Purpose. To evaluate the results of treatment of patients with pseudophakic bullous keratopathy (PBK) by the method of posterior lamellar femto-keratoplasty with ultrathin graft (FS-DSEK) harvested using two different lasers. Material and methods. The results of surgical treatment of 82 patients (82 eyes) underwent posterior lamellar femto-keratoplasty for PBK were analyzed. In the 1st group included 43 patients (43 eyes) with PBK underwent FS-DSEK using FS laser Femto-Visum (Optosystems, Russia). In the 2nd group included 39 patients (39 eyes) with PBK underwent FS-DSEK using LDV Z8 (Ziemer, Switzerland). Observation period was 1 year. Before and after surgery following indicators were evaluated: uncorrected visual acuity (UCVA), best spectacle corrected visual acuity (BSCVA), postoperative astigmatism, central corneal thickness (CCT), graft thickness, center-periphery (C:P) ratio, endothelial cell density (ECD), endothelial cell (EC) loss, optical density. Results. Transparent engraftment at 1-year observation period was observed in 88% of cases. In the 1st group UCVA=0.22±0.11, BSCVA=0.32±0.12, in the 2nd group UCVA=0.18±0.08, BSCVA=0.29±0.1 (p>0.05). The maximum BSCVA in both groups was 0.6. Postoperative astigmatism was comparative in 2nd groups – 1.43±1.1 and 1.38±1.0 D, respectively (p>0.05). In the 1st group, CCT=549±31, graft thickness in the central zone – 83±12, C:P ratio – 0.92±0.05; in the 2nd group CCT=546±28, graft thickness – 80±10, C:P ratio – 0.94±0.06 (p>0.05). In the 1st group ECD – 1326±282, EC loss – 55±6%; ECD in the 2nd – 850±230, EC loss – 70±7% (p<0.05). According to the results of densitometry, optical density of the posterior layers of the cornea and «donor–recipient» interface zone was higher in the 2nd group, both in the central and in the paracentral zones: in the 1st group in the posterior layers of the stroma – 16.4±1.2 (0–2 mm) and 15.8±1.0 (2–6 mm), in the interface zone – 14.5±0.9 (0–2) and 13.9±0.8 (2–6), in the 2nd group in the posterior layers – 18.3±1.3 (0–2 mm) and 17.9±1.1 (2–6 mm), and in the interface zone – 17.3±1.2 (0–2 mm) and 17.0±1.0 (2–6 mm, p<0.05). Conclusions. FS-DSEK showed high efficiency for treating patients with PBK. Functional results were comparative in 2nd groups. Statistical analysis showed highest safety of transplanted endothelium in the 1st group at 1-year observation period. Purpose — to perform a comparative analysis of the clinical efficacy of intrastromal keratoplasty (ISKP) in patients with keratoconus during surgery with and without Verion digital marking device.Patients and methods. 37 patients (50 eyes) with corneal astigmatism up to -5.0 diopters were included in the study. These patients were operated with the femtolaser method of ISKP with implantation of corneal segments. The average age of the patients was 27.96 ± 3.40 years. The observation period is from 3 to 12 months. All patients were divided into two groups depending on the surgical technique: Verion-assisted, using a digital marking device and taking into account the angle of cyclotorsia, or standard, without taking into account the angle of cyclotorsia. The first main group included 28 eyes (56 %), the second control group included 22 eyes (44 %) accordingly.Results. The functional results after ISKP in both groups were quite good. Stabilization of functional parameters and keratometric data was noted in the postoperative period, by 3 months. Patients of the main group showed a statistically significant (p < 0.05) increase in average uncorrected visual acuity to 0.50 ± 0.16, and visual acuity with maximum correction to 0.70 ± 0.12 (p < 0.05). The average keratometry of the weak meridian decreased to 44.3 ± 1.4 diopters, the average keratometry of the strong meridian decreased to 50.8 ± 3.2 diopters. Patients in the control group showed a statistically significant (p < 0.05) increase in average uncorrected visual acuity to 0.45 ± 0.12, and visual acuity with maximum correction to 0.60 ± 0.17. The average values of keratometry of the weak meridian decreased to 45.3 ± 1.8 diopters, the average values of keratometry of the strong meridian decreased to 51.8 ± 2.2 diopters. The results of the vector analysis reliably indicate a significantly higher accuracy of the Verion-assisted ISKP. Which is confirmed by the achieved indicators of the “success” index, the calculated astigmatism vector close to the actual postoperative astigmatism vector, the smaller the magnitude of the astigmatism axis difference vector compared to the control.Conclusion. The use of the Verion-assisted intrastromal technique for keratoconus reduces the risk of erroneous positioning of the corneal segments in the tunnel during transferring diagnostic data to the operating room. Taking into account the angle of cyclotorsia during cutting into the intrastromal tunnel increases the effective correction of the cylindrical component of refraction, which is confirmed by the main coefficients of the vector analysis by N. Alpins.

ObjectiveTo compare clinical outcomes and complications between pre-loaded ultra-thin Descemet stripping automated endothelialkeratoplasty (pl-UT-DSAEK) and pre-loaded Descemet membrane endothelial keratoplasty (pl-DMEK).Methods and analysisComparative study in patients with endothelial dysfunction associated with Fuchs endothelial corneal dystrophy and pseudophakic bullous keratopathy who underwent pl-UT-DSAEK or pl-DMEK transplants. For both groups, the tissues were pre-loaded at the Fondazione Banca degli Occhi del Veneto (Venice, Italy) and shipped to The Royal Liverpool University Hospital (Liverpool, UK). Best corrected visual acuity (BCVA) and re-bubbling rates were the main outcome measures.Results56 eyes of 56 patients were included. 31 received pl-UT-DSAEK and 25 received pl-DMEK. At 12 months, BCVA (LogMAR) was significantly better for pl-DMEK (0.17±0.20 LogMAR) compared with pl-UT-DSAEK (0.37±0.37 LogMAR, p<0.01). The percentage of people that achieved ≥20/30 was significantly higher in the pl-DMEK group. The rate of re-bubbling, however, was significantly higher for pl-DMEK (44.0%) than for Pl-UT-DSAEK (12.9%), p<0.01.ConclusionPl-DMEK offers better BCVA than pl-UT-DSAEK. The higher re-bubbling rate associated with pre-loaded DMEK is of concern.

Abstract Purpose To assess 2-year endothelial cell loss and graft survival after femtosecond laser semi-assisted Descemet stripping endothelial keratoplasty (FLS-DSEK). Methods In this prospective and noncomparative study carried out at Eye Hospital of Shandong First Medical University, 85 eyes (84 patients) with endothelial dysfunction receiving FLS-DSEK (n=62, 75.9%) or FLS-DSEK combined with phacoemulsification cataract surgery and intraocular lens implantation (n=23, 27.1%) from 2013 through 2016 were included. The graft endothelial cell loss, endothelial graft thickness, visual acuity, and complications after surgery were evaluated. Results Thin endothelial grafts were all successfully prepared, with no occurrence of perforation. The rate of endothelial cell loss was 17.4%, 18.8%, 19.9%, and 26.7%, and the central graft thickness was 113±54 μm, 102±40 μm, 101±28 μm, and 96±23 μm at 3, 6, 12, and 24 months, respectively. The median best-corrected visual acuity was 0.4 logMAR (range, 0–2 logMAR) at 24 months, demonstrating a significant difference from that before surgery (2 logMAR; range, 0.2–3 logMAR) (T=187.5, P<.001). Partial graft dislocation was the most common postoperative complication, with an occurrence rate of 14% (n=12), and it was associated with an abnormal iris-lens diaphragm (r=.35, P<.001). The other complications included a high intraocular pressure (n=5, 6%), endothelial graft rejection (n=4, 5%), and pupillary block (n=1, 1%). Endothelial graft decompensation occurred in the two eyes, and 98% (n=83) of the grafts survived at 24 months. Conclusions Data of the study suggest that the treatment using FLS-DSEK seems to be promising and might be considered a feasible choice in patients with endothelial dysfunction. Trial registration 1. Abstract Purpose To evaluate the potential impact of rebubbling on the anterior segment parameters and refractive outcomes in patients with graft detachment following uneventful DMEK for Fuchs endothelial dystrophy (FED). Methods Retrospective institutional cohort study of comparing 34 eyes of 31 patients with rebubbling for graft detachment following Descemet membrane endothelial keratoplasty (DMEK) to 33 eyes of 28 patients with uneventful DMEK. Main outcome parameters were various corneal parameters obtained by Scheimpflug imaging, refractive outcome, corrected distance visual acuity (CDVA), and endothelial cell density (ECD). Results Anterior and posterior corneal astigmatism, corneal densitometry, central corneal thickness, and anterior chamber depth and volume showed no significant differences. Preoperative distribution of astigmatism axis orientations showed a high proportion of anterior corneal with-the-rule astigmatism (71%) in eyes requiring rebubbling. Mean postoperative cylinder in the rebubbling group (1.21 ± 0.85 D) was significantly higher compared to the controls (p = 0.04), while differences in spherical equivalent (SE) were insignificant (p = 0.24). Postoperative CDVA was 0.11 ± 0.11 in the control group compared to 0.21 ± 0.17 in the rebubbling group (p = 0.03). Eyes with subsequent rebubbling demonstrated a significantly higher endothelial cell loss (56% versus 37%) (p < 0.001). Conclusion Apart from higher cylinder values, refractive outcome and corneal parameters assessed by Scheimpflug imaging were comparable in eyes with rebubbling and controls. However, a reduced visual acuity and an increased endothelial cell loss should be taken into consideration prior to rebubbling especially in eyes with circumscribed graft detachment.

Abstract Backgrounds However there have been numerous investigations of intrascleral intraocular lens (IOL) fixation techniques, there is room for improvement in terms of simplifying complicated techniques and reducing the high levels of skill required. This study aimed to report a novel technique for sutureless intrascleral fixation of the IOL using retinal forceps with a 27-gauge trocar. Methods Nineteen eyes of 18 patients underwent intrascleral fixation of the IOL from July 2018 to September 2019 were enrolled in this study. A 27-gauge trocar formed 3-mm scleral tunnels positioned at 4 and 10 o’clock, 2 mm from the corneal limbus. We used a 3-piece IOL haptic grasped by a 27-gauge retinal forceps and pulled from the 27-gauge trocar. The IOL was fixed by making a flange. Main outcome measures were visual acuity, corneal endothelial cell density, IOL tilt, decentration, predicted error of refraction and complications. Results The 19 eyes were followed up for 1 month. The mean pre- and postoperative logMAR uncorrected visual acuity (UCVA) was 1.06 ± 0.63 and 0.40 ± 0.26, respectively (p < 0.01), while the mean pre- and postoperative logMAR best corrected visual acuity (BCVA) was 0.27 ± 0.51 and 0.06 ± 0.15, respectively (p = 0.09). The mean corneal endothelial cell density was 2406 ± 625 to 2004 ± 759 cells/mm2 at 1 month (p = 0.13). The mean IOL tilt was 3.52 ± 3.00°, and the mean IOL decentration was 0.39 ± 0.39 mm. There was no correlation among IOL tilt, decentration and BCVA (p > 0.05). The mean prediction error of the target refraction was − 0.03 ± 0.93 D. The complications were vitreous hemorrhage (3 eyes), hyphema (1 eye), IOP elevation (1 eye), iris capture of the IOL (1 eye) and hypotony (2 eyes). No IOL dislocation occurred. Conclusions IOL intrascleral fixation with a flange achieved good IOL fixation and visual outcome in the scleral tunnels created with the 27-gauge trocar.

Purpose. To report clinical results of a foldable, hydrophilic acrylic, single-piece, injectable, posterior chamber phakic intraocular lens (pIOL).Material and Methods. Medical records of patients who underwent posterior chamber phakic IOL (Eyecryl Phakic IOL, Biotech Vision Care, Ahmedabad, India) implantation for surgical correction of myopia were retrospectively reviewed. Only patients with at least a one-year follow-up were included. Manifest refraction, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), endothelial cell density (ECD), and pIOL vault were analyzed at 1, 3, 6, and 12 months after surgery. Complications observed during and after surgery were also recorded.Results. The study included 58 eyes of 29 patients. Mean patient age was 32 ± 7 years. Spherical equivalent of manifest refraction was −13.41 ± 3.23 D preoperatively and −0.44 ± 0.55 D postoperatively. Preoperative CDVA was 0.29 ± 0.71 logMAR. Postoperative UDVA and CDVA were 0.21 ± 0.66 and 0.15 ± 0.69 logMAR, respectively, at the 12-month visit. At the 12-month visit, the efficacy index was 1.20 and the safety index was 1.39. Mean ECD was 2713 ± 339 cells/mm2at the preoperative visit and 2608 ± 362 cells/mm2at the 12-month visit (3.9% loss,p<0.001). ECD loss from 3 months to 12 months was not statistically significant. No significant cataract formation, significant endothelial cell loss, glaucoma, uveitis, or any other vision-threatening complication was observed.Conclusion. Based on postoperative experience, we have found that Eyecryl Phakic IOL is safe and effective for treating high myopia.
