Semua fasilitas pelayanan kesehatan wajib menyelenggarakan rekam medis, termasuk puskesmas. Dengan pelayanan rekam medis yang berkualitas pasien akan merasa puas, khususnya  karena  pasien  dilayani  dengan  cepat,  tepat  dan  aman  oleh  pihak puskesmas. Permasalahan yang sering ditemui pada bagian penyimpanan rekam medis adalah terjadinya misfile maupun duplikasi nomor rekam medis. Akibatnya, berkas rekam medis pasien lama sulit ditemukan sehingga proses pencarian berkas rekam medis pasien di rak penyimpanan membutuhkan waktu yang cukup lama. Tujuan pelaksanaan Pengabdian Kepada Masyarakat (PKM) agar sistem penyimpanan rekam medis menjadi optimal dalam rangka peningkatan kualitas pelayanan rekam medis pasien rawat jalan di Puskesmas Padang Pasir. Metode yang digunakan berupa sosialisasi tentang optimalisasi sistem penyimpanan rekam medis di Puskesmas Padang Pasir. Sebelum sosialisasi, dilakukan pre-test dan setelah sosialisasi dilakukan kegiatan post-test dengan tujuan mengetahui tingkat pemahaman mitra tentang materi sosialisasi. PKM ini dilakukan pada tanggal 9 bulan Agustus 2019 di Puskesmas Padang Pasir. Peserta dari kegiatan PKM ini adalah semua petugas rekam medis di Puskesmas Padang Pasir sebanyak 6 (enam) orang. Hasil dari kegiatan PKM ini adalah adanya peningkatan pengetahuan mitra tentang penyimpanan rekam medis dari nilai rata-rata 45 menjadi 85 point. With a quality medical record service, patients will feel satisfied, especially because patients are served quickly, precisely and safely by the community health centers. The problem that is often encountered in the medical records storage is the occurrence of misfiling and duplication of medical record numbers. As a result, old patient medical record files are difficult to find, so the process of searching a patient's medical record file on a storage rack takes quite a long time. The purpose of the implementation of Community Service (PKM) so that the medical record storage system becomes optimal to improve the quality of outpatient medical record services at the Padang Pasir Health Center. The method used in the form of socialization about optimizing the medical record storage system at the Padang Pasir Health Center. Before the socialization, a pre-test was carried out and after the socialization, a post-test was carried out to know the level of understanding of partners about the material of the socialization. This PKM was held on August 9, 2019, at the Padang Pasir Communuty Health Center. The participants of this PKM activity are all 6 medical records officers at the Padang Pasir Health Center. The result of this PKM activity was an increase in partner knowledge about medical record storage from an average value of 45 to 85 points. ABSTRACT At this time, information technology is one of technology that is growing rapidly, with the advancement of information technology, data or information that provided can be fast to access, efficient and accurate. For example the results of advances in information technology is the development of the Internet network that allows all mankind worldwide using data provided or connected in the network together, meanwhile for Community Health Centers of Birobuli, data entry system still done manually, only BPJS already using a computerized system, so this study aims to determine the evaluation of health information systems at Community Health Centers of Birobuli Palu in medical record room.The method that used is qualitative, qualitative sense is depth research on employee community health center about training, processing SIK, activities program and benefits as well as developments SIK in community health centers of Birobuli Palu. The number of informants that used was 7 people of one key informant, three informant regular and three additional informants. The analysis of data using analytical approach (analysis content) with the matrix technique where the information was obtained on though in the table. It refers that input (for an educational background in the medical record average DIII of Public Health and for training of SIK at the community health center have never done the training because which provide training was from the Department of Health. Process (in data processing is done in two ways: manually and computerized, with a network system used is a LAN network, and for the delivery of report conducted by the online system and manual system). Output (many benefits of SIK for community health centers, so that the expected for community health center can immediately apply the SIMPUS program for SIK and provide training to SIK employees).The conclusion of this study is the application of SIMPUS program hasn’t been implemented at the community health center of Birobuli Palu because the community health center employee have never been training that given by the Department of Health. To better understand existing capacity and help organizations plan for the strategic and expanded uses of data, a project was initiated that deployed contemporary, Hadoop-based, analytic technology into several multi-site community health centers (CHCs) and a primary care association (PCA). An initial data quality exercise was carried out after deployment, in which a number of analytic queries were executed using both the existing electronic health record (EHR) applications and in parallel, the analytic stack. Each organization carried out the EHR analysis using the definitions typically applied for routine reporting. The analysis using the analytic stack was carried out using those common definitions established for the Uniform Data System (UDS) by the Health Resources and Service Administration.  In addition, interviews with health center leadership and staff were completed to understand the context for the findings.The analysis uncovered many challenges and inconsistencies with respect to the definition of core terms (patient, encounter, etc.), data formatting, and missing, incorrect and unavailable data. At a population level, apparent underreporting of a number of diagnoses, specifically obesity and heart disease, was also evident in the results of the data quality exercise, for both the EHR-derived and stack analytic results.Data awareness, that is, an appreciation of the importance of data integrity, data hygiene and the potential uses of data, needs to be prioritized and developed by health centers and other healthcare organizations if analytics are to be used in an effective manner to support strategic objectives. While this analysis was conducted exclusively with community health center organizations, its conclusions and recommendations may be more broadly applicable.

Abstract - Health services include the recording of the patient's medical record . Medical records were used to aid the treatment process. The number of medical records continues to grow proportional to the number of patients. Tens of thousands of sheets of paper used to record medical record requires effort , time and place great . The amount of effort will continue to grow each day. Search one sheet of medical records among a set of storage shelves requires considerable time and risk data is not found. The risk of error in the search and storing will increase every day. The development of technology allows the implementation of technology in the process of record-keeping. Changes in the form of digital medical records will reduce the need of a previous process. Labor, time and place required by the help of information systems will be reduced significantly . Storage process data stored in the cloud will provide more value for the system as a patient's medical records from a health center can be accessed from other health centers. The development of this system will reduce the risk of inappropriate storage and retrieval of medical records. Grobogan Health Department that oversees health center in Grobogan are office that are ready to migrate business processes into the digital age. Development of medical record information system for the health center expected to improve the quality of service of health centers , especially in health care.

This study aims to determine and analyze the medical record storage system on the effectiveness of patient care at the Cikole Public Health Center. The research method used is quantitative with inferential statistical analysis techniques. Data collection techniques by means of observation, interviews and questionnaires. Cikole Lembang Public Health Center is one of the first level health care facilities. Based on observations, the storage system used is a regional storage system in which each region starts from the first digit. There are 2 officers who work in the medical records section of the Cikole Public Health Center. With the storage system used, there are several shortcomings, namely the occurrence of misfiles and duplication. To find out whether there is an analysis of the medical record storage system on the effectiveness of the service, the author gives a questionnaire to the Cikole Public Health Center officers. The results of the questionnaire also show that the highest percentage level is found in item no.7, namely the decentralized storage system is in accordance with the target of 71%. Then the second is item no. 8, namely a decentralized storage system that allows the provision of medical record files on time by 61%. From the results of interviews with medical records officers, suggestions and input were given: (1). The storage system used was changed to centralization so that the alignment system was not duplicated; (2). The use of bindex as an additional tool to make storage neater and easier.

Nagaswidak Health Center is one of the community health centers that is quite large and has complete facilities. But the problem that is often faced by officers in the puskesmas is the medical record data processing system which is still manual, causing the accumulation of patient medical record file data, in addition to patients who have been checked before and lost their medical records, it is very difficult for officers to find back, so the officer made a new medical record data. To overcome this problem, a Medical Records Filling Application was made at the Nagaswidak Health Center which includes the processing of medical records, patient data, drug data, action data, doctor data, and admin logins. So that the data search problem is not difficult, the turbo boyer moore algorithm method is applied which is expected to later be able to facilitate the search for patient data in the medical record filling application. Based on the test results Boyer Moore's Algorithm successfully applied to search for the beginning of a word, middle word, and final word. And the level of ease and usefulness of medical records application using Boyer Moore's algorithm obtained results that the level of ease is 80% and 100% usability rate.

Puskesmas merupakan fasilitas pelayanan kesehatan yang menyelenggarakan upaya kesehatan masyarakat dan upaya kesehatan perseorangan tingkat pertama. Dalam melaksanakan tugasnya puskesmas berwenang untuk untuk melaksanakan pencatatan dan pelaporan kesehatan pasien serta melakukan evaluasi terhadap mutu dan cakupan pelayanan kesehatan. Pencatatan dan pelaporan pasien dapat dilihat dari berkas rekam medis pasien. Rekam medis pada sarana kesehatan non rumah sakit wajib disimpan sekurang-kurangnya untuk jangka waktu dua tahun terhitung dari tanggal terakhir pasien berobat. Setelah batas waktu penyimpanan dilampaui maka rekam medis dapat dimusnahkan. Puskesmas Kuranji merupakan salah satu puskemas di kota padang yang berlamat dikecamatan Kuranji. Berdasarkan hasil wawancara dengan petugas rekam medis, hingga saat ini Puskesmas Kuranji belum melakukan retensi dan pemusnahan berkas rekam medis sedangkan ruangan penyimpanan hanya ada satu. Berdasarkan pemaparan masalah di atas, maka pengabdi bermaksud untuk melakukan sosialisasi dan praktik langsung mengenai retensi dan pemusnahan berkas rekam medis di Puskesmas Kuranji Kota Padang. Target dan luaran dari pelaksanaan kegiatan pengabdian kepada masyarakat adalah kegiatan ini dapat dijadikan sebagai bahan pertimbangan untuk pelaksanaan retensi dan penghancuran berkas rekam medis. Target kedepannya agar sosialisi ini dapat digunakan sebagai dasar kegiatan retensi dan penghancuran berkas rekam medis. In carrying out its duties PHC is authorized to carry out the recording and reporting of patient health and to evaluate the quality and scope of health services. Patient recording and reporting can be seen from the patient's medical record file. Medical records in non-hospital healthcare facilities must be kept for at least two years from the date the patient was treated. After the storage time limit is exceeded, the medical record can be destroyed. The Kuranji Community Health Center is one of the public health centers in the city of Padang which is well-known in the Kuranji sub-district. Based on the results of interviews with medical record officers, up to now the Kuranji Community Health Center has not retained and destroyed medical record files while there is only one storage room. Based on the explanation of the problem above, the service intends to conduct socialization and direct practice regarding the retention and destruction of medical record files at the Kuranji Health Center in Padang City. The target and output of the implementation of community service activities is that this activity can be used as consideration for the implementation of retention and destruction of medical record files. According to Indonesia’s result of Basic Health Research of 2013, prevalence of acute respiratory infection in 2007 and 2013 were not different (25.5% and 25.0%, respectively). Identifying the cause of acute pharyngitis is a key point in determining the optimal treatment. The main purpose is to evaluate the rational use of drugs and its irrational impact as well as the correlation of the drug use with the incidence and prevalence of acute pharyngitis. This study was a descriptive and observational study, carried out retrospectively and concurrently at two community health centers located in Bandung and Cimahi, Indonesia. There were 80.01% over prescription of antibiotics, with a total of 8.98% is non-treatment option, and 62.43% irrational used of corticosteroids. The incidence and prevalence of acute pharyngitis at one health center in Bandung were 2.45% and 2.31%, respectively, with irrationality rate of 83.82%. Those recorded at one health center in Cimahi were 2.11% of incidence and 2.00% of prevalence with irrational rate of 91.29%. It can be concluded that there were still irrational use of medicines in the treatment of acute pharyngitis in community health center. The higher incidence and prevalence might indicate the declining health services quality.

Vasectomy method has 99% success in preventing pregnancy. Among 13 community health centers in 2017, the widest number of vasectomy contraception users recorded in Bulili Community Health Center with 37 people (57,81%). The studi aims to discover of several factors of contraception use, such as education, knowledge, and family support. This research is a descriptive quantitative. The sample of this research is 37 men, particularly husbands, who are older than 35 years old and have 2 children. In collecting the sample, this research uses total sampling method. The data collection method is done by observation and interview. Furthermore, the obtained data are analyzed statistically by showing frequency distribution in each variable. The research showed that vasectomy contraception based on education (64,9%), knowledge (89,2%), and family support (94,6%) in this area is good. Consequently, citizens are expected to willingly accept information from health worker related to men’s KB
