The national total parental nutrition program with an emphasis on pediatrics started before several ago at Ministry of health hospitals In Kingdom of Saudi Arabia. The program covered several regions and consisted from the foundation of Intravenous Admixture and preparation of pediatric parenteral nutrition to administration and follow up of patients outcomes. In addition to the prior system, the new initiative project with the standardized formulation of pediatric’s parenteral nutrition is the complementary project of the parental nutrition for pediatrics. The project initiated to prevent drug-related problems of parental nutrition, improve patient clinical outcome and reduce the unnecessary economic burden on the healthcare system. It is the new system in the Middle East and Gulf counties in additional to Saudi Arabia. The initiatives are the systemic implementation of standardized pediatrics formulation using management project tools of starting new idea until finding in the ground.

Hipertensi adalah penyakit kardiovaskular yang paling sering terjadi. Diabetes Melitus merupakan suatu kelompok penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena sekresi insulin, kerja insulin atau kedua-duanya. Pasien usia lanjut (geriatri) adalah seseorang yang mencapai usia 60 (enam puluh tahun) ke atas. Pasien usia lanjut umumnya mengalami penurunan fungsi organ dan menerima bebagai macam obat, sehingga rentan Drug Related Problems (DRPs). Drug Related Problems (DRPs) merupakan suatu kondisi terkait dengan terapi obat yang secara nyata atau potensial mengganggu hasil klinis kesehatan yang diinginkan. Penelitian ini bertujuan untuk mengidentifikasi dan menganalisa Drug Related Problems (DRPs) pasien geriatri di Rawat Jalan Rumah Sakit Universitas Andalas. Penelitian ini merupakan penelitian deskriptif dengan pengambilan data secara retrospektif dari 82 rekam medis periode April Juli 2019 di Rawat Jalan Rumah Sakit Universitas Andalas. Hasil penelitian menunjukkan yang mengalami kejadian DRPs. Kategori DRPs yang teridentifikasi, meliputi indikasi tanpa terapi 1 pasien (1,2%), dosis berlebih 1 pasien (1,2%) dan interaksi obat 13 pasien (15,85%).

   Autism spectrum disorders (ASD) adalah gangguan perkembangan saraf dengan penyebab yang kompleks dari banyak fakor Penggunaan obat pada pasien autis harus dimonitoring untuk mencegah terjadinya drug related problems. Intervensi farmasis dengan mengidentifikasi kejadian drug related problem adalah kegiatan pelayanan asuhan kefarmasian untuk meningkatkan keberhasilan terapi. Penelitian ini adalah observasional dengan pengumpulan data secara retrospektif berdasarkan data rekam medik. Evaluasi kejadian drug related problems meliputi indikasi yang tidak diterapi, terapi tanpa indikasi, pemilihan obat yang tidak tepat, overdosis, under dosis, adverse drug reactions dan interaksi obat. Literatur yang digunakan sebagai acuan adalah Drug Information Handbook, 18thed, Stockley Drug Interaction, Drugs Interaction Facts 2001, dan Pharmacotherapy A Pathophysiologic Approach 2005 dan jurnal yang relevan.   Hasil penelitian diperoleh pasien dengan jenis kelamin laki-laki 20 pasien (77%), perempuan 6 pasien (23%). Usia antara 6-11 tahun yaitu 15 pasien (58%), 1-5 tahun terdapat 9 pasien (34%), usia <1 tahun dan 12-17 tahun masing-masing sebanyak 1 pasien (4%). Penyakit penyerta ISPA merupakan kasus yang paling banyak terjadi, terbanyak kedua adalah epilepsi dan gastroenteritis akut (GEA). Kejadian DRPs Indikasi tidak diterapi 9%, Terapi tanpa indikasi 9%, Pemilihan obat tidak tepat 9%, Over dosis 31%, Under dosis 33% dan interaksi obat 9%.   Terdapat 24 pasien ( 92,3%) yang mengalami DRPs potensial dan 2 pasien (7,7%) yang tidak mengalami. Kriteria DRPs dengan persentase tertinggi adalah under dosis sebanyak 33% dan over dosis sebanyak 31%.

Background: Drug-related problems (DRPs) are a frequent reason for emergency departments (EDs) visits. However, data about the risk factors associated with EDs revisits are limited. Objective: To develop and validate a predictive model indicating the risk factors associated with EDs revisit within 30 days of the first visit. Methods: A retrospective cohort study was conducted involving patients who attended an ED for DRPs related to cardiovascular drugs. A 30-day prediction model was created in a derivation cohort by logistic regression. An integer score proportional to the regression coefficient was assigned to the variables with P < .100 in the multivariate analysis. Results: 581 patients (mean age: 80.0 [12.6] years) were included, 133 (22.9%) revisited the ED within 30 days from discharge. Six factors (chronic kidney disease, chronic heart failure, visit to an ED in the preceding 3 months, high anticholinergic burden, DRPs associated with heparin, and safety-related DRPs) were identified as risk factors and combined into a final score, termed the DREAMER score. The model reached an area under the receiver operating curve values of 0.72 (95% confidence interval [CI] = 0.67-0.77) in the referral cohort and 0.71 (95% CI = 0.65-0.74) in the validation cohort ( P = .273). Three risk categories were generated, with the following scores and estimated risks: low risk (0-8 points): 11.6%; intermediate risk (9-14 points): 21.3%; and high risk (>14 points): 41.2%. Conclusion and Relevance: The DREAMER score identifies patients at high risk for ED revisit within 30 days from the first visit for a DRPs, being a useful tool to prioritize interventions on discharge.

<p>The main objective of Economic Cooperation Organization (ECO) is economic development in its region but directly unproductive profit seeking activities such as drug trafficking is the prominent barrier to reach this goal. All members spend lots of money to fight against drug trafficking, as all of them suffer from drug addiction and drug related problems. The first step to cope with this problem is to identify the factors and incentives that make this region vulnerable for drug trafficking activities and to unearth what makes this region a haven for drug traffickers. A bulk of literature supports the concept that organized criminal organizations are not able to operate when there are not any forms of corruption since they are strongly interrelated. In this paper, we analyze the link between drug trafficking and corruption in ECO region in order to develop ECO strategies to hamper and interrupt these transnational crimes. Corruption has posed major challenges to the efforts taken to control drug and also has seriously damaged the ECO members’ image in international community. One of the practical solutions is the responsibility of ECO organization in implementing rule of law in the region. Undoubtedly, fighting against corruption in ECO region is a joint responsibility of international and intercontinental community and this responsibility requires collective action and cooperation among countries in the region.</p>
