The Effect of Uncontrolled Blood Sugar Levels on Macrovascular Complications in Type 2 Diabetes Mellitus Patients: Scoping ReviewDiabetes mellitus (DM) type 2 is a metabolic disorder in which the body is able to produce insulin but not in normal levels or cannot respond to the effects of insulin (insulin resistance), causing hyperglycemia. If blood sugar levels are not properly controlled, it can lead to chronic complications in the form of microvascular diseases such as eye diseases, neuropathy and nephropathy and macrovascular diseases such as coronary artery disease, peripheral arteries and cerebrovascular. In 2013, research showed the high prevalence of DM sufferers in Indonesia, especially in the province of Central Sulawesi. Objective: To see the emergence of macrovascular complications in type 2 diabetes mellitus patietns with uncontrolled blood sugar levels. Research method: Scoping review by identifying, analyzing and evaluating a scientific paper published within the last 10 years, conducted in September—December 2020. Systematic searches were carried out through database sites, i.e Pubmed, Springer Link and Proquest. In accordance with inclusion and exclusion criteria, and screening was carried out using the PRISMA eligibility criteria and presented in the form of a research table containing a reviewed descriptions of research articles (9 articles). Conclusion: Nine articles stated that there was an effect of uncontrolled blood sugar levels on the emergence of macrovascular complications in the form of coronary artery disease, peripheral arteries and cerebrovascular disease in patients with type 2 diabetes mellitus. Uncontrolled blood sugar levels were associated with atherosclerotic plaque formation and impaired endothelial function. Myocardiac infarction, gangrene, ischemic stroke are common presentations in patients with uncontrolled blood sugar levels.

AbstractIncreasing prevalence of type 2 Diabetes Mellitus in developed and developing countries is a health problem. Therefore, type 2 Diabetes Mellitus can be prevented by regular exercise, healthy and regular life. The purpose of this study was to determine the effectiveness of foot exercise on changes in blood sugar levels in elderly people with type 2 diabetes mellitus at the Ciracas District Health Center. This type of research is experimental with one group pre-post test design. The population in this study were 36 elderly people with type 2 Diabetes Mellitus in the Ciracas District Health Center. The sample in this study were 18 elderly with type 2 diabetes mellitus in Ciracas District Health Center. The sampling technique used was convenience sampling. Methods of data collection using an observation sheet before and after being given foot exercises. The statistical test used in this study is the paired T test. The results showed that there was no change in blood sugar levels before and after doing foot exercises, the statistical test results obtained a p value of 0.870, which means 0.870> 0.05, it can be concluded that there is no significant effect between foot exercise and changes in blood sugar levels in elderly patients. Type II diabetes mellitus at the Ciracas Subdistrict Health Center. The absence of any effect of foot exercises on changes in blood sugar levels in the elderly could be due to incomplete exercise, therefore foot exercise training to change blood sugar levels must be done in the elderly. Keywords : Foot exercise, Blood Sugar Levels, DM type 2

Background: Type 2 diabetes mellitus (T2DM) and depressive disorders are among the major health problems in our community. Chronic medical conditions, like diabetes are affectively stressful for patients and may influence pathophysiologic mechanisms as well as mood. This can lead to depression and poor prognosis of T2DM. This study was done to assess the prevalence of depression among T2DM patients and its association with glycemic control. Methods: This descriptive cross sectional study was carried out among 300 T2DM patients attending Urban Health Training Center (UHTC), Anakaputhur, which belongs to the urban field practice area of our institution. Fasting Blood Sugar levels (FBS) were assessed retrospectively from their medical records; and they were screened for depression by administering the nine items, “Patient Health Questionnaire - 9”.Results: Of the 300 T2DM patients, 54% were females. The mean age of the participants was 53±9.7 years. 31% of the study participants had fasting blood sugar (FBS) levels above 125 mg/dl. Depression was present in 39.7% of the individuals, among which, 17.3% had mild depression, 13% had moderate depression, 6.7% had moderately severe depression and 2.7% had severe depression. Depression was found to be significantly associated with increasing fasting blood sugar levels, diabetic complications, female gender and diabetic treatment. Conclusions: This study shows that depression is highly prevalent among T2DM patients and is associated with poor prognosis, as reflected by poor glycemic control (FBS) and the occurrence of diabetic complications. So screening and counselling for depression among T2DM patients have to be carried out for optimum control and treatment of T2DM.
