciences students and has implications for their educational progress. The current study aimed to estimate the prevalence of sleep disorders and investigating the impact of sweet almond on quality of sleep in students of the Tehran University of Medical Sciences (TUMS), Tehran, Iran who live in dormitories. Methods: This is a before-after study conducted in 2017. At first, using the ISI questionnaire prevalence of sleep disorders was determined. Sweet almond was the study intervention. Each day, 10 almonds were given to 446 students for 14 d. At the end of the second week, again ISI questionnaire was filled. SPSS was used to analyze data. The McNemar, Wilcoxson Signed Ranks, and Repeated Measures tests were used. Results: Out of 442 participants, 217 (49.1%) were female. Before intervention, 343 (77.6%) had insomnia and 99 (22.4%) had normal sleep. After intervention, 306 (69.2%) had insomnia and 136 (30.8%) had normal sleep. Having sweet almond for two weeks is associated with reducing insomnia (P<0.05). Investigating the almond impact in different categories also showed that it has a reducing impact on severe, mild, weak and normal sleep categories (P<0.05). Conclusion: Sweet almond has impacts on quality of sleep of those students of the TUMS that are living in dormitories. Intervention programs to improve quality of sleep are necessary and with regard to the high prevalence of insomnia, students must be protected, guided and consulted.

The impact of sleep disorders (SDs) on patients with chronic liver diseases (CLD) is tremendous. SDs are frequently encountered among these patients and interfere with their quality of life. This review aims to present the data available so far about the prevalence, phenotypes, and proposed pathophysiological mechanisms of SDs in CLD. Moreover, we proposed to search the literature regarding the most reliable methods to assess SDs and the possible therapeutic options in patients with CLD. The main results of this review show that when it comes to prevalence, the percentages reported vary widely between studies performed among populations from the USA or Europe and those coming from Asian countries. Furthermore, it has been proven that SDs may also be present in the absence of neurocognitive disorders attributable to hepatic encephalopathy (HE), which contradicts traditional suppositions where SDs were considered part of the clinical scenario of HE. Currently, there are no specific recommendations or protocols to assess SDs in CLD patients and data about the therapeutic management are limited. Taking into consideration their impact, a protocol for diagnosing and managing SDs should be developed and included in the daily practice of hepatologists.

Abstract Introduction COVID-19 epidemic led to great psychological and social stress, related to anxiety, depression, sleep disorders, suicidal risk and behavior, and changes in daily routine. The aim of this study was to assess the psychological impact of COVID-19 lockdown in Italian students. We focused on perceived sleep quality, anxiety and depression symptoms, and mostly on risk of suicide. Methods A total of 307 students (mean age 22.84±2.68) completed Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), Beck Anxiety Inventory (BAI), and Beck Depression Inventory-II (BDI-II). In our study, we focused on item 9 of BDI-II, that is related to suicide. We divided our sample on presence or absence of suicidal ideation based on this item. Results We found that 30.1% showed depressive, while 38.2% anxious symptoms. Concerning item 9 of BDI-II (suicidal thoughts or wishes), 84.7% answered that they do not have any thoughts of killing themselves, while 15.3% answered that they have some suicidal ideation. Concerning sleep variables, we found that 58% of our sample showed a PSQI total score higher than 5 (poor quality of sleep), and a global worsening in sleep quality and increase of insomnia both in student with and without suicidal ideation. Conclusion Our results on risk of suicide are in line with literature. Recent reviews focused on suicidal ideation in medical students found that depressive symptoms and suicidal ideation are common among medical students, finding a prevalence of suicidal ideation of 11%. Several studies suggest that sleep disorders are a risk factor for suicidal thoughts and behaviours. Our findings show that sleep cannot considered a predictive factor of risk of suicide during health emergency, because the risk is polyfactorial. Support (if any) None

Individuals with autism spectrum disorders (ASDs) commonly experience problems with the processing of tactile stimuli and poor quality of sleep. The aim of the present study was to analyze whether tactile stimuli modulation (TSM) disorders might be linked to insomnia prevalence in ASD individuals. We hypothesized that sleep disorders in children with ASD may result from improper tactile under/over responsivity. The study included 27 children diagnosed with ASD, aged 6.8 (±2.9 years) with male dominance (n = 22, 81.5%). To evaluate the pattern of TSM we used a clinical interview with a parent, and guided and spontaneous observation of the patients. Sleep disorders were diagnosed using the Athens Insomnia Scale. Of all the children diagnosed with TSM, 20 patients (74.1%) had an over-responsivity pattern and 7 children (25.9%) had an under-responsivity pattern. Of the patients, 11 children (40.7%) met the diagnostic criteria for insomnia. The data indicated a statistical tendency for higher prevalence of insomnia in individuals diagnosed with tactile under-responsivity (p = 0.051). We concluded that under-responsivity toward tactile stimuli may be partly responsible for poor sleep quality in ASD. There is an urgent need to treat sleep and sensory disruptions which may intensify behavioral difficulties in ASD.

Background:Spondyloarthropathies (SpA) are a group of chronic and progressive diseases, characterized in particular by a progressive stiffening of the spine, spreading to neighboring joints or to certain tissues, which could lead in the long term to progressive stiffening and functional impairment. This conditioncancauseinsomnia problems and impaired sleep quality.Objectives:To assess the impact of SpA on the quality of sleep.Methods:This is a retrospective study over a period of 4 years from January 2015 to December 2019, including all the medical records of patients with SpA followed in the Department of Rheumatology of the University Hospital of Ibn Rochd, Casablanca. We evaluated for each patient two validated scores: the Epworth somnolence scale rated from 0 to 24, and the Pittsburgh sleep score rated from 0 to 21 with 7 components. Patients with a psychiatric history or who were followed up for neurological pathologies were excluded.Results:178 patients were included. 60.67% were men with an average age of 36.32 years (14-68 years). 45.01% had axial SpA, 29.77% had psoriatic arthritis, and 25.22% were followed for SpA associated with inflammatory bowel disease. 45% had associated comorbidities: there were 18 diabetics and 34 hypertensive, 16.58% were smokers. Clinically, 85.42% presented a back pain initially on examination, 55% presented a polyarthralgia, and 39.88% an oligoarthritis. 63% had radiological sacroiliitis, and 35.14% had bilateral coxitis. 13.48% had a positive HLA B27 and 58.89% had a positive inflammatory assessment with very high activity indices,with a mean of 4.6. 64.66% of the patients received NSAIDs,of which 11% responded well. 57% were treated with csDMARDs, and 17.86% were treated with biologics. At the time of our study, the mean visual analog scale was 5.84 ± 1.7 out of 10 (2-9). The mean Epworth score was 8.38 ± 5.2 (0-21). 56.1% of patients had no sleep debt, 33.3% had a sleep deficit, and only 10.6% had signs of drowsiness. For the overall Pittsburgh score, the mean was 7.02 ± 3.6 (1-18). The mean of “subjective quality of sleep” was 1.12, “sleep latency” was 1.22, “duration of sleep” was 1.06, “usual sleep efficiency” was 0.74, “Sleep disturbance” of 1.28, “use of a sleep medication” of 0.54, and the average of the component concerning “poor shape during the day” was 1.03 out of 3. The LEQUESNE index went from an average of 6 to 8, which corresponds to an average handicap (P = 0.2) over a period of 3 years. 68% of the patients had an alteration in the quality of sleep, starting on average three years after the onset of symptoms. 11% reported having experiencedanxiety and depressive symptoms, and reported having used antidepressants or anxiolytics in the past 5 years.Conclusion:Our study showed the negative impact of SpA on the duration and overall quality of sleep. The degree of pain as well as functional impairment can cause and worsen sleep disturbances in SpA. Sleep disorders are a common and poorly treated disease state. This double blind, four arm placebo-controlled, randomized trial compared (1) low dose trazodone, (2) Sentra PM, a neurotransmitter based medical food, (3) the joint administration of trazodone and the medical food Sentra PM and (4) placebo. There were 111 subjects studied in 12 independent sites. Subjects underwent baseline screening, informed consent and an initial sleep questionnaire. After 14 days subjects underwent a second evaluation by questionnaire. At baseline and Day 14 the subjects underwent 24 hour ECG recordings that were analyzed in the frequency domain of heart rate variability. The specific high frequency parasympathetic autonomic nervous system activity was analyzed. The primary endpoints were sleep latency and parasympathetic autonomic nervous system improvement in sleeping hours. The results showed improvement in sleep latency for the Sentra PM and combination of Sentra PM and trazodone (−41 and −56 minutes P < 0.001). There was an improvement in quality of sleep for the amino acid formulation Sentra PM and the combination (3.86 and 6.48 Likert units on a 10 point scale P < 0.001). There was an activation of circadian activity percent at night in the medical food and combination groups while there was no change in parasympathetic activity in either the placebo or trazodone group. These data indicate that Sentra PM can improve the quality of sleep, the response to trazodone as a sleep medication and parasympathetic autonomic nervous system activity.

Background: Several studies on the effects of music on sleep disorders have demonstrated that music listening can improve sleep quality in patients with sleep disorders. To our knowledge, nevertheless, none of them have elucidated the characteristics of such music itself.Objective: The purpose of the present study was to elucidate the characteristics of the types of music that improve sleep quality.Methods: In twenty five tracks used in the previous study, we calculated four analysis indicators: scaling exponent of the spectrum of melody's zero-crossings, redundancy of note values, density of notes and tempo.Results: The characteristics of music to improve sleep quality were slow tempo, small change of rhythm, and moderate pitch variation of melody. Based on the results derived from cluster analysis, the music pieces studied were largely categorized into 3 groups. A comparison of these 3 groups showed no significant differences with respect to the scaling exponent of the melody and the density of notes, whereas it showed significant differences with respect to the redundancy of note values and tempo.Conclusions: Our study revealed several characteristics of the types of music that improve sleep quality. The identification of these characteristics contributes to providing personalized music therapy to patients.
