Background: Diabetes Mellitus is one of the common causes of Chronic Kidney Disease which usually leads to end-stage kidney disease. Thus this study was planned to nd out the outcome of CKD in DM, disease progression, appropriate management and the complications. Methods: A cross sectional study among Diabetes Mellitus patients with Chronic Kidney Disease attending the Department of Nephrology and Department of Medicine in Thanjavur Medical College Hospital during the period of July and August 2018. The duration of study period was 2 months. All patients who attended the outpatient department (OPD) and in-patients departments with Type I and Type II diabetes mellitus associated with chronic kidney disease, on any line of management were included in this study. A total of 101 patients were included. Statistical Package for Social Sciences (SPSS for Windows V20) was used for data analysis. Results: Poor glycemic control, albuminuria, hypertriglyceridemia (greater than 150), increase in LDLlevels (greater than 100), BMI (greater than or equal to 25), poor BP control and a longer duration of diabetes mellitus had a strong positive correlation with decrease in eGFR less than 60 ml/min. Glycemic control, weight reduction and adequate BPcontrol retard the progression of CKD. Conclusion: Albuminuria and decline in eGFR both are independent risk factors for diabetic CKD and are strong predictors of morbidity and mortality from a major vascular event, especially cardiovascular complications and stroke.

BACKGROUND Fatigue has been defined as a feeling of weakness subjectively, also expressed as extreme tiredness and energy loss due to illness or physical or mental exertion. Differences of quality of fatigue between healthy controls and cancer patients have been suggested. Moreover, these are due to heightened degree of expression of fatigue in cancer patients. We wanted to determine the prevalence of fatigue in patients suffering from head & neck cancer and receiving chemotherapy as well as radiotherapy and evaluate the fatigue related impact on quality of life. METHODS This is a cross sectional study done among patients of head & neck cancer, receiving chemotherapy and radiotherapy, in the Department of Oncology, Surgery and Pathology, K.D. Medical College Hospital & Research Center, Mathura, Uttar Pradesh. Department of Radiation Oncology, Grecian Superspeciality Hospital, Mohali. PG Department of Radiotherapy, Acharya Harihar Regional Cancer Centre, S.C.B. Medical College & Hospital, Cuttack. Duration of study is 2013 December 2014 November and 2019 March - 2020 July. Forty-one patients were studied. RESULTS Out of twenty-four patients who were treated with radiotherapy, nine patients had mild fatigue; fifteen patients had moderate fatigue; and no patient had severe fatigue. Among seventeen patients who received chemotherapy, fourteen patients reported mild fatigue, two reported moderate fatigue, and one patient reported severe fatigue. CONCLUSIONS Different anticancer therapies cause Cancer Related Fatigue (CRF); those could be chemotherapy and radiotherapy. Severity of fatigue was more in radiotherapy as compared to chemotherapy group. KEYWORDS Cancer, Chemotherapy, Radiotherapy, Fatigue, Quality of Life (QOL)

Introduction:The aim of the present study was to assess the pattern of lifestyle and knowledge about diabetes mellitus among type 2 diabetic patients at two tertiary level hospitals in Mymensingh Materials and Methods: A descriptive, cross sectional study was conducted from July 2015 to December 2015 among 300 patients attending at Medicine outpatient department of the Community Based Medical College Hospital and Endocrine outpatient department of the Mymensingh Medical College Hospital after obtaining requisite consent from the patients. Data were collected through the interviewing of the patients. The collected data were entered into the computer and analyzed by using SPSS version 20.1. The study was approved by the institutional ethical committee. Results: In a pool of 300 type 2 diabetics, Most of the patients (57.3%) belonged to the middle age group 41-60 years. More than half of the respondents were female (n=223, 74.3%). 97% patients were found to have knowledge about timing of dose regimen. Majority of patient’s (35.7%) knowledge about hypoglycemia was poor. The rate of adherence to diet was 51%. The rate of adherence to exercise was 68.3%. Conclusion: Majority of type 2 DM patients displayed optimal level of diabetes knowledge. There was a high rate of non adherence to diet and exercise recommendations by patients suffering from type 2 diabetes mellitus. So continuous patient education and awareness program are required. Serum creatinine is a very common tool of renal assessment in diabetic nephropathy. Specially due to its availably and cost-effectiveness, serum creatinine has been a popular choice among the clinicians even in rural areas. Also, the staging of diabetic nephropathy is done on the basis of eGFR which is calculated mainly with serum creatinine level. Sometimes, clinicians , even overlook the other investigations ,such as urine routine examinations or ultra sonogram of kidney, when the serum creatinine level is within normal limit. This study aims at observation of existing correlation between serum creatinine and urine albumin in diabetic nephropathy patients. This cross sectional observational study has been conducted on 50 patients in a period of 6 months in the medicine department of Dhaka Medical College Hospital. 50 diabetic patients admitted to Dhaka Medical College Hospital were enrolled in this study after fulfilling inclusion and exclusion criteria. Among the total 50 patients, 34 patients were diagnosed on the basis of microalbuminuria and the rest had raised urinary total protein. No correlation was found between serum creatinine and urine albumin or urine total protein. Staging of the kidney disease was done according to eGFR (calculated by MDRD equation).  Only 41.37% of the patients with microalbuminuria, showed eGFR consistent with its staging while 31.5% of the patients with proteinuria showed respective expected eGFR. The rest of the patient’s eGFR was inconsistent with the staging. And the difference between the mean serum creatinine in two groups was insignificant. This study has shown that, the serum creatinine has no linear correlation with urinary albumin in diabetic nephropathy patients. So, commonly used serum creatinine based formula to calculate the eGFR can misinterpret the staging of the disease which can delay the appropriate treatment thereafter. Objective: To determine subjective outcomes after cholesteatoma surgery. Design: Cross sectional observational study Methods: Study place: Shaheed Ziaur Rahman Medical college Hospital (SZMCH), Bogura, Bangladesh Study population: Chronic otitis media (COM) with extensive cholesteatoma. Interventions: Canal wall down mastoidectomy for cholesteatoma. Main Outcome Measures: In this study, the Chronic Ear Survey (CES) was provided to all patients preoperatively and one year after surgery. The preoperative and postoperative score differences were analyzed. We also assessed correlations between chronic ear survey scores and air conduction threshold. Results: Seventy six patients were enrolled in our study & marked improvements were found in total CES score & all subscale scores postoperatively (P value: 0.00). The total CES score and symptom subscale scores were observed greater improvement (P value: 0.00).Significant negative linear correlations were observed among total CES scores, symptom subscale scores and air conduction thresholds (P<.05). Conclusion: The present study suggests that canal wall down mastoidectomy (CWDM) provides a significant improvement in the post-operative Quality of life (QoL) &b there is a significant association between subjective outcomes &the objective audiometric results. Objective: study will determine clinical presentation, risk, and type of seizures in epileptic patients.  Methodology: This observational study was conducted in Medicine department People Medical College Hospital (PMCH) Nawabshah from January 2020 to December 2020. 110 patients were included for study after informed permission of the patient or their relative. Male and female were selected. Known epileptic patients were included in the study, patients with false seizures were excluded from the study. Statically analysis was done by software SSPS 22 version. Results: Age ranged 18 to 60 years. All the patients included in the study epilepsy was noted. The mean age of patients was 37.40 SD 8.71 years. The minimum age was 28 years while maximum 60 years. Pearson Chi-Square Value 105.000a  Asymp. Sig. (2-sided) .000, Likelihood Ratio Value 135.012  Asymp. Sig. (2-sided) .000, Linear-by-Linear Association Value 16.297 Asymp. Sig. (2-sided) .000 which were statistically significant. Conclusion: Epilepsy is treatable common neurological disease in Pakistan. quality of life can be improved by Education of the patients and their relatives, without socioeconomically  burden.
