The article describes the results of surgical treatment of 323 patients with large hernias of the esophageal aperture of the diaphragm with an area of a hernial defect of 10-20 cm2, which were operated from 2014 to 2017 on the basis of the surgical departments of the Tver region: Regional Clinical Hospital and the State Clinical Hospital No. 7 of the city Tver. In this study, the comparative results of crurorhaphy and alloplasty with large hiatal hernias were determined.

Special features of the Don River flows’ dynamic changes in case of implementation of bank protection various versions in the area of Pavlovsk, Voronezh Oblast, have considered on the basis of computing experiments in two-dimensional definition. Efficiency of the bank protection various versions have been assessed in the conditions of the preset probability floods. Alterations of the velocity fields for different scenarios have been used for assessment. It has been shown that the maximal offshore motion of sediments is not always observed with the maximal water discharge and this has been confirmed by Zheleznyakov kinematic effect.

This paper covers internal migrations in the Republic of Srpska, in the period 2007-2015 and their impact to urban and peri-urban development of urban areas of the Republic. Migration directions show a continuity of permanent abandonment of rural areas and settlement of urban and peri-urban areas of the Republic, also including the migrations (immigration) from the Federation of Bosnia and Herzegovina. The scope of migrations has the trend of decrease as result of decrease in the rural population of the Republic of Srpska and its aging. Urban centres of the northern part of the Republic of Srpska (Banja Luka, Doboj, Bijeljina, Prijedor and Gradiška) have the largest scope of immigration, due to a number of attractive factors. In the southern part of the Republic, in the sub-Mediterranean area, the City of Trebinje is the main immigration area for the population of eastern Herzegovina, but also, more and more, for the population of the rest of the Republic of Srpska, as well for the wider regional environment. Spatial scope of research consists of eight cities in the Republic of Srpska, which defined the status of the city in the period from 1995 to 2019. Those cities are: Banjaluka, Bijeljina, Prijedor, Gradiška, Doboj, Istočno Sarajevo, Zvornik and Trebinje. The specificities in the territorial organization of the Republic of Srpska are the result of complex socio-political processes and the formation of the entity line of demarcation in post-war period (1992-1995), between the Federation of Bosnia and Herzegovina and the Republic of Srpska. Given the physiognomy of space and developmental processes, the Republic of Srpska has complex settlement function. In four urban areas, Istočno Sarajevo, Doboj, Zvornik and Trebinje, the settlements are divided by the entity line, which caused the migrations. In 2014, population migration was also affected by administrative separation of the municipality of Stanari from the Doboj city area. Due to the above mentioned, compared to other cities of the Republic, Doboj has the highest negative values of the migration balance in the observed period (2007-2016). Statistical data from the Federal Bureau of Statistics of BiH and the Republic Institute of the Republic of Srpska were used in research related to migrations, as well as other relevant statistical material produced in the period from 1991 to 2016. The key demographic indicators, internal migrations, immigration and emigration and migration balance were taken into consideration. In order to compare the research areas, data were prepared on the basis of demographic indicators. Conclusions on spatial integration of migration, as well as its direction, have been drawn based on internal migration, spatial distribution of population and change in number of inhabitants in urban and suburban settlements.

Relevance. The article highlights the topical issues of cardiovascular mulimorbidity (CVM) and temporary disability (TD) among medical workers of a large multi-profile city clinical hospital. Aim. To assess the incidence of chronic noncommunicable diseases, CVM and TD in medical workers of the city clinical hospital. Materials and methods. 527 physicians (207 men and 320 women) aged 25 to 79 (42±9.8) years and 857 nurses (32 men and 825 women) aged 19 to 79 (41±9) years were included in a retrospective, open-label study. The medical documentations analysis was performed. Results. Hypercholesterolemia was diagnosed in 38%, overweight - in 26%, obesity - in 4.8%, smoking - in 27,2%, hypertension - in 9.1%, coronary artery disease - in 3%, diabetes mellitus - in 3.2%, CVM - in 2.3% of health care workers. Aim. To conduct a cascade screening and to assess its effectiveness in the diagnosis of familial hypercholesterolemia (FH) in children.Material and methods. The study was conducted from January 2017 to August 2018 on the basis of the City Clinical Hospital № 7 and the Children’s Republican Clinical Hospital (Kazan, the Republic of Tatarstan). It consisted of identifying index cases — primary patients with FH with further examination of first- and second-degree relatives <18 years old. In adults, the diagnosis was established according to Dutch Lipid Clinic Network (DLCN) criteria): FH was diagnosed with a score of ≥6. In children and adolescents <6 years of age, the Simon Broome Registry criteria were used.Results. During this period, 2542 case histories of patients with cardiovascular diseases were analyzed, of which 1220 people with a total cholesterol >5 mmol/L were selected. Next, a targeted screening was carried out aimed at the diagnosis of FH, as a result of which 61 index patients were identified. At the next stage, as a part of cascade screening, 87 first- and second-degree relatives <18 years old were examined. In 43 (49,4%) children, heterozygous HF was diagnosed, of which in 4 patients the disease was detected by re-examination after 1 year.Conclusion. Cascade screening is a necessary and effective method for the diagnosis of АР in first- and second-degree relatives <18 years old. All children of the index patients should be monitored or genetic testing necessary to rule out FH. Today, it is important to increase awareness among clinicians about the diagnosis of FH in adults and children.

The perception of the ecological situation in the Moscow metropolis was studied using an online survey of the city dwellers. Quota sampling, proportional to the age and sex composition for each municipal administrative district, was used to ensure representativeness. The influence of the high-level values (basic, HLV) according to Schwartz and socio-demographic factors on the respondents’ perception of the ecological situation in the metropolis was studied during the research. The results obtained showed no statistically significant differences in the perception of factors that negatively affect the ecological situation in Moscow, depending on the value orientations. At the same time, the perception of changes in the ecological situation in the city differs in different clusters based on socio-demographic indicators. The change in the ecological situation is most acutely recorded in clusters, where young people occupy a significant share. Notably, in the cluster where women prevail, the change in the situation is seen as a deterioration.
