Assessing the quality of public health in the Russian Federation and Israel: Main problems and solutions

Subject. The article addresses the public health in the Russian Federation and Israel. Objectives. The focus is on researching the state of public health in Russia and Israel, using the Global Burden of Disease (GBD) project methodology, identifying problem areas and searching for possible ways to improve the quality of health of the Russian population based on the experience of Israel. Methods. The study draws on the ideology of the GBD project, which is based on the Disability-Adjusted Life-Year (DALY) metric. Results. The paper reveals the main causes of DALY losses and important risk factors for cancer for Russia and Israel. The findings show that the total DALY losses for Russia exceed Israeli values. The same is true for cancer diseases. Conclusions. Activities in Israel aimed at improving the quality of public health, the effectiveness of which has been proven, can serve as practical recommendations for Russia. The method of analysis, using the ideology of the GBD project, can be used as a tool for quantitative and comparative assessment of the public health.

The work shows the importance of rational and efficient assessment of control and supervision in health care for society as a whole and for its individual members. Health care is part of the system that shapes the quality of life of the population, influences the development of human potential, economic performance, on the adoption and diffusion of innovations in Russia. Control and supervision in public administration reform received much attention. The Ministry of Economic Development of the Russian Federation has developed a methodology for assessing and comparing the activities of Supervisory authorities. However, there is a need for not only implementation of the standardized approach in assessing but taking into account the specific features of activity of Supervisory authorities. The article presents the system of socially significant indicators for assessing the activities of Roszdravnadzor and its territorial offices in the RF entities. Currently the authority spends a lot of tests (about 10 thousand per year), however, the effectiveness of control and supervision is low. The health of the population remains low, satisfaction with public health services is reduced. Researching foreign experience of health systems development in countries with socially oriented economy allows to draw a conclusion about the necessity of building a qualitatively new assessment system of control and supervision, which should be based on the interests of society. The author proposes to depart from the principle of "check for validation" and change over to socially significant indicators for the government activities. The article highlights such indicators for healthcare and proposes their decomposition for fractional quantitative and qualitative assessment of activities. This system of indicators is aimed at improving the efficiency of public health care management and growth of the citizens’ health and their quality of life in a whole. The work is performed in as part of research of Financial University under the Government of the Russian Federation by order of the Government of the Russian Federation.

This paper discusses the current issues of legal regulation in the field of artificial intelligence in the state and municipal service in the Russian Federation in order to ensure and protect the rights and freedoms of man and citizen. The article highlights the current problems that arise in the implementation of certain state functions, such as – the work of state bodies with citizens' appeals and the lack of regulatory regulation of the use of artificial intelligence technology in this process, the use of which will contribute to meeting the deadlines for working with citizens' appeals, as well as increase the overall level of quality of interaction between citizens and public authorities. Specific measures are proposed for the development of legislation in order to introduce artificial intelligence in solving the problems facing the public authorities. The article formulates the main conclusion regarding the trend of using the artificial intelligence system in the issue under consideration.

Annotation Subject. Digital competencies of State civil servants of the Russian Federation Topic. The importance of digital competencies of civil servants of the Russian Federation in the system of public administration and the main approaches to their formation and development. Goals. Analysis of the results of the development of digital competencies in the public sector in order to form a unified approach to the methodology of teaching the basics of digitalization in the public sector. Methodology. Methods of statistical, graphical, system analysis, grouping methods, comparative analysis of statistical data, expert evaluation, as well as assessment of the regulatory framework in the field of competencies in the civil service of the Russian Federation were used in the research. Results. Based on the analysis of the results of the development of digital competencies in the public sector, it is possible to draw conclusions about the need for a high-quality study of the composition of digital transformation teams, a responsible approach to the appointment of digital transformation managers, as well as the need to develop a unified methodology for teaching the basics of digital transformation, both the teaching staff and civil servants of the Russian Federation. To solve this problem, it is proposed to form a single categorical and terminological apparatus, to define unified requirements and approaches for the appointment of those responsible for digital transformation in state bodies, as well as to develop methodological recommendations that define the main aspects of the organization of the learning process. In order to improve the quality of teaching the basics of digital transformation in the field, it is advisable to develop a course aimed at training the teaching staff. Scope of application. State authorities of the Russian Federation and the subjects of the Russian Federation. State Civil Service of the Russian Federation. Conclusions. The transition to the information society is characterized by the formation of new relationships between the individual and the government, the improvement of the quality of life, the emergence of conditions for business development, the use of innovative solutions. Today, there is a high demand for information technologies in the public sector, government agencies are focused on customer orientation, and actively implement an innovative approach. The process of mastering the digital competencies of state civil servants of the Russian Federation will allow optimizing the organizational work of state structures. The article discusses the urgent problems of increasing life expectancy and involving older people in the active social space in accordance with the Decree of the President of the Russian Federation No.204 of May 7, 2018 “On national goals and strategic tasks for development of the Russian Federation for the period until 2024”. The article considers the term ‘productive longevity’ as a comprehensive idea of the tasks facing the authorities in this direction. It is noted that the regulatory framework provides the basis for the development and implementation of the program of systemic support and improvement of the quality of life of older citizens. The author proposes to consolidate the main signs of active longevity, which could be considered as criteria for the involvement and participation of various ministries and departments in fulfilling the tasks formulated by the Presidential Decree. In addition, the features of public administration by solving the problem of active longevity are considered. The need for interagency cooperation, as well as the creation of a federal portal on the Internet, is noted, where all available opportunities for the older generation will be laid out in the public domain: for the field of healthcare, education, and creative implementation. The need for targeted use of capital investments and systematic use of the resources laid down for the implementation of the program is emphasized.

In any wastewater recycling scheme, the protection of public health is of primary importance. In Australia, the public health requirements applying to the treatment of recycled water are stringent. They use the Disability-Adjusted Life Year (DALY) metric to set a level of negligible public health risk. The target maximum risk of 10–6 DALY per person per year has been adopted in Australian water recycling guidelines since 2006. A key benefit of the DALY approach is its ability to standardise the understanding of risk across disparate areas of public health. To address the key challenge of translating the results of monitoring of microorganisms in the recycled water into this quantitative public health metric, we have developed a novel method. This paper summarises an approach where microbial surrogate organisms indigenous to wastewater are used to measure the efficiency of water recycling treatment processes and estimate public health risk. An example of recent implementation in the Greater Sydney region of Australia is provided.
