Abstract The present article discusses the creation of an elderly care model entitled Caring Senior. Population aging caused by demographic and epidemiological changes in Brazil, a relatively recent phenomenon, requires an efficient response. Based on a critical analysis of healthcare models for the elderly, the text presents a proposal for the healthcare of this age group, with emphasis on low intensity levels of care, focusing on health promotion and prevention, in order to avoid overload in the system. Integrated care models aim to solve the problem of fragmented and poorly coordinated care in current health systems. The more health professionals know about the history of their patients, the better the results. This is how the contemporary and resolutive models of care recommended by most major national and international health agencies should function. A higher quality, more resolutive and cost-effective care model is the focus of the present study.

Abstract Background: Dementia special care unit (D-SCU) is a main care mode for the elderly with dementia, and has increased steadily internationally. However, little is known about the D-SCUs of China. The present study aimed to analyze and summarize the types and standards of D-SCUs in China, and evaluate the care efficiency of the D-SCU care model.Methods: A cross-sectional survey was conducted between June 2018 and July 2019 in 6 cities of China. Quantitative data about the basic information and the care satisfaction were collected from 25 care facilities in the 6 cities. Qualitative data about the care status of the elderly with dementia, the standards, the management and the operation of the D-SCUs were collected among the 25 care facilities and the 3 cities that had issued standards for setting D-SCUs. Grounded theory method was used to analyze the qualitative data, and data envelopment analysis (DEA) method was applied to evaluate the care efficiency of the D-SCU care model.Results: The technical efficiency, pure technical efficiency and scale efficiency of care facilities with D-SCUs were higher than those of care facilities without D-SCUs (P=0.016, P=0.022, and P=0.021, respectively). The types of D-SCUs could be divided into independent, semi- independent, semi-dependent and dependent according to the architecture, physical space, and management mode. The standards for the area, environment, beds, service items of the D-SCUs were various among the 3 cities that had issued standards for setting D-SCUs.Conclusions: The care model of D-SCUs is effective and professional, and can be further promoted in China. The exploration and standard research on the D-SCUs should be strengthened in future.

Abstract The study addresses a health care model of the elderly practiced by a health care provider in the city of Rio de Janeiro, RJ, Brazil, focusing on the age group of the elderly, population segment in which the greatest misconceptions are identified. The current assistance models did not consider the deep transformations observed in the new epidemiological and demographic reality of the country. Considering that the aging process in Brazil is relatively recent, the article presents a proposal for a contemporary care model, recommended by the most important national and international health agencies as the most suitable for better care, focusing on the promotion, prevention of health care and the coordination of care, in order to avoid excesses, waste and fragmentation. The assistance and financial results of this study display very positive figures and indicate the path to be taken by healthcare companies. The model of remuneration for service providers and the indicators used for the establishment of bonuses are also presented, since they function as an instrument that stimulates and values good health care practices. This text is concerned with a higher quality, more resolutive and cost-effective care model, which is corroborated by the operator’s results presented here.

Earlier research by Gardell and Gustafsson indicates a general discrepancy between perceived needs and organizational structure in Swedish somatic hospitals; the work organization directs the work process as if cure and medical treatment were the only appropriate goals in almost all kinds of health care settings. The standard organizational model for general hospitals, here named “the acute care modei”–which is a merger of medical and administrative hierarchies–forces great segments of the staff into a work content that is neither appropriate for patients' needs nor satisfying for the personnel. The present study is a historical-sociological discourse in which the structural antecedents of the acute care model are traced. It gives an expose of the main stages in the formation of the Swedish health care system from the middle ages to the present. In 1864 a regulation of the hospital boards was issued. This meant the definite consolidation of the acute care model and was in line with earlier developments, which were characterized by an incremental interorganizational activity demarcation that divided the core of institutional care into three branches: somatic hospitals, mental hospitals, and homes for the elderly. The driving forces in the formation of the total health care system are shown to be closely related to premedical and extramedical factors, such as military needs, mercantilism, and the emergence of the middle class.

Resumo: Este estudo de revisão bibliográfica teve como objetivos identificar, em diferentes fontes de dados, literaturas acerca do aumento de idosos submetidos à terapia renal substitutiva e verificar o modelo de cuidado de enfermagem que vem sendo direcionado a essa população. Na contemporaneidade, em decorrência do prolongamento do processo de viver e envelhecer humano, observa-se a doença renal crônica como implicação da hipertensão e do diabetes, tornando-se uma epidemia entre as diferentes idades. A enfermagem motivada para a compreensão das necessidades de saúde da população idosa tem avançado na produção de conhecimento em gerontologia, despontando estudos voltados ao cuidado do idoso renal crônico, o que denota que os profissionais percebem estar frente a uma nova demanda de pacientes, a qual requer a construção de novas abordagens. Abstract: This study of bibliographic review aimed to identify, in different data sources, literatures about the increasing number of elders who undergo substitutive renal therapy and verify the nursing care model that has been directed to this population. Currently, due to the longer human life span and aging process, the chronic renal disease is observed as implication of high blood pressure and diabetes, becoming an epidemic among the different ages. The nursing motivated for the understanding of health needs of the elderly population has advanced in the production of knowledge in Geronthology, bringing out studies related to the care of the chronic renal elder, which shows that professionals are aware of this new demand of patients, which requires the construction of new approaches.
