Korelasi Profil Darah Dengan CRP Serum pada Pasien Diabetes Mellitus dengan Ulkus Diabetikum

Diabetes mellitus is a metabolic disorder that experiences hyperglycemia and causes various complications, including diabetic foot ulcers (DFU). Hyperglycemia can cause chronic inflammation and decreased function of immune cells. This study aims to determine the correlation between blood profiles and serum C-Reactive Protein (CRP) in diabetes mellitus patients with diabetic ulcers. A total of 30 blood samples of diabetes mellitus patients with blood tests and serum CRP. Blood profile examination used an automatic method with a hematology analyzer, erythrocyte sedimentation rate (ESR) using the Westergreen method, and serum CRP using a semi-quantitative method. In the  correlation regression multiple test knowed that the parameters associated with CRP serum are hemoglobin (r = 23.553; p = 0.015), hematocrit (r = -8.678; p = 0.016), leukocyte count (r = 2.098; p = 0.004), and platelet count (r = -0.112; p = 0.002) because it has significance (p < 0.05). Meanwhile, the parameters of erythrocyte count (r = -8.717; p = 0.359), and ESR (r = -0.142; p = 0.109) had no effect on serum CRP levels because they had significance (p> 0.05). The conclusion of this study shows there is a strong correlation (r = 0.777) between blood profile and serum CRP in diabetes mellitus patients with diabetic foot ulcers.

Abstract Background and aims: Infected foot ulcer is one of the most feared complications of diabetes mellitus. Staphylococcus aureus is the most frequently isolated pathogen in diabetic foot infections. The aim of this study was to evaluate the prevalence of S. aureus strains involved in producing foot infections in diabetic patients and the antibiotic resistance pattern of these strains. Material and methods: The study included 33 S. aureus strains isolated from 55 diabetic foot ulcers. The subjects were selected from the 2465 patients with diabetes mellitus hospitalized in the Timişoara Diabetes Clinic, between 2011 and 2013. Germs’ identification relied on cultural and biochemical characteristics. Final identification and antimicrobial testing were performed using the Vitek 2 (Bio Merieux France) automatic analyzer. Results: All the 55 samples collected from diabetic foot ulcers were positive. We isolated 64 bacterial strains (some samples were positive for 2 microorganisms). The most frequently isolated germ was S. aureus, in 33 samples (51.56%). All these S. aureus strains showed resistance to benzylpenicillin, while only 33.33% were methicillin-resistant (MRSA). Conclusions: The most frequently isolated germ in the wound secretions from diabetic foot ulcers was S. aureus. The highest percentage of antimicrobial resistance was recorded to benzylpenicillin and erythromycin.

The prevalence of diabetes mellitus in Manggarai Regency in 2019 is 535 people and some of them have diabetic foot ulcers. Patients with diabetic foot ulcers had a higher risk of falling than others.This study was to determine the risk factors for diabetic foot ulcers and the risk of falls in patients with type 2 diabetes mellitus. A quantitative study using a cross-sectional study was conducted at BLUD RSUD dr. Ben Mboi  in April-June 2020 (n = 51).The sampling technique used was purposive sampling with a questionnaire of diabetic foot screening and risk stratification form and Morse scale. Logistic regression results showed that there was a significant relationship between claudication (AOR: 8.409, 95% CI 1.664-42.500, p value 0.010), history of previous diabetic foot ulcers (AOR: 5,680, 95% CI 1,151-28,035, p value 0.033) with diabetic foot ulcers. There was a significant relationship between hypertension (AOR: 0.152, 95% CI 0.028-0.834, p value 0.030), diabetic foot ulcers (AOR: 11.392, 95% CI 1.277-101.651, p value 0.029) with the risk of falling for patients with type 2 diabetes mellitus.  Assessment of the risk of diabetic foot ulcers and the risk of falls should be done in patients with diabetes mellitus with or without neuropathy peripheral

ABSTRAK Jumlah penderita DM (diabetes melitus) saat ini semakin meningkat. Salah satu komplikasi yang terjadi yaitu DFU (diabetic foot ulcers). Banyak cara yang dapat dilakukan untuk mencegah DFU, salah satunya dengan diabetic foot exercise. Penelitian ini bertujuan untuk mengetahui efektifitas diabetic foot exercise terhadap risiko dfu (diabetic foot ulcers) pasien diabetes mellitus. Penelitian menggunakan design pre-eksperimen dengan pendekatan one-group pra-post test design. Populasi penderita diabetes yang berjumlah 60 orang, besar sampel 40 orang yang diambil menggunakan teknik purposive sampling. Resiko DFU dinilai menggunakan inlow’s 60-second diabetic foot screening tool dengan metode observasi. Analisa data menggunakan uji statistik Wilcoxon. Hasil penelitian ini didapatkan bahwa sebelum dilakukan diabetic foot exercise sebagian besar reponden mempunyai risiko sedang sebanyak 30 orang (75%), sesudah dilakukan diabetic foot exercise diperoleh bahwa sebagian besar responden risiko rendah sebanyak 32 orang (80%). Uji statistik Wilcoxon diperoleh nilai p value = 0,001 dimana nilai p value<α (0,05) yang berarti ada pengaruh diabetic foot exercise terhadap risiko diabetic foot ulcers. Diabetes foot exercise sangat efektif untuk penderita diabetes dalam mencegah risiko DFU. Penderita diabetes dapat melakukan diabetic foot exercise 2 kali dalam seminggu secara teratur.  Kata Kunci : Diabetes mellitus, Diabetic foot ulcers, diabetic foot exercise

International  Diabetes  Federation  (IDF) reported that the number of diabetes mellitus patients worldwide increases every year. Diabetes mellitus is a chronic disease due to damage to the pancreas in producing enough insulin and characterized by impaired metabolism of fats, increased blood sugar, carbohydrates and protein both absolutely and relatively. Diabetic foot ulcers (DFU) are among the most common complications in diabetic patients and are associated with high mortality, morbidity, and health costs.   This study was to determine the effect of health education on knowledge improvement about diabetic ulcers in the regional hospital of Tidore Islands.  This research is a quasi-experimental type with a pretest-posttest without a control group design. The research sample consisted of 30 respondents who met the inclusion criteria. Data were analyzed using a paired test and processed with statistic version 21. Based on the research results, there was an improved knowledge in pre and post-health education counseling, where the average score before health education counseling was 11.6. After health counseling, the average value was 15.0. The results showed the effect of knowledge in the prevention of diabetic ulcers with a p-value =0,000.  Health education is an effort to improve patient's ability to prevent diabetic foot ulcers that have been proven in several scientific studies. Health education methods provide additional information so that patients who previously wondered about their disease can directly ask health workers. Health education affects the knowledge improvement of Diabetes Mellitus patients in the Regional hospital of Tidore Islands

INTRODUCTION : Diabetes mellitus is a common metabolic disorder, prevalence steadily increasing over the past few decades. The complications associated with it , hence , has also increased. Diabetic foot ulcer is one of the most serious complications , utilizing resources, significantly contributing to the morbidity of the patient. There is hence, a need to correctly identify the severity of the diabetic foot ulcer so as to plan the appropriate management and to help in counselling of such patients. AIM : To assess severity in diabetic foot ulcer using diabetic ulcer severity score. MATERIAL AND METHODOLOGY : This is a hospital based longitudinal study , conducted on 93 study subjects admitted with diabetic foot ulcers. Diabetic ulcer severity score was calculated for each patient . The score was calculated by adding scores of the respective parameters constituting site of ulcer, number of ulcers, presence/absence of pedal pulsations, presence/ absence of bone involvement. Each patient was followed up for a period of 6 months , or earlier in case of patient undergoing minor/major amputation. After the study was conducted , analysis was done by calculating various percentages of healing /amputation with respect to the score. RESULTS : Out of the total 93 study subjects , 74.2% were males. The mean age was calculated to be 59.6 years with maximum number of subjects being in 55-60 years of age group range. Majority of them had diabetic ulcer severity score of 2 (42%). Out of the total study subjects , 58% had a complete healing , 28% underwent minor amputation whereas 14% underwent major amputation. 100% of the study participants with score 0 had healing of ulcer which decreased to 85% for score 1 , 53.8% for score 2 , 6.25% for score 3 and 0% for score 4. This was suggestive of poorer chances of healing as the diabetic ulcer severity score increases. CONCLUSION : With the increasing incidence of patients diagnosed with diabetes mellitus , the rate of complications of diabetes has also increased over the past few decades including the risk and occurence of diabetic foot ulcers There is an increasing need for diabetic foot ulcer prognostication systems and universal use of the same. Thus ,we recommend the use of diabetic ulcer severity score as a prognostic tool to assess the severity of the diabetic foot which will further enhance communication and counselling of the patient and will help in providing the appropriate treatment to such patients.

At the moment, a lot of scientific research focused on the role of immune mechanisms in diabetic foot ulcers development and impaired healing. A 3D skin culture system as a relevant skin model may prove valuable in investigating these mechanisms and may be a useful tool to study interactions between different cell types such as keratinocytes, fibroblasts, and immune cells. The aim of our research was to study keratinocytes and fibroblasts viability in co-culture with immune factors of patients with diabetes mellitus type 2 (DM2) and patients with diabetes and chronic foot ulcers in a 3D skin culture system. In this study, the multilayer 3D immunocompetent model of human skin comprising keratinocytes, fibroblasts, and mononuclears in an agarose-fibronectin gel was used. The human immortalized keratinocyte cell line, HaCaT, and primary fibroblast cell culture isolated from skin samples of healthy man in abdominal surgery were used for the 3D system. For the experiment 20 % serum of 9 patients with chronic diabetic foot ulcers (without active inflammation signs), 9 diabetic type 2 patients and 9 healthy people, and mononuclears of the same groups of patients were used. 9 experimental series with 3 repeats were carried out. Mononuclears of patients with DM2 and DM2 and diabetic foot syndrome (DFS) had a greater inhibitory effect on fibroblasts, significantly inhibiting their proliferation to a level of 83.78 [79.03; 89.53] % vs 70.18 [66.38; 72.10] % vs 95.40 [91.75; 99.05] %, H = 21.259, p <0.001 – DM2, DFS, and the control group, respectively. There was no significant difference in the cytoinhibitory effect of mononuclears on keratinocytes between different groups: 96.40 [92.82; 100.50] % vs 93.61 [86.80; 97.10] % vs 92.87 [85.15; 95.25] %, H = 4.459, p = 0.108 – control, DM2 and DFS group, respectively. Adding serum to the culture system influenced significantly the viability of neither keratynocytes – 99.40 [95.35; 102.05]  % vs 98.60 [90.55; 100.40] % vs 94.79 [91.65; 98.16] %, H = 3.030, p = 0.220 nor of fibroblasts – 95.61 [92.39; 100.19] % vs 95.80 [88.99; 102.15] % vs 96.20 [99.69; 88.70] %, H = 0.353, p = 0.838, control, DM2 and DFS group, respectively. It was determined that the fibroblasts vialability significantly decreases after introducing mononuclears of patients with DM and patients with DM and chronic diabetic foot ulcers to the co-culture system. Adding serum of these patient groups to the culture system doesn’t influence significantly the viability of skin cells.

Pendahuluan:Ulkus kaki diabetik merupakan salah satu komplikasi yang sering terjadi pada penderita Diabetes Mellitus. Telemedicinemerupakan pennggunaan sarana teknologi komunikasi untuk mendapatkan dan mendesiminasi data dan pelayanan medis. Sistematik review ini bertujuan mengkaji efektivitas telemedicine untuk pengelolaan perawatan ulkus kaki diabetik.Metode:kata kunci yang digunakan adalah "telemedicine" "healing" dan "diabetic foot" dan "diabetic foot ulcers", yang dihubungankan dengan Boolean operator "AND".  Pencarian artikel dengan menggunakan kerangka PICOT pada database internasional; EBSCO, Science Direct, Scopus, ProQuest, selama lima tahun, 2017-2021. Hasil:Diperoleh 15 artikel yang sesuai dengan kriteria inklusi yaitu berfokus pada telemedicine pada pasien dengan perawatan ulkus kaki diabetic, penelitian jenis Randomized Control Trialdan quasi-experiment. Studi desain lainnya dimasukkan dalam kriteria eksklusi. Artikel yang didapatkan dianalisa dengan Preferred Reporting Items for Systematic Review and Meta-Analyzes(PRISMA). Simpulan:Telemedicine memiliki beberapa inovasi yang dapat digunakan dalam mengamati, mengantisipasi, dan mengobati penderita kaki diabetik. Inovasi dalam bentuk Telemedicine skin thermography, photo imaging, dan mobile phone onlinedapat memberikan hasil yang bagus dalam pemulihan kelangsungan hidup untuk pasien dengan perawatan ulkus kaki diabetik.
